Myofibrillar Myopathy.

Myofibrillar myopathy (MFM) is characterized by segmental disarray of myofibrils and ectopic accumulation of a protein called desmin. Previously thought to be a glycogen storage disease, MFM is now recognized as a stand-alone myopathy. Endurance Arabians with MFM usually present with exertional rhabdomyolysis (MFM-ER) at the end of races, elevated serum muscle enzymes, and myoglobinuria. Warmblood horses with MFM (MFM-WB) usually present with pain-associated behaviors such as exercise intolerance, reluctance to engage hind quarter muscles, shifting lameness and normal serum muscle enzymes. Disease Activity After Switching From Eculizumab to Ravulizumab in Patients With Paroxysmal Nocturnal Haemoglobinuria: Real World Data From Denmark and Finland.

INTRODUCTION: Ravulizumab replaced eculizumab as the preferred complement inhibitor for Paroxysmal Nocturnal Haemoglobinuria (PNH) in Denmark and Finland after 2020. Data on real-world disease activity after switching are limited. METHODS: We conducted a case series of 20 PNH patients who transitioned from eculizumab to ravulizumab, assessing transfusion needs and breakthrough haemolysis episodes (BTH). RESULTS: In 19 of 20 patients, transfusions and BTH were stable or reduced after switching. Particularly, patients with high transfusion or BTH burden had a marked reduction in these paramete

Sex-dependent metabolic and behavioural alterations in a rat model of forced exertion-induced myopathy.

BACKGROUND: Mass boma capture (MBC) of ungulates may trigger a metabolic condition known as capture myopathy (CM), resulting in myoglobinuria and hyperthermia (rhabdomyolysis). Its pathobiology is poorly understood, especially the role of contextual reminders; a preclinical model system could thus be useful. Sixty (60) adult Sprague Dawley rats (30 rats per sex), divided into three experimental series (n = 12-24), were exposed to MBC-like exertion, viz., forced treadmill running (FTR) at 75% of VO2MAX (30 m/min) with and without aversive noise (context) until physical exhaustion. Acute kidney injury after swimming: a pediatric case of McArdle disease.

McArdle disease, a rare metabolic disorder, can lead to muscle breakdown and acute kidney injury (AKI) following physical exertion. We report a 12-year-old girl who developed AKI after several hours of swimming. Laboratory findings showed elevated blood urea nitrogen, creatinine, and creatine phosphokinase levels, indicating myoglobinuria. Genetic testing confirmed a pathogenic c.1708C > T mutation in the PYGM gene, diagnosing McArdle disease. After four sessions of hemodialysis, her kidney function fully recovered. This case highlights the need to consider metabolic disorders in the different

Clinical Outcomes and Disease Management for Patients with Paroxysmal Nocturnal Haemoglobinuria in China: Results from a Real-World Study.

BACKGROUND: Paroxysmal nocturnal haemoglobinuria (PNH) is a chronic haematological disorder caused by uncontrolled complement activation, leading to intravascular haemolysis, indicated by increased lactate dehydrogenase levels (LDH). Standard-of-care complement factor 5 inhibitors have only recently been approved in China. Using real-world data, this study aimed to understand treatment pattern and burden of disease, including thrombosis risk, for Chinese patients with PNH. METHODS: Data were from the Adelphi Real World PNH Disease Specific Programme (DSP)™, a cross-sectional retrospective sur

Decrease in Total Leukocyte Count is Associated with Acute Kidney Injury After Severe Burn.

Leukocytosis is common after burn injury from profound systemic inflammatory response. Total leukocyte count (TLC) often decreases 72-96 h post-injury. The incidence of early (<72 h) leukopenia has not been previously described; this analysis sought to determine if early and extreme decreases in TLC were associated with increased fluid requirements or resuscitation-related outcomes in patients after burn injury. The burn navigator (BN) database is composed of 300 patients with >20% TBSA and >40 kg that were resuscitated utilizing the BN clinical decision support system. Patients were split int

Pharmacological Therapies in Paroxysmal Nocturnal Haemoglobinuria: Focus on Complement Inhibition.

Paroxysmal nocturnal haemoglobinuria (PNH) is an ultra-rare acquired genetic stem cell disorder based on a mutation in the PIGA gene that results in susceptibility of resulting blood cells to complement-mediated intravascular haemolysis (IVH). In countries where anti-complement therapy is available, pharmacological treatments have transformed this disease from a highly morbid and sometimes lethal disorder. The first treatment developed was the terminal complement (C5) monoclonal antibody inhibitor eculizumab, in 2002. This has been largely supplanted by a longer-acting antibody, ravulizumab, t

Exercise-induced mild rhabdomyolysis associated with oclacitinib in a dog.

