Dark Urine After Nonfatal Drowning: Rhabdomyolysis-Associated Acute Kidney Injury and Freshwater Aspiration Pneumonia.

Nonfatal drowning is managed primarily as a respiratory emergency, but clinically important kidney injury may be overlooked, particularly with delayed presentation. A previously healthy 53-y-old man fell into a freshwater pond that may have been contaminated by wastewater, struggled for ∼30 to 60 min, aspirated water, and presented 2 to 3 d later with fever, dyspnea, oliguria, and dark brown urine. Respiratory testing identified Aeromonas hydrophila complex and Escherichia coli, supporting drowning-associated pneumonia after freshwater aspiration. Serum creatinine was 986 μmol·L-1 (11.2 mg·dL-

Association of early qualitative myoglobinuria with in-hospital mortality and time to hospital discharge in burn patients: A 5-year retrospective cohort study.

BACKGROUND: Early urinary myoglobin may indicate extensive tissue injury after burns, but its prognostic value remains uncertain. We evaluated its association with in-hospital mortality and time to hospital discharge. METHODS: This single-centre retrospective cohort included 721 burn patients with an available urinary myoglobin result among 1005 potentially eligible admissions from 2020 to 2024. Myoglobinuria was assessed during the initial hospital evaluation using a qualitative ammonium sulphate precipitation test. Multivariable logistic regression evaluated in-hospital mortality. Delayed Postoperative Malignant Hyperthermia After Total Intravenous Anesthesia: A Case Report.

Malignant hyperthermia (MH) is a rare pharmacogenetic disorder typically triggered by volatile anesthetics or succinylcholine, but its occurrence after total intravenous anesthesia (TIVA) remains exceptional and underreported. Propofol-based TIVA is recommended for MH-susceptible patients to avoid known triggers, yet absolute prevention cannot be guaranteed. A 24-year-old male with prior suspected MH underwent spine surgery under propofol-remifentanil TIVA after strict preventive preparation. Despite stable intraoperative parameters, he developed fever, tachycardia, rigidity, and myoglobinuria

Updates on Rhabdomyolysis: A Clinically Oriented Narrative Review.

TOPIC IMPORTANCE: Rhabdomyolysis is the clinical syndrome resulting from the release of skeletal muscle cellular contents into the bloodstream, typically manifesting as limb weakness, myalgias, swelling, myoglobinuria, electrolyte abnormalities, and, critically, acute kidney injury (AKI). Recognition of this complex of clinical and biochemical features is necessary for accurate diagnosis and subsequent management. REVIEW FINDINGS: A precise consensus definition of rhabdomyolysis remains elusive. The McMahon score has proven useful for identifying patients at the highest risk for serious compl

[Pink-red discoloration in the diaper of a neonate].

Pink-red discoloration in the diaper of neonates is often attributed to harmless urate crystals, formed from uric acid. However, clinicians should remain alert to other possible causes of pink-red discoloration. Here we present two cases of pink-red discoloration in neonates: one due to a congenital mesoblastic nephroma and the other one due to renal vein thrombosis as underlying cause. Additional diagnostics should be considered at a low threshold, starting with a simple urine dipstick. If the dipstick is negative for haem, hematuria can be excluded. If positive, urine sediment analysis is th

Acute Biochemical Muscle Damage Responses to a Single Session of Whole-Body Electromyostimulation.

PURPOSE: This study aimed to investigate the acute effects of whole-body electromyostimulation (WB-EMS) on markers of skeletal muscle damage, renal function, and metabolic stress in sedentary adults. Given increasing reports of severe muscle damage after a single WB-EMS session, this study examined the time course and magnitude of biochemical and urinary responses after such an event. METHODS: Twenty-six participants were randomly assigned to a WB-EMS group or a control (exercise-only) group. While both groups completed the same physical exercise protocol, only the WB-EMS group received low-f

When Dark Urine Is Not Nephritis: A Life-Threatening Case of Lipin-1 Deficiency in Infancy.

Lipin-1 deficiency is a rare inherited metabolic disorder and a major genetic cause of severe, recurrent rhabdomyolysis in early childhood, carrying a significant risk of acute kidney injury and mortality if unrecognized. We report the case of a male toddler who first presented at 19 months of age with dark urine and intercurrent viral illness. Initial investigations demonstrated hematuria/myoglobinuria, proteinuria, and markedly elevated transaminases, leading to a presumptive diagnosis of glomerulonephritis. Creatine kinase (CK) was not measured at that time, and the episode was retrospectiv

Critical care considerations in rhabdomyolysis-associated acute kidney injury and kidney replacement therapy.

