Metamorphosis of dermal cylindroma: possible relation to malignant transformation. Case report of cutaneous cylindroma with direct intracranial invasion.

A case of locally invasive cutaneous cylindroma is reported. This neglected scalp tumor, 6.5 cm in greatest dimension, initially invaded the calvaria at the time of the first operation. A recurrence infiltrated the dura and presented as an intracranial space-occupying lesion. No brain invasion was present and distant metastases were not documented. The patient remains well 2 1/2 years after tumor resection. Light and electron microscopic appearance of the recurrent tumor revealed distinct morphologic features, which may be of use in predicting the biologic behavior of cutaneous cylindromas. Th

Congenital meningeal sarcoma--a case report.

A 30-year-old multigravida woman was admitted to the obstetrical unit in the third trimester of gestation, because of a large-for-date uterus. Repeated ultrasonic examinations over a 2-week period revealed an increase in biparietal diameter far too large to be normal. This progressive asymmetrical hydrocephalus was diagnosed as caused by an intracranial space-occupying lesion in the middle cranial fossa. Because of the only minimal thickness of the cerebral cortex prognosis was judged so poor, that labor was induced. A stillborn female infant was delivered vaginally after an ultrasonically gui

[Cerebral abscess and pulmonary arteriovenous fistula. A clinical and respiratory function study].

Liz... Josiane, a 9 year old girl, was admitted with a 24 hours history of severe headache and vomiting. On admission she was conscious, irritable and complained of a severe headache. Clinical examination revealed a right hemiparesis with cyanosis of the lips and extremities and clubbing of the fingers, all consistent with chronic hypoxia. Cardiovascular examination was normal apart from a systolic murmur which could be heard posteriorly under the left scapula. There were no angiomatous or telangiectatic lesions of the skin. A blood examination revealed a raised ESR, a marked polycythaemia wit

[Totally thrombosed large arteriovenous malformation--a case report].

A totally thrombosed large arteriovenous malformation (AVM) which operated as an intracranial space occupying lesion was presented. The patient was a 24-year-old woman who had had frequent attacks of loss of consciousness since the age of 13 years and had been on anticonvulsant medication at the psychiatric department of our hospital. She was referred to our clinic with bitemporal girdle pain, nausea and vomiting. Neurological examination revealed right homonymous hemianopsia, sensory aphasia and choked discs in both ocular fundi. Computed tomogram demonstrated a well defined large high densit

Fluorescein retinal angiography in the early diagnosis of optic disc edema.

33 patients hospitalized for suspected intracranial space-occupying lesion with negative or dubious ophthalmoscopic findings underwent fluorescein retinal angiography (FRA). The final diagnosis was based on the clinical and instrumental evidence. In 18 patients FRA showed late fluorescence of the optic disc with blurring of its margins. 17 of them had an intracranial space-occupying lesion and 1 had had a transient attach of cerebral ischemia. The FRA was negative in 15 patients: in 13 the final diagnosis ruled out raised intracranial space-occupying lesion. Comparison of the FRA results with

Angiography and computerized tomography in the diagnosis of aneurysmal bone cyst of the skull: case report.

An aneurysmal bone cyst of the occipital bone, presenting as an intracranial space-occupying lesion, is reported. Clinical and neuroradiological findings are described in detail. The significance of angiographic and computerized tomographic findings is discussed.

Cryptococcal granuloma presenting as an intracranial mass.

A case of cryptococcosis of the central nervous system presenting as an intracranial space occupying lesion is described. The world literature and epidemiology of this unusual disease are surveyed. Problems in the diagnosis and the use of modern diagnostic techniques are discussed. The authors suggest that cryptococcosis should be included in the differential diagnosis of an intracranial space occupying lesion.

Discovery of an epidermoid of the skull concomitant with a homolateral subacute traumatic subdural hematoma.

The case of man of 32 years with an epidermoid of the left side of the skull is reported. The tumor itself was asymptomatic and was discovered accidentally because of a subdural hematoma on the same side. After a closed head injury, this patient had symptoms of an intracranial space occupying lesion (hematoma) on the left after a symptom-free interval. The left carotid angiogram demonstrated the characteristic picture of a subdural hematoma in the left parieto-temporo-occipital region. At the same time, characteristic changes in the skull on the same side, which were more apparent after remova

Computerised axial tomography and acute neurological problems of childhood.

The results of computerised axial tomography (CAT) in 80 children with neurological symptoms and/or signs of less than 3 months' duration are discussed in relation firstly to intracranial pathology and secondly to clinical presentation. 26 children had intracranial space-occupying lesions (tumour, abscess, haemorrhage, infarct). CAT was abnormal in 25 of these and diagnostic in 18. A further 20 children had meningitis or encephalitis, and CAT was abnormal in 12. In contrast with this high rate of scans showing pathology, CAT was abnormal in only 4 of the remaining 34 children who had less defi

Optic disc drusen and tumours of the chiasmal region.

Two patients with optic disc drusen and tumour of the chiasmal region are presented. The association of optic disc drusen and intracranial space-occupying lesion is probably a chance occurrence. In spite of the recognition of the optic disc drusen, certain findings indicate further examinations. Progressive loss of central visual acuity unexplained by retinal pathology is an indication for a neurological investigation. Bitemporal and homonymous hemianopic visual field defects as well as evidence of papilloedema warrant a neurological evaluation.

The neurology of metastatic chorion carcinoma.

Although chorion carcinoma of the uterus is the seventh commonest in the comparative frequency of malignant tumours seen at the Cancer Registry of the University College Hospital, Ibadan, Nigeria (being exceeded in order of commoness by the reticuloendothelial tumours, carcinoma of the cervix, liver, breast, stomach and ovary), it is the most frequent source of tumour deposit of the brain in this hospital. Between 1960 and 1969, 197 Nigerians with chorion cancer of the uterus were admitted to UCH; in twenty-five of them the nervous system was involved during the course of their disease. The ne

Meningiomas: treated and untreated.

Meningiomas are frequently amenable to total resection. On review of a 10-year series of these tumours in Western Australia it was found that 18 out of total 78 were undiagnosed during life. Analysis of the present series is undertaken as well as comparison with other series in the literature in respect of the overall clinical picture. It is suggested that, in cases of gradual psychological deterioration, the presence of a benign intracranial space-occupying lesion ought to be excluded.

[Reliability of positive findings in serial cerebral scintigraphy. Evaluation of a prospective series of 1700 cases].

The cerebral sequential scintigraphy enables a process to be described according to its hemodynamics (Stage I), its degree of vascularization (Stage II), and the extent of the localized disturbance of the blood-brain barrier function (Stage III). For a given lesion, typical scintigraphic behaviour patterns can be described. This report presents the results of a prospective series with 1722 patients examined using this method. The accuracy of the different scintigraphic diagnoses, according to tumor type, was: cerebrovascular accident with brain infarction - 92% (= CVA), metastasis - 90%, bone

Cerebral hemorrhage following heart surgery.

The authors report a study of the problem of intracranial hemorrhage in 16 children following cardiac surgery, four studied clinically, and 12 by postmortem pathological review. Eleven children sustained subdural hematomas of varying sizes, one had a massive extradural clot, and four had intracerebral clots. The pre-, intra-, and postoperative data of these 16 patients are presented, but the specific factors causing the intracranial hemorrhage remain unexplained. The neurological course was similar to that of patients with an intracranial space-occupying lesion, and fundamental neurosurgical m
