Fixator-Assisted Plating and Nailing for Multiplanar Lower Limb Deformities in Adolescents: Indications, Technical Considerations, and Radiographic Outcomes.

BACKGROUND: Uncorrected lower limb deformities in adolescents, particularly those with severe multiplanar deviations, can lead to early joint degeneration and functional impairment. When growth modulation is no longer feasible, corrective osteotomy becomes necessary. This study aims to demonstrate the efficacy of fixator-assisted techniques-fixator-assisted locked plating (FALP) and fixator-assisted nailing (FAN)-in achieving accurate correction of complex femoral and tibial deformities across the frontal, sagittal, and torsional planes. METHODS: A prospective study was conducted involving 27

Dysmorphic child with cone-shaped epiphyses: a diagnostic approach and novel trichorhinophalangeal syndrome gene variant.

Trichorhinophalangeal syndrome (TRPS) is a very rare genetic disorder comprised of three recognised types, all characterised by short, deformed fingers with cone-shaped epiphyses (CSE) visible on radiographs. TRPS type I is the most common subtype. We report the case of a girl in her mid-teens who presented with the characteristic clinical and radiological features of TRPS type I. The diagnosis was confirmed by whole-exome sequencing, which identified a novel heterozygous variant in the TRPS1 gene. This case highlights a structured diagnostic approach to CSE and may further expand the mutation

Extreme trochlear morphology in pediatric patients with fixed lateral patellar dislocation: a radiographic study.

BACKGROUND: Fixed lateral patellar dislocations (FLPD) are a rare subtype of chronic patellofemoral instability (PFI) where the patella remains dislocated in flexion and extension. The aim of this study was to describe radiographic morphological features in pediatric patients with FLPD and determine how they differ from those with acute patellar dislocation (APD) and unaffected controls. METHODS: Pediatric patients who underwent medial patellofemoral ligament reconstruction for FLPD or APD were identified and age- and sex-matched to controls without PFI. On axial magnetic resonance imaging, b

Acupuncture Prescription Based on Thermal-Sensitized Acupoints for the Treatment of Knee Osteoarthritis: Protocol for a Randomized Controlled Pilot Trial.

BACKGROUND: Treatment options for knee osteoarthritis (KOA) remain limited. Although previous studies suggest that acupuncture can alleviate pain and functional impairment associated with KOA, there is significant variation in acupoint selection across studies and a lack of objective criteria to guide this choice. Infrared thermography (IRT) is recognized as a reliable method for identifying inflammatory regions in KOA. Our previous research revealed abnormal skin temperature at specific acupoints and surrounding areas in the lower limbs of patients with KOA, indicating potential treatment tar

Update on Adult Acquired Flat Foot Deformity Using MR Imaging.

Adult acquired flatfoot deformity (AAFD) is a progressive condition characterized by collapse of the medial longitudinal arch, pain, and functional impairment. It predominantly affects middle-aged women and is associated with obesity, diabetes, hypertension, and anatomic variants. MR imaging is the gold standard for evaluating osseous, tendinous, and ligamentous structures, enabling accurate diagnosis and staging. This review summarizes the role of MR imaging in AAFD, detailing normal anatomy, pathophysiological mechanisms, classification systems, and advanced imaging protocols. Key MR imaging

Corticospinal tract involvement in Spinocerebellar ataxias type 1, 2 and 3 characterized by motor evoked potentials and diffusion MRI.

INTRODUCTION: Spinocerebellar ataxias (SCAs) types 1, 2, and 3 are the most common autosomal dominant SCAs, characterized by neurodegeneration in the cerebellum, brainstem, and basal ganglia. However, corticospinal tract involvement remains underexplored using combined electrophysiological and neuroimaging approaches. This study aimed to investigate the electrophysiological and imaging characteristics of SCA subtypes. METHODS: A total of 23 SCA1, 21 SCA2, and 40 SCA3 patients, along with 27 healthy controls, were enrolled. All subjects underwent neurological examination and motor evoked poten

Effects of bone deformation on patient-specific finite element predictions of hip chondrolabral mechanics.

Patient-specific finite element models of hip cartilage and labrum (chondrolabral) mechanics have improved the understanding of form-function relationships underpinning hip osteoarthritis. While these models often assume the pelvis and femur are rigid bodies to reduce development and computational time, contact stresses may be overestimated when the pelvis and femur cannot deform. Recent advancements in element formulations, constitutive models, and integration of patient-specific boundary/loading conditions warrant a re-examination of this rigid body assumption. We assessed the influence of m

Spinal hypomyelination in a child with classic hypomyelination with atrophy of the basal ganglia and cerebellum (H-ABC): expanding the neuroradiological spectrum, a case report.

BACKGROUND: Hypomyelination with atrophy of the basal ganglia and cerebellum (H-ABC) is a progressive hypomyelinating leukodystrophy caused by pathogenic variants in the tubulin beta-4A gene (TUBB4A). Some patients exhibit clinical symptoms suggestive of spinal cord involvement; however, spinal cord magnetic resonance imaging (MRI) findings have rarely been described. CASE PRESENTATION: We performed spinal cord MRI in an 11-year-old boy carrying the most common, prototypical H-ABC-associated TUBB4A variant (p.D249N) and identified hypomyelinating changes in the spinal cord. In addition to the

The Spine-Foot Connection: Investigating Compensatory Mechanisms in Degenerative Spine Disease Through Foot Deformity Patterns.

Background and Objectives: In degenerative spine disease, compensatory mechanisms are activated to maintain upright posture, extending beyond the spine to involve the pelvis, lower limbs, and feet. These adaptations may be accompanied by differences in foot alignment, which could be associated with sagittal balance. The aim of this study is to investigate the relationship between foot alignment and spinal posture in patients with degenerative spine disease and evaluate whether foot deformities are associated with sagittal imbalance in degenerative spine disease. Materials and Methods: We retro

Hypertrophic pulmonary osteoarthropathy in bilateral distal tibia and fibula.

Hypertrophic pulmonary osteoarthropathy (HPOA) is a rare syndrome involving periosteal new bone formation, joint pain and digital clubbing, commonly secondary to pulmonary disease. It is frequently misdiagnosed as a primary orthopaedic disorder, which may delay appropriate systemic evaluation. A man in his late 60s with bilateral ankle pain visited our outpatient foot and ankle clinic. Radiographs and CT scans revealed symmetric periosteal new bone formation in both tibiae and fibulae. MRI showed no bone marrow abnormalities, supporting the diagnosis of HPOA. Further investigation revealed a l

MRI and US of Normal and Abnormal Digital Nerves.

Digital nerves provide essential sensory innervation to the fingers and toes, playing a crucial role in tactile sensation and proprioception. Injury, inflammation, or entrapment of these nerves can result in sensory deficits, paresthesia, and neuropathic pain. Imaging the digital nerves is technically challenging due to their small caliber, adjacent and similarly appearing vascular bundles, and distal location in the extremities. Accurate evaluation requires detailed anatomic knowledge and optimized protocols using high-frequency US or MRI to ensure proper nerve visualization. Common pathologi

HACD1-related congenital myopathy skeletal muscle MRI findings in a Brazilian cohort.

HACD1-related congenital myopathy is an ultra-rare congenital myopathy recently described in families from the Middle East or Asia. Clinical phenotype has been described, but little is known about the pattern of muscle MRI abnormalities. We describe four Brazilian patients from unrelated families including not only clinico-pathological, but also muscle MRI findings. All patients shared the known pattern of early onset of motor deficits combined with respiratory distress, later followed by remarkable improvement. Muscle biopsy revealed congenital fiber type disproportion. The same pathogenic bi
