KEY POINTS: The insulin-regulated kinase, mammalian target of rapamycin complex 2 (mTORC2), coordinates regulation of sodium-glucose cotransport and gluconeogenesis in the kidney proximal tubule. Dietary potassium can bypass mTORC2 to regulate sodium-glucose cotransport and gluconeogenesis in mTORC2 knockout mice. The transcription factor forkhead box O4 may have an unexpected role in mediating mTORC2 effects on renal tubule glucose homeostasis.
BACKGROUND: The kidney is uniquely responsible for reabsorption of filtered glucose and gluconeogenesis. Insulin stimulates glucose transport and sup

PURPOSE: While chronic damage to the lower urinary tract in diabetic patients is well-known, the specific effects of acute glucosuria on the bladder remain unclear. This study, being the first of its kind in the available literature, aims to exclusively investigate the early effects of acute glucosuria on bladder function and morphology, independent of the broader impacts of diabetes.
METHODS: Wistar Albino rats were divided into four groups: CONTROL, Empagliflozin (EMPA), Diabetes Mellitus + Empagliflozin (DM + EMPA), and DM. Acute glucosuria was induced using empagliflozin in the EMPA and D

Cancer therapy-induced cardiotoxicity (CTIC) is a serious and increasingly recognized cause of death and disability among cancer survivors. It frequently necessitates the withdrawal or dose reduction of effective anticancer drugs, limiting therapeutic options and affecting patient outcomes. While CTIC poses a major health risk, the precise cellular and molecular mechanisms responsible for this toxicity remain elusive, which complicates the development of preventive and therapeutic strategies. In recent years, natural products derived from Traditional Chinese Medicine (TCM) have gained attentio

Inflammatory response, oxidative stress, and pyroptosis play important roles in the pathogenesis of diabetic kidney disease (DKD), and the NOD-like receptor protein 3 (NLRP3) inflammasome complex and pyroptosis are possible cellular regulators dependent on these processes. Treatment of DKD relies on sodium-glucose cotransporter-2 inhibitors (SGLT2is); however, its effects on oxidative stress and the NLRP3 complex have not yet been fully elucidated. This study aimed to evaluate the role of a SGLT2i in the regulation of the redox system, inflammatory profile, and NLRP3 inflammasome in an experim

Background: Sodium-glucose cotransporter-2 inhibitors (SGLT2i) are increasingly used in patients with type 2 diabetes, chronic kidney disease, and heart failure. However, their safety and efficacy in patients with congenital or surgically altered urogenital anatomy remains underexplored. Methods: We conducted a narrative review of current evidence regarding the use of SGLT2i in patients with urinary tract malformations, urinary diversions, and functional voiding disorders. Key risks, clinical considerations, and management strategies were synthesized from the existing literature and case repor

BACKGROUND: Standard management of acute immune thrombocytopenic purpura (ITP) remains debated, with some advocating observation for mild cases, while others recommend pharmacological intervention to accelerate platelet recovery in children with severe thrombocytopenia (TCP) (platelet count < 20,000/mm3) or significant mucosal bleeds. Corticosteroids, particularly prednisolone (PDN), are a widely used first-line treatment due to their rapid immunosuppressive effect. This prospective cohort study evaluated the effectiveness of short-course high-dose PDN (4 mg/kg/day for 7 days) in treating chil

AIMS/HYPOTHESIS: Despite recent therapeutic advances, achieving optimal glycaemic control remains a challenge in managing type 2 diabetes. Sodium-glucose cotransporter 2 (SGLT2) inhibitors have emerged as effective treatments by promoting urinary glucose excretion. However, the full scope of their mechanisms extends beyond glycaemic control. At present, their immunometabolic effects remain elusive.
METHODS: To investigate the effects of SGLT2 inhibition or deletion, we compared the metabolic and immune phenotype between high-fat-diet-fed control mice, mice treated chronically with dapaglifloz

OBJECTIVE: Pathogenic variants of HNF1A cause maturity-onset diabetes of the young type 3 (HNF1A-MODY; also known as MODY3). Individuals with HNF1A-MODY are primarily treated with sulfonylureas; however, little is known about the effect of sodium-glucose cotransporter 2 (SGLT2) inhibitors in HNF1A-MODY. Interestingly, HNF1A-MODY is associated with increased glucosuria, which has been attributed to lower expression of SGLT2 as observed in HNF1A-knockout mice. We investigated the impact of acute SGLT2 inhibition on glucosuria in individuals with HNF1A-MODY or type 2 diabetes.

INTRODUCTION: Hepatocyte nuclear factor-1α (HNF-1α) and hepatocyte nuclear factor-4α (HNF-4α) are transcription factors highly expressed in β-cells, hepatocytes, intestinal epithelial cells, and renal tubular cells. Variants in both HNF1A and HNF4A gene have been linked to maturity-onset diabetes of youth (MODY) and congenital hyperinsulinism (HI). To date, the association between HI, renal tubulopathy, and hepatopathy has been described only in patients with HNF-4α deficiency. HI due to HNF-1α deficiency has not been linked to extra-pancreatic features.

