Identification of an unusual variant of Fanconi-Bickel syndrome presenting as proximal tubulopathy and short stature.

Fanconi-Bickel syndrome (FBS) is caused by biallelic pathogenic variants in the SLC2A2 gene, which encodes the glucose transporter protein 2 (GLUT2), leading to a rare disorder that affects glucose homeostasis. The exact mechanisms by which FBS leads to dysglycaemia are not clearly understood. The clinical manifestations are those related to dysglycaemia, proximal tubulopathy (glycosuria, galactosuria, aminoaciduria, proteinuria, phosphaturia), hepatomegaly, galactose intolerance, rickets and short stature. We report a teenage girl with persistent glycosuria and short stature. Laboratory findin

Diabetes and hyperglucosuria exacerbate the severity of urinary tract infection caused by uropathogenic E. coli in the mouse model.

Diabetes and urinary tract infection (UTI) are major global public health problems. We investigated the effects of glucose on bacterial colonization and host response to UTI caused by uropathogenic Escherichia coli (UPEC) in a mouse model of type 2 diabetes mellitus (T2DM) of polygenic origin. Dapagliflozin was used to induce hyperglucosuria in diabetic and healthy adult mice before and during experimental UTI with UPEC. Glucose, bacterial load, cytokines, tissue damage scores, and myeloid cell composition during acute UTI were determined. A UPEC mutant lacking a gluconeogenesis pathway was co

Uncoupling glucosuria from infection: SGLT2 inhibition modulates kidney immune response during pyelonephritis.

Sodium-glucose cotransporter 2 inhibitors increase urinary glucose excretion, theoretically predisposing to urinary tract infection, yet clinical trials have not shown higher rates of severe infection. In this issue of Kidney International, Sendtner et al. provide a plausible mechanistic framework for this paradox. Combining human cohort data with experimental pyelonephritis, they demonstrate that sodium-glucose cotransporter 2 inhibition with empagliflozin is associated with reduced C1q expression, preserved antibacterial defenses, and altered inflammatory signaling. These findings suggest th

Rethinking Zinc Requirements in Diabetes Mellitus: Comprehensive Review of Experimental and Clinical Studies Evidence.

Dysregulated zinc homeostasis is a common feature of diabetes mellitus (DM), exacerbating β-cell dysfunction, impairing glycemic control, and promoting vascular, renal, neural, and cognitive complications. This review summarizes experimental and clinical evidence on hyperzincuria and impaired intestinal zinc absorption in type 1 (T1DM) and type 2 (T2DM) DM, focusing on their mechanisms and implications for revising dietary zinc recommendations. Experimental models showed tissue-specific zinc depletion, with pancreas, liver, and femur zinc levels reduced by 17-50%, while kidney, muscle, and int

ABO-incompatible kidney transplantation: impact of apheresis on graft and patient survival in recipients with low isoagglutinin titer.

ABO-incompatible (ABOi) kidney transplantation carries a high risk of acute antibody-mediated rejection due to the presence of isoagglutinins. To mitigate this risk, current protocols recommend performing apheresis before transplantation. Our objective was to evaluate outcomes in ABOi recipients with low isoagglutinin titers, comparing those who did and did not undergo pre-transplant apheresis. We conducted a multicenter study including recipients of ABOi kidney transplants between 2012 and 2022. A total of 78 patients with baseline isoagglutinin titers ≤1:8 were included; 41 received pre-tran

Fanconi syndrome developing and worsening during anti-myeloma therapy in multiple myeloma: a case report and literature review.

BACKGROUND: Fanconi syndrome (FS) is a rare renal complication of monoclonal gammopathies including multiple myeloma (MM), typically diagnosed before or at the time of MM presentation. Onset and progression of FS during anti-myeloma treatment are extremely rare and easily misdiagnosed, resulting in delayed electrolyte correction and clinical deterioration. CASE PRESENTATION: We report a 55-year-old woman with κ light chain MM who developed FS during induction therapy and experienced recurrence after autologous stem cell transplantation (ASCT). Initial workup showed subclinical tubular dysfunc

Cushing's Syndrome associated with diabetes mellitus in a Persian cat: clinical features, diagnosis, and therapeutic management.

