[Compound heterozygous plasminogen mutations causing hereditary plasminogen deficiency: a family study and mechanistic analysis].

Objective: To investigate the molecular mechanisms underlying compound heterozygous mutations in a patient with hereditary plasminogen (PLG) deficiency. Methods: The proband presented to the First Affiliated Hospital of Wenzhou Medical University with a 2-day history of left-sided limb weakness. Plasminogen activity (PLG∶A) and plasminogen antigen (PLG∶Ag) were measured by chromogenic substrate and enzyme-linked immunosorbent assays, respectively, in the proband and family members (eight individuals across three generations). Sanger sequencing was performed to identify the PLG mutation sites.

High-density EEG in the eyes closed condition reveals early reduction of alpha oscillations after galcanezumab: A pilot study in migraine.

OBJECTIVE: This study investigates the impact of galcanezumab on spontaneous cortical electrical activity, focusing on its potential to reduce abnormal alpha oscillations. METHODS: We recorded high-density spontaneous EEG (hd-EEG) in 10 migraine patients before (T0) and 7 days after the first galcanezumab administration (T1) in eyes open (EO) and closed (EC) conditions. For each EEG frequency band, we assessed total power, including both aperiodic and periodic components, of the Power Spectral Density (PSD), and compared values between T0 and T1. For the alpha band, we further estimated the c

Autoimmune heparin-induced thrombocytopenia following cardiac surgery.

An elderly man underwent cardiac surgery with heparin anticoagulation; postoperatively, the platelet counts steadily fell, reaching 50×109/L on postoperative day 5 (POD5), prompting testing for heparin-induced thrombocytopenia (HIT) antibodies; brain imaging showed parietal stroke. Screening immunoassay for HIT antibodies yielded a strong-positive result; the platelet activation test (serotonin-release assay) gave an atypical profile (strong serotonin release at 0, 0.1 and 0.3 U/mL heparin with only partial inhibition by a platelet Fc receptor-blocking monoclonal antibody). Anticoagulation was

In utero exposure to NMDA receptor autoantibodies disrupts hippocampal circuit maturation.

Maternal anti-neuronal autoantibodies (ABs) have been linked to abnormal brain maturation and may increase the risk of neurodevelopmental disorders, yet their pathogenetic mechanisms remain elusive. Here, we show that in utero exposure to ABs targeting the NR1 subunit of N-methyl-D-aspartate (NMDA) receptors (NR1 ABs) disrupts hippocampal circuit maturation in mice. Using a passive-transfer model, we find that transplacental NR1 ABs impair GABAergic transmission in CA1 pyramidal cells (PCs) ex vivo. Ca2+ imaging and computational modeling reveal that this deficit compromises early synchronized

Severe polyneuropathy in HIV-negative human herpesvirus 8-associated multicentric Castleman disease successfully treated with rituximab and liposomal doxorubicin.

Human herpesvirus 8-associated multicentric Castleman disease (HHV-8-associated MCD) is a rare lymphoproliferative disorder that typically occurs in immunocompromised patients with HIV infection. HHV-8-associated MCD is characterized by systemic inflammatory manifestations driven by viral interleukin-6, where neurological complications are uncommon. This contrasts with POEMS-associated MCD, which is HHV-8 negative and frequently complicated by polyneuropathy. A 56-year-old HIV-negative man presented with prolonged high-grade fever, severe anemia, multiple mildly enlarged lymph nodes, hepatospl

When cold complicates: the impact of cryoglobulins and pre-analytical conditions on monoclonal protein analysis.

Cryoprecipitation of monoclonal protein in two patients from separate hospitals led to falsely reduced concentrations. In one case, this was only later discovered because the hematology analyzer falsely flagged the cryoglobulins as abnormal platelets and lymphocytosis. This resulted in the generation of a peripheral blood smear in which the cryoprecipitation was microscopically identified. In both cases, initially unnoticed discrepancies existed between the total immunoglobulin and albumin concentrations derived from the general chemistry analyzer and those from serum protein electrophoresis a

A case of IgA vasculitis developing in a patient with Crohn's disease treated with adalimumab.

This study aimed to investigate a suggestive case of IgA vasculitis that developed following acute tonsillitis in a patient with Crohn's disease treated with adalimumab. IgA vasculitis and Crohn's disease can both cause abdominal pain, joint pain, and erythema, making an early diagnosis difficult. When they occur together, it can be challenging to determine which is the primary condition. Due to similar symptoms, a careful assessment and thorough understanding of each disease's characteristics are essential for an accurate diagnosis and proper management. In this case, the overlapping of immun

Cryo-EM structures of light chain fibrils from abdominal fat biopsies of multiple myeloma patients.