Fanconi syndrome is a disorder of the proximal renal tubules characterized by metabolic acidosis, renal glycosuria, and aminoaciduria. Recently, Fanconi syndrome caused by red yeast rice supplements has been reported. We encountered a 60-year-old woman with acute kidney injury and Fanconi syndrome caused by a red yeast rice supplement. The patient discontinued the supplements, leading to an improvement in her serum creatinine and urinary protein levels. Early detection and discontinuation of the suspected drugs are important for the treatment of drug-induced Fanconi syndrome.

Sodium-glucose cotransporter-2 (SGLT-2) inhibitors, while providing cardiorenal benefits in heart failure, have been associated with an elevated risk of urinary tract infections (UTIs). This study aimed to identify independent risk factors for UTIs in HF patients receiving SGLT-2 inhibitor therapy. In this multicenter retrospective cohort study, 110 heart failure patients treated with SGLT-2 inhibitors were included, among whom 41 developed UTIs. Comparative analyses between UTI and non-UTI groups were performed for demographic, clinical, and laboratory variables. Statistically significant fac

Fungal urinary tract infections (fUTIs) are emerging as a public health concern, notably in South Asia, where a convergence of ecological, genetic, and clinical factors underlies a rising burden of antifungal-resistant disease. This review synthesizes epidemiological, mechanistic, and genetic evidence implicating Candida tropicalis as a dominant uropathogen in South Asia's diabetic and immunocompromised populations. We examine the shift from Candida albicans to non-albicans Candida (NAC) species, driven by selective antifungal pressure and nosocomial transmission. Emphasis is placed on the vir

Urine biomarker detection with high accuracy and sensitivity is crucial for advancing point-of-care diagnostics. This paper developed a microfluidic paper-based analytical device (μPAD) for electrochemical glucose detection in urine. The proposed μPAD employs a filter paper substrate to enable pump-free sample transport to a detection zone integrated with the screen-printed electrode (SPE), allowing rapid quantification within 60 seconds. A composite of reduced graphene oxide (RGO)/platinum-palladium nanoparticles (Pt-Pd NPs) with good analytical performance has been electrodeposited on the su

A five-month-old female mixed-breed dog presented with a two-week history of polyuria, polydipsia, and vomiting. Clinical examination revealed poor body condition, growth retardation, pale oral mucous membranes, weak pulse, and prolonged capillary refill time. Laboratory findings included neutrophilic leukocytosis with a regenerative left shift, fasting hyperglycemia, elevated fructosamine, glycated hemoglobin, and β-hydroxybutyrate concentrations, while the acid-base balance remained normal. Canine-specific pancreatic lipase and trypsin-like immunoreactivity concentrations ruled out an underl

OBJECTIVE: Sodium-glucose cotransporter-2 inhibitors (SGLT2is) are recommended as first-line cardiorenal protective therapy in type 2 diabetes. Because SGLT2is cause glycosuria and increase urine volume, they may exacerbate incontinence symptoms among patients with pre-existing urinary incontinence. Our objective was to determine how many adults meeting guideline indications for SGLT2i have frequent urinary incontinence.
RESEARCH DESIGN AND METHODS: We conducted a cross-sectional analysis of National Health And Nutrition Examination Survey (NHANES) participants aged ≥55 with type 2 diabetes i

Staphylococcus aureus (S. aureus) is a major opportunistic pathogen, causing acute and chronic infections including urinary tract infection (UTI). S. aureus relies on the Accessory gene regulator (Agr), a central quorum sensing (QS) system and the urease genes ureABCEFGD, for the regulation of urease expression and its pathogenicity. Urease is a key virulence factor for S. aureus, modulating immune responses by altering the local urinary pH and impairing immune defenses in the hostile urinary environment. QS is regulated by the external stimuli and in urological disease, the role of different

Diabetes mellitus is a global health issue affecting millions of people and requires regular glucose level monitoring. Current non-invasive methods, such as urinalysis (including colorimetry and biosensors), are primarily laboratory-based and lack user-friendliness, limiting their practicality for continuous glucose monitoring. Although promising, research on smartphone-integrated laser refractometry for glucose detection remains limited. To address this gap, a non-contact, smartphone-based laser refractometer for glucose monitoring was developed. This prototype measures the refractive index o

Background: Sodium-glucose cotransporter-2 inhibitors (SGLT2is), initially developed as antihyperglycemic agents, have emerged as multifunctional therapeutics with profound cardiorenal and metabolic benefits. Their unique insulin-independent mechanism, targeting renal glucose reabsorption, distinguishes them from conventional antidiabetic drugs. Mechanisms and Clinical Evidence: SGLT2is induce glycosuria, reduce hyperglycemia, and promote weight loss through increased caloric excretion. Beyond glycemic control, they modulate tubuloglomerular feedback, attenuate glomerular hyperfiltration, and

The therapeutic scope of sodium-glucose cotransporter 2 (SGLT2) inhibitors has expanded beyond glycaemic regulation in the management of diabetes mellitus. Studies published in the past few years highlight their substantial effect on cardiovascular outcomes, notably in decreasing mortality and the need for heart failure-related hospitalization. These agents also lead to pronounced improvements in a range of renal outcomes. The primary actions of SGLT2 inhibition, glycosuria and natriuresis, are pivotal in enhancing glucose control, promoting weight loss and lowering blood pressure. These effec
