Emerging biomedical imaging applications of tumor-derived extracellular vesicles for premetastatic niche detection: Biological rationale, engineering strategies, and translational challenges.

BACKGROUND: Accurate fetal imaging is essential to the practice of maternal-fetal medicine. While ultrasonography has been the traditional mainstay of fetal imaging, its ability to resolve critical features of central nervous system (CNS) anatomy remains limited. As interest in intrauterine therapy for myelomeningocele has increased, so has the need for more accurate, noninvasive imaging of the CNS. Fetal magnetic resonance imaging (MRI) promises to fill the gap left by ultrasound. METHODS: Thirty-seven MRI scans of fetuses previously diagnosed with myelomeningocele were reviewed by 2 neurora

Chorea after cardiac surgery.

Chorea after cardiac surgery (formerly post-pump chorea) is a common complication in children (estimated prevalence of 0.6%-3%) but is rare in adults; when it does occur, it is often permanent and less responsive to treatment. A 47-year-old woman developed chorea in all limbs following cardiac surgery for a pseudoaneurysm of the anastomosis along with thrombectomy of the prosthetic aortic valve (after Bentall's procedure in the past). MR scan of brain showed bilateral basal ganglia abnormalities, which partially resolved over 3 months. Despite initial treatments, her condition worsened until s

Physiology-Guided Heart Rate Modulation in Cardiogenic Shock With Myocardial Bridging: Reversing the Inotrope Paradox.

Myocardial bridging is a dynamic coronary anomaly in which systolic compression may extend into early diastole, particularly under conditions of increased heart rate and contractility. In patients with cardiogenic shock, this creates a potential physiological conflict, as catecholaminergic support may worsen coronary flow impairment rather than improve haemodynamics. A 66-year-old man presented with inferolateral ST-segment elevation myocardial infarction due to thrombotic occlusion of the proximal left circumflex artery, successfully treated with primary percutaneous coronary intervention. Larger lung volumes in fetuses with transposition of the great arteries and an intact ventricular septum compared to heart healthy fetuses and fetuses with transposition of the great arteries and a ventricular septal defect.

BACKGROUND: Complex CHDs may impair organ development. One proposed mechanism is an altered relationship between blood flow, oxygen delivery, and subsequent organ growth. In this study, we examined whether fetal lung, intracranial, liver, and kidney volumes differ among fetuses with transposition of the great arteries with an intact ventricular septum, transposition of the great arteries with a ventricular septal defect, and healthy controls. METHODS: Eleven fetuses with transposition of the great arteries (6 with a ventricular septal defect and 5 with an intact ventricular septum) and 22 hea

Supra Occipital Flattening: A Key Sign for Antenatal Diagnosis of Isolated Sagittal Craniosynostosis.

Craniosynostosis is an isolated finding in most cases, involving either the metopic or the sagittal suture. It is detected only sporadically during prenatal ultrasound, in the third trimester for non-isolated cases. When synostosis of the sagittal suture occurs, head shape growth proceeds in an anteroposterior direction, leading to scaphocephaly (long, narrow, boat-shaped skull). Moreover, the reduced vertical growth of the skull vault causes a supra-occipital flattening, detectable at the profile view. Fetuses' head shape diagnosed as dolicocephalic should be carefully studied for the presenc

Characterization and treatment of symptomatic bipartite patella in pediatric, adolescent, and young adult patients: a large case series.

INTRODUCTION: Bipartite patella (BPP) results from ossification center fusion failure during development. Treatment of symptomatic BPP varies and may include nonoperative management or surgical interventions. However, characteristics distinguishing symptomatic from asymptomatic and operative from nonoperative BPP remain unclear. This study presents a large case series of pediatric, adolescent, and young adult patients with symptomatic BPP and assesses the association between radiographic characteristics and symptomatology. METHODS: We retrospectively reviewed patients ≤21 years diagnosed with

Assessment of the Diagnostic Value of the Sit Test as a Screening Test for Cranial Cruciate Ligament Rupture in Dogs.

OBJECTIVE: Diagnosis of cranial cruciate ligament rupture (CCLR) is primarily through physical examination, imaging, and intra-articular assessment. Assessment of sit position where affected dogs abduct and extend the stifle to reduce discomfort has been proposed to aid diagnosis but not evaluated as a diagnostic test. This study aimed to evaluate the relationship between sit position and CCLR and assess the sit test as a diagnostic tool. STUDY DESIGN: Dogs with CCLR or hip dysplasia (HD) diagnosis were prospectively recruited and allocated to affected (CCLR) and unaffected (HD and normal sti

Indocyanine green lymphography imaging of normal lymphatic drainage in the lower limbs.

OBJECTIVES: Indocyanine green lymphography (ICGL) has emerged as a potentially powerful tool for the study of the superficial lymphatic system and to support the diagnosis of lymphoedema. However, detailed descriptions of ICGL findings in healthy individuals are limited. In this study, we imaged a series of healthy participants using ICGL, attempting to establish quantitative and qualitative ICGL parameters of the lower limb. METHODS: Sixteen healthy individuals aged 20-55 years were recruited to undergo lower limb ICGL after 0.1 mL injections of 1 g/L ICG were administered intradermally to 5

Explainable deep learning-based multiclass classification of foot radiographs into normal, plantar fasciitis, and flatfoot.

BACKGROUND: The medial longitudinal arch plays a critical role in foot biomechanics, and its abnormalities are associated with conditions such as flatfoot and plantar fasciitis. Early and accurate diagnosis of these disorders is clinically important, but plain radiographs have limitations in visualizing soft tissue pathology. PURPOSE: This study aimed to develop and interpret a deep learning model capable of classifying foot radiographs into normal, plantar fasciitis, and flatfoot categories. METHODS: A DenseNet-121 architecture was trained on 9500 synthetic lateral foot X-ray images from th

Peronea Arteria Magna: a rare but surgically important variant for prevention of post-surgical limb loss.

PURPOSE: A large number of anatomic variants occur in the lower limb arteries and the Peronea Arteria Magna (PAM) is a rare congenital variant where the fibular/peroneal artery becomes the dominant arterial supply to the leg foot, replacing hypoplastic anterior and posterior tibial arteries. It is imperative that the operative surgeon be aware of this anomaly prior to obtaining the fibula osteocutaneous free tissue flap (OFF) so that post-surgical limb loss can be prevented. METHODS: We present a case of a young male who was posted for reconstructive surgery with OFF, after undergoing right he

Positive association of lipoprotein(a) and the prevalence of lower extremity arterial disease in MASLD: a cross-sectional study.

BACKGROUND: Lipoprotein(a) [Lp(a)] has been recognized as a genetically determined and independent contributor to atherosclerotic cardiovascular disease. However, its role in lower extremity arterial disease (LEAD) among individuals with metabolic dysfunction-associated steatotic liver disease (MASLD) remains insufficiently studied. Given the overlapping metabolic disturbances in both conditions, such as insulin resistance and lipid abnormalities, a potential relationship between Lp(a) and peripheral vascular injury in MASLD is biologically plausible. This study aimed to investigate the cross-

Proximal origin variants of the plantaris muscle: a classification-based review with surgical and radiologic implications.

BACKGROUND: The plantaris muscle (PM) is a highly variable structure whose proximal origin demonstrates significant morphological diversity. While traditionally considered clinically insignificant, recent evidence highlights its role in sports injuries, Achilles tendinopathy, and even vascular entrapment syndromes. A structured classification-based approach is therefore essential to improve diagnostic accuracy and surgical safety. METHODS: A classification-focused narrative review of anatomical, radiological and clinical literature was performed using PubMed and Web of Science, supplemented by
