Chyluria Status Post Microwave Ablation of Renal Cell Carcinoma: A Case Report.

Microwave ablation is a minimally invasive alternative to nephrectomy for select patients with renal cell carcinoma (RCC), particularly those with solitary kidneys or significant comorbidities. We present a case of a 55-year-old male with stage 3B chronic kidney disease and biopsy-confirmed clear cell RCC who underwent successful microwave ablation. Post-procedure imaging demonstrated a stable lesion size without recurrence. However, follow-up revealed persistent chyluria, likely as a rare and delayed consequence of ablation. This report discusses the pathophysiology, diagnostic considerations

Evaluation of Cervical Thoracic Duct Exploration in Idiopathic Chyluria: A Retrospective Comparative Study.

PURPOSE: The purpose of this study is to evaluate a novel surgical method for idiopathic chyluria. METHODS: Data from 63 patients with idiopathic chyluria who underwent direct lymphangiography (DLG) were retrospectively analyzed. Of these, 50 patients received cervical thoracic duct exploration (CTDE) as a treatment, while 13 were managed with a low-fat diet. The treatment efficacy of the two groups was compared, and a univariate analysis of the CTDE treatment effect was performed. RESULTS: DLG revealed that the contrast agent passed upwards and outwards through paravertebral lymphatics into

A Case Report of Membranoproliferative Glomerulonephritis: Infection-Related or Immune-Related?

Membranoproliferative glomerulonephritis (MPGN) has previously been used as an umbrella term to describe a spectrum of hypocomplementemic glomerular diseases, which are rare causes of end stage kidney disease (ESKD). We present a 22-year-old man with a well-established medical history who had been complaining of 4 days of frothy dark urine, bilateral lower limb swelling, and puffiness on his face. For a month before his presentation, he had many bilateral skin lesions on his lower limbs that were leaking pus. Aside from the scars from the prior skin lesions, he had no other significant medical

Rhabdomyolysis in a young adult following chronic non-prescribed pregabalin use.

Rhabdomyolysis is a serious condition caused by skeletal muscle breakdown and can present with severe generalised pain, elevated creatine kinase and myoglobinuria. We report a rare case of rhabdomyolysis likely induced by chronic pregabalin abuse (15-year history of non-prescribed use) in a physically active man in his 20s. Despite the absence of trauma, infection or other known causes, he presented with typical symptoms and laboratory findings of rhabdomyolysis. He improved significantly with supportive care, including intravenous fluids and analgesia, and was discharged after stabilisation.

Outbreak of subacute necrotic myopathy caused by ionophore poisoning in water buffaloes (Bubalus bubalis).

This study reports an outbreak of subacute necrotic myopathy in buffaloes associated with the ingestion of a commercial feed formulated for cattle containing monensin. Twenty-nine buffaloes were fed a monensin containing ration, and after two weeks, nine animals developed apathy, anorexia, dark-red urine, recumbency, and ventral abdominal distention. Four of them died and one was euthanized in extremis within two to five days after the onset of clinical signs. Three buffaloes were necropsied, and the main lesions included focally extensive areas of pallor, mainly in the musculature of the thor

Intravascular haemolysis following pulsed field ablation pulmonary vein isolation for atrial fibrillation: a systematic review.

BACKGROUND: Pulsed field ablation (PFA) has emerged as a promising non-thermal alternative to conventional atrial fibrillation (AF) ablation techniques. However, intravascular haemolysis has been increasingly recognised as a potential complication, with variable incidence and clinical significance. OBJECTIVE: To systematically review the available clinical evidence on PFA-related haemolysis, focusing on biochemical markers, clinical manifestations and device-specific differences. METHODS: PubMed, Embase and Cochrane databases were searched until 20 May 2025 for clinical studies evaluating pr

Case Report: Something Is Brewing.

Kava plant root (piper methysticum) is an ingredient used in ceremonial drinks throughout Pacific and South Sea Island communities. Kava is made up of kavalactones; psychoactive compounds that act as central nervous system depressants. The rare occurrence of kavalactone toxicity manifests most commonly with hepatotoxicity, and cases of myoglobinuria have been reported. Our case describes an association of significant kava tea ingestion with acute tubulo-interstitial nephritis. An 18-year-old male, without any medical comorbidities, presented with oliguric acute kidney injury after ingesting 15

Nonexertional Rhabdomyolysis.

Although horses most commonly develop exertional rhabdomyolysis, there are numerous causes for nonexertional rhabdomyolysis (nonER) that pose a serious health threat to horses. Their etiologies can be broadly categorized as toxic, genetic, inflammatory/infectious, nutritional, and traumatic and a variety of diagnostic tests are available to discern among them. This study discusses causes of nonER as well as diagnostics and treatments that are specific to each etiology. General treatment of acute rhabdomyolysis is covered in detail in the study in this issue on sporadic and recurrent exertional

Challenges in Treating Dermatomyositis-Related Rhabdomyolysis: A Case Report of Steroid-Induced Myopathy.

