Does the inferior facial angle predict respiratory status in Robin sequence infants?

UNLABELLED: Robin sequence (RS) is a rare congenital condition characterized by retrognathia, glossoptosis, and upper airway obstruction, with highly variable respiratory severity at birth. Prenatal measurement of the inferior facial angle (IFA) is used to support diagnosis, but its prognostic value for respiratory outcomes remains uncertain. This study assessed whether fetal MRI-measured IFA predicts early respiratory status in infants with RS. We conducted a retrospective monocentric study including consecutive infants with prenatal suspicion and postnatal confirmation of RS who underwent fe

Simultaneous Orthognathic Surgery and Condylectomy for Active Unilateral Condylar Hyperplasia: Indications and Outcomes of a Single-Stage Protocol.

BACKGROUND: Active unilateral condylar hyperplasia (AUCH) leads to progressive facial asymmetry and occlusal imbalance and may coexist with preexisting skeletal discrepancies. In such cases, a single-stage approach combining condylectomy and orthognathic surgery can address both pathological growth and the associated dentofacial deformity. PURPOSE: The aim of this study was to present a treatment protocol and surgical algorithm for simultaneous orthognathic surgery and condylectomy in subjects with AUCH and to evaluate clinical outcomes in a retrospective case series. STUDY DESIGN, SETTING,

Morphological analysis of posterior fossa in Apert and Crouzon syndromes before and after posterior cranial vault expansion.

BACKGROUND: Syndromic craniosynostosis, such as Apert and Crouzon syndromes, involve premature fusion of cranial sutures, leading to cranial deformities and potential neurological complications. Posterior cranial vault expansion (PCVE) is commonly used to address intracranial volume (ICV) anomalies in these cases, especially in the posterior fossa region. However, the morphological variation of this region between normal, Apert and Crouzon individuals remains understudied. This study aimed to carry out a detailed morphological analysis of the posterior fossa between the aforementioned groups b

Tricortical versus paramedian bicortical palatal anchorage in maxillary atresia: A CBCT study.

BACKGROUND: Maxillary transverse deficiency limits the choice of safe and stable palatal miniscrew sites. Cone-beam CT (CBCT) mapping of the posterior palatal supra-alveolar insertion site (PPSAIS) may support bicortical/tricortical anchorage planning. OBJECTIVE: To quantify bone height available for tricortical and posterior bicortical insertion in patients with maxillary atresia using CBCT. METHODS: In this observational cross-sectional study, 90 full-head CBCT scans (ages 14-40 years; equal sexes) with maxillary atresia were analyzed. Palatal bone height was measured at paramedian bicorti

Transcranial acoustoelectric imaging (tABI) of seizure activity in human head model with neuronavigation.

Objective. The human brain consists of multiple interacting neuronal networks that interleave at fine spatial and temporal scales. This complexity presents a challenge for scalp EEG and other noninvasive mapping techniques to accurately identify seizures and abnormal current patterns from background activity. To address this unmet need, this study investigates transcranial acoustoelectric brain imaging (tABI) with neuronavigation as a new method for mapping EEG-derived currents through the skull.Approach.In tABI, ultrasound (US) is focused and steered in the brain as surface electrodes record a

Coronal Clival Cleft: Estimated Prevalence and Clinical Associations in a Pediatric Cohort.

BACKGROUND AND PURPOSE: Coronal clival cleft is a congenital corticated defect traversing the basioccipital portion of the clivus, beneath the spheno-occipital synchondrosis. It has been reported in cases of CHARGE syndrome, Cornelia de Lange syndrome, anencephaly, hemifacial microsomia, Chiari deformities, and in asymptomatic patients, but it may be underdiagnosed and underestimated on imaging. This study aims to estimate the prevalence of coronal clival cleft and expand its genetic and clinical associations. MATERIALS AND METHODS: In this retrospective study, the imaging report database fro

The impact of previous head trauma and history of cranial surgery on clinical findings in acute pediatric head injury.

BACKGROUND: This study aimed to identify independent predictors of intracranial pathology in children presenting with acute blunt head trauma, with a particular focus on patients who had a history of previous head trauma or cranial surgery and did not meet the PECARN (Pediatric Emergency Care Applied Research Network) criteria for cranial computed tomography (CCT). The study further sought to evaluate the impact of these historical factors on acute clinical findings and to inform more objective decision-making for CCT use in this specific subgroup. METHODS: In this prospective cohort study, i

Association between cranial morphology and dysgnathias in adolescents and adults: A prospective case-control study.

BACKGROUND: Cranial deformities in infancy, such as positional plagiocephaly and brachycephaly, have been suggested to influence the development of jaw malpositions. However, the long-term relationship between cranial shape and dysgnathia remains unclear. OBJECTIVE: This prospective case-control study investigates whether differences in cranial morphology, measured by the Cranial Index (CI), Cranial Vault Asymmetry Index (CVAI), Diagonal Difference (DD), and ear axis offset (EO), are associated with malocclusions classified according to the Angle system in adolescents and adults. MATERIALS A

Pediatric growing orbital roof fracture with ptosis: a case report.

The majority of pediatric skull fractures are minimally displaced and can usually be managed conservatively. A rare but serious complication is the development of a growing skull fracture (GSF), that may lead to significant sequelae including orbital meningoencephalocele or neurological impairments. We report a 6-year-old boy who initially presented with right-sided traumatic ptosis and a minimally displaced right orbital roof "blow-up" fracture following minor head trauma. This injury subsequently progressed into a growing orbital roof fracture, complicated by the development of an orbital me

En-bloc posterior vault distraction including the foramen magnum for Chiari I malformation in severe syndromic craniosynostosis: First cases and literature review.

