Usability, acceptability, and cost of the SD BIOLINE Ov16 rapid diagnostic test for onchocerciasis surveillance in endemic communities in the middle belt of Ghana.

BACKGROUND: Previous studies in the Bono Region (middle belt) of Ghana have reported persistent Onchocerca volvulus infection and associated morbidities after nearly three decades of ivermectin treatment. This study aimed to assess the usability, acceptability, and cost of the Ov16 SD BIOLINE rapid diagnostic test (Ov16 RDT) in onchocerciasis surveillance activities in the middle belt of Ghana. METHODOLOGY: A cross-sectional study was conducted in 6 endemic communities in the Tain District and Wenchi Municipality. A total of 254 individuals (54% females; median age (range)=31 (5-83) years), a

The last mile in onchocerciasis elimination: diagnostic challenges.

Global elimination of onchocerciasis is progressing. However, challenges persist in achieving elimination across all endemic regions, particularly in sub-Saharan Africa, due to diagnostic limitations and operational complexity in coendemic, resource-constrained settings. This review synthesizes the historical and programmatic trajectory of onchocerciasis control, outlines the WHO elimination framework and evaluates diagnostic tools across programmatic phases. We examine clinical, molecular, serological, and entomological tools, highlighting the strengths and limitations while emphasizing the u

Repurposing of Anti-Infectives for the Management of Onchocerciasis Using Machine Learning and Protein Docking Studies.

There is a need to improve the discovery of new drugs for neglected tropical diseases (NTDs), as the lack of financial incentives has slowed their development. Currently, ivermectin and moxidectin are used in the management of onchocerciasis. We present a proof-of-concept study based on computational methods to find anti-infectives that can be repurposed or serve as lead compounds for onchocerciasis. A combination of exploratory data analysis, machine learning (ML), and molecular docking studies was used to evaluate 58 anti-infective agents. Out of the 58 test drugs, 14 were predicted by at le

A cross-sectional investigation of the ophthalmological impact of loiasis in Cameroon, Central Africa.

BACKGROUND: Current knowledge of ocular manifestations of loiasis is limited to the transient subconjunctival passage of the adult filaria and anecdotal reports of posterior segment lesions. Therefore, the ocular burden of loiasis is likely underestimated since it has never been systematically assessed at the population level. We aimed to evaluate the relationship of Loa loa microfilaremia and recent eye worm passage with chronic ocular lesions identified through comprehensive ophthalmological assessment in an endemic area. METHODOLOGY/PRINCIPAL FINDINGS: Subjects aged ≥ 15 years, residing in

Community-directed treatment with ivermectin in Maridi, South Sudan: Impact of an onchocerciasis awareness campaign and bi-annual treatment on therapeutic coverage.

BACKGROUND: A high onchocerciasis disease burden and a low coverage of community-directed treatment with ivermectin (CDTI) have been observed in many parts of South Sudan. In the Maridi County, CDTI was re-introduced in 2017 and various interventions implemented to improve coverage. METHODS: Through successive community-based surveys, we investigated whether an onchocerciasis awareness campaign and a switch from annual to bi-annual distribution of ivermectin in Maridi County increased CDTI coverage. We also reviewed the evolution of ivermectin distribution in Maridi since 2017 and identified

Safety and efficacy of 3- and 5-day regimens of levamisole in loiasis: a randomized, placebo-controlled, double-blind clinical trial.

Individuals with high Loa loa microfilarial densities (MFD) risk serious adverse events (SAEs) following ivermectin treatment. A single dose of levamisole (LEV) induces a temporary, progressive MFD decrease. This double-blind, randomized, placebo-controlled trial evaluated the safety and efficacy of 3-day and 5-day LEV regimens for the treatment of loiasis in the Republic of the Congo. Participants were randomly assigned to receive placebo (PLA), LEV-3, or LEV-5. Safety was assessed by the occurrence of SAEs and the frequency and severity of AEs. Efficacy was measured by changes in Loa loa MFD

Strategies and Recommendations for the Prevention and Control of War-Facilitated Onchocerciasis Reemergence in Sudan.

