A disease that is difficult to predict: regional distribution and phenotypic, histopathological and genetic findings in McArdle disease.

OBJECTIVES: McArdle disease (also known as glycogen storage disease type V) is a rare metabolic myopathy that is caused by myophosphorylase deficiency, leading to impaired glycogenolysis in skeletal muscles. This study explored the clinical, histopathological, and genetic landscape of McArdle disease in a regional cohort from Turkey, emphasizing diagnostic and management challenges. METHODS: A retrospective analysis was conducted on 350 muscle biopsies performed between 2013 and 2024 in a tertiary care center. RESULTS: Seven patients (2.1 %) were diagnosed with McArdle disease. The clinical

Low-carbohydrate ketogenic diet in Mc Ardle's disease: a single-blinded randomized controlled trial.

Glycogenosis type 5 (GSD5, Mc Ardle's disease), the most common muscle glycogen storage disease characterized by exercise intolerance, recurrent myoglobinuria and possible myopathic evolution, is still missing and effective treatment. A dietetic approach to circumvent the metabolic failure in GSD5 seems attractive and has been attempted in various forms but with mixed results. We ran a single-blind randomized controlled trial comparing 6 months of individualized 3:1 low-carbohydrate ketogenic diet (LCKD) to diet as usual in 21 adult patients with GSD5. Primary endpoints were safety and tolerab

Dysphagia following transcatheter mitral valve-in-ring replacement complicated by neo-left ventricular outflow tract obstruction: a case report.

BACKGROUND: Neo-left ventricular outflow tract (LVOT) obstruction is a dreaded complication following transcatheter mitral valve replacement (TMVR). Dynamic LVOT obstruction has been reported to cause mechanical intravascular haemolysis due to red cell fragmentation. Intravascular haemolysis can result in a rare but well-described phenomenon in which patients experience dysphagia due to oesophageal spasm. This phenomenon is classically associated with paroxysmal nocturnal haemoglobinuria and has never been reported following TMVR. CASE SUMMARY: A 77-year-old female presented 8 days following

Recurrent Chyluria Revisited: Lymphangiographic Diagnosis of Pyelolymphatic Fistula With Insights on Endolymphatic Interventions.

Chyluria, the passage of chyle-laden lymph into urine, is most frequently linked to filarial infection but may also occur in diverse nonparasitic settings. We describe a 43-year-old man with diabetes, hypertension, and rheumatic heart disease who was admitted with prolonged fever and dyspnea and found to have infective endocarditis. The patient developed persistent milky urine during hospitalization that was unresponsive to a high-protein, low-fat diet supplemented with medium-chain triglycerides. Filariasis serology was negative. Although his comorbidities are not primary etiologic factors fo

Refractory chyluria warranting surgical managementa case report.

INTRODUCTION: Chyluria is a rare condition characterized by milky urine usually occurring due to rupture of lymphatic channels into the urinary system. Common symptoms include milky urine, clot colic, urinary retention, weight loss, edema, etc. Diagnosis is based on clinical suspicion, laboratory tests and imaging techniques such as USG, CT, MRI, and lymphangioscintigraphy. Management is based on dietary modifications, pharmacological treatment, minimally invasive endoscopic sclerotherapy, and laparoscopic chylolymphatic disconnection. Other procedures such as lymphovenous anastomosis or simpl

Higher Doses of Eculizumab may be Beneficial in Paroxysmal Nocturnal Haemoglobinuria in Pregnancy.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare bone marrow failure disorder that results in complement-mediated haemolysis (mostly intravascular), thrombosis and bone marrow suppression. Terminal complement inhibitors, such as eculizumab, have improved outcomes in pregnant and non-pregnant individuals. However, the optimal management of PNH during pregnancy has not been fully established. CASE DESCRIPTION: We describe the case of a 30-year-old Caucasian woman with classical PNH who became pregnant while on eculizumab therapy. Despite complete terminal complement blockade,

Efficacy and safety of HRS-5965 monotherapy in complement inhibitor-naïve patients with paroxysmal nocturnal haemoglobinuria.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening disorder characterized by complement-mediated haemolysis, leading to anaemia and thrombosis. HRS-5965 is a novel, oral, selective complement factor B inhibitor targeting the alternative pathway, potentially reducing both intra- and extravascular haemolysis. In this randomized, open-label phase II study, we evaluated the efficacy and safety of HRS-5965 monotherapy in complement inhibitor-naïve adult PNH patients. Twenty-six patients were randomized 1:1 to receive HRS-5965 50 mg orally twice daily (BID) (with potential up-tit

Iptacopan/LNP023 and rituximab as rescue therapy in a patient with systemic lupus erythematosus-associated atypical haemolytic uraemic syndrome.

