The incidence of idiopathic intracranial hypertension in Israel from 2005 to 2007: results of a nationwide survey.

BACKGROUND AND PURPOSE: Idiopathic intracranial hypertension (IIH), also known as pseudotumor cerebri, is a disorder related to increased intracranial pressure without clinical, laboratory or radiological evidence of an intracranial space occupying lesion or cerebral sinus vein thrombosis, predominantly affecting obese women of childbearing age. Our aim was to determine the incidence and clinical features of IIH in Israel. METHODS: In a cross-sectional study, all medical records of patients discharged from the hospital with a primary diagnosis of IIH during 2005-2007 were reviewed. RESULTS:

Surgicel application in intracranial hemorrhage surgery contributed to giant-cell granuloma in a patient with hypertension: case report and review of the literature.

BACKGROUND: Surgicel is an oxidized cellulose preparation that is widely applied in neurosurgery due to its hemostatic effect and good tissue compatibility. Tumor-like lesions induced by Surgicel application in cerebral surgery have been rarely reported, especially for intracranial hemorrhage debridement surgery in patients with hypertension. CASE PRESENTATION: This case report describes a rare case in which Surgicel application led to a foreign body reaction, contributing to the development of an intracranial giant-cell granuloma. A 49-year-old female hypertensive patient was diagnosed with

Gigantic ossified chronic epidural haematoma and contralateral postoperative subdural haematoma: A case report and literature review.

A 21-year-old woman was found to have a dramatic intracranial space-occupying lesion found via CT scan performed to investigate headache. Craniotomy was performed and the mass found to be a chronic extradural haematoma, whose aetiology was likely to have been spontaneous, due to thrombocytopenia in infancy. Although this was removed, her recovery was complicated by the need to treat a contralateral delayed subdural haematoma.

Classical Hodgkin Lymphoma involving the central nervous system (brain) - an unusual presentation.

Intracranial Hodgkin lymphoma (HL) is considered so atypical that an intracranial space-occupying lesion in a patient with known HL should be thoroughly investigated to rule out a second disease process.

POEMS syndrome in a 20-year-old patient diagnosed following a complaint of reduced visual acuity.

We report a case of POEMS syndrome in a 20-year-old patient diagnosed after visiting an eye clinic with a chief complaint of reduced visual acuity. A male university student aged 20 years was referred to our department complaining of blurred vision in both eyes that had persisted for 1 month. He also noted headache, nausea, and paresthesia in the lower extremities around the same time. The visual acuity of his right and left eye was 20/40 and 20/20, respectively. Optic disc edema and serous retinal detachment were present. Brain magnetic resonance imaging showed no intracranial abnormalities,

Anaesthetic management of a pregnant patient with intracranial space occupying lesion for craniotomy.

Intracranial space occupying lesion [SOL] during pregnancy presents several challenges to the neurosurgeons, obstetricians and anaesthesiologists in not only establishing the diagnosis, but also in the perioperative management as it requires a careful plan to balance both maternal and foetal well-being. It requires modification of neuroanaesthetic and obstetric practices which often have competing clinical goals to achieve the optimal safety of both mother and foetus. Intracranial tuberculoma should be considered in the differential diagnosis of intracranial SOL in pregnant women with signs an

Surgical treatment of elevated intracranial pressure: decompressive craniectomy and intracranial pressure monitoring.

Surgical techniques that address elevated intracranial pressure include (1) intraventricular catheter insertion and cerebrospinal fluid drainage, (2) removal of an intracranial space-occupying lesion, and (3) decompressive craniectomy. This review discusses the role of surgery in the management of elevated intracranial pressure, with special focus on intraventricular catheter placement and decompressive craniectomy. The techniques and potential complications of each procedure are described, and the existing evidence regarding the impact of these procedures on patient outcome is reviewed. Surgi

Astrocytoma development following complete multiple myeloma remission in a 49-year-old patient: A case report.

