Placentas From COVID-19-Positive Mothers: A Morphological Study of 20 Cases in Marrakesh, Morocco.

Background Due to COVID-19, hospitals have seen a large influx of COVID-19-positive pregnant women. In order to study the impact of this virus on the infant and the possibility of its vertical transmission, we led a morphological study of 20 placentas from women infected or with a history of SARS-CoV-2 infection during pregnancy. Objective The aim of our study was to present the morphological characteristics of the placentas from these women and to correlate them with the clinical and biological findings, highlighting their relevance to pregnancy outcome and vertical transmission. Study design

Prevalence and clinical significance of proteinuria in intrahepatic cholestasis of pregnancy: A retrospective cohort study.

Intrahepatic cholestasis of pregnancy (IHCP) is the most common pregnancy-specific liver disease. This study aimed to determine the prevalence of proteinuria in patients with IHCP and to evaluate its association with adverse pregnancy outcomes. This retrospective cohort study included pregnant patients with gestational age > 24 weeks who were diagnosed with IHCP and completed 24-hour urine protein collection at Haseki Training and Research Hospital (January 2018-December 2024). Proteinuria was defined as ≥300 mg/24-h urine collection. Patients were categorized into 3 groups: non-proteinuric, i

Identification of an unusual variant of Fanconi-Bickel syndrome presenting as proximal tubulopathy and short stature.

Fanconi-Bickel syndrome (FBS) is caused by biallelic pathogenic variants in the SLC2A2 gene, which encodes the glucose transporter protein 2 (GLUT2), leading to a rare disorder that affects glucose homeostasis. The exact mechanisms by which FBS leads to dysglycaemia are not clearly understood. The clinical manifestations are those related to dysglycaemia, proximal tubulopathy (glycosuria, galactosuria, aminoaciduria, proteinuria, phosphaturia), hepatomegaly, galactose intolerance, rickets and short stature. We report a teenage girl with persistent glycosuria and short stature. Isolated Proteinuria in Uncomplicated Twin Pregnancies vs. Singleton Pregnancies; Measured by Spot Protein-Creatinine Ratio.

BACKGROUND AND OBJECTIVES: Proteinuria is used for prediction of pre-eclampsia; however, the cutoff values are suggested for singleton pregnancies and might not be appropriate for twin pregnancies. In this study, we examined whether the urine spot protein-creatinine ratio (SPCR) is higher in twin than singleton pregnancies and what factors are associated with this difference. MATERIALS AND METHODS: In this prospective longitudinal study with repeated measures, pregnant mothers (aged 15-45 years) who referred to Mahdiye Hospital, Tehran, Iran, from April 2021 to November 2022 for routine prena

Diabetes and hyperglucosuria exacerbate the severity of urinary tract infection caused by uropathogenic E. coli in the mouse model.

Diabetes and urinary tract infection (UTI) are major global public health problems. We investigated the effects of glucose on bacterial colonization and host response to UTI caused by uropathogenic Escherichia coli (UPEC) in a mouse model of type 2 diabetes mellitus (T2DM) of polygenic origin. Dapagliflozin was used to induce hyperglucosuria in diabetic and healthy adult mice before and during experimental UTI with UPEC. Glucose, bacterial load, cytokines, tissue damage scores, and myeloid cell composition during acute UTI were determined. A UPEC mutant lacking a gluconeogenesis pathway was co

Frequency and short-term persistence of haematuria and/or proteinuria in neonates: a cohort study.

PURPOSE: Haematuria and proteinuria may point to kidney diseases, but may also be found incidentally. Among schoolchildren, many studies suggest their prevalence to around 1%. In neonates, the frequency and persistence of haematuria and/or proteinuria in the general population have so far not been investigated systematically. METHODS: In the course of the prospective KUNO-Kids Health Study, urine samples were collected and analysed by dipstick in asymptomatic neonates on days three to five after birth. Those with positive findings underwent a maximum of two follow-ups until 16 weeks of age an

Uncoupling glucosuria from infection: SGLT2 inhibition modulates kidney immune response during pyelonephritis.