A four-year-old Toy Poodle treated with oclacitinib for one year presented with transient myoglobinuria associated with elevated serum creatine kinase (CK) levels only after exercise. An association between myoglobinuria and oclacitinib was suspected. An exercise stress test with or without oclacitinib was repeated twice on different days. Myoglobinuria and increased serum CK levels occurred only after testing with oclacitinib. The dog was diagnosed with exercise-induced mild rhabdomyolysis, which was presumably provoked by oclacitinib based on the results of the exercise stress tests. Rhabdom

A single-centre, real-world study on the efficacy and recovery of inflammatory cytokine levels of C5 complement inhibitor therapy in patients with paroxysmal nocturnal haemoglobinuria.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare clonal disorder caused by PIG-A mutations and characterized by complement-mediated haemolysis. C5 inhibitors are recommended as first-line therapy for PNH with high disease activity. We retrospectively analysed 57 patients treated with eculizumab or crovalimab. Luminex® liquid-phase chip technology was used to detect changes in complement and cytokine levels in patients pre- and post-treatment. Haemolysis control (LDH ≤ 1.5 × ULN) was achieved in 78.9% of patients, and 68.3% became transfusion-independent. Mean haemoglobin rose from 76 to 99

A Case Report and Review of Literature on Spinning-Induced Rhabdomyolysis Among First-Time Female Spinners.

Spinning is an indoor static cycling training program, and spinning-induced rhabdomyolysis (SIR) is a type of exercise-induced rhabdomyolysis causing systemic and local complications. In this case report, we would like to address its relevance to young women, especially first-time or unfit spinners, and the precautions that should be taken. A 24-year-old woman presented with bilateral thigh pain and dark brown-colored urine after a 45-minute first-time spinning session. She had a creatinine phosphokinase level of 108,166 U/L and myoglobinuria. She was diagnosed with SIR, treated with adequate

The efficiency of blood cell counts and inflammatory indices in prediction of need for acute kidney injury in patients with crush syndrome.

BACKGROUND: Disasters are inevitable causes of trauma, resulting in a high number of deaths and morbidity in survivors requiring prolonged treatment. Crush syndrome (CS), also known as traumatic rhabdomyolysis, is a syndrome with a wide clinical spectrum. A series of clinical complications caused by CS, hyperkalemia, myoglobinuria and especially acute kidney injury (AKI) are the main causes of death. OBJECTIVES: This study has been designed to investigate the effectiveness of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), lymphocyte-monocyte ratio (LMR) and systemic immun

Role of NK cells in immune escape in patients with classical paroxysmal nocturnal haemoglobinuria.

BACKGROUND: Paroxysmal nocturnal haemoglobinuria (PNH) is an acquired clonal haematopoietic stem cell disorder. Immune escape is crucial in PNH, and our previous studies revealed that natural killer (NK) cells potential participate in the immune escape of PNH. This study aimed to investigate the subtypes and functional changes of NK cells in PNH patients. METHODS: We analysed CD59+ and CD59- bone marrow mononuclear cells using single-cell RNA sequencing (scRNA-seq). The results were validated through flow cytometry and co-culture experiments. RESULTS: We classified NK cells into seven subtyp

Persistent Hematuria With Late Diagnosis of Cardiac Origin: A Case Report.

Dark-colored urine is often referred to as hematuria; however, this is an assumption that can often be misleading. In the evaluation of dark-colored urine, distinguishing between true hematuria, hemoglobinuria, and myoglobinuria is essential, as each indicates different underlying pathologies and requires targeted diagnostic and management strategies. Visualization of dark-colored urine often leads to an isolated urologic evaluation, and assessment for more infrequent causes of this phenomenon may be obscured. Clinicians tend to anchor on common urologic causes of dark-colored urine or rely on

Athletic pseudonephritis in male cross-country ultra-marathoners: a comparative observational study.

BACKGROUND/OBJECTIVE: Athletes have commonly reported hematuria, cylindruria, and proteinuria, which are consistent with "athletic pseudonephritis." To date, little is known about the overall consequences of 100 km (62.5-mile) cross-country ultra-marathons on Asian male runners. This study aimed to examine (i) acute urinary changes in runners with athletic pseudonephritis following a 100 km cross-country ultra-marathon and (ii) whether this phenomenon is associated with the runners average running speed. METHODS: Twenty male Taiwanese ultra-marathoners were prospectively recruited. Urinary pa

Incidence and Impact of Elevated Creatine Kinase Following Living Donor Nephrectomy.

BACKGROUND: The clinical significance of elevated creatine kinase (CK) levels following living kidney donation (LKD) remains poorly characterized. METHODS: In this single-center retrospective study, we analyzed 264 consecutive donors undergoing hand-assisted laparoscopic donor nephrectomy (HALDN) between March 2021 and December 2023. CK levels were monitored postoperatively on days 0 (POD0) and 1 (POD1), with POD2 testing performed only if clinically indicated. Primary outcomes included serum creatinine and estimated glomerular filtration rate (eGFR) at six months post donation. RESULTS: The

Rhabdomyolysis.

Muscle injury resulting in markedly elevated creatine kinase levels is termed rhabdomyolysis. The muscle damage can occur because of direct or indirect trauma, or it can be related to the use of certain medications, toxins, illicit drugs, or the presence of underlying inflammatory, metabolic, or genetic disorders with a predisposition for rhabdomyolysis. A detailed review of history can help in identifying the underlying etiology. Identification and removal of the triggering/inciting factor and noxious stimuli should be attempted as soon as possible. Careful evaluation for any complications an
