Interference-free detection of glucose in human urine based on a core-shell structured SERS nanoprobe.

Non-invasive glucose monitoring plays a key role in the diagnosis and management of diabetes and related diseases. However, the application of conventional SERS glucose sensors in complex biofluids is often limited by spectral overlap between Raman signals from the reporter molecules and those of endogenous biomolecules. To overcome this issue, we developed a SERS nanoprobe capable of interference-free glucose detection. The probe comprises a gold-core silver-shell nanostructure (Au@Ag NPs), with 4-mercaptobenzonitrile (MBN) as the Raman reporter embedded between the core and the shell, and gl

Glycemic burden-adjusted urinary glucose excretion deficit is associated with hyperuricemia in patients with type 2 diabetes.

OBJECTIVE: To investigate the associations of glycemic burden-adjusted urinary glucose excretion deficit (GB-UGE deficit) with serum uric acid levels and prevalent hyperuricemia in patients with type 2 diabetes mellitus (T2DM). METHODS: A total of 887 patients with T2DM were enrolled. GB-UGE deficit was defined as the inverse standardized residual from a linear regression of ln(urinary glucose excretion + 1) on glycated hemoglobin, fasting glucose, 2-hour postprandial glucose and eGFR; a higher GB-UGE deficit indicates less urinary glucose excretion than expected for a given glycemic burden.

Kidney Epithelial AKT Controls Potassium Homeostasis and Metabolic Adaptation.

BACKGROUND: Potassium intake is strongly associated with cardiometabolic health, yet the mechanisms linking electrolyte balance to systemic metabolism remain poorly defined. We hypothesized that renal epithelial AKT acts as a potassium-responsive signaling node that coordinates potassium homeostasis and metabolic adaptation. METHODS: Inducible nephron-wide and proximal tubule-specific AKT1/AKT2 knockout mice were generated to define the role of epithelial AKT in renal potassium handling and metabolism. Physiologic, molecular, proteomic, and metabolic analyses were performed under normal and p

SGLT2 inhibitors: mechanisms, physiology and pharmacology, and clinical applications.

Sodium-glucose cotransporter 2 (SGLT2) is a low-affinity, high-capacity transporter expressed predominantly in the renal proximal tubule, where it mediates the sodium-coupled reabsorption of most filtered glucose. SGLT2 inhibitors have evolved from glucose-lowering agents into disease-modifying therapies across the cardio-renal-metabolic continuum. Blockade of proximal tubular transport induces glycosuria, osmotic diuresis, restoration of tubuloglomerular feedback, resetting of renal haemodynamics, and altered fluid compartmentalisation. These early renal and circulatory effects interact with

Exploring the Therapeutic Potential of SGLT-2 Inhibitors in Managing Metabolic Syndrome: A Narrative Review.

Metabolic syndrome, a cluster of interconnected metabolic risk factors such as central obesity, insulin resistance, hypertension, and dyslipidemia, significantly increases the risk of type 2 diabetes mellitus (T2DM) and cardiovascular disease (CVD). Despite its rising prevalence and serious health consequences, metabolic syndrome remains underdiagnosed and undertreated. Sodium-glucose co-transporter-2 (SGLT-2) inhibitors, initially developed for T2DM management, have demonstrated promising therapeutic potential for addressing multiple components of metabolic syndrome. These drugs lower blood g

A Case Series Evaluating the Correlation Between Glycosuria Measurement and Serum Fructosamine in Two Diabetic De Brazza's Monkeys (Cercopithecus neglectus) and a Diabetic Tufted Capuchin (Sapajus apella).

BACKGROUND: Diabetes mellitus (DM) is a common condition affecting non-human primates (NHPs). The use of serum fructosamine in NHPs is a useful monitoring tool. Data does not exist to show whether there is any correlation between glycosuria and serum fructosamine. The purpose of this case series was to evaluate if there is a correlation between these two measurements to inform future monitoring of diabetic NHPs. METHODS: Two De Brazza's monkeys (Cercopithecus neglectus) and one tufted capuchin (Sapajus apella) previously diagnosed with DM underwent daily urinalysis monitoring. The average of

Acute Kidney Injury Associated With Dehydration Protocol Used by Combat Sports Athletes.