BACKGROUND Rhabdomyolysis, although typically associated with immobilization, trauma, or the use of certain medications, can also manifest as a severe and potentially life-threatening complication of dermatomyositis. This condition results from the breakdown of skeletal muscle, presenting with symptoms such as myalgia, muscle weakness, and myoglobinuria. Although high-dose steroids are the treatment of choice in cases of dermatomyositis-related rhabdomyolysis, they can trigger the onset of steroid-induced myopathy. CASE REPORT A 50-year-old woman with bilateral muscle weakness, myalgia, erythe

[The night has many faces: When schistocytes step out of line].

We present a case study of a patient with paroxysmal nocturnal haemoglobinuria (PNH) which first presented clinically as a haemolytic crisis, potentially triggered by a SARS-CoV-2 infection. This case highlights the diagnostic challenges involved in distinguishing between PNH and thrombotic microangiopathy (TMA), particularly in emergency situations. Administering eculizumab early on, as it is effective in treating both diseases, was crucial in stabilising the patient before a definitive diagnosis could be confirmed.

Oral iptacopan monotherapy in paroxysmal nocturnal haemoglobinuria: final 48-week results from the open-label, randomised, phase 3 APPLY-PNH trial in anti-C5-treated patients and the open-label, single-arm, phase 3 APPOINT-PNH trial in patients previously untreated with complement inhibitors.

BACKGROUND: The factor B inhibitor iptacopan improved 24-week outcomes in adult patients with paroxysmal nocturnal haemoglobinuria in the phase 3 APPLY-PNH and APPOINT-PNH trials; the trial extension periods assessed clinical activity and safety up to 48 weeks. Here, we report the final 48-week data from APPLY-PNH and APPOINT-PNH. METHODS: In both APPLY-PNH and APPOINT-PNH trials, patients were aged 18 years or older, with paroxysmal nocturnal haemoglobinuria (red and white blood cell population sizes ≥10%) and without laboratory evidence of bone marrow failure. In APPLY-PNH (an open-label, r

Suspected Thyroid Storm in a Patient Undergoing Internal Fixation for a Left Mandibular Fracture Under General Anesthesia: A Case Report.

Thyroid storm is a life-threatening condition in which patients with untreated or poorly controlled thyroid disease experience excessive thyroid hormone action triggered by severe stress, resulting in multiple organ dysfunction. We present a case of suspected thyroid storm during surgery for a left mandibular fracture under general anesthesia. The patient was a 23-year-old male, and a preoperative interview revealed no notable medical history. During open reduction and internal fixation under general anesthesia, the patient developed sinus tachycardia, hypertension, elevated end-tidal carbon d

Intravascular hemolysis as an initial complication of acute hyponatremia from water intoxication.

Acute hyponatremia in emergency care settings can be life-threatening due to neurological complications such as cerebral edema. While water intoxication is a well-recognized cause of acute severe hyponatremia with prominent neurologic manifestations, non-neurologic complications such as intravascular hemolysis are rarely reported. We report a rare case of acute hyponatremia, initially manifesting as intravascular hemolysis, due to water intoxication over just two hours. A 67-year-old woman presented to the emergency department in an agitated state (Glasgow Coma Scale score of 13). She had a hi

Protocol for a mouse tubuloid model of myoglobinuric acute kidney injury.

Myoglobinuric acute kidney injury (AKI) occurs as a result of rhabdomyolysis. Here, we present a protocol based on mouse primary renal tubular epithelial organoids, or "tubuloids", to model myoglobinuric AKI in vitro. We describe how to establish and maintain the culture of mouse tubuloids and how to trigger myoglobin-induced injury. Moreover, we provide instructions on how to quantify tubuloid damage. For complete details on the use and execution of this protocol, please refer to Zhou et al.1.

Case Report: Perioperative Management of a Patient with Glycogen Storage Disease Type IXd.

INTRODUCTION: Glycogen storage disease type IX (GSD type IX) is caused by a deficiency in phosphorylase b kinase (PHK) and is classified into hepatic (IXa-c) and muscular (IXd) subtypes. GSD type IXd leads to exercise intolerance, rhabdomyolysis, and myoglobinuria owing to impaired glycogen breakdown. It is a rare and mild metabolic disorder, with only 19 reported cases of PHKA1 mutations. To the best of our knowledge, this is the 1st report on the perioperative management of a patient with GSD type IXd.
