Oral iptacopan monotherapy in paroxysmal nocturnal haemoglobinuria: final 48-week results from the open-label, randomised, phase 3 APPLY-PNH trial in anti-C5-treated patients and the open-label, single-arm, phase 3 APPOINT-PNH trial in patients previously untreated with complement inhibitors.

BACKGROUND: The factor B inhibitor iptacopan improved 24-week outcomes in adult patients with paroxysmal nocturnal haemoglobinuria in the phase 3 APPLY-PNH and APPOINT-PNH trials; the trial extension periods assessed clinical activity and safety up to 48 weeks. Here, we report the final 48-week data from APPLY-PNH and APPOINT-PNH. METHODS: In both APPLY-PNH and APPOINT-PNH trials, patients were aged 18 years or older, with paroxysmal nocturnal haemoglobinuria (red and white blood cell population sizes ≥10%) and without laboratory evidence of bone marrow failure. In APPLY-PNH (an open-label, r

Dysphagia following transcatheter mitral valve-in-ring replacement complicated by neo-left ventricular outflow tract obstruction: a case report.

BACKGROUND: Neo-left ventricular outflow tract (LVOT) obstruction is a dreaded complication following transcatheter mitral valve replacement (TMVR). Dynamic LVOT obstruction has been reported to cause mechanical intravascular haemolysis due to red cell fragmentation. Intravascular haemolysis can result in a rare but well-described phenomenon in which patients experience dysphagia due to oesophageal spasm. This phenomenon is classically associated with paroxysmal nocturnal haemoglobinuria and has never been reported following TMVR. CASE SUMMARY: A 77-year-old female presented 8 days following

Higher Doses of Eculizumab may be Beneficial in Paroxysmal Nocturnal Haemoglobinuria in Pregnancy.

INTRODUCTION: Paroxysmal nocturnal haemoglobinuria (PNH) is a rare bone marrow failure disorder that results in complement-mediated haemolysis (mostly intravascular), thrombosis and bone marrow suppression. Terminal complement inhibitors, such as eculizumab, have improved outcomes in pregnant and non-pregnant individuals. However, the optimal management of PNH during pregnancy has not been fully established. CASE DESCRIPTION: We describe the case of a 30-year-old Caucasian woman with classical PNH who became pregnant while on eculizumab therapy. Despite complete terminal complement blockade,

Efficacy and safety of HRS-5965 monotherapy in complement inhibitor-naïve patients with paroxysmal nocturnal haemoglobinuria.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, life-threatening disorder characterized by complement-mediated haemolysis, leading to anaemia and thrombosis. HRS-5965 is a novel, oral, selective complement factor B inhibitor targeting the alternative pathway, potentially reducing both intra- and extravascular haemolysis. In this randomized, open-label phase II study, we evaluated the efficacy and safety of HRS-5965 monotherapy in complement inhibitor-naïve adult PNH patients. Twenty-six patients were randomized 1:1 to receive HRS-5965 50 mg orally twice daily (BID) (with potential up-tit

Iptacopan/LNP023 and rituximab as rescue therapy in a patient with systemic lupus erythematosus-associated atypical haemolytic uraemic syndrome.

Systemic lupus erythematosus (SLE) is rarely associated with atypical haemolytic uraemic syndrome, leading to a poor prognosis with multi-system involvement. The pathogenesis seems to be driven by hyperactivation of alternate pathways of complement cascade and patients should be treated with prompt administration of complement inhibitors and immunosuppressive therapy. Iptacopan/LNP023, a novel, orally administered C3 convertase inhibitor that has shown efficacy in C3 glomerulopathy and paroxysmal nocturnal haemoglobinuria, is going to be tested in patients with atypical haemolytic syndrome. Rotational Thromboelastometry (ROTEM)-Assisted Anaesthetic Management of a Parturient With Paroxysmal Nocturnal Haemoglobinuria and Aplastic Anaemia for Caesarean Section: A Case Report.

Paroxysmal nocturnal haemoglobinuria (PNH) is a rare, acquired, clonal haematopoietic disorder. It is life-threatening and is characterized by intravascular haemolytic anaemia, large vessel thrombosis and bone marrow failure. Aplastic anaemia (AA) and PNH are intimately linked; however, concurrent treatment of PNH and AA is uncommon. Pregnancies in patients with PNH are rare and associated with increased maternal and fetal morbidity and mortality. Nevertheless, with the advent of eculizumab, a monoclonal antibody used to treat PNH, pregnancy outcomes have become more favourable. The obstetric

Epidemiology, clinical spectrum, and outcomes of severe malaria in Eastern Uganda: a prospective study.

