BACKGROUND: Pediatric polydactyly is a relatively common congenital condition of hand and foot, affecting physical and mental development of children. Hand polydactyly has been brought into focus recently. By contrast, the studies on foot polydactyly are limited. Among the types, there is little international research on preaxial foot polydactyly since it is a relatively rare type, and the domestic research on it is fewer. The purpose of our study is to propose a new classification for guiding operation.
METHODS: We collected all patients with preaxial foot polydactyly who had preoperative ra

BACKGROUND: POEMS syndrome (polyneuropathy, organomegaly, endocrinopathy, M-protein, skin changes) is a paraneoplastic syndrome caused by a plasma cell proliferative disorder. Although POEMS syndrome can cause polyserous effusions, the report of pleural effusion as the main manifestation is not common. Here, we describe a case of POEMS syndrome misdiagnosed as tuberculous pleurisy due to massive pleural effusion.
CASE PRESENTATION: A 70-year-old man was admitted with chest tightness and was diagnosed with tuberculous pleurisy. Despite receiving anti-tuberculosis treatment, patient exhibited i

BACKGROUND: Coronary microvascular dysfunction (CMD) can exacerbate myocardial fibrosis, which may further impair microvascular function. However, the relationship between CMD and the extracellular volume fraction (ECV) in patients with ischemia with no obstructive coronary artery disease (INOCA) who are late gadolinium enhancement (LGE)-negative remains undetermined. We used T1 mapping and ECV to assess diffuse myocardial tissue abnormalities and extracellular compartment expansion in LGE-negative patients with CMD. We then investigated how these markers correlate with global myocardial flow

Pyogenic sacroiliitis (PSI) is an uncommon form of septic arthritis and is frequently diagnosed late in older adults because initial symptoms are nonspecific and imaging findings may be misattributed to other sources of infection. We report two older patients with PSI complicated by massive retroperitoneal and gluteal abscesses, highlighting the importance of early recognition and, when indicated, timely surgical source control. Case 1 was an 83-year-old woman in whom a conspicuous retroperitoneal abscess and abnormal urinalysis led to suspicion of an intra-abdominal or urinary tract source, d

A superior gluteal artery pseudoaneurysm is a rare vascular lesion that can mimic soft tissue masses, creating diagnostic uncertainty and posing significant risk during biopsy or intervention. This case describes a patient with a superior gluteal artery pseudoaneurysm and associated arteriovenous fistula (AVF) initially mistaken for a piriformis mass. We discuss the clinical presentation, emphasize the imaging findings that facilitated accurate diagnosis, and outline the endovascular techniques used to achieve occlusion of the pseudoaneurysm and associated AVF.

BACKGROUND: Aplasia cutis congenita (ACC) is a rare congenital disorder characterized by localized absence of skin, most commonly affecting the scalp. Extensive lesions involving the calvarium and dura, particularly when the superior sagittal sinus is exposed, are uncommon and may be complicated by hemorrhage, infection, thrombosis, neurological injury, and mortality. Management may be conservative or surgical and must be individualized according to defect size and depth, the structures exposed, the clinical condition of the neonate, and the resources and expertise available.

BACKGROUND: Diabetic peripheral neuropathy (DPN) affects 20%-50% of patients with type 2 diabetes mellitus (T2DM), yet early screening is limited by restricted access to nerve conduction studies (NCS). We aimed to develop and validate a dual-modality ultrasound radiomics model combining B-mode ultrasound (BMUS) and shear wave elastography (SWE) of the medial gastrocnemius (MG) - an accessible surrogate reflecting neurogenic muscle atrophy - for noninvasive DPN detection.
METHODS: A total of 190 T2DM patients (380 limbs) enrolled between June 2025 and February 2026 were allocated to training (

BACKGROUND: Long-term observation of patients with complaints involving the distal phalanx of the hallux (disturbances of nail plate growth, periungual fold changes, and abnormal alignment of the distal phalanx relative to the proximal phalanx) has drawn attention to recurrent bony changes on its lateral aspects. These observations provided part of the rationale for earlier anatomical studies, which confirmed the presence of the collateral ligaments of the distal phalanges of the foot macroscopically and histologically. Such bony changes have previously been classified as exostoses, osteophyte

PURPOSE: A substantial number of patients with autosomal dominant polycystic kidney disease (ADPKD) undergo nephrectomy. A decrease in intra-abdominal pressure after nephrectomy might hypothetically accelerate the growth of other polycystic organs and accelerate the rate of eGFR decline of the remaining kidney. In this case-series, we investigated the change in kidney and liver volume in patients before and after nephrectomy, and in a control group without nephrectomy.
METHODS: In this retrospective case series kidney and liver volumes were measured using an automated stereological approach (

BACKGROUND: Coronary microvascular dysfunction (CMD) contributes to myocardial ischemia in patients with angina and non-obstructive coronary artery disease. Quantitative positron emission tomography (PET) allows noninvasive assessment of myocardial blood flow and identification of CMD.
OBJECTIVES: To investigate the association between PET-assessed coronary microvascular dysfunction (CMD), coronary plaque burden, and vascular remodeling, integrating microvascular resistance reserve (MRR).

