Metabolomic Biomarkers for the Human Diagnosis of Neglected Tropical Diseases: A Systematic Review.

Neglected tropical diseases (NTDs) affect more than 1.6 billion people worldwide and remain major contributors to morbidity, disability, stigma, and poverty. Metabolomics has emerged as a promising approach for investigating disease-associated biochemical alterations and identifying biomarkers for diagnosis, prognosis, and treatment monitoring. This systematic review evaluated the available evidence on metabolomic profiles in WHO-defined NTDs and their potential clinical applications. MEDLINE/PubMed, Web of Science, and Scopus were searched through March 2026. Observational studies investigati

Spatial and seasonal dynamics of Simulium damnosum s.l. and entomological indicators of onchocercosis in the provinces of Huambo and Cuanza-Sul in Angola.

BACKGROUND: Onchocerciasis remains a public health problem in Angola. Despite the implementation of onchocerciasis control programs based on mass treatment with ivermectin, current entomological data remain limited, making it difficult to accurately assess the potential risk of transmission. Our objectives were to analyze the spatial and seasonal dynamics of Simulium damnosum s.l. and the main entomological indicators of onchocerciasis in the provinces of Huambo and Cuanza Sul, Angola, to assess the potential entomological risk of transmission. METHODS: We captured black flies using the human

Retinal manifestations of neglected tropical diseases.

PURPOSE OF REVIEW: Neglected tropical diseases (NTDs) are increasingly encountered outside endemic regions due to global migration, international travel, and climate change. This review addresses the timely need for ophthalmologists to recognize the retinal manifestations of NTDs, as these conditions now appear in developed nations where clinicians may have limited prior exposure. RECENT FINDINGS: Modern multimodal imaging has revealed distinct retinal patterns that are frequently seen in NTDs. Dengue and chikungunya present with maculopathy, foveolitis, and retinal hemorrhages. Onchocerciasi

Discovery of GPCR Antagonists with Potent In Vitro and In Vivo Macrofilaricidal Activity.

Chronic filarial infections remain a global health burden, with limited adulticidal therapies highlighting the need for new drugs. We explored repurposing human GPCR antagonists, including clemizole, pimozide, proroxan, and suloctidil, along with 45 rationally designed analogs for activity against Brugia pahangi and two Onchocerca species. Of those, 27 showed in vitro adulticidal activity. In vivo, clemizole and analog C-8 significantly reduced worm burden by 60.5% and 50.8%, respectively, while S-17 and PMZ-6 modestly reduced worm burden (36% and 25%, respectively); however, all three signifi

Modelling the impact of mass drug administration with ivermectin or moxidectin on human onchocerciasis when there is sub-optimal response of parasites to treatment.

Onchocerciasis is a disease caused by the filarial nematode parasite Onchocerca volvulus and is transmitted by blackflies after they ingest larval offspring, microfilariae, found in the skin of infected people. The elimination of transmission within endemic communities in sub-Saharan Africa, the Americas, and Yemen depends on mass drug administration of ivermectin (MDAi) at high treatment coverage until the prevalence of the parasite in people or in blackflies reaches below a threshold value. In Africa, there are a number of communities where worms that are less susceptible to the effects of i

Sustained Drug Exposure Drives Efficacy in Mice: PK/PD Analysis of Corallopyronin A against Wolbachia using Physiologically Based Absorption Modeling.

Corallopyronin A (CorA) depletes essential Wolbachia endosymbionts of filarial nematodes and is therefore a promising candidate for treating the neglected tropical diseases lymphatic filariasis and onchocerciasis. To optimize anti-Wolbachia therapy, the pharmacokinetic/pharmacodynamic (PK/PD) relationship was investigated. Due to the intra-nematode and -cellular target, conventional minimal inhibitory concentration could not be determined. Instead, an iterative approach was used to define PD efficacy thresholds and compare them with IC₅₀/IC₉₀ values from infected cells. A physiologically based

Modelling the effects of immigration on the re-introduction of onchocerciasis.

BACKGROUND: Onchocerciasis is a filarial neglected tropical disease targeted by the World Health Organization for elimination (interruption) of transmission (EOT), principally by mass drug administration (MDA) with ivermectin. Variable effectiveness and success of MDA, among other factors, has led to a markedly heterogeneous contemporary spatial landscape of endemicity and transmission, with some foci having achieved or nearing EOT, while in others, transmission persists despite decades of MDA or has only recently been identified. Communities reaching EOT or free from infection are thus vulner

Assessment of the efficacy and safety of two albendazole regimens for the treatment of hypermicrofilaraemic loiasis in adults in Woleu-Ntem Province, Gabon: A phase IIb single-blind randomised controlled trial.

BACKGROUND: Loa (L.) loa hypermicrofilaraemia (≥ 8,000 mf/mL) increases the risk of severe adverse events during mass ivermectin administration for onchocerciasis control. Albendazole has been proposed as a potential alternative for reducing microfilaraemia prior to ivermectin administration. METHODOLOGY AND PRINCIPAL FINDINGS: This prospective study was conducted in northern Gabon from November 2021 to April 2022. Individuals infected with L. loa were screened and allocated to three groups: two treatment arms receiving 400 mg or 800 mg of albendazole daily for 30 days among hypermicrofilarae

Nuclear, mitochondrial, and Wolbachia endosymbiont genomes of Onchocerca lupi, Portugal.

Zoonotic Onchocerca lupi (Spirurida, Onchocercidae) has attracted the interest of the scientific community worldwide, by causing severe ocular infections in domestic animals (dogs, cats) and can infect wild carnivores (wolves, coyotes), as well as humans. Though recent advancements in scientific knowledge have been gained, gaps still remain about the biology of this filarioid, as well as its genetic structure. Based on mitochondrial genes, two highly divergent genotypes were identified, in the Iberian Peninsula (genotype 2) and Europe, Asia, and the United States (genotype 1), meanwhile only a

Helminth Antigens Modulate Virus-Induced Activation of CD154 (CD40L) Expression on T Cells in Onchocerca volvulus-Infected Individuals.

BACKGROUND: The interaction between helminth and viral infections has important implications for understanding viral disease outcomes and vaccine efficacy in helminth-endemic regions. We previously demonstrated that helminth seropositivity is associated with reduced Th1/Th17 cytokine levels and reduced COVID-19 severity; however, the underlying immunological mechanisms remain unclear. This study further investigated these mechanisms by assessing how helminth antigens influence SARS-CoV-2-induced T-cell responses in individuals infected with filarial parasites in vitro. METHODS: Peripheral blo

Impact of seasonal changes on human onchocerciasis transmission and disease elimination.

The current guidelines for stopping treatment and verifying elimination of onchocerciasis recommend entomological and epidemiological surveys to be performed during the peak transmission season, which is generally assumed to occur during the rainy season, although this may not necessarily hold true in all transmission zones (TZs). This review has synthesized data, from a literature search, on the effects of seasonality on transmission in West and Central Africa. Seasonality affects vector population dynamics by altering terrestrial and aquatic ecology. In TZs with perennial rivers, peak transm
