Influenza A is primarily a respiratory pathogen but may rarely cause severe extrapulmonary manifestations, including myositis, rhabdomyolysis, and acute kidney injury (AKI). Recognition of this complication is often delayed because respiratory symptoms may be absent or overshadowed by systemic manifestations. We present a case of a previously healthy 31-year-old man who presented with fever, generalized myalgia, oliguria, and dark urine. Laboratory evaluation demonstrated extreme rhabdomyolysis with a peak creatine kinase (CK) concentration of 468,130 U/L, severe transaminitis consistent with

McArdle disease (McA) is a rare metabolic disorder of autosomal recessive inheritance caused by pathogenic variants in the PYGM gene, which lead to a deficiency of the myophosphorylase enzyme. This enzymatic defect impairs muscle glycogenolysis, typically resulting in exercise intolerance, premature fatigue, and exertional cramps triggered by anaerobic or high-intensity physical activity starting in childhood or adolescence. However, the diagnosis is frequently delayed due to the heterogeneous and non-specific presentation of these symptoms. The authors report a case of a 61-year-old woman wit

Malignant hyperthermia (MH) is a rare, life-threatening inherited disorder triggered by volatile inhalational anesthetics and/or the depolarizing muscle relaxant suxamethonium. In susceptible individuals, calcium release from the sarcoplasmic reticulum in the skeletal muscle becomes abnormally accelerated, leading to a hypermetabolic state. Early signs of MH include unexplained hypercarbia (end-tidal carbon dioxide > 55 mm Hg), tachycardia, and muscle rigidity, particularly in the masseter. Rapid increases in core temperature (> 0.5 °C/15 min, with temperatures often exceeding 40 °C) are typic

Rhabdomyolysis is a rare but serious adverse effect of isotretinoin therapy, especially when combined with strenuous physical activity. We present a case of drug-induced rhabdomyolysis with secondary intramuscular abscesses in an adolescent. A 16-year-old male on standard-dose isotretinoin presented with a four-day history of right gluteal and thigh pain and fever, without a history of trauma, but with recent intense physical activity. Laboratory tests showed elevated inflammatory markers, creatine kinase, and myoglobinuria. MRI demonstrated extensive myositis with multiple intramuscular and i

Background and Clinical Significance: Tarui disease, or glycogen storage disease type VII, is a rare autosomal recessive metabolic myopathy caused by muscle phosphofructokinase deficiency. Its manifestations include exercise intolerance, exertional myalgia, muscle cramps, myoglobinuria, rhabdomyolysis, hyperuricemia, and compensated hemolysis. The hematologic phenotype may obscure the underlying metabolic disorder and raise concern for a clonal myeloid neoplasm. Case Presentation: A 23-year-old man was referred for persistent mild thrombocytopenia following evaluation for jaundice and hepatosp

Statin-associated myopathy represents a well-recognised adverse effect of 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase inhibitors, ranging from myalgia to life-threatening rhabdomyolysis. Neurological involvement in the form of peripheral neuropathy mimicking Guillain-Barré syndrome (GBS) is rare and diagnostically challenging. We report a case of a 72-year-old man with diabetes mellitus, hypertension, and single-vessel coronary artery disease who underwent percutaneous transluminal coronary angioplasty (PTCA) and was initiated on atorvastatin 40 mg once daily as secondary prevent

Dermatomyositis (DM) is often associated with malignancies but less commonly with plasma cell dyscrasias. Myoglobin nephropathy is an uncommon complication of inflammatory myopathies. This case describes a patient who presented with proteinuria because of myoglobin nephropathy caused by dermatomyositis. The dermatomyositis was paraneoplastic with an underlying smouldering multiple myeloma (SMM) diagnosed on investigation. Case report. A 58-year-old mixed-race gentleman was referred for sub-nephrotic proteinuria. Further history taking revealed a 1-month history of proximal muscle weakness and

BACKGROUND: Aplastic anaemia (AA) and paroxysmal nocturnal haemoglobinuria (PNH) are closely related acquired bone marrow failure syndromes sharing an immune-mediated pathogenesis. Cytogenetic abnormalities are uncommon in AA-PNH overlap syndrome, and their significance remains uncertain. We report a case of non-severe AA-PNH overlap syndrome harbouring a low-level chromosome 17p deletion in the absence of morphologic or molecular evidence of myeloid neoplasm.

Glycogen storage disease type VII (GSD-VII), or Tarui disease, is a rare autosomal recessive disorder caused by biallelic loss-of-function variants in the PFKM gene encoding the muscle isoform of phosphofructokinase (PFK), a key enzyme of the glycolytic pathway. PFK deficiency impairs glycogen and glucose metabolism in skeletal muscle and erythrocytes, causing exercise intolerance, exertional myalgia, and myoglobinuria, and, in some cases, fixed proximal muscle weakness, as well as haemolytic anaemia. We report the case of an Italian woman with genetically confirmed GSD-VII harbouring a homozy

Naphthalene poisoning is a rare but potentially life-threatening cause of haemolysis and methaemoglobinaemia due to severe oxidative stress. We report a case of a previously healthy man in his 30s who presented at the emergency department with scleral icterus, peripheral oxygen saturation of 85% on pulse oximetry, haemoglobinuria and chocolate-coloured blood following the accidental ingestion of a 'white air freshener ball'. Arterial blood gas analysis revealed elevated methaemoglobin (MetHb) levels. Laboratory results showed severe haemolysis. Bedside solubility and flotation tests on the whi

