Exploring individualized crovalimab dosing in PNH through in silico modelling: Potential for improved convenience and cost efficiency.

AIM: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired haematopoietic stem cell disorder. Crovalimab, a complement C5-inhibitor, is approved for PNH and can be self-administered subcutaneously every 4 weeks, offering a more convenient route than intravenous C5-inhibitors. Since exposure often exceeds pharmacokinetic targets and varies with body weight, we aimed to explored individualized dosing of crovalimab using clinical trial simulations to optimize therapy. METHODS: We compared standard, weight-based and therapeutic drug monitoring (TDM)-guided regimens in treatment-naïve PNH

Acute Biochemical Muscle Damage Responses to a Single Session of Whole-Body Electromyostimulation.

PURPOSE: This study aimed to investigate the acute effects of whole-body electromyostimulation (WB-EMS) on markers of skeletal muscle damage, renal function, and metabolic stress in sedentary adults. Given increasing reports of severe muscle damage after a single WB-EMS session, this study examined the time course and magnitude of biochemical and urinary responses after such an event. METHODS: Twenty-six participants were randomly assigned to a WB-EMS group or a control (exercise-only) group. While both groups completed the same physical exercise protocol, only the WB-EMS group received low-f

Successful Treatment of Idiopathic Chyluria by Direct Percutaneous Puncture and Embolization of the Fistula Site: A Case Report.

We report a case of idiopathic chyluria in a woman in her 70s, successfully treated by direct percutaneous puncture and embolization of the fistula site. The patient had experienced persistent milky-white urine, progressive weight loss, and voiding difficulties for >3 years. Inguinal intranodal lymphangiography identified a lymphatic-urinary fistula near the lower pole of the left renal calyx. Later, under fluoroscopic guidance, the fistula site was directly punctured and embolized using n-butyl cyanoacrylate without complications. Her symptoms resolved immediately, and there has been no recur

Case report: morbid obesity and lithotomy positioning: a high-risk triad with GCS leading to severe rhabdomyolysis with impending acute compartment syndrome following ovarian cancer cytoreduction.

BACKGROUND: Venous thromboembolism (VTE) prophylaxis with graduated compression stockings (GCS) is a standard of care in gynecologic oncology surgery; however, the mechanical risks in specific patient populations require critical attention. CASE PRESENTATION: A 24-year-old morbidly obese patient (BMI 41.7 kg/m²) with ovarian carcinoma developed severe rhabdomyolysis and impending acute compartment syndrome (ACS) after a 7.5-hour fertility-preserving cytoreduction performed in the Trendelenburg position with lithotomy, during which routine GCS were applied. Postoperative symptoms included sever

Physician and Patient Perception of Disease, Symptomatology, and Treatment Administration Preference in Paroxysmal Nocturnal Haemoglobinuria: Results from a Real-World Survey.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is an ultra-rare condition, characterised by complement-mediated red blood cell breakdown, resulting in haemolytic anaemia. Associated symptoms, such as fatigue, impact on patient quality of life. Little is known about the comparative perceptions of symptom burden and treatment preferences among patients and physicians. METHODS: This study analysed secondary data from the Adelphi Real World PNH Disease Specific Programme™, a cross-sectional survey of physicians and patients with PNH in France, Germany, Italy, Spain, China, and Japan con

Recurrent bilateral chyluria after lymphatic embolization successfully treated with renal pelvic instillation sclerotherapy: a case report.

Chyluria is a rare condition caused by abnormal communication between the lymphatic system and the urinary tract. We report a case of recurrent bilateral chyluria in a 65-year-old woman who previously underwent lymphatic embolization but developed persistent symptoms. Retrograde pyelography demonstrated bilateral renal lymphatic reflux. The patient was treated with bilateral renal pelvic instillation sclerotherapy (RPIS) using povidone iodine for three consecutive days. Symptoms resolved rapidly and no complications occurred. The patient remained symptom-free during 36 months of follow-up. Thi

When Dark Urine Is Not Nephritis: A Life-Threatening Case of Lipin-1 Deficiency in Infancy.

