Polygenicity shapes distinct brain patterns and clinical profiles of bipolar disorder.

BACKGROUND: Bipolar disorder (BD) manifests both genetic predispositions and brain imaging abnormalities. Genetic analyses provide a powerful approach to disentangling the associations between BD and brain imaging-derived phenotype (IDP). However, in East Asian (EAS) populations, the genetic basis of these neuroimaging changes and their clinical relevance remains underexplored due to limited integrative evidence. METHODS: We investigated the genetic architecture and clinical relevance of BD and brain IDPs in East Asian populations using individual-level data from the Chinese Bipolar Disorder

Structural network alterations and peripheral immune-inflammatory dysregulation define distinct subtypes of major depressive disorder with prognostic implications.

BACKGROUND: Immune-inflammatory dysregulation and cortical structural vulnerability have been increasingly implicated in major depressive disorder (MDD). However, how they interact with each other, and whether immune heterogeneity in MDD is systematically organized by structural networks, remain unclear. METHODS: Genome-wide association studies data of MDD, gray matter volume (GMV) phenotypes, and 19 immune-related traits were utilized to assess causal relationships. A prospective cohort comprising 122 patients with MDD and 66 healthy controls underwent structural magnetic resonance imaging a

Association of Left Ventricular Outflow Tract Obstruction with Left Atrial Remodeling and Dysfunction in Hypertrophic Cardiomyopathy: A Three-Dimensional Echocardiography and Left Atrial Strain Study.

Adolescents with major depressive disorder (MDD) and non-suicidal self-injury (NSSI) face serious challenges to their mental and physical health. The development of this condition is influenced by genetic, environmental, and brain development factors. In this study, we used the Morphometric Inverse Divergence Gradient Network (MIND-Gd) for the first time to explore brain structural network abnormalities and their transcriptional mechanisms in adolescents with MDD and NSSI. A total of 241 adolescents with MDD participated in this study. Among them, 134 had NSSI, while 107 did not. Using high-re

Central nervous system, cranial, and cerebrovascular conditions beyond the Kimonis criteria in patients with Gorlin syndrome: a retrospective cohort study.

BACKGROUND: Gorlin syndrome is a multisystem developmental and cancer-predisposing disorder commonly caused by germline PTCH1 variants. The Kimonis clinical criteria capture the canonical phenotype; however, central nervous system (CNS), cranial, and cerebrovascular conditions documented beyond these criteria remain poorly defined. METHODS: We conducted a retrospective descriptive cohort study of 32 patients who fulfilled the Kimonis clinical criteria and received care at either or both hospitals in Japan. Clinical information through March 31, 2026, including sufficiently documented care els

Possible Epstein-Barr virus-associated central nervous system involvement with reversible electroencephalographic abnormalities in a child: a case report.

BACKGROUND: Epstein-Barr virus (EBV)-associated central nervous system (CNS) involvement in children is uncommon and may present with nonspecific neurological symptoms. Brain magnetic resonance imaging (MRI) may remain normal, making diagnosis challenging. Cerebrospinal fluid (CSF) targeted next-generation sequencing (tNGS) has emerged as a useful diagnostic tool, although positive EBV results require careful clinical interpretation. CASE DESCRIPTION: A previously healthy 5-year-old boy presented with fever (38.5 ℃), headache, dizziness, and nausea for 3 days without seizures, altered conscio

Late-Detected Sinus Venosus Atrial Septal Defect and Subsequent Enteroviral Central Nervous System Infection: A Case Report.

Sinus venosus atrial septal defect (SVASD) is frequently associated with partial anomalous pulmonary venous return and may be missed on routine transthoracic echocardiography because of its superior and posterior location. We report a 68-year-old woman with atrial fibrillation who presented with dyspnea and marked right-sided chamber enlargement. Cardiac magnetic resonance imaging identified a 33 × 21-mm superior SVASD, anomalous drainage of the right upper and middle pulmonary veins, and a persistent left superior vena cava. The Qp/Qs was 3.7 by cardiac magnetic resonance and 3.2 by right hea

Association Between Neuropsychological Test Performance and Neuroimaging Findings in Cognitive Impairment: A Retrospective Cohort Study.