This case series describes two patients with severe Crouzon syndrome and persistent Chiari I malformation. Initial treatment with standard posterior vault distraction osteogenesis and frontoorbital advancement failed to resolve the patient's symptoms. One patient developed severe syringomyelia and myelopathy refractory even to standard suboccipital decompression. Given the persistent posterior fossa constriction, an en-bloc posterior vault distraction incorporating the foramen magnum was performed over 21 days, achieving an expansion of 20 mm. This approach resulted in complete symptom resolut

Severe Plagiocephaly Due to Isolated Left Frontosphenoidal Craniosynostosis: An Exceptional Presentation.

Craniosynostosis is a condition characterized by premature fusion of cranial sutures, leading to abnormal head shapes and potential developmental and neurological complications. Although craniosynostosis typically involves major cranial sutures, isolated frontosphenoidal suture fusion represents an exceptionally rare entity. In this case report, we present a 2-year-old girl with a rare occurrence of early closure of the left frontosphenoidal suture, resulting in significant left-sided plagiocephaly. After frontal-orbital advancement surgery with barrel-stave osteotomies, the patient experience

Pre- and postoperative cranial shape analysis in Craniosynostosis: The role of mechanical loading during CT imaging.

BACKGROUND: Craniosynostosis is a congenital disorder characterized by premature fusion of one or more cranial sutures, resulting in abnormal skull growth and, in some cases, elevated intracranial pressure. Accurate assessment of cranial morphology is essential for diagnosis, surgical planning, and evaluation of postoperative outcomes. Three-dimensional computed tomography allows detailed visualization of cranial shape; however, measurements may be influenced by external forces during image acquisition, such as contact between the occipital region and the supporting surface. It remains unclear

Insights into the pathogenesis and differential diagnosis of clival lesions in an individual from a 16th-century-CE mass grave at Mohács (Southwestern Hungary).

The aim of our paper is to present and discuss in detail the pathological lesions suggestive of tuberculosis observed in a skeleton (MMG3-75) that was excavated from the 16th-century-CE mass grave No. 3 of the Mohács National Memorial Site (Sátorhely, southwestern Hungary). The differential diagnoses of the observed bony changes, with special attention to the clival alterations, are presented. During the macromorphological, radiological, and digital microscopic examination of MMG3-75, the skull base showed mild cortical erosion and multiple, well-circumscribed osteolytic lesions at the clivus

Decompression with or without Duraplasty for Chiari I and Syringomyelia.

BACKGROUND: In children with Chiari type I malformation and syringomyelia, neurosurgical posterior fossa decompression (PFD) provides clinical improvement, but whether duraplasty (incising the dura and placing a dural graft) improves outcomes is unclear. METHODS: We conducted a multicenter, cluster-randomized, controlled trial of PFD with duraplasty (PFD-D) as compared with PFD alone. Persons 21 years of age or younger with cerebellar tonsillar ectopia of at least 5 mm and a maximum syrinx diameter of 3.0 to 9.9 mm were enrolled at 38 centers. Centers were cluster-randomized: all the particip

Three-Dimensional Histopathologic Correlates of Cochlear Hypoplasia Type II Versus Incomplete Partition Type II.

HYPOTHESIS: Cochlear hypoplasia type-II (CH-II) and incomplete partition type-II (IP-II) may display measurable differences in modiolar: cochlear area ratios and interscalar ridge angles correlating with spiral ganglion neuron (SGN) counts. BACKGROUND: Inner ear malformations (IEM) can cause variable sensorineural hearing loss and cochlear implant outcomes. CH-II and IP-II share overlapping imaging features. Subtle differences in bony cochlear architecture may underlie distinct SGN patterns. METHODS: IP-II (n=11), CH-II (n=5), and normal controls (n=16) were identified from a digitised human

A One-Finger-Width "Baklava Dough" Dural Thinning Technique in Dura-Preserving Posterior Fossa Decompression for Chiari Type I Malformation.

BACKGROUND: The optimal surgical extent of posterior fossa decompression in Chiari type I malformation remains controversial. This study aimed to evaluate the clinical and radiological outcomes of a modified dura-splitting technique based on controlled thinning of the meningeal layer. METHODS: This modification, referred to as the "One-Finger-Width 'Baklava Dough' Dural Thinning Technique," involves performing a minimal skin incision of one-finger width (about 2 cm), followed by splitting the dura into thin layers and microscopically thinning the meningeal layer to a thickness comparable to b

TCF12-related bicoronal craniosynostosis complicated by a large middle fossa arachnoid cyst and developmental regression: a case report.

Pathogenic variants in Transcription Factor 12 (TCF12) have been identified as genetic causes of craniosynostosis. The phenotypic spectrum of TCF12-related craniosynostosis is broad and remains incompletely understood. Herein, we report a case of a 2-month old male with a heterozygous pathogenic variant of TCF12 (c.1267C > T; pArg423*) and bicoronal craniosynostosis. He underwent bicoronal suturectomy at 4 months of age, followed by post-operative molding helmet therapy. He returned at 2 years of age with developmental regression including loss of expressive language and fine motor skills. Sho
