A Large Superior Gluteal Artery Pseudoaneurysm Masquerading as a Pelvic Mass: When Not to Biopsy.

A superior gluteal artery pseudoaneurysm is a rare vascular lesion that can mimic soft tissue masses, creating diagnostic uncertainty and posing significant risk during biopsy or intervention. This case describes a patient with a superior gluteal artery pseudoaneurysm and associated arteriovenous fistula (AVF) initially mistaken for a piriformis mass. We discuss the clinical presentation, emphasize the imaging findings that facilitated accurate diagnosis, and outline the endovascular techniques used to achieve occlusion of the pseudoaneurysm and associated AVF.

Extensive Aplasia Cutis Congenita with Calvarial Defect and Superior Sagittal Sinus Exposure in a Term Neonate: A Case Report of Successful Healing with Conservative Management.

BACKGROUND: Aplasia cutis congenita (ACC) is a rare congenital disorder characterized by localized absence of skin, most commonly affecting the scalp. Extensive lesions involving the calvarium and dura, particularly when the superior sagittal sinus is exposed, are uncommon and may be complicated by hemorrhage, infection, thrombosis, neurological injury, and mortality. Management may be conservative or surgical and must be individualized according to defect size and depth, the structures exposed, the clinical condition of the neonate, and the resources and expertise available. Dual-modality ultrasound radiomics of the medial gastrocnemius muscle for detecting diabetic peripheral neuropathy: a multicenter prospective study.

BACKGROUND: Diabetic peripheral neuropathy (DPN) affects 20%-50% of patients with type 2 diabetes mellitus (T2DM), yet early screening is limited by restricted access to nerve conduction studies (NCS). We aimed to develop and validate a dual-modality ultrasound radiomics model combining B-mode ultrasound (BMUS) and shear wave elastography (SWE) of the medial gastrocnemius (MG) - an accessible surrogate reflecting neurogenic muscle atrophy - for noninvasive DPN detection. METHODS: A total of 190 T2DM patients (380 limbs) enrolled between June 2025 and February 2026 were allocated to training (

Linear arrangement of bony outgrowths of the distal phalanges of the foot in relation to ligamentous structures: a radiological analysis.

BACKGROUND: Long-term observation of patients with complaints involving the distal phalanx of the hallux (disturbances of nail plate growth, periungual fold changes, and abnormal alignment of the distal phalanx relative to the proximal phalanx) has drawn attention to recurrent bony changes on its lateral aspects. These observations provided part of the rationale for earlier anatomical studies, which confirmed the presence of the collateral ligaments of the distal phalanges of the foot macroscopically and histologically. Such bony changes have previously been classified as exostoses, osteophyte

The impact of nephrectomy on liver and contralateral kidney volume in patients with ADPKD; a case series.

PURPOSE: A substantial number of patients with autosomal dominant polycystic kidney disease (ADPKD) undergo nephrectomy. A decrease in intra-abdominal pressure after nephrectomy might hypothetically accelerate the growth of other polycystic organs and accelerate the rate of eGFR decline of the remaining kidney. In this case-series, we investigated the change in kidney and liver volume in patients before and after nephrectomy, and in a control group without nephrectomy. METHODS: In this retrospective case series kidney and liver volumes were measured using an automated stereological approach (

PET-based assessment of coronary microvascular dysfunction and its relation to plaque burden and vascular remodeling: integration with microvascular resistance reserve.

BACKGROUND: Coronary microvascular dysfunction (CMD) contributes to myocardial ischemia in patients with angina and non-obstructive coronary artery disease. Quantitative positron emission tomography (PET) allows noninvasive assessment of myocardial blood flow and identification of CMD. OBJECTIVES: To investigate the association between PET-assessed coronary microvascular dysfunction (CMD), coronary plaque burden, and vascular remodeling, integrating microvascular resistance reserve (MRR). METHODS: This post hoc analysis of the PACIFIC-1 trial included coronary arteries from symptomatic patie

Vertical Maxillary Movement Is Associated With Incisal Display in Orthognathic Surgery.

BACKGROUND: Maxillary incisal display (MID) is a key feature in orthodontic and orthognathic surgical planning that critically impacts esthetics and forms the basis for the vertical positioning of the maxilla. In orthognathic surgical planning, several approaches are taken to position the maxilla to determine the MID, but little evidence exists in the relationship between maxillary movement and MID. PURPOSE: The purpose of this study was to measure the association between MID and maxillary movements in maxillary orthognathic surgery. STUDY DESIGN, SETTING, AND SAMPLE: This was a retrospectiv

Navigation-Assisted Pure Endoscopic Transoral Styloidectomy for Recurrent Eagle Syndrome: A Pictorial Essay.