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired clonal haematopoietic stem cell disorder characterised by intravascular haemolysis, thrombosis and bone marrow failure. It results from an acquired somatic mutation in the X-linked phosphatidylinositol glycan Class A (PIGA) gene, leading to deficiency of glycosylphosphatidylinositol-anchored complement regulatory proteins, particularly CD55 and CD59, and uncontrolled complement-mediated haemolysis. Although sickle cell disease is the most prevalent haemolytic anaemia in Kenya, other aetiologies need to be considered. We describe a

UNLABELLED: Congenital cytomegalovirus (cCMV) infection is the leading non-genetic cause of sensorineural hearing loss and an important cause of neurodevelopmental impairment [1-3, 6-14]. Targeted screening strategies frequently fail to detect infected infants who are asymptomatic at birth but remain at risk for late sequelae [9]. This study aims to evaluate the diagnostic yield of universal saliva PCR screening compared with targeted screening strategies and to describe neuroimaging findings in identified cases. We conducted a prospective observational cohort study including 4000 neonates bor

BACKGROUND: Plasmodium vivax remains a challenge for malaria elimination in Nepal due to its ability to relapse. Radical cure with primaquine is effective but limited by poor adherence to the standard 14-day low-dose regimen. In 2022, the WHO recommended administering the same total dose (3.5 mg/kg) over 7 days to improve adherence. This study aimed to evaluate the 7-day low-dose primaquine regimen in G6PD-normal patients with uncomplicated P. vivax and/or P. falciparum malaria in the pre-elimination context of Nepal.
METHODS: A randomised study was conducted in south-west Nepal. Adult patien

A 28-year-old male with paroxysmal nocturnal haemoglobinuria (PNH) presented with headache, nasal bridge discomfort and haemoglobinuria. He developed a macular-papular rash which rapidly progressed into purpura, necrosis and peri-orbital oedema. Investigations demonstrated severe haemolytic anaemia and acute parvovirus B19 infection. A skin biopsy confirmed extensive necrotising vasculitis with intravascular thrombi, affecting the dermal and subcutaneous vessels. Eculizumab, corticosteroid and intravenous immunoglobulin were given, and within 24 h, symptoms markedly improved. The patient later

BACKGROUND: Dengue haemorrhagic fever (DHF) may occasionally manifest with atypical or severe complications under the umbrella of expanded dengue syndrome (EDS). Haemophagocytic lymphohistiocytosis (HLH) and intravascular haemolysis are rare but potentially fatal complications that require a high index of suspicion, particularly when clinical deterioration persists beyond the critical phase.
CASE PRESENTATION: We report a previously healthy 19-year-old woman who presented with DHF and subsequently developed persistent fever, worsening transaminitis, rising bilirubin levels, haemoglobinuria, a

BACKGROUND: Malarial retinopathy refers to a constellation of changes seen in severe or complicated malaria cases. These include: retinal whitening, vessel changes-whitening, tramlining, retinal hemorrhages, and papilledema. There are very few Indian studies on this entity. Since retina can be easily visualized by direct ophthalmoscopy, this study was done to determine prevalence of malarial retinopathy among malaria cases and to determine relationship between malarial retinopathy and severity of the disease.
MATERIALS AND METHODS: The study was done at Indoor and Outdoor Departments of Tropi

In November 2025, the American Academy of Orthopaedic Surgeons (AAOS) released the updated Clinical Practice Guideline for the Management of Acute Compartment Syndrome (ACS). This guideline addresses key issues including early diagnostic strategies, surgical decision-making, and perioperative management, presenting 8 recommendations and 7 consensus statements. A systematic interpretation highlights the core recommendations regarding the diagnostic value of biomarkers, the important role of compartment pressure monitoring, surgical decision-making, and wound management. A major update is the up

BACKGROUND: In July 2024, our hospital confirmed a rare case of facial infection with Mycobacterium scrofulaceum. The patient visited our hospital due to pain and pus discharge from the right orbital incision for one month. The patient suffered multiple facial fractures due to trauma three months ago. He underwent systemic anti infection treatment and open reduction and internal fixation surgery at an external hospital. After the surgery, there was repeated swelling around the orbit, and the patient did not fully recover. One month ago, the infraorbital area was swollen again, locally ruptured

BACKGROUND: Foot orthoses (FOs) are prescribed as an important conservative treatment option in patients with foot problems related to rheumatoid arthritis. However, a broad variation in FOs is used, both in clinical practice and in research. To date, there is no overview on the outcomes of the treatment with different kinds of FOs in patients with rheumatoid arthritis and a specific foot problem. The objectives of the present study were to summarize the comparative effectiveness of FOs in the treatment of various foot problems in patients with rheumatoid arthritis, on the primary outcomes foo

As per Internet world stats, with 560 million Internet users, India is the second largest online market in the world, ranked only behind China. Out of these 560 million Internet users in India, almost 375 million people use social media. These facts are enough to highlight the deep penetration of Internet and social media in Indian society. This deep penetration of social media among the Indian workers has further more deepened in the era of COVID-19. The employees are working from home, so they are more prone to use mobile and social media platforms for non-work-related purposes. It is a prov
