Although a skeletal scan is an effective means of detecting early metastatic deposits from carcinoma of the breast, its indiscriminate use in screening patients with early tumors does not seem justified because of the low yield of abnormal results. Selection of patients for this procedure should be based on the size of the tumor and the clinical stage of the disease. Scintigraphic criteria for hepatic metastases were studied by examination of 333 liver scintigrams performed on 275 patients with primary cancers of the colon or breast. Focal defects in radiocolloid distribution correctly signaled the presence of metastatic colon carcinoma in 88% of the patients with that disease and incorrectly pointed to only 6% of the patients without such metastases. Seventy-five female patients with early primary cancer of the breast were studied by bone scanning with 99mTc labeled phosphate at the time of initial presentation and serially during follow-up examination. Eleven patients had an abnormal bone scan at the time of presentation. To provide insight into the significance of findings reported from screening asymptomatic women for breast cancer, we reviewed 19,928 mammographic studies with the accompanying physical examination and correlated these findings with 554 breast biopsies. The hallmark of the normal breast thermogram is relative symmetry of vascular configuration and thermal content with preservation of the breast contour. Accepted criteria of abnormality are predicated upon graphic and thermal asymmetry with emphasis placed upon elevated temperature, an increase in the number of discernible vessels, and distorted vascular patterns. The thermal and graphic criteria which should be used to evaluate breast thermograms are outlined. Thermograms are then categorized as normal, suspicious, or abnormal on the basis of the criteria outlined. It is hoped that these criteria can be used widely to standardize breast thermographic evaluation.

A consecutive series of 70 exudates from 45 patients with clinically suspected malignancy was examined by cytology, cytophotometric measurement of DNA, short-term cell culture, scanning and transmission electron microscopy. In a prospective evaluation, 1,000 consecutive breast thermograms were categorized as either normal or having 1 of 4 abnormal patterns: diffuse, asymmetrical, focal, or peri-areolar. In a series of 177 anovulatory patients, x-ray studies of sella turcica revealed 4 patients with pituitary adenomas and 1 with an empty sella. Galactorrhea was present in 15 patients (8.5%) of the total series, but 4 of the 5 patients with pituitary abnormalities also showed galactorrhea. The study of 635 breast cancers before and after radical radiation therapy, allows us to define 4 different ways of evolution: progressive linear cooling or sterilization, persistent thermal abnormalities or non-sterilization, relighting following a cooling phase or recurrence, persistent tepid breasts, meaning a radiation sequela (radiotherapic breast). Three patients with advanced breast cancer are described who developed lymphangitic pulmonary metastases with normal chest x-rays and abnormal perfusion lung scans. The abnormal lung scans can probably be attributed to embolization of the pulmonary vasculature, first described in association with lymphangitic pulmonary metastases in 1903. The importance of recognizing this condition in patients with breast cancer is shown by their frequently good responses to combination chemotherapy.

Radionuclide breast scintigraphy was evaluated as a noninvasive tumor-localizing modality. Technetium-99m-pertechnetate (99mTcO4) demonstrated good correlation between malignancy and postive scintigraphy (88% accuracy in 16 cases of breast carcinoma). Strontium 87m bone scanning was used in the assessment of 162 patients with breast cancer. Seventy-two patients had abnormal bone scans and in 63 (87 percent) these findings were subsequently confirmed. More metastases were detected by scanning and radiology than by radiology alone. There was a false positive rate of 7 percent. Of the 90 negative scans 23 showed evidence of metastasis either radiologically or at autopsy. This represented a false negative rate of 26 percent. The reasons for the false results are discussed particularly in relation to the problems of imaging the dorsal spine.

A computer-based technique was developed for the automated diagnosis of breast thermogram. Eighty-five thermograms were digitized and analyzed, and 23 parameters computed for each thermogram. A statistical decision program, based on a linear discriminant analysis technique, classified the thermograms into normal and abnormal categories. The accuracy of classification was evaluated by several techniques and found to be comparable to that of trained thermographers.

Occlusion of the left subclavian artery was demonstrated following exposure to two courses of radiation, the patient having received 4,200 rads in 4 weeks 9 years previously, and 4,000 rads in 8 weeks almost 5 years previously. Despite other intercurrent medical conditions, this seems to be a cause-and-effect relationship, since the only segment of abnormal artery was confined to the limited volume receiving the double exposure, whereas other arteries nearby did not demonstrate abnormality.