BACKGROUND: Structural magnetic resonance imaging (MRI) and [18F]fluorodeoxyglucose positron emission tomography (FDG-PET) provide different information about cognitive impairment, but the additional contribution of FDG-PET beyond MRI across cognitive outcomes remains unclear. This study examined associations between neuropsychological performance and MRI/FDG-PET findings. METHODS: This retrospective cohort included 200 patients with mild cognitive impairment or dementia. Neuropsychological assessment covered global cognition and memory, executive, visuospatial and language domains. Structura

Head-to-head comparison of ⁶⁸Ga-FAPI and ¹⁸F-FDG PET/CT in intracranial space-occupying lesions.

PURPOSE: Intracranial space-occupying lesions (SOL), including primary brain tumors, metastases, and inflammatory lesions, require accurate characterization for diagnosis and management. Although ¹⁸F-FDG PET/CT reflects tumor glucose metabolism, its performance in the brain is limited by high physiological uptake. ⁶⁸Ga-FAPI PET/CT targets fibroblast activation protein in the tumor microenvironment and may provide complementary biological information with improved lesion conspicuity. This study aimed to compare ⁶⁸Ga-FAPI and ¹⁸F-FDG PET/CT for the evaluation of intracranial SOL. METHODS: Thirt

Imaging of Vascular Diseases of the Spinal Cord.

Spinal cord vascular diseases are rare but pose a significant diagnostic challenge and can lead to devastating neurologic injury. Timely diagnosis is challenging given their heterogeneous presentations and overlapping features. This article provides an anatomic framework for understanding spinal vascular pathologies, key imaging patterns, and clinical nuances to improve diagnostic accuracy and guide treatment for a variety of conditions including spinal cord ischemia, spinal vascular malformations, spinal dural arteriovenous fistulas, spinal aneurysms, and cerebrospinal fluid venous fistulas.

Neurological Manifestations in Whipple's Disease: A Scoping Review of Clinical Features, Laboratory Findings, and Neuroimaging Characteristics of 111 Cases.

BACKGROUND: Whipple disease (WD) is a rare, potentially fatal infection caused by Tropheryma whipplei (TW). Central Nervous System involvement (CNS-WD) is a severe complication of WD with a challenging diagnosis, due to limited data integrating neurological features, imaging, cerebrospinal fluid analysis, PCR, and intestinal histopathology. Prompt recognition and treatment are essential to improve patient outcomes. METHODS: A scoping review of the literature from 2000 to 2025 was conducted, including all CNS-WD case reports and series, to outline its clinical spectrum and diagnostic workup.

Staged bilateral MR-guided focused ultrasound thalamotomy in a patient with cerebral cavernous malformations.

MRI-guided focused ultrasound (MRgFUS) thalamotomy is an effective ablative procedure for medication-refractory essential tremor (ET). However, limited data exist regarding its safety in patients with coexisting intracranial disease such as cerebral cavernous malformations. We present a man in his late 60s with medication-refractory ET who underwent staged bilateral MRgFUS thalamotomy. Preoperative MRI demonstrated heterogeneously T1-hyperintense and T2-hyperintense lesions in the left caudate head and basal ganglia consistent with cavernous malformations, with an associated developmental veno

Shunting Arterio-Venous Lesions of The Cervical Spine: Diagnosis Across Imaging Modalities.

BACKGROUND: Shunting arteriovenous lesions of the central nervous system historically have been challenging to diagnose on non-invasive imaging given their subtle findings and complex anatomy. For this reason, catheter directed angiography has remained the gold standard for diagnosing and characterizing these lesions. While this should always remain a core component of working up suspected vascular lesions, other imaging modalities can be useful for making the diagnosis. METHODS: We identified patients with shunting arteriovenous lesions of the cervical spine at our institution between the 19

Thermodynamic impedance mismatch in neurodegeneration: A biophysical framework for the prediction of amyloid-related imaging abnormalities.