Multiple myeloma (MM), one of the B-cell non-Hodgkin lymphomas, is a bone marrow-derived, antibody-producing cancer of the plasma cells. In the advanced stages, the cancer cells frequently cause widespread osteolytic bone damage; however, in rare cases, MM also manifests as an intracranial plasmacytoma. In the present study, we describe a case in which a patient, initially treated for MM and with subsequent complete remission, was admitted to hospital with a lesion in the right cerebellar hemisphere and neurological symptoms of a brain tumor. Our initial diagnosis was an MM relapse with the ra

Vitamin D deficiency rickets presenting as pseudotumor cerebri.

Pseudotumor cerebri is a condition of elevated intracranial pressure in the absence of clinical, laboratory or radiological evidence of an intracranial space-occupying lesion. Various associations with pseudotumor cerebri have been made in literature. We report the case of a five-month-old female infant with vitamin D deficiency rickets, who presented with pseudotumor cerebri. Her cerebrospinal fluid examination was normal, with a high opening pressure of 330 mmH2O. Her computed tomography scan was normal. After lumbar puncture the anterior fontanelle came at level. Her investigations revealed

Anaesthetic concerns for large intracranial cyst excision: expect the unexpected!

Neurosurgical procedures in infants poses various challenges such as difficulty in venous cannulation, securing invasive vascular lines, difficult airway, controlling intra-cranial tension, managing large fluid shifts to positioning- related issues and temperature maintenance. We hereby present an case of a large intracranial space occupying lesion, suspected to be a hydatid cyst, which later turned out to be an infected ventricular cyst and intraoperative problems. A thorough preparation prior to operation of infected cystic lesion of the brain is required including anticipation of massive bl

Fentanyl co-administration decreases the induction dose requirement of propofol in patients with supratentorial tumors and not in patients with spinal lesions.

BACKGROUND: The requirement of anesthetic drugs in a patient with an intracranial space-occupying lesion is of relevance to the neuroanesthetist. The requirement is often presumed to have reduced or at least altered. However, not much research has focused on this issue. Hence, we conducted this study to examine whether intracranial tumors reduce the induction dose of propofol in patients undergoing craniotomy based on plasma and effect site concentrations (Ce) of propofol and the effect of additional fentanyl. METHODS: A total of 80 patients were recruited into the study. The study group incl

Ocular complications of malaria treatment.

Malaria is endemic in Nigeria. With the emergence of chloroquine resistance various modes of treatment including parenteral quinine are employed with consequent untoward effects. This article reports two cases of severe ocular toxicity, including mimicry of intracranial space-occupying lesion, from treatment of malaria with various drugs including quinine. Medical practitioners are advised to exhaust other less toxic modes of therapy before using drugs with great potential for severe untoward effects such as quinine.

Optic disc oedema: a diagnostic dilemma.

The authors describe a patient who presented with bilateral disc oedema and left eye visual loss. The patient was evaluated for intracranial space occupying lesion, however, a CT and cerebrospinal tap was normal. No definitive diagnosis could be established and patient developed left eye optic atrophy which was thought to be post papilloedema atrophy. Six months later, the patient had similar visual loss in right eye and he was diagnosed as anterior ischemic optic atrophy (AION) by the ophthalmologist. The inferior altitudinal visual field loss, delayed visual evoked response and clinical pict

Pediatric idiopathic intracranial hypertension (pseudotumor cerebri).

Idiopathic intracranial hypertension (IIH), or pseudotumor cerebri, is a condition of elevated intracranial pressure in the absence of clinical, laboratory or radiological evidence of an intracranial space occupying lesion that can occur in the pediatric population. While IIH is more commonly recognized as a disorder of adults, it affects children of all ages and can have distinctive characteristics when presenting in the prepubertal age group. This review discusses the demographics, clinical presentation, diagnosis, neuroimaging, and management of pediatric IIH. Common etiologies of IIH in ch

Fibrosarcoma of the meninges.

Meningeal fibrosarcomas are rare tumors. Only 41 cases have been reported in the literature to date. Primary central nervous system fibrosarcomas are very aggressive neoplasms and have a poor prognosis. Hence they need to be correctly diagnosed. This is a case of a 13-year old boy with intracranial space occupying lesion. The mass was completely removed and histological examination was characteristic of meningeal fibrosarcoma. The pathological diagnosis is usually made on routine light microscopic examination; however, occasionally these may be difficult to distinguish from other malignant neo
