Seeing is believing-on the utility of CT in phenotyping chronic obstructive pulmonary disease.

Chronic obstructive pulmonary disease (COPD) is a heterogeneous condition with complicated structural and functional impairments. For decades now, chest CT has been used to quantify various abnormalities related to COPD. More recently, with the newer data-driven approaches, biomarker development and validation have evolved rapidly. Studies now target multiple anatomical structures including lung parenchyma, the airways, the vasculature, and the fissures to better characterize COPD. This review explores the evolution of chest CT biomarkers in COPD, beginning with traditional thresholding approa

Modified lung ultrasound score to assess extent and prognosis in systemic autoimmune rheumatic disease-associated interstitial lung disease.

OBJECTIVES: Lung ultrasound (LUS) assesses lung lesion severity in systemic autoimmune rheumatic disease-associated interstitial lung disease (SARD-ILD). The ultrasound manifestations of SARD-ILD include pleural line (PL) abnormalities and heterogeneous B-line (BL) distribution. We aimed to investigate the feasibility of a modified ultrasound scoring system based on refined grading of BL and PL features in patients with SARD-ILD, to determine whether BLs and PLs can effectively reflect ILD patterns across different lung compartments. METHODS: One hundred and ninety-five patients with nine SAR

Incidental Cardiovascular Findings in Lung Cancer Screening and Noncontrast Chest Computed Tomography.

While the primary goal of lung cancer screening CT is to detect early-stage lung cancer in high-risk populations, it often reveals asymptomatic cardiovascular abnormalities that can be clinically significant. These findings include coronary artery calcifications (CACs), myocardial pathologies, cardiac chamber enlargement, valvular lesions, and vascular disease. CAC, a marker of subclinical atherosclerosis, is particularly emphasized due to its strong predictive value for cardiovascular events and mortality. Guidelines recommend qualitative or quantitative CAC scoring on all noncontrast chest C

Phenotyping asthma and/or chronic obstructive pulmonary disease using 129Xe MRI and comprehensive physiologic testing.

RATIONALE: Asthma and chronic obstructive pulmonary disease (COPD) significantly overlap by conventional diagnostic criteria, yet important treatment differences remain, and people with both asthma and COPD (asthma + COPD) have worse clinical outcomes than people with a single diagnosis. Hyperpolarized xenon-129 magnetic resonance imaging (129Xe MRI) and pulmonary function tests (PFTs) are sensitive to lung function and structure. OBJECTIVE: To determine whether 129Xe MRI alongside PFTs can aid phenotyping of real-world patients with asthma and/or COPD. METHODS: Patients ≥16 years with physi

Characterizing chronic obstructive pulmonary disease using quantitative MRI biomarkers.

Chronic obstructive pulmonary disease (COPD) is a heterogeneous lung disease that occurs due to structural changes to the parenchyma, airways and pulmonary vasculature, and consequent functional impairments to ventilation and perfusion. Although computed tomography (CT) imaging is the standard for assessing structural lung changes in COPD, it requires ionizing radiation and is unable to provide functional information without contrast agents. Conversely, there have been numerous developments for MRI of the lungs in the last several decades, allowing for the quantification of structural and func

Bronchopulmonary malinosculations.

Congenital bronchopulmonary malinosculations encompass a spectrum of disorders characterized by anomalous connections among the 3 components in the lung-airways, parenchyma, and vessels. This review describes these anomalies using a simplified systematic approach to facilitate understanding and diagnosis. The classification framework, based on the presence or absence of arterial, venous, and bronchial anomalies, organizes malinosculations into 7 groups. Cross-sectional imaging techniques such as CT angiography plays a critical role in delineating anatomical abnormalities, enabling precise iden

Heterogeneity of short-term elexacaftor-tezacaftor-ivacaftor response in cystic fibrosis using 129Xe MRI.

BACKGROUND: Personalised management of cystic fibrosis (CF) lung disease in the era of CF transmembrane conductance regulator (CFTR) modulator therapy will require novel approaches to assess treatment response and monitor longitudinal progression. METHODS: We evaluated the heterogeneity of short-term response to elexacaftor-tezacaftor-ivacaftor (ETI) using 129Xe MRI, multiple breath washout (MBW) and spirometry. For CFTR modulator-naïve people ≥6 years old starting commercial ETI, we measured same-day 129XeMRI ventilation defect per cent (VDP), lung clearance index (LCI), forced expiratory vo

Gas chromatography-mass spectrometry exhaled breath analysis for phenotyping interstitial lung disease- an exploratory study.

