Prenatal Ultrasound Features of Biliary Atresia: Diagnostic Significance of Abnormal Gallbladder Size and Hepatic Hilar Cyst.

OBJECTIVE: To explore the predictive potential of prenatal ultrasound features and their capacity to differentiate biliary atresia (BA) in fetuses exhibiting biliary abnormalities. METHODS: Data of pregnant women who underwent routine fetal ultrasound screening during the second trimester (18-27+6 weeks) were retrospectively analyzed in the 2018-2022 period. Fetuses with suspected biliary abnormalities in the second trimester were identified, and the cases were categorized into 5 groups: enlarged fetal gallbladder, small fetal gallbladder, non-visualized fetal gallbladder, hepatic hilar cyst,

Application of endoscopic retrograde cholangiopancreatography in children with common bile duct microdilation: a single-center retrospective cohort study of 36 cases.

OBJECTIVES: Limited data exists on endoscopic retrograde cholangiopancreatography (ERCP) for children with common bile duct microdilation (CBDM). We evaluate the benefits of ERCP applications in CBDM, with or without pancreaticobiliary maljunction (PBM). METHODS: Retrospective analysis of 36 children with CBDM undergoing therapeutic ERCP. Patients were stratified by the presence (PBM group, n = 15) or absence (non-PBM group, n = 21) of PBM. Data included demographics, clinical/imaging/endoscopic findings, and post-ERCP outcomes. RESULTS: A total of 36 children underwent therapeutic ERCP proced

From Diagnosis to Treatment of an Extreme Case of Sump Syndrome Presenting With Cholangitis.

This case report presents an extreme sump syndrome complicated by acute cholangitis. The patient, who underwent a Roux-en-Y hepaticojejunostomy (HJ) for a Type 1 choledochal cyst 10 years ago, had imaging findings of debris filling both lobes of the intrahepatic bile ducts (IHBD) and obstructing the anastomosis. Surgical or endoscopic intervention was not feasible, so percutaneous biliary drainage was performed. Two sequential procedures were performed. Initially, a 12F biliary drainage catheter was used, and saline irrigation was applied to facilitate debris resolution. However, after the rem

Outcomes of Endoscopic Ultrasound-Guided Gallbladder Drainage Using Lumen-Apposing Metal Stent Performed by Early Career Advanced Endoscopists: A Multicenter Study.

BACKGROUND AND AIMS: Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) has gained popularity for acute cholecystitis (AC) management in patients deemed unfit for surgery. Recent studies have found > 95% technical success and > 90% clinical success when performed by experienced advanced endoscopists. Recently, the U.S. Food and Drug Administration (FDA) approved the use of lumen-apposing metal stent (LAMS) for EUS-GBD. The aim of this study is to evaluate the safety and efficacy of EUS-GBD performed by early-career advanced endoscopists in North America. METHODS: This was a retrospec

Rapid development of diffuse hepatic calcification in a patient with congenital hepatic fibrosis and secondary hyperparathyroidism.

A man in his 20 s with congenital hepatic fibrosis and polycystic kidney disease (PKD) presented with symptoms of anemia. Upper gastrointestinal endoscopy revealed mild exudative hemorrhage due to gastric antral vascular ectasia, which was treated with argon plasma coagulation. Computed tomography (CT) at admission revealed diffuse hepatic calcification and ectopic calcifications in the left buttock and bilateral inguinal regions. CT performed five months earlier showed no hepatic calcification, indicating rapid progression. The patient had been receiving maintenance hemodialysis for four mont

Open cholecystectomy via liver evisceration with a history of neonatal malrotation and omphalocele.

With the growth of laparoscopy, open cholecystectomies are often reserved for exceedingly inflamed and difficult gallbladders. We present a case of a patient in her 70s with a history of malrotation and omphalocele requiring silo management with recurrent admissions for acute cholecystitis. She had failed a prior laparoscopic converted to open attempt at cholecystectomy. She presented with right upper quadrant abdominal pain and Computed Tomography (CT) showing numerous anomalies. She was taken to the operating room where her liver was mobilised and eviscerated out of the abdomen; gallbladder

Portohepatic fusion mimics biliary aplasia.

We report a rare case of biliary aplasia in an infant who presented with persistent jaundice and elevated direct bilirubin despite having cholic stools. Imaging confirmed the absence of extrahepatic bile ducts, consistent with biliary atresia. Intraoperatively, there was a complete absence of an extrahepatic biliary tree consistent with biliary hypoplasia. We identified a cryptogenic hepatic plate directly fused to the right portal vein on further dissection. Despite the atypical anatomy, a modified Kasai portoenterostomy was successfully performed. The patient showed progressive clinical impr

Two cases of gallbladder torsion presenting with right lower quadrant pain: Clinical and imaging characteristics.

RATIONALE: Gallbladder torsion (GT) is a rare cause of acute abdominal pain. Due to its rarity and nonspecific clinical presentation, GT is frequently misdiagnosed preoperatively. We report 2 cases of GT in patients initially suspected of acute appendicitis, both presenting with right lower abdominal pain. We conducted a retrospective analysis of the clinical and radiological findings in these 2 patients to enhance our understanding of and management for this condition. PATIENT CONCERNS: Case 1 was a thin 80-year-old woman who was admitted to the hospital with migration of pain to the right l

[A Case of Double Cancers of the Gallbladder and Duodenal Papilla].

