Rhabdomyolysis is a potentially life-threatening condition caused by acute skeletal muscle injury leading to myocyte destruction and release of intracellular contents. While trauma and medications are common triggers, autoimmune-mediated necrotizing myopathies are rare. Diagnostic evaluation becomes challenging in seronegative cases. We report a 51-year-old man with multiple cardiovascular comorbidities presenting with one week of progressive proximal muscle weakness, severe back pain, and dark urine. Initial labs revealed a creatine kinase (CK) of 24,700 U/L, transaminitis, and acute kidney i

Nelarabine is a purine nucleoside analog used in relapsed or refractory T-cell acute lymphoblastic leukemia (T-ALL) and in up-front pediatric therapy. Although neurologic toxicity is dose-limiting, skeletal muscle toxicity, including rhabdomyolysis, has been reported. Rhabdomyolysis occurred in approximately 2% of nelarabine-treated patients in pediatric COG AALL0434, although the clinical features of these events were not detailed. We report a 7-year-old patient with T-ALL who developed rapidly progressive diffuse myalgia, muscle stiffness, impaired ambulation, dark urine, presumptive myoglob

Exertional rhabdomyolysis has been described in military recruits and endurance athletes, whereas recognition in adults with severe obesity performing routine activities remains limited. Diagnostic delays may occur when initial clinical features resemble those of more common conditions and creatine kinase is omitted from early evaluation. A 30-year-old man with class III obesity (body mass index 51 kg/m²) developed severe right flank pain over three days after lawn mowing in hot weather. On presentation, he was febrile to 39.1°C, tachycardic, met systemic inflammatory response syndrome criteri

Rhabdomyolysis is a rare but potentially life-threatening complication of statin therapy, typically occurring within days to weeks (mean onset approximately nine days) after initiation or dose escalation. Delayed-onset presentations after a period of uneventful use are uncommon and may delay diagnosis. We report a 62-year-old man with ischemic heart disease and chronic kidney disease (CKD stage 3a) who developed a five-day history of bilateral lower limb pain, difficulty ambulating, and dark-colored urine. During a routine post-percutaneous coronary intervention (PCI) follow-up visit, subtle l

RATIONALE: Early diagnosis and prompt treatment of rhabdomyolysis (RM) in children are crucial to prevent potential complications, such as acute renal failure (ARF). Given that the clinical manifestations of RM in pediatric patients are often atypical, especially in infants, this case report aims to improve the recognition of RM by analyzing a specific clinical case and reviewing relevant literature.
PATIENT CONCERNS: A 10-month-old girl was admitted to the hospital with a 3-day history of fever and a 1-day history of cough and rash. The child had no obvious cause of fever and showed no typic

Rhabdomyolysis in children is frequently attributed to viral myositis or trauma; however, severe or recurrent episodes precipitated by febrile illnesses often indicate an underlying genetic myopathy. Variants in the LPIN1 gene, which encodes the phosphatidic acid phosphatase Lipin-1, have recently emerged as a major cause of early-onset, life-threatening rhabdomyolysis (Autosomal Recessive Acute Recurrent Myoglobinuria). We present the case of a 5-year-old female who presented with acute onset profound lethargy, generalized muscle weakness, and cola-colored urine following a viral prodrome, on

Routine urine toxicology screening may be misleading when adolescents present with acute psychotic symptoms and systemic toxicity after exposure to drugs not included in the screening panel. We report a 17-year-old girl with no known psychiatric or neurologic illness who developed agitation, persecutory delusions, fever, diaphoresis, tachycardia, limb rigidity, and a brief generalized tonic-like episode after suspected repeated nonprescribed pregabalin misuse and misuse of an amantadine-containing over-the-counter cold preparation. Influenza A is primarily a respiratory pathogen but may rarely cause severe extrapulmonary manifestations, including myositis, rhabdomyolysis, and acute kidney injury (AKI). Recognition of this complication is often delayed because respiratory symptoms may be absent or overshadowed by systemic manifestations. We present a case of a previously healthy 31-year-old man who presented with fever, generalized myalgia, oliguria, and dark urine. Laboratory evaluation demonstrated extreme rhabdomyolysis with a peak creatine kinase (CK) concentration of 468,130 U/L, severe transaminitis consistent with

McArdle disease (McA) is a rare metabolic disorder of autosomal recessive inheritance caused by pathogenic variants in the PYGM gene, which lead to a deficiency of the myophosphorylase enzyme. This enzymatic defect impairs muscle glycogenolysis, typically resulting in exercise intolerance, premature fatigue, and exertional cramps triggered by anaerobic or high-intensity physical activity starting in childhood or adolescence. However, the diagnosis is frequently delayed due to the heterogeneous and non-specific presentation of these symptoms. The authors report a case of a 61-year-old woman wit