Lipin-1 deficiency is a rare inherited metabolic disorder and a major genetic cause of severe, recurrent rhabdomyolysis in early childhood, carrying a significant risk of acute kidney injury and mortality if unrecognized. We report the case of a male toddler who first presented at 19 months of age with dark urine and intercurrent viral illness. Initial investigations demonstrated hematuria/myoglobinuria, proteinuria, and markedly elevated transaminases, leading to a presumptive diagnosis of glomerulonephritis. Creatine kinase (CK) was not measured at that time, and the episode was retrospectiv

Critical care considerations in rhabdomyolysis-associated acute kidney injury and kidney replacement therapy.

Rhabdomyolysis is a frequently seen and potentially severe condition in intensive care, resulting from causes like trauma, infection, drug toxicity, or prolonged immobility. When muscle cells break down, substances such as myoglobin enter the bloodstream, increasing the risk of acute kidney injury (AKI), which greatly affects patient outcomes. Prompt diagnosis, effective fluid management, and supportive care are crucial for reducing kidney-related complications. In more critical situations, standard treatments may not be enough, and kidney replacement therapy (KRT) may be required. This review

From the Field to the Emergency Room: A Systematic Case-Level Review of Exertional Rhabdomyolysis and Acute Kidney Injury in Athletes.

Exertional rhabdomyolysis is a potentially life-threatening syndrome caused by skeletal muscle breakdown with release of myoglobin and creatine kinase (CK) into the circulation. Clinical severity ranges from transient enzyme elevation to severe acute kidney injury (AKI) requiring renal replacement therapy (RRT). Although increasingly recognized, the published literature remains fragmented and largely descriptive. This review systematically aggregated case-level data to characterize biochemical severity and renal outcomes across different athlete populations and exertional contexts. PubMed, Emb

Exertional Rhabdomyolysis Following Concurrent Exercise Including Pelvic Floor (Kegel) Training in a Previously Healthy 36-Year-Old Male Patient: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition characterized by skeletal muscle breakdown and release of intracellular contents into the systemic circulation. Exertional rhabdomyolysis (ER) most commonly occurs after high-intensity exercise involving large muscle groups, but cases related to lower intensity or repetitive activity have also been reported. We describe a case of acute ER in a previously healthy 36-year-old man who developed the condition after initiating a workout regimen for erectile dysfunction that included gluteal exercises and pelvic floor muscle training (PFMT),

Hypokalemia-Induced Rhabdomyolysis Following Prolonged Diarrhea in a Child With Congenital Heart Disease Receiving Loop Diuretics: A Case Report and Literature Review.

Hypokalemia is a rare but important cause of rhabdomyolysis in children. Unlike typical rhabdomyolysis, hyperkalemia may be absent because the triggering factor is hypokalemia, potentially delaying diagnosis. We report a five-year-old boy with hypoplastic left heart syndrome after a Fontan procedure who developed severe hypokalemia and rhabdomyolysis following prolonged diarrhea while receiving loop diuretics. He presented with acute lower extremity weakness and inability to stand. Laboratory evaluation revealed potassium 1.9 mmol/L and creatine kinase 9,013 U/L, peaking at 32,908 U/L. Urinaly

A multi-step screening algorithm for fecal microbiota transplantation donors: integrating metagenomic and metabolomic profiling for improved safety and donor quality assessment.

BACKGROUND: Fecal Microbiota Transplantation (FMT) has emerged as a highly effective treatment for recurrent Clostridioides difficile infection. It is also a promising therapeutic approach for other microbiome-related disorders. However, the safety and efficacy of FMT depend critically on rigorous donor screening and selection protocols. In this study, we developed and evaluated an optimized donor screening and selection workflow that integrates multistep pathogen detection, metagenomic and metabolomic profiling, and clinical compatibility assessment to enhance FMT safety and donor quality ass

AI-driven diagnostic algorithm enhances early detection of paroxysmal nocturnal hemoglobinuria in real-world settings.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening hematologic disease with diagnostic delays exceeding 5 years in 24% of cases. We developed and deployed an artificial intelligence algorithm analyzing structured and unstructured electronic health record data across 14 healthcare organizations in Poland. Screening of 1,307,140 patients identified 356 high-risk individuals; of 119 referred for flow cytometry, 13 were diagnosed (positive predictive value: 10.92%; 95% CI, 9.68%-12.30%), comparing favourably to 6.9% conventional screening hit rate. High-risk patients were signi