Sodium-glucose cotransporter 2 inhibitors increase urinary glucose excretion, theoretically predisposing to urinary tract infection, yet clinical trials have not shown higher rates of severe infection. In this issue of Kidney International, Sendtner et al. provide a plausible mechanistic framework for this paradox. Combining human cohort data with experimental pyelonephritis, they demonstrate that sodium-glucose cotransporter 2 inhibition with empagliflozin is associated with reduced C1q expression, preserved antibacterial defenses, and altered inflammatory signaling. These findings suggest th

Rethinking Zinc Requirements in Diabetes Mellitus: Comprehensive Review of Experimental and Clinical Studies Evidence.

Dysregulated zinc homeostasis is a common feature of diabetes mellitus (DM), exacerbating β-cell dysfunction, impairing glycemic control, and promoting vascular, renal, neural, and cognitive complications. This review summarizes experimental and clinical evidence on hyperzincuria and impaired intestinal zinc absorption in type 1 (T1DM) and type 2 (T2DM) DM, focusing on their mechanisms and implications for revising dietary zinc recommendations. Experimental models showed tissue-specific zinc depletion, with pancreas, liver, and femur zinc levels reduced by 17-50%, while kidney, muscle, and int

Genetic diagnosis of hereditary kidney disease in pediatric patients through whole-exome sequencing and mitochondrial DNA analysis.

BACKGROUND: Identifying the genetic cause of hereditary kidney disease is essential for appropriate management. However, pediatric patients often present with nonspecific renal symptoms such as proteinuria, hematuria, or both, making accurate diagnosis difficult. Mitochondrial diseases are also an important but underrecognized etiology of hereditary kidney disorders. This study aimed to assess the diagnostic utility of whole-exome sequencing (WES) combined with mitochondrial DNA (mtDNA) analysis and to describe the clinical and genetic features of pediatric patients presenting with isolated pr

ABO-incompatible kidney transplantation: impact of apheresis on graft and patient survival in recipients with low isoagglutinin titer.

ABO-incompatible (ABOi) kidney transplantation carries a high risk of acute antibody-mediated rejection due to the presence of isoagglutinins. To mitigate this risk, current protocols recommend performing apheresis before transplantation. Our objective was to evaluate outcomes in ABOi recipients with low isoagglutinin titers, comparing those who did and did not undergo pre-transplant apheresis. We conducted a multicenter study including recipients of ABOi kidney transplants between 2012 and 2022. A total of 78 patients with baseline isoagglutinin titers ≤1:8 were included; 41 received pre-tran

Modified cardiopulmonary bypass strategy for severe glucose-6-phosphate dehydrogenase deficiency during coronary artery bypass grafting: A practical perfusion technique.

BackgroundSevere glucose-6-phosphate dehydrogenase (G6PD) deficiency presents unique challenges during cardiopulmonary bypass (CPB) because of increased susceptibility to oxidative injury and haemolysis. Published guidance regarding optimal perfusion management remains limited, particularly in patients with a history of severe haemolytic complications and renal dysfunction.TechniqueA 63-year-old male with severe G6PD deficiency, a previous dialysis-requiring haemolytic crisis, chronic renal impairment and residual hemiparesis following a cerebrovascular accident underwent urgent three-vessel c

AI-driven diagnostic algorithm enhances early detection of paroxysmal nocturnal hemoglobinuria in real-world settings.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening hematologic disease with diagnostic delays exceeding 5 years in 24% of cases. We developed and deployed an artificial intelligence algorithm analyzing structured and unstructured electronic health record data across 14 healthcare organizations in Poland. Screening of 1,307,140 patients identified 356 high-risk individuals; of 119 referred for flow cytometry, 13 were diagnosed (positive predictive value: 10.92%; 95% CI, 9.68%-12.30%), comparing favourably to 6.9% conventional screening hit rate. High-risk patients were signi

Severe Rhabdomyolysis Without Electrolyte Abnormalities or Renal Failure and With Disproportionately Low AST.

