A systematic review comparing the performance of alternative blackfly (Simulium) trapping methods against the standard human landing catch (HLC) for onchocerciasis surveillance.

Despite decades of control efforts, onchocerciasis remains a major public health concern in Africa, the Americas, and Yemen. Human Landing Catch (HLC) is the primary method for collecting blackflies and is central to surveillance. However, HLC raises ethical concerns due to collectors' exposure to painful and potentially infectious bites and faces operational challenges in areas of very low or high transmission. Consequently, several alternative blackfly trapping methods have been investigated, but no comprehensive synthesis comparing their effectiveness against standard HLC across studies has

Perceived barriers and opportunities to the continuous distribution of ivermectin in the Cameroon primary health care system: an exploratory study of major stakeholders.

BACKGROUND: Onchocerciasis, a debilitating parasitic disease transmitted by blackflies, remains a significant public health concern in Cameroon. While mass drug administration (MDA) campaigns with ivermectin offer success in controlling transmission, logistical and financial challenges threaten long-term sustainability. Integrating ivermectin distribution into the existing primary healthcare system (PHC) could offer a promising alternative. We aimed to explore challenges and opportunities associated with this integration. METHODS: This study employed a mixed-methods approach conducted across

Assessment of scabies diagnosis and the impact of MDA for onchocerciasis on the burden of scabies in an endemic community in Ghana.

BACKGROUND: Scabies is a neglected tropical disease with substantial public health impact, especially in resource-limited countries like Ghana. Worldwide, more than 300 million people are affected by scabies. In Ghana, scabies outbreaks have been occurring frequently, especially among school-going children. This study assessed scabies burden, diagnostic accuracy, and the effect of ivermectin-based Mass Drug Administration (MDA) on scabies prevalence in the Sefwi Wiawso Municipality of the Western North Region of Ghana. METHODS: A total of 341 participants were recruited at health facilities w

Preclinical animal models for onchocerciasis and loiasis: A systematic review of applications in drug screening.

BACKGROUND: Onchocerciasis and loiasis are co-endemic filarial neglected tropical diseases in Central and West Africa. While ivermectin-based mass drug administration has reduced onchocerciasis burden, it can trigger severe neurological adverse events in individuals with high Loa loa microfilaraemia. This limitation highlights the urgent need for safe macrofilaricidal therapies and appropriate preclinical models. METHODOLOGY/PRINCIPAL FINDINGS: We conducted a systematic review following PRISMA guidelines to evaluate animal models used for preclinical drug screening in onchocerciasis and loias

From the Laboratory to the Field: Assessing the Usability of the NTDscope in the Tropical Rainforest Region of Gabon.

In regions where Loa loa is coendemic with lymphatic filariasis and onchocerciasis, mass drug administration of ivermectin has posed a risk for serious adverse reactions in individuals with high microfilarial loads. To overcome this problem, easy-to-use point-of-care diagnostic tools are crucial for identifying individuals at risk in population-based control programs. The NTDscope is a device that uses smartphone components for video microscopy and may support decentralized L. loa microfilariae screening under field conditions. This study evaluated the usability and operational feasibility of

Persistent onchocerciasis after 24 years of ivermectin mass treatment in the Democratic Republic of the Congo: a cross-sectional analytical study.

OBJECTIVES: Despite more than two decades of community-directed treatment with ivermectin (CDTI), onchocerciasis remains endemic in several parts of sub-Saharan Africa, including the Democratic Republic of the Congo (DRC). Suboptimal therapeutic coverage and environmental factors may hinder progress toward elimination. METHODS: We conducted a cross-sectional analytical study between June and December 2025 in five health zones of Kasai Central, DRC. Individuals aged ≥5 years and residing in the area for at least five years were selected using multistage stratified sampling. Onchocerciasis infe

Proposed rodent models for Onchocerciasis-associated epilepsy: Integrating Infection, microfilarial load, and parasite secretome biology.

Onchocerciasis-associated epilepsy (OAE) clusters where transmission of Onchocerca volvulus is intense, yet the causal steps to epileptogenesis remain unresolved. Rodent models can close this gap by enabling standardized, brain-anchored readouts that are impossible in humans. Here we synthesize current platforms and outline a pragmatic path to high construct validity. Implantation models using Onchocerca ochengi establish feasibility and reveal longitudinal physiological and behavioral effects, but lack natural infection routes and rarely generate sustained skin microfilariae. Gerbil models ar

Multibead-based immunoassay provides one-step species-specific serodiagnosis of filarial infections and Strongyloides stercoralis in travelers and immigrants.