Rhabdomyolysis is a frequently seen and potentially severe condition in intensive care, resulting from causes like trauma, infection, drug toxicity, or prolonged immobility. When muscle cells break down, substances such as myoglobin enter the bloodstream, increasing the risk of acute kidney injury (AKI), which greatly affects patient outcomes. Prompt diagnosis, effective fluid management, and supportive care are crucial for reducing kidney-related complications. In more critical situations, standard treatments may not be enough, and kidney replacement therapy (KRT) may be required. This review

From the Field to the Emergency Room: A Systematic Case-Level Review of Exertional Rhabdomyolysis and Acute Kidney Injury in Athletes.

Exertional rhabdomyolysis is a potentially life-threatening syndrome caused by skeletal muscle breakdown with release of myoglobin and creatine kinase (CK) into the circulation. Clinical severity ranges from transient enzyme elevation to severe acute kidney injury (AKI) requiring renal replacement therapy (RRT). Although increasingly recognized, the published literature remains fragmented and largely descriptive. This review systematically aggregated case-level data to characterize biochemical severity and renal outcomes across different athlete populations and exertional contexts. PubMed, Emb

Exertional Rhabdomyolysis Following Concurrent Exercise Including Pelvic Floor (Kegel) Training in a Previously Healthy 36-Year-Old Male Patient: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition characterized by skeletal muscle breakdown and release of intracellular contents into the systemic circulation. Exertional rhabdomyolysis (ER) most commonly occurs after high-intensity exercise involving large muscle groups, but cases related to lower intensity or repetitive activity have also been reported. We describe a case of acute ER in a previously healthy 36-year-old man who developed the condition after initiating a workout regimen for erectile dysfunction that included gluteal exercises and pelvic floor muscle training (PFMT),

Hypokalemia-Induced Rhabdomyolysis Following Prolonged Diarrhea in a Child With Congenital Heart Disease Receiving Loop Diuretics: A Case Report and Literature Review.

Hypokalemia is a rare but important cause of rhabdomyolysis in children. Unlike typical rhabdomyolysis, hyperkalemia may be absent because the triggering factor is hypokalemia, potentially delaying diagnosis. We report a five-year-old boy with hypoplastic left heart syndrome after a Fontan procedure who developed severe hypokalemia and rhabdomyolysis following prolonged diarrhea while receiving loop diuretics. He presented with acute lower extremity weakness and inability to stand. Laboratory evaluation revealed potassium 1.9 mmol/L and creatine kinase 9,013 U/L, peaking at 32,908 U/L. Urinaly

Severe Rhabdomyolysis Without Electrolyte Abnormalities or Renal Failure and With Disproportionately Low AST.

Rhabdomyolysis is a condition in which damaged muscle tissue breaks down and typically presents with muscle pain, weakness, and dark-colored urine, along with markedly elevated serum creatine kinase (CK) levels. Common lab findings include hyperkalemia, hyperphosphatemia, hypocalcemia, elevated lactate dehydrogenase (LDH), acute kidney injury (AKI), and elevated aspartate aminotransferase (AST), with a typical CK:AST ratio of approximately 20:1. This report describes a 26-year-old man with no prior medical history who developed dark urine and lower back pain after a high-intensity functional t

Imatinib-induced rhabdomyolysis: A case report.

Imatinib, a tyrosine kinase inhibitor used for chronic myeloid leukemia (CML) and gastrointestinal stromal tumors (GIST), is associated with myalgia and creatine kinase (CK) elevations, though severe rhabdomyolysis and myopathy are rare. We report an 83-year-old woman with CML who developed progressive proximal weakness, dark urine, and acute kidney injury after three years of imatinib therapy. Laboratory evaluation revealed CK >24,000 U/L, transaminitis, and myoglobinuria. MRI showed diffuse muscle and fascial edema, while autoimmune testing was negative. Imatinib and rosuvastatin were discon

Delayed onset of myotoxicity following red-bellied black snake envenoming: a case report.

BACKGROUND: Myotoxicity is an important and recognized effect of red-bellied black snake envenoming that remains poorly understood but can result in severe illness. Prior observational data have shown that myotoxicity develops less frequently and later than other envenoming effects, including systemic symptoms and anticoagulant coagulopathy, which are likely to coexist in an envenomed victim. Current recommendations for managing snake bite victims in Australia include serial clinical and laboratory assessments for envenoming for at least 12 hours post-bite. CASE PRESENTATION: We report the ca

Diagnostic Approach to Seronegative Autoimmune-Mediated Rhabdomyolysis: A Case Report.

Rhabdomyolysis is a potentially life-threatening condition caused by acute skeletal muscle injury leading to myocyte destruction and release of intracellular contents. While trauma and medications are common triggers, autoimmune-mediated necrotizing myopathies are rare. Diagnostic evaluation becomes challenging in seronegative cases. We report a 51-year-old man with multiple cardiovascular comorbidities presenting with one week of progressive proximal muscle weakness, severe back pain, and dark urine. Initial labs revealed a creatine kinase (CK) of 24,700 U/L, transaminitis, and acute kidney i