Multiple myeloma (MM) is a hematological malignancy characterized by the abnormal proliferation of plasma cells in the bone marrow and is closely associated with systemic light chain amyloidosis (AL), a disease triggered by the amyloid aggregation of antibody light chain (LC). High-resolution structures of amyloid LC fibrils from MM patients remain largely unexplored. Here, we extracted LC amyloid fibrils directly from abdominal fat biopsies of two living MM patients. Using cryo-electron microscopy (cryo-EM), we further determined the structures of three distinct LC fibril morphologies, which

Immune-Related Thyroid Dysfunction in PD-L1 High Non-Oncogene-Addicted NSCLC Treated with First-Line Pembrolizumab: Incidence, Timing, and Predictive Impact.

In metastatic NSCLC with high PD-L1 expression (TPS ≥ 50%), pembrolizumab monotherapy yields durable benefit in a subset of patients. Immune-related thyroid dysfunction (irTD) is common during PD-1/PD-L1 blockade, but its predictive value remains uncertain. We conducted a retrospective, single-center study including 363 patients with metastatic NSCLC, PD-L1 TPS ≥ 50%, and no actionable oncogenic drivers treated with first-line pembrolizumab. Thyroid function tests were performed at baseline and every six weeks. irTD was defined based on laboratory abnormalities with or without clinical symptom

Normal delivery following use of galcanezumab until the first and second trimesters of pregnancy: A report of 2 cases.

RATIONALE: Clinical trials have demonstrated the efficacy, rapid action, and safety of calcitonin gene related peptide-related antibody drugs for migraine prevention, and subcutaneous galcanezumab was launched in Japan in April 2021. However, the safety of this drug in pregnant migraine patients has not been demonstrated in humans, with evidence of safety available only from animal studies. We report 2 cases in which subcutaneous galcanezumab was administered continuously until the first and second trimesters of pregnancy, resulting in normal births without any complications for either the mot

The evolving landscape of amyloid-targeting therapies in Alzheimer's disease: progress and challenges.

BACKGROUND: Alzheimer's disease (AD) is the leading cause of dementia worldwide. Amyloid-β (Aβ) accumulation has long guided therapeutic development, though early β- and γ-secretase inhibitors failed to demonstrate clinical benefit. METHODS: A narrative review was conducted based on a structured search of MEDLINE, Embase, and ClinicalTrials.gov, supplemented by manual screening of key reviews and Phase 3 trial reports on amyloid-targeting therapies. RESULTS: Monoclonal antibodies including aducanumab, lecanemab, and donanemab achieve significant amyloid clearance and modest slowing of cognit

Antibody-based nanoparticles in Alzheimer's disease: Innovations in diagnosis and therapy.

BACKGROUND: Alzheimer's disease (AD) is a progressive neurodegenerative disorder characterized by accumulation of amyloid-β (Aβ) plaque, tangles of tau neurofibres, chronic neuroinflammation, and dysfunction of the blood brain barrier (BBB). Monoclonal antibodies targeting Aβ and tau have focused disease-modifying potential, but their clinical impact is limited in brain penetration, immunogenicity, amyloid-related imaging abnormalities (ARIA), high treatment costs, and the need for repeated intravenous administration. These limitations has lead to discover in antibody-based nanoparticle platfo

Memory Consolidation and ARIA in Individuals Receiving Anti-amyloid Monoclonal Antibodies.

Amyloid related imaging abnormalities (ARIA) are the most significant risk associated with the use of anti-amyloid monoclonal antibodies (MAB) for Alzheimer's disease (AD). Currently, the presence of the APOE ε4 allele is the best predictor for the development of ARIA. However, the degree of baseline memory impairment has not been fully explored as a risk factor for ARIA. Here, we examined MAB outcomes in a memory clinic population and compared patients with AD who developed ARIA to a case-matched group who did not develop ARIA. Participants who developed ARIA had greater numbers of recall int

Temporal association of diffuse large B-cell lymphoma with PD-1 inhibitor therapy in a patient with gastric adenocarcinoma: a case report.

BACKGROUND: Immune checkpoint inhibitors (ICIs) have demonstrated substantial clinical benefit across a wide range of malignancies. With their expanding use, uncommon immune-related events, including hematologic abnormalities and lymphoid proliferations, are increasingly recognized. However, a causal relationship between ICI exposure and lymphoma development remains unproven. CASE DESCRIPTION: We report a 70-year-old woman with moderately to poorly differentiated gastric adenocarcinoma who was diagnosed with diffuse large B-cell lymphoma (DLBCL) during postoperative treatment with FOLFOX chem