BACKGROUND: Maxillary incisal display (MID) is a key feature in orthodontic and orthognathic surgical planning that critically impacts esthetics and forms the basis for the vertical positioning of the maxilla. In orthognathic surgical planning, several approaches are taken to position the maxilla to determine the MID, but little evidence exists in the relationship between maxillary movement and MID.
PURPOSE: The purpose of this study was to measure the association between MID and maxillary movements in maxillary orthognathic surgery.

BACKGROUND: Complete resection of the styloid process at its skull-base origin is challenging in recurrent Eagle syndrome because of postoperative fibrosis, distorted anatomy, and proximity to the internal carotid artery (ICA).
METHODS: Bilateral dissections of two silicone-injected cadaveric heads defined a pure endoscopic transoral trans-medial pterygomandibular corridor to the styloid base. The technique was applied in a patient with recurrent Eagle syndrome after previous transoral styloid shortening.
RESULTS: Cadaveric dissections demonstrated a reproducible route to the styloid base th

BACKGROUND: Takotsubo syndrome (TTS) features transient left ventricular (LV) dysfunction, often with myocardial edema and impaired coronary microcirculation. The relationships between myocardial edema, blood flow, and LV contractility remain unclear. We retrospectively examined these associations using cardiac magnetic resonance (CMR) and positron emission tomography (PET).
METHODS: TTS patients were included if they underwent CMR and PET within 30 days of admission and ≤7 days apart. Correlations between CMR-derived T2 and PET-derived stress myocardial blood flow (MBF), myocardial flow rese

OBJECTIVE: To characterize the weight-bearing three-dimensional (3D) lower-limb phenotype associated with knee osteoarthritis (KOA) and assess whether noncoronal measurements provide information beyond conventional coronal alignment.
DESIGN: This retrospective cohort study used weight-bearing EOS images from a single-center database. Participants were classified by clinical diagnosis and radiographic assessment. Exact sex matching plus 1:1 nearest-neighbor propensity score matching for age and BMI (logit-scale caliper=0.10) generated 279 KOA-control pairs (n=558). Seventeen pelvic and lower-l

BACKGROUND: Cerebellar tonsils normally extend up to 5 mm below the foramen magnum (FM). Some individuals with tonsillar ectopia (TE) measuring under 5 mm may present with symptoms of Chiari type 1 malformation (CM1). We analyzed MRI data of the posterior cranial fossa (PCF) in affected patients to assess TE's impact on clinical manifestations.
METHODS: The retrospective study examined 83 adults with 2-4 mm symptomatic cerebellar TE without syringomyelia (SM), 16 with syndromic CM1 without SM, and 19 controls. Participants with TE were divided into three groups: syndromic (TEs, n = 15), incom

BACKGROUND: Single coronary artery is an exceptionally rare congenital anomaly, occurring in ∼0.02% of patients undergoing coronary angiography. Hypoplastic coronary artery disease represents another uncommon congenital abnormality characterized by underdevelopment of epicardial coronary arteries. The coexistence of these anomalies resulting in a functionally single coronary circulation is extremely rare and has important diagnostic and clinical implications.
CASE SUMMARY: A woman in her late 60s presented with atypical chest pain. Electrocardiography and transthoracic echocardiography demons

CASE: We present 3 patients with anterior interosseous nerve syndrome (AINS) who demonstrated thumb interphalangeal flexion despite complete denervation of the flexor pollicis longus. Advanced imaging revealed a Gantzer muscle in all cases that was confirmed at surgery in the 2 patients who required microneurolysis.
CONCLUSION: In patients with suspected AINS, retention of thumb IP flexion should prompt suspicion for a Gantzer muscle. Preservation or return of thumb flexion does not rule out AINS nor indicate recovery.

BACKGROUND: Hand and upper extremity complaints frequently prompt evaluation for systemic inflammatory conditions, leading surgeons to refer patients to rheumatology. However, referral patterns are highly variable and not standardized, and access to rheumatologic care is often limited by prolonged wait times and workforce constraints. Existing literature on rheumatology referrals has largely focused on access and disease-specific pathways, with little attention to referral patterns within surgical subspecialties. Concordance between referral indication and confirmed rheumatologic diagnosis is

Caudal regression syndrome (CRS) refers to a spectrum of congenital structural defects of the caudal region. We present a case of caudal regression syndrome with the objective of correlating the symptoms of caudal regression syndrome with the detailed imaging findings. A 05-year-old boy who was admitted to Mymensingh Medical College Hospital in August 2025 , presented with a post-urethroplasty state of hypospadias, complaining of urinary incontinence through the normal meatus and a urethrocutaneous fistula. He had dribbling of urine through hypospadias and deformed feet since birth. He also de