Malignant hyperthermia (MH) is a rare, life-threatening inherited disorder triggered by volatile inhalational anesthetics and/or the depolarizing muscle relaxant suxamethonium. In susceptible individuals, calcium release from the sarcoplasmic reticulum in the skeletal muscle becomes abnormally accelerated, leading to a hypermetabolic state. Early signs of MH include unexplained hypercarbia (end-tidal carbon dioxide > 55 mm Hg), tachycardia, and muscle rigidity, particularly in the masseter. Rapid increases in core temperature (> 0.5 °C/15 min, with temperatures often exceeding 40 °C) are typic

Rhabdomyolysis is a rare but serious adverse effect of isotretinoin therapy, especially when combined with strenuous physical activity. We present a case of drug-induced rhabdomyolysis with secondary intramuscular abscesses in an adolescent. A 16-year-old male on standard-dose isotretinoin presented with a four-day history of right gluteal and thigh pain and fever, without a history of trauma, but with recent intense physical activity. Laboratory tests showed elevated inflammatory markers, creatine kinase, and myoglobinuria. MRI demonstrated extensive myositis with multiple intramuscular and i

Background and Clinical Significance: Tarui disease, or glycogen storage disease type VII, is a rare autosomal recessive metabolic myopathy caused by muscle phosphofructokinase deficiency. Its manifestations include exercise intolerance, exertional myalgia, muscle cramps, myoglobinuria, rhabdomyolysis, hyperuricemia, and compensated hemolysis. The hematologic phenotype may obscure the underlying metabolic disorder and raise concern for a clonal myeloid neoplasm. Case Presentation: A 23-year-old man was referred for persistent mild thrombocytopenia following evaluation for jaundice and hepatosp

Glycogen storage disease type VII (GSD-VII), or Tarui disease, is a rare autosomal recessive disorder caused by biallelic loss-of-function variants in the PFKM gene encoding the muscle isoform of phosphofructokinase (PFK), a key enzyme of the glycolytic pathway. PFK deficiency impairs glycogen and glucose metabolism in skeletal muscle and erythrocytes, causing exercise intolerance, exertional myalgia, and myoglobinuria, and, in some cases, fixed proximal muscle weakness, as well as haemolytic anaemia. We report the case of an Italian woman with genetically confirmed GSD-VII harbouring a homozy

In November 2025, the American Academy of Orthopaedic Surgeons (AAOS) released the updated Clinical Practice Guideline for the Management of Acute Compartment Syndrome (ACS). This guideline addresses key issues including early diagnostic strategies, surgical decision-making, and perioperative management, presenting 8 recommendations and 7 consensus statements. A systematic interpretation highlights the core recommendations regarding the diagnostic value of biomarkers, the important role of compartment pressure monitoring, surgical decision-making, and wound management. A major update is the up

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Myofibrillar myopathy (MFM) is characterized by segmental disarray of myofibrils and ectopic accumulation of a protein called desmin. Previously thought to be a glycogen storage disease, MFM is now recognized as a stand-alone myopathy. Endurance Arabians with MFM usually present with exertional rhabdomyolysis (MFM-ER) at the end of races, elevated serum muscle enzymes, and myoglobinuria. Warmblood horses with MFM (MFM-WB) usually present with pain-associated behaviors such as exercise intolerance, reluctance to engage hind quarter muscles, shifting lameness and normal serum muscle enzymes. BACKGROUND: Mass boma capture (MBC) of ungulates may trigger a metabolic condition known as capture myopathy (CM), resulting in myoglobinuria and hyperthermia (rhabdomyolysis). Its pathobiology is poorly understood, especially the role of contextual reminders; a preclinical model system could thus be useful. Sixty (60) adult Sprague Dawley rats (30 rats per sex), divided into three experimental series (n = 12-24), were exposed to MBC-like exertion, viz., forced treadmill running (FTR) at 75% of VO2MAX (30 m/min) with and without aversive noise (context) until physical exhaustion. McArdle disease, a rare metabolic disorder, can lead to muscle breakdown and acute kidney injury (AKI) following physical exertion. We report a 12-year-old girl who developed AKI after several hours of swimming. Laboratory findings showed elevated blood urea nitrogen, creatinine, and creatine phosphokinase levels, indicating myoglobinuria. Genetic testing confirmed a pathogenic c.1708C > T mutation in the PYGM gene, diagnosing McArdle disease. After four sessions of hemodialysis, her kidney function fully recovered. This case highlights the need to consider metabolic disorders in the different