Onchocerciasis is a neglected tropical disease that has severe health and socioeconomic impacts on the mostly poor and underserved communities in disease-endemic areas. Despite significant global progress toward the elimination of onchocerciasis transmission there remains a high risk of reemergence in areas where the disease was previously eliminated. Here we discuss the potential risk factors for the reemergence of onchocerciasis in disease-free zones in Sudan and nearby countries resulting from war-induced massive population displacement. We urge public health, humanitarian, and development

Positive allosteric modulation of emodepside sensitive Brugia malayi SLO-1F and Onchocerca volvulus SLO-1A potassium channels by GoSlo-SR-5-69.

Human lymphatic filariasis and onchocerciasis are Neglected Tropical Diseases (NTDs), of major public health concern. Prophylaxis and treatment rely on anthelmintics that effectively eliminate migrating microfilariae but lack efficacy against adult filarial worms. To expedite the elimination of both diseases, drugs with adulticidal activity are needed. The broad-spectrum anthelmintic emodepside, a nematode selective SLO-1 K channel activator, is a promising candidate for the treatment of onchocerciasis due to its macrofilaricidal activity against Onchocerca volvulus. Nevertheless, it is less e

Constructing Success: The World Bank, Onchocerciasis Control, and What Lies Beneath Triumphalist Global Health Narratives.

Using a historical case study of the World Bank and World Health Organization (WHO)'s Onchocerciasis Control Programme (OCP, 1972-2002), I explore how success is conceptualised in global health and why it matters for policy and priority-setting. First, I summarise the 'dominant' OCP success narrative that has emerged since the 1980s, which is based on public health, socio-economic and humanitarian justifications for the programme's effectiveness. Next, I analyse how socio-economic metrics linking the programme's disease control to increased labour productivity and agricultural land availabilit

Ivermectin: a therapeutic strategy to treat patients with resistant epilepsy.

Drug-resistant epilepsy (DRE) remains a major clinical challenge, with up to one-third of patients experiencing uncontrolled seizures despite polytherapy. Ivermectin (IVM), a macrocyclic lactone with antiparasitic, neuromodulatory, and anti-inflammatory properties, has recently emerged as a candidate adjunctive therapy. We conducted an observational study including 146 patients with highly refractory epilepsy treated with IVM in addition to standard antiseizure medication (ASM). Clinical outcomes were assessed before and after IVM initiation, with seizure frequency recorded from patient diarie

Vaccination against Onchocerca volvulus induces IgG-mediated protective immunity dependent on neutrophils and complement.

Onchocerciasis remains a significant cause of morbidity and economic loss in sub-Saharan Africa. Despite the existence of effective therapeutics, a prophylactic vaccine targeting the etiologic agent, Onchocerca volvulus, is needed to control ongoing disease and transmission. Mice were vaccinated against O. volvulus with a fusion of the recombinant antigens Ov-103 and Ov-RAL-2 (Ov-FUS-1) with Advax-CpG adjuvant. Immunized mice developed protective immunity with the killing of third-stage larvae (L3) within 36 h of challenge infection. IgG from immunized mice passively transferred protective imm

Preliminary Evaluation of the Diagnostic Performance of OvMANE1 and OvMCBL02 Multiepitope Antigens for the Non-Invasive Diagnosis of Onchocerciasis Exposure.

A shift in the public health goal for onchocerciasis from control to elimination implies that the treatment of onchocerciasis must be extended to communities that are hypoendemic for the disease. However, in such communities, the majority of the population may not manifest the symptoms of onchocerciasis. As a result, they may be reluctant to take part in epidemiological surveys aimed at monitoring parasite transmission, particularly due to the invasive nature of the currently approved diagnostic tests. This reluctance is compounded by the absence of visible, severe manifestations of the diseas

Effects of emodepside on single-channel properties of Onchocerca volvulus SLO-1A (BK) potassium channels.

BACKGROUND: Control of onchocerciasis (river blindness of humans due to infection with the filarial nematode, Onchocerca volvulus) remains a challenge because of the lack of effective adulticides and vaccines. Emodepside is a broad-spectrum veterinary anthelmintic that has been found to inhibit nematode muscle activity by activating their tetrameric SLO-1K channels. Emodepside has adulticidal activity and is being trialed for onchocerciasis treatment, but the molecular mode of action of emodepside is still being elucidated. Here, we examine the single-channel currents of Ovo-SLO-1A, a SLO-1K s