Systemic lupus erythematosus (SLE) is rarely associated with atypical haemolytic uraemic syndrome, leading to a poor prognosis with multi-system involvement. The pathogenesis seems to be driven by hyperactivation of alternate pathways of complement cascade and patients should be treated with prompt administration of complement inhibitors and immunosuppressive therapy. Iptacopan/LNP023, a novel, orally administered C3 convertase inhibitor that has shown efficacy in C3 glomerulopathy and paroxysmal nocturnal haemoglobinuria, is going to be tested in patients with atypical haemolytic syndrome. Rotational Thromboelastometry (ROTEM)-Assisted Anaesthetic Management of a Parturient With Paroxysmal Nocturnal Haemoglobinuria and Aplastic Anaemia for Caesarean Section: A Case Report.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired, clonal haematopoietic disorder. It is life-threatening and is characterized by intravascular haemolytic anaemia, large vessel thrombosis and bone marrow failure. Aplastic anaemia (AA) and PNH are intimately linked; however, concurrent treatment of PNH and AA is uncommon. Pregnancies in patients with PNH are rare and associated with increased maternal and fetal morbidity and mortality. Nevertheless, with the advent of eculizumab, a monoclonal antibody used to treat PNH, pregnancy outcomes have become more favourable. The obstetric

Chyluria With Coexisting Minimal Change Disease: An Uncommon Clinical Association.

Chyluria is a rare clinical entity defined by the presence of chyle in the urine, classically presenting as milky or cloudy urine. While typically associated with parasitic infections such as Wuchereria bancrofti in endemic regions, non-parasitic causes are less common and diagnostically challenging. We report the case of a 56-year-old male presenting with milky urine, dysuria, urinary hesitancy, and rectal discomfort. Urine triglyceride level was found to be >525 mg/dL (reference range <10 mg/dL). An extensive infectious and parasitic workup was negative. Renal biopsy revealed minimal change

Primary HIV-1 infection presenting with nephrotic-range proteinuria and severe acute kidney injury mimicking imported Lassa fever.

PURPOSE: Primary HIV-1 infection (PHI) can present with protean clinical manifestations. We report a rare presentation of PHI that underscores that a high index of suspicion is required for diagnosis of PHI. METHODS: We report on a 54-yearold previously healthy woman of African descent who presented with sudden-onset nephrotic-range proteinuria and acute kidney injury (AKI) requiring hemodialysis in the setting of febrile multiple organ dysfunction syndrome. Both the epidemiological and clinical features initially pointed to imported Lassa fever, but this was ruled out. She was eventually dia

Epidemiology, clinical spectrum, and outcomes of severe malaria in Eastern Uganda: a prospective study.

BACKGROUND: In sub-Saharan Africa, malaria remains a public health problem despite some reports of declining incidence in the period 2000-2018. Since 2019, there have been some reports of disease epidemics and resurgences in areas that had registered steep declines and unusual clinical presentations. This study aimed to describe the epidemiology, clinical spectrum, and outcomes of severe malaria in children among malaria-endemic Eastern Uganda, a region that has recently experienced disease epidemics. METHODS: This prospective study was conducted at Mbale Regional Referral Hospital, Uganda, f

A Case of Mycobacterium Marinum Infection in the Middle Finger of the Left Hand.

BACKGROUND: In April 2024, our hospital confirmed a case of Mycobacterium marinum infection in the middle finger of the left hand. The patient sought medical attention at our hospital due to swelling of the left middle finger with tendon contracture for one and a half years and subcutaneous induration of the left palm for one month. In September 2022, the patient's left middle finger was pricked by raw fish bones, resulting in punctate erythema and pain. Self-application of erythromycin ointment did not improve. In November 2022, the left middle finger showed redness and swelling. The local ho

Acute Leptospirosis Outbreak in Cattle: A Case Report.

Leptospirosis is a globally distributed re-emerging zoonotic disease caused by Leptospira species. In cattle, the clinical course varies from a subclinical-chronic infection, commonly found in adult animals, to a severe acute syndrome mostly found in calves. The present study reports an outbreak of acute leptospirosis, following favourable climacteric conditions. Seven affected farms were monitored. Clinical observations from infected animals (n = 30) revealed two distinct patterns: lactating calves experienced hyperacute courses, often culminating in death, and older animals, mainly steers, d
