RATIONALE & OBJECTIVE: Autoantibodies targeting complement factor B (anti-FB) are among the most recently described alternative pathway anomalies. Although they have been strongly associated with post-infectious glomerulonephritis in children, the clinical phenotypes associated with such antibodies, along with their effects on the alternative pathway in adults, remain elusive and require further characterization, which was the goal of this study.
STUDY DESIGN: Retrospective case series.
SETTING & PARTICIPANTS: Patients for whom anti-FB immunoglobulin G (IgG) was systematically detected by th

We report the cases of 3 patients with biopsy-proven light-chain cardiac amyloidosis (AL-CA) presenting with varying clinical manifestations and abnormal findings on Tc-based bone-avid scintigraphy, illustrating the diagnostic challenges of distinguishing AL-CA from transthyretin-related CA. All patients demonstrated typical electrocardiographic and echocardiographic features of CA, including low voltage and apical-sparing strain pattern. Despite abnormal scintigraphy suggestive of transthyretin-related CA, concurrent abnormal light-chain assays raised diagnostic uncertainty. Endomyocardial bi

OBJECTIVE: To stratify the progression rate to clinical stage 3 type 1 diabetes in children with early-stage disease.
RESEARCH DESIGN AND METHODS: The Fr1da study tested 211,464 children aged 1.75-10 years for islet autoantibodies. Children with early-stage type 1 diabetes were classified as stage 1 or stage 2 by oral glucose tolerance test (OGTT) and hemoglobin A1c (HbA1c) using current American Diabetes Association criteria and were followed 3-6 months. We applied our previously developed progression likelihood score (PLS), a composite of HbA1c, 90-min OGTT glucose, and islet antigen 2 anti

Beta-2 glycoprotein I (β2GPI) as a core target antigen of antiphospholipid antibodies (aPLs), is a multifunctional plasma protein with phospholipid-binding properties. Under physiological conditions, β2GPI not only binds to negatively charged phospholipids via its domain V but also interacts with various molecules, such as angiostatin4.5 (AS4.5) and annexin II. These interactions play key roles in maintaining the balance between procoagulant and anticoagulant processes and in promoting angiogenesis. β2GPI binds to pathophysiological ligands, such as apoptotic cells, oxidized low-density lipopr

BACKGROUND AND OBJECTIVES: Although anti-Ma2 antibodies (Ma2-Abs) typically associate with paraneoplastic encephalitis, a subset of patients with Ma2-Abs does not have any detectable tumor. The clinical specificities of these idiopathic cases are unknown. The aim of this study was to describe clinical phenotypes and outcomes of patients with idiopathic Ma2-Abs (I-Ma2) compared with patients with paraneoplastic Ma2-Abs (PNS-Ma2).
METHODS: A retrospective review of the French Reference Center of Paraneoplastic Neurologic Syndromes and Autoimmune Encephalitis database was conducted to identify c

UNLABELLED: This study aimed to identify the candidate genes of dihydroartemisinin (DHA) in acting against hepatocellular carcinoma (HCC), as well as its association with lipid metabolism-related genes (LMRGs), thereby providing a basis for the application of DHA in anti-HCC therapy.
METHODS: Potential candidate genes of DHA were predicted using databases. Differentially expressed genes (DEGs) were screened from HCC datasets in the Gene Expression Omnibus (GEO) database. Common candidate genes of DHA and HCC were obtained through Weighted Gene Co-expression Network Analysis (WGCNA) and inters

Our center had a case of a 20-year old female with severe bi-ventricular dysfunction, leading to heart transplantation. Strong GSTT1 antibody was detected pre-transplant, which encouraged us to further study its possible significance. Glutathione S-transferases (GSTs) are a class of enzymes that metabolize xenobiotics. We hypothesized that high-titer non-HLA antibodies to GSTT1 can be generated in recipients who have a complete GSTT1 gene deletion and are exposed to GSTT1 antigen. When a donor organ is transplanted, the anti-GSTT1 antibodies could bind to functional GSTT1 which has been expose

Anti-IgLON5 disease is an autoimmune encephalitis that presents with a heterogenous clinical phenotype, including sleep disorders, movement abnormalities and bulbar involvement. It is characterized by autoantibodies against IgLON5, 85% association with HLA-DQB1*05:∼ and a brainstem-dominant tauopathy. Cellular and murine models report pathogenic effects of the autoantibodies, and neurodegenerative factors suggest progressive atrophy as a common sequela. However, evidence from in vivo patient data and long-term follow-up is limited, and the degree of progression remains elusive. In this multice

Targeting dysregulated lipid metabolism within the tumor microenvironment (TME) has emerged as a promising strategy for restoring anti-tumor immunity and reversing immune suppression to improve therapeutic efficiency. This study reports a nanomaterial platform for co-delivery of irisin and manganese ions (Mn2+) to exert cumulative effects of modulating lipid metabolism dysregulation and ameliorating the immunosuppressive TME in triple-negative breast cancer (TNBC). Irisin stimulates lipolysis and inhibits lipogenesis, whereas Mn2+ strengthen irisin binding to integrin αVβ5 over-expressed in va

We report the case of a 45-year-old woman who was referred to our Cardiology Department because of persistent exertional dyspnea and peripheral edema. She had an established clinical diagnosis of hereditary hemorrhagic telangiectasia with multiple gastrointestinal telangiectasias that had been submitted to repeat embolization in the past and arteriovenous malformations in the liver and lungs. Complete blood count was diagnostic for severe anemia (hemoglobin 5-6 g/dL). Since the hereditary hemorrhagic telangiectasia diagnosis 3 years prior, the patient informed us that she has undergone 27 bloo

Tubular atrophy and interstitial fibrosis are basic renal pathological changes in autosomal dominant tubulointerstitial kidney disease (ADTKD). Reduced secretion or abnormal structure of uromodulin (UMOD) are recognised pathogenic factors of ADTKD. Studies show uromodulin binds complement factor H (cFH), enhancing its ability to inhibit complement activation. Overactivation of the complement system contributes to tubulointerstitial injury. Therefore, exploring the UMOD-tubulointerstitial fibrosis link may aid in the development of treatment for ADTKD-UMOD. Immunofluorescence staining detected

A raised serum ferritin (>300 µg/l in men or >200 µg/l in women) is a common finding during routine investigations in primary care. It is often non-specific since it is both an iron-storage protein and acute-phase reactant. As such, a raised ferritin level should be interpreted primarily as a context-dependent signal of disease activity rather than a direct measure of iron burden. Common causes of hyperferritinaemia include liver pathologies, metabolic syndrome and autoimmune conditions and infection. Hereditary haemochromatosis (HH) is also a cause of elevated ferritin levels but less common

Alström syndrome (AS) is a rare autosomal-recessive ciliopathy caused by biallelic variants in ALMS1, with an incidence of 1 in 10,000 to 1 in 1,000,000 live births. We report a female diagnosed at age 5 with a previously unreported homozygous nonsense variant in ALMS1: c.4740C>G (p.Tyr1580Ter), located in exon 8, a known hotspot for pathogenic variants. The variant aligns with the loss-of-function mechanism seen in most ALMS1-related AS cases and is found within a 15 Mb run of homozygosity, consistent with her history of parental consanguinity. Her clinical picture included progressive hearin
