Modified cardiopulmonary bypass strategy for severe glucose-6-phosphate dehydrogenase deficiency during coronary artery bypass grafting: A practical perfusion technique.

BackgroundSevere glucose-6-phosphate dehydrogenase (G6PD) deficiency presents unique challenges during cardiopulmonary bypass (CPB) because of increased susceptibility to oxidative injury and haemolysis. Published guidance regarding optimal perfusion management remains limited, particularly in patients with a history of severe haemolytic complications and renal dysfunction.TechniqueA 63-year-old male with severe G6PD deficiency, a previous dialysis-requiring haemolytic crisis, chronic renal impairment and residual hemiparesis following a cerebrovascular accident underwent urgent three-vessel c

AI-driven diagnostic algorithm enhances early detection of paroxysmal nocturnal hemoglobinuria in real-world settings.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening hematologic disease with diagnostic delays exceeding 5 years in 24% of cases. We developed and deployed an artificial intelligence algorithm analyzing structured and unstructured electronic health record data across 14 healthcare organizations in Poland. Screening of 1,307,140 patients identified 356 high-risk individuals; of 119 referred for flow cytometry, 13 were diagnosed (positive predictive value: 10.92%; 95% CI, 9.68%-12.30%), comparing favourably to 6.9% conventional screening hit rate. High-risk patients were signi

Severe Rhabdomyolysis Without Electrolyte Abnormalities or Renal Failure and With Disproportionately Low AST.

Rhabdomyolysis is a condition in which damaged muscle tissue breaks down and typically presents with muscle pain, weakness, and dark-colored urine, along with markedly elevated serum creatine kinase (CK) levels. Common lab findings include hyperkalemia, hyperphosphatemia, hypocalcemia, elevated lactate dehydrogenase (LDH), acute kidney injury (AKI), and elevated aspartate aminotransferase (AST), with a typical CK:AST ratio of approximately 20:1. This report describes a 26-year-old man with no prior medical history who developed dark urine and lower back pain after a high-intensity functional t

Imatinib-induced rhabdomyolysis: A case report.

Imatinib, a tyrosine kinase inhibitor used for chronic myeloid leukemia (CML) and gastrointestinal stromal tumors (GIST), is associated with myalgia and creatine kinase (CK) elevations, though severe rhabdomyolysis and myopathy are rare. We report an 83-year-old woman with CML who developed progressive proximal weakness, dark urine, and acute kidney injury after three years of imatinib therapy. Laboratory evaluation revealed CK >24,000 U/L, transaminitis, and myoglobinuria. MRI showed diffuse muscle and fascial edema, while autoimmune testing was negative. Imatinib and rosuvastatin were discon

Delayed onset of myotoxicity following red-bellied black snake envenoming: a case report.

BACKGROUND: Myotoxicity is an important and recognized effect of red-bellied black snake envenoming that remains poorly understood but can result in severe illness. Prior observational data have shown that myotoxicity develops less frequently and later than other envenoming effects, including systemic symptoms and anticoagulant coagulopathy, which are likely to coexist in an envenomed victim. Current recommendations for managing snake bite victims in Australia include serial clinical and laboratory assessments for envenoming for at least 12 hours post-bite. CASE PRESENTATION: We report the ca

Paroxysmal Cold Haemoglobinuria: A Chilling Case of Acute Renal Failure.

We describe a rare case of a 64-year-old man with acute renal failure secondary to paroxysmal cold haemoglobinuria (PCH), presenting with haemoglobinuria, jaundice, and fevers. PCH is an intravascular haemolytic anaemia with polyphasic IgG antibody binding of red blood cells at cold temperatures, with complement-mediated haemolysis on rewarming. Renal biopsy confirmed acute tubular necrosis without thrombotic microangiopathy. The patient received supportive management including intermittent haemodialysis with a warm circuit to prevent further haemolysis and achieved full renal recovery. Optima

Diagnostic Approach to Seronegative Autoimmune-Mediated Rhabdomyolysis: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition caused by acute skeletal muscle injury leading to myocyte destruction and release of intracellular contents. While trauma and medications are common triggers, autoimmune-mediated necrotizing myopathies are rare. Diagnostic evaluation becomes challenging in seronegative cases. We report a 51-year-old man with multiple cardiovascular comorbidities presenting with one week of progressive proximal muscle weakness, severe back pain, and dark urine. Initial labs revealed a creatine kinase (CK) of 24,700 U/L, transaminitis, and acute kidney i

Nelarabine-associated rhabdomyolysis with severe creatine kinase elevation in a pediatric patient with T-cell acute lymphoblastic leukemia: a case report.