Amyloid-related imaging abnormalities (ARIA) represent a significant iatrogenic complication of anti-amyloid monoclonal antibody therapies. To address this, we propose the Impedance Mismatch Theory as a theoretical framework and hypothesis, reframing neurodegeneration as a network-level thermodynamic failure characterized by localized thermal runaway (Neural Physiological Load Index (NLI) greater than or equal to 1.0). Within this framework, amyloid-β (Aβ) is modeled not only in its established biological context, but as a potential thermodynamic heat sink and bio-conductive electrical shunt d

Shared Genetic Architecture Between Common Epilepsies and Subcortical Brain Volumes Is Associated With Cognition and Mental Health.

AIMS: Epilepsy is a common neurological disorder, often presenting with psychiatric comorbidities and cognitive impairment. Neuroimaging studies have suggested associations between epilepsy and subcortical volumetric abnormalities. The genetic basis between epilepsy and subcortical volume abnormalities is, however, poorly understood, particularly if they share a common genetic architecture. METHODS: To investigate polygenic overlap, we employed the bivariate causal mixture model (MiXeR) to analyze genome-wide association data from various epilepsy subtypes (genetic generalized epilepsy [GGE]:

Glymphatic dysfunction as a potential mechanistic link to cognitive ımpairment in Behçet's disease: a hypothesis.

Behçet's disease (BD) is a chronic multisystem inflammatory disease in which cognitive impairment is increasingly recognized but remains incompletely understood. Although cognitive dysfunction is more common in patients with Neuro-Behçet's disease (NBD), it has also been reported in patients without clinically apparent neurological involvement, suggesting that mechanisms beyond focal inflammatory lesions may contribute to cognitive impairment. The glymphatic system is a brain-wide perivascular clearance pathway that maintains cerebral homeostasis by facilitating the exchange of cerebrospinal a

Von Hippel-Lindau syndrome presenting as a lateral ventricular space-occupying lesion: a case report.

Von Hippel-Lindau disease is a rare autosomal dominant genetic disorder caused by mutations in the VHL tumor suppressor gene located on chromosome 3. Its core feature is the development of vascular-rich tumors or cysts in multiple organ systems at different ages, mainly manifesting as cerebellar hemangioblastoma, retinal hemangioblastoma, renal cell carcinoma and/or multiple renal cysts, and adrenal pheochromocytoma, among others. The disease is rare and involves multiple systems, making diagnosis challenging. Genetic testing has gradually become the gold standard for its diagnosis. This case

Sex-dependent mild cardiac dysfunction and neurometabolic abnormalities in a model of Huntington's disease.

Huntington's disease (HD) is a neurodegenerative disease with progressive decline in psychiatric, motor and cognitive symptoms. Hallmarks of HD include brain volume loss and abnormal metabolism. However, HD is a multi-system disorder, and the impact on other organ systems is poorly understood. Using high-frequency ultrasound and nuclear magnetic resonance, we studied cardiac function (n=9-11 mice/genotype/sex) and brain metabolism (n=10 mice/genotype/sex) in a humanized transgenic mouse model of HD (Hu97/18 mice) at 12 months of age. The assessments were conducted in different cohorts and anal

Graph theory-based machine learning for automated fibromyalgia diagnosis using fMRI during an emotional task.

BACKGROUND: Fibromyalgia (FM) is a multifaceted chronic pain disorder presenting with pain throughout the musculoskeletal system, alongside chronic fatigue, sleep disturbances, and cognitive impairments. Current diagnostic criteria primarily rely on symptom questionnaires, with no specific laboratory markers or definitive tests, making diagnosis subjective and often delayed. Moreover, the overlap with other central pain syndromes further complicates accurate diagnosis. Neuroimaging studies, particularly functional magnetic resonance imaging (fMRI), have shown promise in elucidating brain abnorm