Rhabdomyolysis is a condition in which damaged muscle tissue breaks down and typically presents with muscle pain, weakness, and dark-colored urine, along with markedly elevated serum creatine kinase (CK) levels. Common lab findings include hyperkalemia, hyperphosphatemia, hypocalcemia, elevated lactate dehydrogenase (LDH), acute kidney injury (AKI), and elevated aspartate aminotransferase (AST), with a typical CK:AST ratio of approximately 20:1. This report describes a 26-year-old man with no prior medical history who developed dark urine and lower back pain after a high-intensity functional t

Imatinib-induced rhabdomyolysis: A case report.

Imatinib, a tyrosine kinase inhibitor used for chronic myeloid leukemia (CML) and gastrointestinal stromal tumors (GIST), is associated with myalgia and creatine kinase (CK) elevations, though severe rhabdomyolysis and myopathy are rare. We report an 83-year-old woman with CML who developed progressive proximal weakness, dark urine, and acute kidney injury after three years of imatinib therapy. Laboratory evaluation revealed CK >24,000 U/L, transaminitis, and myoglobinuria. MRI showed diffuse muscle and fascial edema, while autoimmune testing was negative. Imatinib and rosuvastatin were discon

Delayed onset of myotoxicity following red-bellied black snake envenoming: a case report.

BACKGROUND: Myotoxicity is an important and recognized effect of red-bellied black snake envenoming that remains poorly understood but can result in severe illness. Prior observational data have shown that myotoxicity develops less frequently and later than other envenoming effects, including systemic symptoms and anticoagulant coagulopathy, which are likely to coexist in an envenomed victim. Current recommendations for managing snake bite victims in Australia include serial clinical and laboratory assessments for envenoming for at least 12 hours post-bite. CASE PRESENTATION: We report the ca

Paroxysmal Cold Haemoglobinuria: A Chilling Case of Acute Renal Failure.

We describe a rare case of a 64-year-old man with acute renal failure secondary to paroxysmal cold haemoglobinuria (PCH), presenting with haemoglobinuria, jaundice, and fevers. PCH is an intravascular haemolytic anaemia with polyphasic IgG antibody binding of red blood cells at cold temperatures, with complement-mediated haemolysis on rewarming. Renal biopsy confirmed acute tubular necrosis without thrombotic microangiopathy. The patient received supportive management including intermittent haemodialysis with a warm circuit to prevent further haemolysis and achieved full renal recovery. Optima

Diagnostic Approach to Seronegative Autoimmune-Mediated Rhabdomyolysis: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition caused by acute skeletal muscle injury leading to myocyte destruction and release of intracellular contents. While trauma and medications are common triggers, autoimmune-mediated necrotizing myopathies are rare. Diagnostic evaluation becomes challenging in seronegative cases. We report a 51-year-old man with multiple cardiovascular comorbidities presenting with one week of progressive proximal muscle weakness, severe back pain, and dark urine. Initial labs revealed a creatine kinase (CK) of 24,700 U/L, transaminitis, and acute kidney i

Exertional Heat Rhabdomyolysis in Class III Obesity Mimicking Nephrolithiasis and Hepatobiliary Disease.

Exertional rhabdomyolysis has been described in military recruits and endurance athletes, whereas recognition in adults with severe obesity performing routine activities remains limited. Diagnostic delays may occur when initial clinical features resemble those of more common conditions and creatine kinase is omitted from early evaluation. A 30-year-old man with class III obesity (body mass index 51 kg/m²) developed severe right flank pain over three days after lawn mowing in hot weather. On presentation, he was febrile to 39.1°C, tachycardic, met systemic inflammatory response syndrome criteri