Nelarabine is a purine nucleoside analog used in relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL) and in up-front pediatric therapy. Although neurologic toxicity is dose-limiting, skeletal muscle toxicity, including rhabdomyolysis, has been reported. Rhabdomyolysis occurred in approximately 2% of nelarabine-treated patients in pediatric COG AALL0434, although the clinical features of these events were not detailed. We report a 7-year-old patient with T-ALL who developed rapidly progressive diffuse myalgia, muscle stiffness, impaired ambulation, dark urine, presumptive myoglob

Exertional Heat Rhabdomyolysis in Class III Obesity Mimicking Nephrolithiasis and Hepatobiliary Disease.

Exertional rhabdomyolysis has been described in military recruits and endurance athletes, whereas recognition in adults with severe obesity performing routine activities remains limited. Diagnostic delays may occur when initial clinical features resemble those of more common conditions and creatine kinase is omitted from early evaluation. A 30-year-old man with class III obesity (body mass index 51 kg/m²) developed severe right flank pain over three days after lawn mowing in hot weather. On presentation, he was febrile to 39.1°C, tachycardic, met systemic inflammatory response syndrome criteri

A fusion protein of the two tick-derived C5 inhibitors OmCI and RaCI: complete C5 inhibition despite strong complement activation.

The complement system must be tightly regulated to prevent tissue damage and disease, yet current C5 inhibitors like the anti-C5 monoclonal antibody Eculizumab - though clinically approved (e.g., for paroxysmal nocturnal haemoglobinuria, PNH) - fail to achieve complete terminal pathway blockade under conditions of strong complement activation. To address this limitation, we investigated tick-derived C5 inhibitors and discovered that combinatorial targeting of non-overlapping C5 epitopes enables full inhibition. Leveraging this insight, we engineered a bifunctional fusion protein RaCI-gly-OmCI

Intermittent White Urine With Nephrotic-Range Proteinuria and Preserved Renal Function: A Diagnostic Dilemma Between Chyluria, Pseudochyluria, and Amyloidosis.

Milky or "white" urine is usually attributed to chyluria but may also result from lipid-rich proteinuric urine or crystals ("pseudochyluria"). We describe a 47-year-old woman with a 6-month history of intermittent white urine, bilateral edema, nephrotic-range proteinuria (20.3 g/24 h), preserved renal function, near-normal serum albumin, and chronic bronchiectasis. Evaluation for parasitic and non-parasitic chyluria, tuberculosis, and systemic amyloidosis was repeatedly negative. A fasting urine sample showed markedly raised triglycerides but negative tests for chyle. The kidney biopsy was non

Between principles and practice: exploring transformative discourses in a Dutch National Park.

Transformative change is essential to halt the worldwide decline in biodiversity. Therefore, conservation efforts must extend beyond the traditional approach of protected areas to address the complex drivers of this decline. Landscape-oriented partnerships, such as partnership-based national parks, are emerging as potential instruments for this change. To be truly transformative, we assume that such partnerships must be underpinned by a shared discourse reflecting the principles of transformative governance. Drawing on transformative change and transformative governance theory combined with Q-

Evaluation of Clinical Safety of 4% Articaine in Inferior Alveolar Nerve Block for Non-surgical Mandibular Third Molar Extractions.

