The glucagonoma syndrome is a rare clinical entity characterized by a glucagon-producing tumor of the pancreas, necrolytic migratory erythema, weight loss and usually decreased glucose tolerance. Anergy in tuberculosis is of considerable clinical and immunological interest. Although negative skin reactions may be secondary to improper tuberculin testing or to certain diseases, drugs, vaccinations and constitutional factors known to affect cell-mediated immunity, often no underlying explanation is apparent. The local graft-versus-host reaction (GVHR) following intradermal injection of peripheral blood lymphocytes (PBL) in Lewis rats was examined in order to test the cell-mediated immunity (CMI) in 31 patients with renal disease and in healthy controls. It was found that patients with lipoid nephrosis (LN) in the nephrotic stage and systemic lupus erythematosus (SLE) had less mean lesion of the local GVHR and a smaller proportion with positive reactions as compared to normal subjects. Circulating immune complexes (CIC) were demonstrated in sera of Guatemalan patients with onchocerciasis by Raji cell radioimmunoassay. 44% of patients but none of controls had abnormally high concentrations of CIC in their sera. X-ray films of the thorax in two levels were prepared of 15 children of 3-14 years of age who had not been BCG-inoculated, after the tuberculin test had shown a positive reaction. Besides bacteriology and bronchoscopy, tomography of the hilus and of the anterior mediastinum was performed to exclude hilar lymph node enlargements. In this manner, it became possible to disclose an enlargement of lymphatic nodes in four children in whom plain radiography of the thorax had not yielded any abnormal findings.

A broad spectrum of immunologic parameters was investigated in 13 cases of myotonic muscular dystrophy (MyD), including those for cellular immunity which has attracted little attention in the past. Joint symptoms in sarcoidosis are early manifestations of this systemic disease; patients with such symptoms, however, are seldom examined by a rheumatologist. A highly statistically significant correlation was found between asbestosis and impaired responsiveness in skin reaction to intermediate and second strength tuberculin and SK-SD. Considering ANA incidence these antibodies were found with higher frequency in asbestos workers who lacked a cutaneous response to the recall antigens. In asbestosis cases peripheral blood lymphocyte profiles are also abnormal demonstrating low proportions of E-RFC. Moreover, MIF test results showed the impairment of this cytokine generation when lymphocytes were stimulated with SK-SD, PPD and PHA in asbestosis cases.

A 31-year-old black woman had acquired ichthyosis. Histologic examination of her skin revealed a sarcoidal reaction in the dermis. The diagnosis of sarcoidosis was supported by negative tuberculin (intermediate strength purified protein derivative) and Candida albicans extract intradermal skin tests; by a positive Kveim test; and by roentgenographic evidence of bilateral hilar adenopathy, paratracheal node enlargement, and diffuse pulmonary parenchymal changes. Sarcoidosis must be considered when acquired ichthyosis develops in a patient.

To investigate whether steroids influence the course and prognosis of acute sarcoidosis (Löfgren syndrome) 35 patients who developed this disease during the period 1960-1970 were examined and retrospectively divided into two groups, one of which had received steroids and the other not. From 1966 to 1973, a total of 30 cases of tuberculous peritonitis were seen in Seattle-King County. Abdominal pain, swelling, and constitutional symptoms were the most frequent initial complaints. Radiographic abnormalities consistent with tuberculosis were present in 25 cases, and pulmonary disease was proven in ten. An initial tuberculin test with intermediate-strength purified protein derivative of tuberculin was negative in 19 of 27 patients. Six of 13 initial nonreactors still had negative reactions on repeat testing, and four appeared to be anergic when retested one to four months later.

Personal experience on the PPD-test in 154 patients with chronic liver disease is reported. The results show a frequent hyperergic response. In chronic active hepatitis (CAH, n=58) 70% of the HBsAg negative and 48% of the HBsAg positive cases showed a CMI against human liver specific proteins (HLPI). Five patients with advanced breast cancer were treated with pooled dialyzable transfer factor from healthy adult donors. The period of treatment ranged from 21 to 310 days, the total dose from 20 to 257 ml. Transfer factor did not elicit inflammatory or hypersensitivity reactions or detectable formation of antibody to itself, nor any hematological or biochemical abnormalities or other side effects. Three patients became responsive (by skin test) to tuberculin and/or streptococcal antigens. Marked partial regression of the breast cancer, lasting 6 months, was observed in one patient.