BACKGROUND: Previously, antifilarial antibody testing in the evaluation of returned travelers and immigrants to North America has relied on IgG- and IgG4-specific responses to crude filarial extracts (BmA), tests that suffer from significant cross-reactivity with Strongyloides and cannot distinguish among the infecting filarial species. Nevertheless, between 1992 and 2016 we have tested >11000 samples from providers across North America in whom a filarial infection was being considered; close to 20% were able to be definitively diagnosed with a filarial infection (based on IgG4 anti-BmA reacti

Assessing the efficacy of high-dose rifampicin plus albendazole against onchocerciasis.

BACKGROUND: Onchocerciasis, also known as river blindness, remains a public health challenge in tropical regions. Despite significant gains through mass drug administration (MDA) with ivermectin, the infection remains persistent in many endemic foci. While the disease burden has reduced, elimination may require alternative strategies, including macrofilaricidal therapies. Rifampicin (RIF) has demonstrated strong anti-Wolbachia activity in vitro and in animal models, but its clinical efficacy in humans remains uncertain. METHODS: We conducted a phase II randomised, controlled trial in Ghana to

Declining Onchocerca volvulus transmission despite limited ivermectin delivery in the Kakoi-Koda focus, Ituri, Democratic Republic of the Congo: An epidemiological, entomological and landscape evidence synthesis.

BACKGROUND: The Kakoi-Koda focus in Ituri Province was historically hyperendemic for onchocerciasis, yet screening for a recent trial suggested a marked decline, including in Logo Health Zone, which had never received routine ivermectin. We explored whether this decline extended across the focus and how infection indicators corresponded spatially with ivermectin delivery, entomological observations and deforestation. METHODOLOGY: We conducted a scoping evidence synthesis of epidemiological, programmatic, entomological and geospatial sources. Against a 2003 nodule-mapping baseline, change was

Modelling anti-Ov16 seroprevalence for the control and elimination of onchocerciasis.

The World Health Organization aims to verify elimination of onchocerciasis transmission in 12 endemic countries by 2030. Serological assays detecting IgG4 antibodies against the Onchocerca volvulus Ov16 antigen guide decision-making regarding start- and stop-mass ivermectin treatment, post-treatment and post-elimination surveillance. Transmission models are crucial for understanding anti-Ov16 IgG4 seroprevalence dynamics. Uncertainty associated with the parasite stage(s) stimulating seroconversion and duration of antibody persistence has hitherto hindered progress. We use the individual-based

Diethylcarbamazine: A review of its modes of action.

Filariases are neglected tropical diseases caused by Clade III parasitic nematodes transmitted by biting insects as microfilaria which mature into female and male adults in the host. One example of filariasis is lymphatic filariasis or elephantiasis, caused by three species of filaria: Brugia malayi, Wuchereria bancrofti, and/or Brugia timori. Elephantiasis results from the occlusion of lymphatic vessels by adult parasites and subsequent swelling of the limbs. It is a disfiguring disease that occurs in 120 million people worldwide. Another filarial disease is onchocerciasis caused by Onchocerc

BACKGROUND: Maudsley Family Therapy (MFT), and its manualised version, Family-Based Therapy (FBT), are the only well-established treatment interventions for adolescent anorexia nervosa (AN), with treatment efficacy primarily measured by improvements in eating behaviours and weight restoration. A crucial component of this therapy is an intensive home-based refeeding intervention that requires a substantial commitment from parents for up to one year. While this treatment works to restore weight in a proportion of adolescents, very little is known about its impacts on family distress, relationshi

ABC-RF-rejection: A two-stage machine-learning-enhanced framework for efficient likelihood-free inference.

Accurate parameter estimation is fundamental to quantitative epidemiology as it provides the foundation for robust modelling and evidence-based decision-making. We present a novel two-stage framework, that is designed to enhance computational efficiency for parameter estimation of complex, stochastic, or spatially explicit models. Traditional Approximate Bayesian Computation (ABC) methods often face prohibitive costs when likelihoods are analytically intractable and acceptance rates are low. Our hybrid approach, ABC-RF-rejection, integrates ABC rejection sampling with Random Forest (RF) classi

The influence of human population movement on mass drug administration for neglected tropical diseases: a scoping review.

BACKGROUND: Neglected tropical diseases (NTDs), including but not limited to lymphatic filariasis, onchocerciasis, trachoma, schistosomiasis, and soil-transmitted helminths, remain a major public health challenge in Africa. Mass drug administration (MDA) is a key strategy for NTD control and elimination. However, mobile populations such as internally displaced persons (IDPs), nomadic groups, and seasonal workers often face major constraints that reduce MDA effectiveness defined by coverage. Understanding how mobility and its underlying drivers affect MDA implementation is therefore critical to