BACKGROUND: Complete resection of the styloid process at its skull-base origin is challenging in recurrent Eagle syndrome because of postoperative fibrosis, distorted anatomy, and proximity to the internal carotid artery (ICA). METHODS: Bilateral dissections of two silicone-injected cadaveric heads defined a pure endoscopic transoral trans-medial pterygomandibular corridor to the styloid base. The technique was applied in a patient with recurrent Eagle syndrome after previous transoral styloid shortening. RESULTS: Cadaveric dissections demonstrated a reproducible route to the styloid base th

Myocardial edema, blood flow, and contractility in Takotsubo syndrome: Comparative evaluation with cardiac magnetic resonance and positron emission tomography.

BACKGROUND: Takotsubo syndrome (TTS) features transient left ventricular (LV) dysfunction, often with myocardial edema and impaired coronary microcirculation. The relationships between myocardial edema, blood flow, and LV contractility remain unclear. We retrospectively examined these associations using cardiac magnetic resonance (CMR) and positron emission tomography (PET). METHODS: TTS patients were included if they underwent CMR and PET within 30 days of admission and ≤7 days apart. Correlations between CMR-derived T2 and PET-derived stress myocardial blood flow (MBF), myocardial flow rese

Clinical and MRI morphometric characterization of symptomatic cerebellar tonsillar ectopia.

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations. METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

The lone lifeline-a rare case report of a single right coronary circulation with hypoplastic left coronary system.

BACKGROUND: Single coronary artery is an exceptionally rare congenital anomaly, occurring in ∼0.02% of patients undergoing coronary angiography. Hypoplastic coronary artery disease represents another uncommon congenital abnormality characterized by underdevelopment of epicardial coronary arteries. The coexistence of these anomalies resulting in a functionally single coronary circulation is extremely rare and has important diagnostic and clinical implications. CASE SUMMARY: A woman in her late 60s presented with atypical chest pain. Electrocardiography and transthoracic echocardiography demons

Anterior Interosseus Nerve Syndrome Masqueraded by a Functional Gantzer Muscle Variant: A Report of 3 Cases.

CASE: We present 3 patients with anterior interosseous nerve syndrome (AINS) who demonstrated thumb interphalangeal flexion despite complete denervation of the flexor pollicis longus. Advanced imaging revealed a Gantzer muscle in all cases that was confirmed at surgery in the 2 patients who required microneurolysis. CONCLUSION: In patients with suspected AINS, retention of thumb IP flexion should prompt suspicion for a Gantzer muscle. Preservation or return of thumb flexion does not rule out AINS nor indicate recovery.

Diagnostic yield and outcomes of hand surgery referrals to rheumatology.

BACKGROUND: Hand and upper extremity complaints frequently prompt evaluation for systemic inflammatory conditions, leading surgeons to refer patients to rheumatology. However, referral patterns are highly variable and not standardized, and access to rheumatologic care is often limited by prolonged wait times and workforce constraints. Existing literature on rheumatology referrals has largely focused on access and disease-specific pathways, with little attention to referral patterns within surgical subspecialties. Concordance between referral indication and confirmed rheumatologic diagnosis is

Emerging biomedical imaging applications of tumor-derived extracellular vesicles for premetastatic niche detection: Biological rationale, engineering strategies, and translational challenges.

BACKGROUND: Accurate fetal imaging is essential to the practice of maternal-fetal medicine. While ultrasonography has been the traditional mainstay of fetal imaging, its ability to resolve critical features of central nervous system (CNS) anatomy remains limited. As interest in intrauterine therapy for myelomeningocele has increased, so has the need for more accurate, noninvasive imaging of the CNS. Fetal magnetic resonance imaging (MRI) promises to fill the gap left by ultrasound. METHODS: Thirty-seven MRI scans of fetuses previously diagnosed with myelomeningocele were reviewed by 2 neurora

Chorea after cardiac surgery.

Chorea after cardiac surgery (formerly post-pump chorea) is a common complication in children (estimated prevalence of 0.6%-3%) but is rare in adults; when it does occur, it is often permanent and less responsive to treatment. A 47-year-old woman developed chorea in all limbs following cardiac surgery for a pseudoaneurysm of the anastomosis along with thrombectomy of the prosthetic aortic valve (after Bentall's procedure in the past). MR scan of brain showed bilateral basal ganglia abnormalities, which partially resolved over 3 months. Despite initial treatments, her condition worsened until s