BACKGROUND: In sub-Saharan Africa, malaria remains a public health problem despite some reports of declining incidence in the period 2000-2018. Since 2019, there have been some reports of disease epidemics and resurgences in areas that had registered steep declines and unusual clinical presentations. This study aimed to describe the epidemiology, clinical spectrum, and outcomes of severe malaria in children among malaria-endemic Eastern Uganda, a region that has recently experienced disease epidemics. METHODS: This prospective study was conducted at Mbale Regional Referral Hospital, Uganda, f

Acute Leptospirosis Outbreak in Cattle: A Case Report.

Leptospirosis is a globally distributed re-emerging zoonotic disease caused by Leptospira species. In cattle, the clinical course varies from a subclinical-chronic infection, commonly found in adult animals, to a severe acute syndrome mostly found in calves. The present study reports an outbreak of acute leptospirosis, following favourable climacteric conditions. Seven affected farms were monitored. Clinical observations from infected animals (n = 30) revealed two distinct patterns: lactating calves experienced hyperacute courses, often culminating in death, and older animals, mainly steers, d

A rare case of dialysis associated reaction presenting with urticarial vasculitis and haemoglobinuria.

Dialysis associated reactions presenting with urticarial vasculitis is rarely reported in medical literature. We report a 61-year-old gentleman who developed sudden onset dyspnea with diffuse erythema within 20 min of haemodialysis. Patient was started on Azilsartan 3 days prior to this clinical event. Labs revealed features of hemolysis and urine was positive for hemoglobinuria. All dialysis related factors responsible for this reaction were ruled out. Due to non-resolution of skin rash, skin biopsy was attempted which revealed fibrinoid necrosis of occasional vessels with predominant lymphoc

Classic paroxysmal nocturnal haemoglobinuria presenting with intestinal malabsorption syndrome, acute abdomen and acute kidney injury.

A male patient in his 30s presented with complaints of acute abdominal pain, black stools and red-coloured urine. CT revealed thrombi in the splenic and left renal veins, leading to infarctions. An endoscopy displayed scalloping of the duodenal folds, indicative of intestinal malabsorption syndrome (IMS). Histopathological examination confirmed IMS. Due to the presence of intravascular haemolysis, haemoglobinuria and thrombotic complications, paroxysmal nocturnal haemoglobinuria (PNH) was suspected and subsequently confirmed by flow cytometry. Thus, a diagnosis of classic PNH with IMS and thro

The histone demethylase JMJD1C regulates CPS1 expression and promotes the proliferation of paroxysmal nocturnal haemoglobinuria clones through cell metabolic reprogramming.

Paroxysmal nocturnal haemoglobinuria (PNH) is a disorder resulting from erythrocyte membrane deficiencies caused by PIG-A gene mutations. While current treatments alleviate symptoms, they fail to address the underlying cause of the disease-the pathogenic PNH clones. In this study, we found that the expression of carbamoyl phosphate synthetase 1 (CPS1) was downregulated in PNH clones, and the level of CPS1 was negatively correlated with the proportion of PNH clones. Using PIG-A knockout K562 (K562 KO) cells, we demonstrated that CPS1 knockdown increased cell proliferation and altered cell metab

Acute Necrotizing Pancreatitis Complicated with Paroxysmal Nocturnal Haemoglobinuria: A Case Report.

This report presents a unique case of acute necrotizing pancreatitis(ANP) concomitant with paroxysmal nocturnal hemoglobinuria(PNH), a combination that has not been documented in existing literature. The impact of PNH on ANP and its treatment remains uncertain due to the lack of consensus. The case described herein involves a patient who exhibited both ANP and PNH, subsequently experiencing splanchnic vein thrombosis (SVT), resulting in substantial intra-abdominal and gastrointestinal hemorrhaging. We attempted to analyze the role of PNH in the formation of SVT in ANP and propose some new insi

The metabolic fuel of paroxysmal nocturnal haemoglobinuria.

Metabolic reprogramming has been investigated in haematological malignancies. To date, a few studies have analysed the metabolic profile of paroxysmal nocturnal haemoglobinuria (PNH). The study by Chen and colleagues sheds light on the involvement of metabolic changes in the proliferation of PNH clones. Commentary on: Chen et al. The histone demethylase JMJD1C regulates CPS1 expression and promotes the proliferation of PNH clones through cell metabolic reprogramming. Br J Haematol 2024;204:2468-2479.