Interstitial lung disease (ILD) encompasses pulmonary disorders characterised by varying degrees of inflammation and/or fibrosis. The presence and extent of these pulmonary abnormalities on (high-resolution) computed tomography (CT) have consequences for diagnosis and treatment; however, inter-observer assessment varies. Analysis of exhaled volatile organic compounds (VOCs) through gas chromatography-mass spectrometry (GC-MS) offers a noninvasive approach to biomarker discovery and pathophysiology understanding. Our study aims to explore the ability of GC-MS-driven exhaled breath analysis to d

Practical application of lung ultrasound during discharge echocardiography to predict outpatient effusions after septal defect and Fontan surgery.

INTRODUCTION: Lung ultrasound findings in cardiac patients correlate with mortality, hospital length of stay, and rehospitalisation after surgery. We report a lung ultrasound protocol integrated with echocardiography and its ability to predict adverse events in children after discharge following congenital heart surgery. METHODS: A prospective, single-blinded observational trial was performed. Subjects were consecutively identified after Fontan or septal defect repairs. Performed by cardiac sonographers at discharge, lung ultrasound scores were based on the number of B-lines. The primary outc

4D Flow Yields Similar Clinical Results Compared To 2D Phase Contrast for Decision Making Regarding Pulmonary Valve Replacement in Repaired Tetralogy of Fallot.

2D phase contrast (PC) CMR is critical for monitoring pulmonary regurgitant fraction (PRF) in patients with a history of repaired tetralogy of Fallot (rTOF) and guides decisions about pulmonary valve replacement (PVR). However, newer techniques such as 3D time-resolved phase contrast (4D flow) MRI are gaining momentum due to the added clinical value beyond conventional flow metrics. While agreement between 2D PC CMR and 4D flow is well-established, the real-world implications for clinical decision-making have not been fully studied and are thus the primary aim of this study. All patients with

Prevalence, Co-occurrence, and Prognostic Implications of S Modifiers in the Korean National Lung Cancer Screening Program.

Background The documentation of clinically significant incidental findings (S modifiers) in low-dose CT lung cancer screening varies among radiologists. Although the Korean National Lung Cancer Screening Program adopted structured reporting for seven standardized S modifiers, the prognostic value of standardized S modifiers has not been evaluated comprehensively. Purpose To evaluate the implementation of structured reporting for prespecified S modifiers by analyzing their prevalence, mortality associations, and co-occurrence patterns. Materials and Methods This retrospective study included bas

Reframing spontaneous pneumothorax: A practical guide to the PLEX variant classification.

The conventional primary-secondary classification of spontaneous pneumothorax fails to capture the complexity encountered during thoracoscopic surgery, where CT imaging often misses subtle apical scarring, early emphysematous changes, or fibrotic, noncompliant lung tissue. To provide a more operative-relevant clinical framework, we hereby propose the PLEX Classification, based on Pattern of lung abnormality, Location of leak and lung reserve, Extent of disease, and eXpected surgical complexity and surgical outcome. This system categorizes pneumothorax into four different variants: Type I (apic

Bidirectional cortical gyrification alterations in chronic obstructive pulmonary disease: links to cognitive impairment and global initiative for chronic obstructive lung disease staging.

PURPOSE: Cognitive impairment is a common but poorly understood comorbidity in chronic obstructive pulmonary disease (COPD). Although gray matter abnormalities have been observed in this population, the contribution of cortical gyrification-a structural feature linked to cognitive development and brain plasticity-remains unclear. This study aimed to characterize region-specific cortical gyrification alterations and examine their associations with domain-specific cognitive function and disease severity. METHODS: We enrolled 59 patients with stable COPD and 49 healthy controls who underwent pul

Pulmonary Venous Atresia as a Mimicker of Chronic Thromboembolic Pulmonary Disease: Diagnostic Utility of Ventilation/Perfusion Scanning.

A man in his 60s was referred to our institution for evaluation of suspected chronic thromboembolic pulmonary disease in the context of progressive exertional dyspnea and a mismatched ventilation/perfusion defect on scintigraphy. The ventilation/perfusion scan revealed complete absence of perfusion to the left hemithorax with preserved ventilation. CT pulmonary angiography demonstrated unilateral pulmonary vein atresia, characterized by absent left pulmonary venous drainage, a hypoplastic pulmonary arterial tree on the affected side, and compensatory hyperinflation of the contralateral lung. Central nervous system complications: atypical manifestation of Lemierre syndrome.

Lemierre syndrome is a systemic suppurative infectious disease caused by septic thrombophlebitis of the internal jugular vein. Because multiple organs can be involved, diagnosis is often challenging, and treatment may be delayed. Cerebral infarction and cavernous sinus thrombosis are rare complications. We describe the case of a 65-year-old man who initially presented with headache and vomiting. Based on cerebrospinal fluid analysis and cranial magnetic resonance imaging, viral infection was first suspected. Despite antiviral therapy, his condition worsened. He developed fever, left ptosis, vi