A 65-year-old man was diagnosed with gallbladder cancer during a medical checkup that revealed liver dysfunction. He was referred to our hospital seeking bloodless treatment due to religious beliefs. Blood tests revealed elevated hepatobiliary enzyme levels, and abdominal echocardiography revealed a broad nodular protuberant lesion in the body of the gallbladder and marked dilatation of the common bile duct. PET revealed marked abnormal accumulation of FDG in the gallbladder mass and duodenal papilla. Based on the imaging findings, double cancers of the gallbladder and duodenal papilla were di

[Anatomical variations of vessels and bile ducts in hepatobiliary surgery : What must be considered in gallbladder and liver surgery?].

Anatomical variations of both the vascular structures of the liver and gallbladder as well as the bile ducts, are very common. Detailed knowledge of the different courses of these vessels and ducts as well as their topographical relationships to one another, is essential in hepatobiliary procedures and surgery involving the upper gastrointestinal tract. Whenever possible, these variations should be identified preoperatively. This is best achieved through contrast-enhanced computed tomography (CT) with both portal venous and arterial phases, and for bile ducts, additionally via magnetic resonan

An ultrasound approach to visualize the "duct at the hilum" in infants undergoing evaluation for biliary atresia.

OBJECTIVES: Ultrasound exams are commonly used to evaluate infants for biliary atresia. However, ultrasound findings may be inconclusive, and more invasive diagnostic testing is often required. We hypothesized that another ultrasound finding-absence of the extrahepatic bile duct at the liver hilum (the "duct at the hilum")-could be useful. METHODS: Ultrasound exams were reviewed from infants undergoing evaluation for biliary atresia at Texas Children's Hospital during two periods. First, exams performed before 2021 were reviewed to develop a systematic approach to visualize the duct at the hi

Pancreaticobiliary Maljunction With a Non-Dilated Extrahepatic Bile Duct Masked by Adenomyomatosis: A Potential Cause of Unresectable Gallbladder Carcinoma.

Adenomyomatosis of the gallbladder (ADM) is generally considered benign. The presented cases were initially diagnosed with ADM via ultrasound during an annual health check-up and monitored for several years without further investigations due to the absence of significant changes or symptoms. Subsequently, one case was diagnosed with pancreaticobiliary maljunction (PBM) with a non-dilated extrahepatic bile duct (EBD) after symptom onset, and underwent prophylactic laparoscopic cholecystectomy to avoid the risk of carcinogenesis. Although the other case remained asymptomatic, ultrasound later re

Two diagnoses, similar clinical presentation: unexpected hepatic sarcoidosis during laparoscopic cholecystectomy.

A man in his late 20s presented to the emergency department with 4 months of progressive colicky pain, suspected of cholecystolithiasis. Abdominal ultrasonography confirmed the presence of gallstones. Laboratory tests showed no abnormalities. Due to the severity of his symptoms, an emergency laparoscopic cholecystectomy was scheduled. Diffuse intra-capsular, solid, oval, white lesions were observed on the liver intraoperatively. Histological analysis showed granulomatous inflammation consistent with sarcoidosis. Chest CT scan showed mediastinal and hilar lymphadenopathy, also suggestive of sar

Ciliated foregut cysts involving the hepatopancreaticobiliary system: a clinicopathological evaluation with focus on atypical features.

AIMS: Foregut cystic malformations are rare developmental abnormalities, which may involve the hepatopancreaticobiliary tract (HPBT). These cysts are composed of inner ciliated epithelium; subepithelial connective tissue layer; smooth muscle layer; and an outer fibrous layer. While radiopathologic findings are often diagnostic, atypical location and histologic features can pose a diagnostic challenge. We aimed to study ciliated foregut cysts (CFCs) in the HPBT, assess their clinicopathological features with a focus on atypical features. METHODS: We collected cases of CFCs involving the HPBT f

Ultrasonographic evaluation of the gallbladder motor function in the diagnosis and prognosis of intrahepatic cholestasis of pregnancy.

BACKGROUND: Intrahepatic cholestasis of pregnancy (ICP) is characterized by skin pruritus, elevated liver enzymes, and increased serum total bile acids. Several previous studies have revealed that the fasting and ejection volumes of the gallbladder in cholestasis of pregnancy are greater than those in normal pregnancy. The goal of this study was to explore the gallbladder volume and evaluate the diagnostic and prognostic value of ultrasound in ICP. METHODS: We prospectively recruited a cohort of 60 ICP patients at the First Affiliated Hospital of Xi'an Jiaotong University, Shaanxi, China from

Characteristics and malignancy rates of adult patients diagnosed with choledochal cyst in the West: a systematic review.

BACKGROUND: The approach to patients with choledochal cysts (CCs) remains varied and subject to institutional practices. Owing to the rarity of the disease, the optimal treatment remains poorly defined, particularly in the adult population. This study aimed to review the literature on adult patients with CCs to evaluate trends of diagnosis and management in Western countries. METHODS: A literature search of 3 electronic databases was performed on adult patients diagnosed with CCs in Western institutions. A review of published literature was completed with comprehensive screening by 2 independ

Unusual Presentation of a Hepatic Neuroendocrine Tumor With Elevated CEA and CA 19-9: A Case Report.

Neuroendocrine tumors (NETs) of the gastrointestinal tract (GIT) are rare malignancies, which may have unique presentations. The diagnostic process predominantly relies on immunohistochemical analysis. While tumor markers are extensively utilized in diagnosing and monitoring GI malignancies, their specific role in NETs has not been fully explored. This case describes an 83-year-old male presenting with jaundice and general weakness. Diagnostic imaging through MRI and CT angiography (CTA) revealed a nodular texture on the liver's surface suggesting cirrhosis. The presence of elevated tumor mark
