The epidemiology of ceftriaxone and cotrimoxazole-resistant Escherichia coli from humans, poultry, and their environment in central Malawi: A cross-sectional study.

The poultry industry represents an important reservoir for clinical antibiotic resistance, especially in low- and middle-income-countries where food security pressures drive intensified farming and antibiotic use. However, evidence linking poultry production to public health risks remains limited in settings such as Malawi. This study examined Escherichia coli resistance to ceftriaxone and cotrimoxazole, two antibiotics with distinct stewardship priorities. We conducted a cross-sectional study between July and September 2022 in Malawi's Central region, targeting poultry farms. Our mixed-method

Analysis of the dual role of amyloid-beta in Alzheimer's disease through multi-omics integration.

Accumulation of amyloid-beta is highly important in the development of Alzheimer's disease. Given the limitations of the amyloid cascade hypothesis and the repeated clinical failures of anti-amyloid-beta therapies, researchers are increasingly exploring the infection hypothesis. This review explores the dual behaviors of amyloid-beta in Alzheimer's disease, with a particular focus on its protective role against infection by microorganisms and its complicated connections with innate immune system. This new opinion holds that amyloid-beta can play an antimicrobial peptide role. During microbial

Circulating cell-free DNA as a biomarker for immune function and disease progression in HIV-infected patients: a cross-sectional observational study.

BACKGROUND: Human immunodeficiency virus type 1 (HIV-1) causes immune deficiency, particularly CD4+T-cell depletion. This cross-sectional study explored circulating cell-free DNA (ccfDNA) as a biomarker for immune function and disease progression in HIV-1-infected individuals. METHODS: We measured ccfDNA levels, CD4+T-cell counts, immune activation/apoptosis markers, and viral load in 79 patients receiving antiretroviral therapy (37 immune responders, 42 non-responders) and 35 controls. Correlation and receiver operating characteristic (ROC) curve analysis were performed to assess relationshi

Challenges in Balancing Hemostasis and Thrombosis in Therapy Tailoring for Hemophilia: A Narrative Review.

Hemostasis and thrombosis reflect a delicate balance, regulated by the interplay between procoagulant and anticoagulant mechanisms. Hemophilia is traditionally viewed as a bleeding disorder, but emerging evidence highlights the paradoxical risks of thrombosis in hemophilia patients. We explore the landscape of hemophilia management, emphasizing challenges of balancing hemostasis in the context of aging, novel non-factor replacement therapies (NRTs), and comorbidity-driven thrombotic complications. Therapeutic approaches, including innovative NRTs, such as emicizumab, or rebalancing agents (e.g

Prevalence and determinants of unintended pregnancy among female sex workers (FSW) in Jashore, Bangladesh.

BACKGROUND: Unintended pregnancy among female sex workers (FSW) is a pressing reproductive health concern attributable to risky sexual behaviors, healthcare inequities and poor negotiation powers with male sex partners. However, evidence is scarce on the prevalence and determinants of unintended pregnancies among FSW, which is crucial for enhancing reproductive healthcare. This analysis aims to measure the prevalence of lifetime unintended pregnancies and their associated factors. METHODS: A cross-sectional study was conducted on 327 FSW in Jashore (a border belt district of Bangladesh) from

Ischemic Stroke as the First Manifestation of Primary Antiphospholipid Syndrome in a Patient With a Recent Diagnosis of HIV: A Case Report.

The importance of stroke in young adults lies in the clinical challenge it represents, because of the frequent association with uncommon causes and non-atherosclerotic causes, such as the antiphospholipid syndrome (APS). This last one plays a very important role in strokes in this population, especially in patients with the absence of conventional vascular risk. We report a 34-year-old male adult with no prior history of thrombotic events or cardiovascular risk factors, who started with focal neurological symptoms. Many studies were performed; a simple coronal skull computed tomography scan re

Acceptability Limits for Inter-instrument Variability in Viral Serological Tests on Two Enhanced Chemiluminescence Immunoassay Instruments: Evidence From a Hospital Laboratory-Based Study.

Background Reliable viral serological testing is essential for accurate diagnosis and public health management of HIV, hepatitis B, and hepatitis C. Chemiluminescent immunoassay platforms, particularly enhanced chemiluminescence (eCLIA) systems, are widely used; however, inter-instrument variability remains a challenge in laboratories operating multiple analyzers. Defining acceptable limits of variation is crucial for ensuring diagnostic accuracy, laboratory quality assurance, and clinical decision-making. Objectives To evaluate the correlation and agreement between two eCLIA platforms (Vitros

Effectiveness of early warning scores in predicting in-hospital mortality and adverse events in adult general-ward patients in low- and middle-income countries: A systematic review.