Group A streptococcus (GAS) causes diseases ranging from benign to severe infections such as necrotizing fasciitis (NF). The reasons for the differences in severity of streptococcal infections are unexplained. We developed the polymorphic-tag-lengths-transposon-mutagenesis (PTTM) method to identify virulence genes in vivo. We applied PTTM on an emm14 strain isolated from a patient with NF and screened for mutants of decreased virulence, using a mouse model of human soft-tissue infection. A mutant that survived in the skin but was attenuated in its ability to reach the spleen and to cause a let

Delayed-Onset Severe Statin-Induced Rhabdomyolysis Due to Atorvastatin-Ticagrelor Interaction in a Patient With Acute Coronary Syndrome: A Case Report.

Rhabdomyolysis is a rare but potentially life-threatening complication of statin therapy, typically occurring within days to weeks (mean onset approximately nine days) after initiation or dose escalation. Delayed-onset presentations after a period of uneventful use are uncommon and may delay diagnosis. We report a 62-year-old man with ischemic heart disease and chronic kidney disease (CKD stage 3a) who developed a five-day history of bilateral lower limb pain, difficulty ambulating, and dark-colored urine. Rhabdomyolysis in a 10-month-old child: A case report and literature review.

RATIONALE: Early diagnosis and prompt treatment of rhabdomyolysis (RM) in children are crucial to prevent potential complications, such as acute renal failure (ARF). Given that the clinical manifestations of RM in pediatric patients are often atypical, especially in infants, this case report aims to improve the recognition of RM by analyzing a specific clinical case and reviewing relevant literature. PATIENT CONCERNS: A 10-month-old girl was admitted to the hospital with a 3-day history of fever and a 1-day history of cough and rash. The child had no obvious cause of fever and showed no typic

A case report of CD4- CD8+ CD56- TCRγδ+ T-large granular lymphocytic leukaemia with severe aplastic anemia and a PNH clone.

T-large granular lymphocytic leukemia (T-LGLL) is a rare, indolent lymphoproliferative disorder of lymphocytes. Clonal expansion of T cells, driven by dysregulation of Fas-mediated apoptosis following activation, is a hallmark of this disease. In recent years, research on large granular T-cell leukemia has grown substantially compared with the past; however, most reports have focused on TCRαβ+ cases, while studies on other subtypes remain limited, and diagnostic experience with coexisting conditions requires further refinement. This study reports, for the first time, a case of CD4- CD8+ CD56-

Renal mucormycosis caused by Apophysomyces species: case report and literature review.

INTRODUCTION: Mucormycosis represents an uncommon yet aggressive and life-threatening fungal infection, typically occurring in immunocompromised individuals. Unlike most Mucorales infections, Apophysomyces frequently infects otherwise healthy hosts, raising significant clinical concern. Alarmingly its atypical manifestation, necrotizing fasciitis, is often misdiagnosed as a bacterial infection due to overlapping clinical features. CLINICAL CASE: Herein we present a case of Apophysomyces infection in kidney in an immunocompetent 52-year-old man, whose condition deteriorated swiftly, leading to

A real-world, multi-center, prospective, observational study for paroxysmal nocturnal haemoglobinuria (PNH) in China: Baseline characteristics, disease burden and treatment patterns.

BACKGROUND: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare life-threatening hematologic disorder with high thromboembolic mortality. In the context of large patient population and limited use of complement inhibitors, the clinical characteristics and disease progression of patients in China have not been well studied. OBJECTIVES: The China PNH Registry was initiated to advance understanding of the disease by collecting data and describing PNH disease burden, progression, and clinical outcomes with different medical interventions. DESIGN: This is a multi-center, prospective observational

Early identification of LPIN1 variant in a child with severe myoglobinuria and a history of sibling deaths.

Rhabdomyolysis in children is frequently attributed to viral myositis or trauma; however, severe or recurrent episodes precipitated by febrile illnesses often indicate an underlying genetic myopathy. Variants in the LPIN1 gene, which encodes the phosphatidic acid phosphatase Lipin-1, have recently emerged as a major cause of early-onset, life-threatening rhabdomyolysis (Autosomal Recessive Acute Recurrent Myoglobinuria). We present the case of a 5-year-old female who presented with acute onset profound lethargy, generalized muscle weakness, and cola-colored urine following a viral prodrome, on
