Recent data have suggested that there are racial differences in the susceptibility to infection by Mycobacterium tuberculosis. An opportunity to test this suggestion was afforded by an outbreak of tuberculosis in a racially mixed elementary school in St. Louis County, Missouri. The diagnosis of tuberculosis infection is based on a pathological dermal induration (positive Mantoux reaction) after injection of a standard dose of tuberculin (2 U). Patients with positive tuberculin reaction, abnormal chest radiograph, and negative bacteriology are often treated empirically for tuberculosis (TB) after exclusion of other causes. Therapy generally consists of two bactericidal drugs (rifampin [RIF] and isoniazid [INH]) for 9 months or INH for 9 to 12 months. With such a small bacillary population, even less therapy might suffice. Thus we began in January 1980 to discontinue therapy at 4 months when there was sufficient evidence of a paucity of bacilli demonstrated by at least three negative smears and cultures for TB at the start of therapy.

In patients with lung tuberculosis--depending on the severity of the disease--delayed-type hypersensitivity reaction is weakened, also in cases of non-miliary tuberculosis. On analysing the mononuclear cells of peripheral blood, abnormalities are found, in particular an increase in the monocyte count. In addition to increasing levels of serum immunoglobulin A and G, circulating immune complexes were detected in 50% of the patients. In 50% of the patients we found elevated IgE-serum levels, which decreased in response to treatment but did not return to normal.

To identify the correlation between incidence of anti-phospholipid antibodies and fetal prognosis in pregnant SLE patients, we measured the amount of anti-cardiolipin antibody in their sera, using solid-phase enzyme immunoassay (EIA) methods. In 1986 and 1987 nine patients with a raised antibody titre to Yersinia pseudotuberculosis were found. Two of these results were almost certainly due to the cross reactivity between some Y pseudotuberculosis serogroups and some salmonella serotypes. Of the other seven patients with otherwise unexplained Y pseudotuberculosis serology, three suffered from severe arthritis and two had symptoms suggestive of sacroiliitis. Our data and published reports suggest that reactive arthritis might follow both Y enterocolitica and Y pseudotuberculosis infections.

Morbidity caused by tuberculosis was determined in a cohort of 9,328 refugees from Southeast Asia during the first 5 yr after their immigration to the United States. Generalized lymphadenopathy in intravenous drug abusers (IVDAs) at risk for AIDS has not been well studied. We have retrospectively analyzed the results of lymph node biopsies obtained from 27 patients referred to the Infectious Diseases Service for evaluation of generalized lymphadenopathy and suspected AIDS during a recent 18-month period. Subacute thyroiditis is extremely rare during the first decade of life. We describe a case of subacute thyroiditis in a 2 year old child whose initial clinical, sonographic, and radioisotopic features were indistinguishable from acute suppurative thyroiditis. The diagnosis was established by a raised antibody titre against adenovirus and typical increased thyroid function tests.