BACKGROUND: Early Warning Score systems (EWSs) based on bedside physiological parameters are widely implemented in high-income countries, yet their performance and utility in low- and middle-income countries (LMICs) remain uncertain, particularly in emergency and acute care settings, due to limited evidence and health system constraints. We systematically reviewed the effectiveness of EWSs for predicting in-hospital mortality and adverse events among adult general-ward patients in LMICs. METHODS: This systematic review followed PRISMA 2020 and PRISMA-S guidance and was prospectively registere

Acceptability of Cardiovascular Disease Point-of-Care Diagnostics in Primary Care Settings: A Scoping Review.

BACKGROUND AND AIMS: Cardiovascular disease (CVD) remains the leading global cause of death, with 80% of deaths occurring in low- and middle-income countries (LMICs). Improving access to screening and early diagnosis is essential. Point-of-care testing (POCT), which provides rapid results near the patient, is particularly valuable in resource-limited settings. While POCT has been successfully implemented for infectious diseases like HIV and TB, investment in CVD-focused POCT remains limited. This scoping review maps global evidence on the acceptability of CVD POCT, guided by the World Health O

The roles of salivary secretory IgA on the development of oral candidiasis.

Oral candidiasis, an opportunistic fungal infection mainly caused by Candida albicans, is highly prevalent in immunocompromised individuals. Saliva acts as the oral cavity's first line of defense, with secretory immunoglobulin A (sIgA) as its key specific immune component. In this review, we systematically clarify sIgA's multifaceted roles in oral immunity and its significance in the pathogenesis, progression, and management of oral candidiasis. We detail sIgA's biological characteristics (synthesis, secretion) and core mechanisms: immune exclusion (inhibiting fungal adhesion/invasion), virule

Classic Kaposi Sarcoma: Current Treatment Strategies and Emerging Therapeutic Approaches.

Classic Kaposi sarcoma (CKS) is a rare vascular neoplasm driven by infection with Kaposi sarcoma-associated herpesvirus (KSHV) and characterized by a heterogeneous geographic distribution. While the etiology of other Kaposi's sarcoma (KS) variants is well established, the underlying mechanisms of CKS appear multifactorial, with several risk factors, among which advanced age and male sex are most significant. Due to the rarity of CKS and its often indolent clinical course, clinical trials are sparse, and current knowledge of therapeutic approaches remains limited. Most available clinical trials

Association Between Gut Microbiota and HIV Infection Risk: Insights from Mendelian Randomization and 16S rRNA Amplicon Sequencing.

Observational evidence links gut microbiota (GM) dysbiosis to HIV infection; however, the causal relationship between them has not been established. Mendelian randomization (MR) and 16S rRNA gene sequencing analyses were performed to identify gut microbial taxa associated with HIV infection risk. MR analysis results identified 18 gut microbial taxa associated with HIV infection (p values < 0.05), of which 16 taxa were detected in the 16S rRNA gene sequencing data. Following the exclusion of seven taxa with low relative abundance, eight taxa with potential relationships with HIV infection were

Feasibility and acceptability of task sharing collection of HIV viral load dried blood spot samples with community lay cadres: A cross-sectional diagnostic validation study in Zimbabwe.

Viral load (VL) testing is a critical tool for clinical management of HIV, yet healthcare worker (HCW) shortages remain a key barrier to VL sample collection. This study assessed the feasibility and acceptability of VL dried blood spot (DBS) sample collection by community lay cadres (CLCs) compared to collection by trained HCWs. We implemented a cross-sectional diagnostic validation study across 10 purposefully selected public health facilities in Zimbabwe over six weeks in March-April 2024. Two DBS samples were collected from 374 participants: a reference sample collected by a HCW and a valid

Stress granules at the crossroads of retroviral replication and antiviral immunity: mechanisms and therapeutic opportunities.

Stress granules (SGs) are dynamic cytoplasmic ribonucleoprotein aggregates that form in response to cellular stress and function as key regulators of mRNA translation, stability, and antiviral defense. Increasing evidence demonstrates that retroviruses, including HIV-1, HTLV-1, and other oncogenic retroviruses, interact extensively with stress granule pathways to promote viral replication, persistence, and immune evasion. This review summarizes current knowledge of the molecular mechanisms governing stress granule assembly and highlights how retroviruses manipulate SG components, such as G3BP1

Determinants of Quality of Life of People Living with Human Immunodeficiency Virus in Africa: A Systematic Review.

Human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome continues to shape the lives of millions in Africa and remains a major public health concern. Despite advances in prevention and treatment, the quality of life (QoL) of people living with HIV (PLHIV) has often been overlooked. For many PLHIV, survival is no longer the only concern; emotional well-being, social ties, economic security, and dignity are now equally vital. QoL is increasingly recognized as a key outcome of HIV care, indicating whether treatment enables PLHIV not only to survive but to live well and participate f