The increase in combat sports practice and the creation of weight divisions for fairer competitions led to dehydration practice as a strategy of inclusion in inferior divisions. However, this technique can damage kidney and heart functions due to alterations in blood volume. This study evaluated the acute effects of weight loss through dehydration on the kidney function of Mixed Martial Arts and Muay Thai fighters. The sample was composed of 30 athletes of both Mixed Martial Arts (n = 15) and Muay Thai (n = 15) fighters. Both groups went through two protocols for collecting data about the athl

From glucosuria to dialysis: a case report of osmotic nephropathy due to an SGLT2 inhibitor.

BACKGROUND: Osmotic nephropathy is an infrequent drug‑induced complication characterized by vacuolization of renal tubular cells secondary to lysosomal uptake, which increases osmotic pressure within the proximal tubule. This lesion pattern has been linked to administration of intravenous immunoglobulins, iodinated contrast agents, mannitol, dextrans and glucose solutions, and more recently to the use of sodium-glucose cotransporter‑2 (SGLT2) inhibitors. CASE PRESENTATION: A 49‑year‑old man with a history of hypertension, type 2 diabetes mellitus and obesity, chronically treated with empaglif

More than Glucose Elimination: Additional Benefits of SGLT2 Inhibitors in Glomerular Diseases.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors have consistent nephroprotective effects across diverse patient populations, including those with glomerular disease without diabetes mellitus. These somehow unexpected benefits cannot be solely explained by glycosuria and intrarenal hemodynamic effects. Experimental evidence largely supports the effects of SGLT2 inhibitors on several pathways, many of them outside the kidneys. This review explores the mechanisms underlying these benefits, focusing on those of importance in primary and secondary glomerulonephritis. In addition to glucose homeos

SGLT2 inhibitors reduce the salt sensitivity of blood pressure in type 2 diabetes via enhanced postprandial natriuresis.

BACKGROUND: Sodium-glucose cotransporter-2 inhibitors (SGLT2i) lower blood pressure (BP) and provide cardiovascular protection. In animal models, SGLT2i blunt sodium-induced BP elevation, but this effect remains unexplored in humans. We investigated whether SGLT2i reduce the salt sensitivity of BP in patients with type 2 diabetes (T2D) and explored the underlying mechanisms. METHODS: In an open-label, non-randomized study, 26 patients with T2D (14 on treatment with SGLT2i, 12 controls) completed two sequential 7-day dietary phases consisting of a very-low-sodium background diet with the addit

Enhanced urine glucose sensing using two-dimensional TMDCs-based SPR biosensor.

Surface plasmon resonance (SPR) sensing technology has been extensively applied in biomedical applications, but traditional SPR sensors often suffer from limited sensitivity. In this study, a bi-metallic/ Two-Dimensional (2D) Transition-Metal-Dichalcogenides (TMDCs)-based SPR biosensor has been proposed to achieve improved performance in urine glucose detection utilizing the Lumerical Finite-Difference-Time-Domain (FDTD) platform. The design incorporates different TMDCs materials, including WS2, WSe2, MoS2, MoSe2, and MoTe2 to enhance the sensor's sensitivity, whereas the MoS₂-based structure

Effects of Oral Single-Dose Empagliflozin on Urinary Glucose, Serum β-Hydroxybutyrate, and Plasma Empagliflozin Concentrations in Healthy Adult Dogs.

Empagliflozin is a sodium-glucose cotransporter-2 inhibitor used for treating humans with heart failure, among other conditions. This drug could have applications in veterinary cardiology, but whether empagliflozin has biological effects in dogs when administered at dosages approximating human therapeutic dosages is unknown. This study investigated whether empagliflozin (target dosage, 0.3 mg/kg) could produce significant increases in urinary glucose concentrations and measurable plasma drug concentrations, without increasing serum β-hydroxybutyrate (β-HA) concentrations in dogs. Six healthy,

Urinary tract infections, risk factors and antimicrobial resistance patterns in heart failure patients on sodium-glucose transporter 2 inhibitors: Evidence from Jakaya Kikwete Cardiac Institute in Tanzania.

BACKGROUND: Sodium-glucose co-transporter 2 (SGLT2) inhibitors such as dapagliflozin and empagliflozin are increasingly used in heart failure (HF) management due to their cardiovascular benefits. However, glycosuria induced by SGLT2 inhibition may increase the risk of urinary tract infections (UTIs). Limited data exist on UTI burden among HF patients on SGLT2 inhibitors in low-resource settings. OBJECTIVE: This study aimed to determine the prevalence of UTIs, associated factors, and antimicrobial susceptibility patterns of uropathogens in HF patients receiving SGLT2 inhibitors at the Jakaya K