The association between Cushing's syndrome (CS) and diabetes mellitus (DM) in cats is rarely reported. Complications related to hypercortisolism and persistent hyperglycemia may occur, which may complicate the control of both endocrinopathies. This report describes the case of a 10-year-old Persian cat diagnosed with CS and DM. The patient presented with polyuria, polydipsia, chronic dry seborrhea on the dorsal region and flanks, and lack of hair regrowth on the abdomen. Through the dexamethasone suppression test (0.1 mg/kg) and the presence of persistent hyperglycemia with elevated fructosami

The Potential Role of SGLT2 Inhibitors in Cushing's Disease.

Cushing's disease (CD) is a rare endocrine disorder caused by the overproduction of adrenocorticotrophin hormone (ACTH) from a pituitary tumour, leading to excessive secretion of cortisol. The chronic exposure to cortisol leads to endocrine and cardiovascular complications that persist even after remission of hypercortisolism. CD is associated with a significant clinical burden characterised by insulin resistance, renal dysfunction and cardiovascular complications. In addition, patients frequently experience impaired health-related quality of life (HRQoL) and have an increased risk of mortalit

More than meets the eye: a case of tubulointerstitial nephritis and uveitis syndrome.

Tubulointerstitial nephritis with uveitis (TINU) syndrome is a rare oculorenal inflammatory disorder characterized by concurrent tubulointerstitial nephritis (TIN) and uveitis in the absence of an alternative systemic disease. We present a 57-year-old woman with acute kidney injury, euglycemic glycosuria, and simul-taneous non-granulomatous anterior uveitis. Kidney biopsy demonstrated eosinophil-rich chronic active tubuloin-terstitial nephritis with non-caseating granulomas. After excluding infectious, drug-induced, and systemic autoimmune etiologies, TINU syndrome was diagnosed. Corticosteroi

Fanconi syndrome induced by zoledronic acid: Two case reports.

RATIONALE: Zoledronic acid (ZA) is a potent third-generation bisphosphonate widely used to manage bone metastases in prostate cancer. Although effective, ZA is associated with nephrotoxicity, which in rare cases may lead to Fanconi syndrome (FS): a form of acquired proximal renal tubular dysfunction. Clinicians may overlook early signs of FS, especially in patients receiving long-term bisphosphonate therapy. PATIENT CONCERNS: We report 2 cases of prostate cancer complicated by FS following ZA administration. Patient #1, a 74-year-old male without prior renal dysfunction, developed hypouricaem

Perioperative management of SGLT2 inhibitors: balancing euglycaemic ketoacidosis risk and cardiorenal benefits.

Sodium-glucose cotransporter 2 (SGLT2) inhibitors are established therapies for type 2 diabetes, heart failure, and chronic kidney disease, creating a perioperative need to balance prevention of euglycaemic diabetic ketoacidosis (euDKA) against cardiorenal protection. Fasting, surgical stress, infection, reduced carbohydrate intake, insulin dose reduction, and hypovolaemia can amplify glycosuria and ketogenesis and precipitate euDKA. Most guidelines recommend withholding SGLT2 inhibitors for 3-4 days before elective surgery, although recommendations vary by agent, procedure, fasting duration,

Paralytic Ileus as the Initial Manifestation of New-Onset Type 1 Diabetes Without Diabetic Ketoacidosis: A Case Report.

Type 1 diabetes (T1D) is an autoimmune disease characterized by pancreatic β-cell destruction and absolute insulin deficiency. In pediatric patients, T1D typically presents with classic symptoms such as polydipsia, polyuria, weight loss, or diabetic ketoacidosis (DKA). Although gastrointestinal symptoms may occur at disease onset, paralytic ileus as an initial manifestation of T1D, particularly in the absence of DKA, is extremely uncommon. A 15-year-old boy presented with severe abdominal pain and vomiting and was diagnosed with paralytic ileus based on abdominal radiographic findings. Mild hy
