Choroid plexus cyst of the spinal cord presenting with signs of spinal cord compression in a 55-year-old male: a case report.

BACKGROUND: Choroid plexus cysts are benign, typically asymptomatic cystic lesions found almost exclusively in the brain's lateral ventricles, often noted during second-trimester fetal ultrasounds and usually resolving spontaneously before the 28th week of gestation. Spinal intramedullary choroid plexus cysts remain extremely rare, with only few cases documented in  the literature to date. This case describes a 55-year-old man with thoracic spinal cord compression due to a choroid plexus cyst, highlighting a rare differential diagnosis for adult spinal cord lesions. CASE PRESENTATION: A 55-ye

A rare case of POEMS syndrome with unique endoscopic presentation.

POEMS (Polyneuropathy, Organomegaly, Endocrinopathy, Monoclonal gammopathy, Skin changes) syndrome is a rare multisystem disease, often presenting diagnostic challenges. Here, we report a case of POEMS syndrome in an elderly patient with refractory peritoneal effusion and hypoalbuminemia as the main manifestations. Physical examination revealed cutaneous hyperpigmentation of upper and lower extremities. Tests revealed elevated vascular endothelial growth factor (VEGF) and hypogonadism, while imaging showed enlargement of armpits and groin lymph nodes. Lymph node biopsy excluded tuberculosis an

King-Denborough Syndrome-Dysmorphic Features Suggesting Risk of Malignant Hyperthermia.

BACKGROUND: King-Denborough syndrome is a rare disorder within the spectrum of RYR1-related myopathies. It is a congenital myopathy, with characteristic dysmorphic features, and susceptibility to malignant hyperthermia. Early recognition is clinically important because the diagnosis may affect anesthetic management and perioperative safety. CASE PRESENTATION: We report a 4.5-year-old boy with delayed motor development, generalized hypotonia, characteristic dysmorphic features, and mildly elevated serum creatine kinase levels. The MRI showed features of asymmetrical atrophy and fatty degenerat

Diagnostic challenges in internal carotid artery fenestration: a 78-year-old woman presenting with transient ischemic attack-a case report.

BACKGROUND: Internal carotid artery fenestration is a rare vascular anomaly that may present with transient ischemic attack symptoms due to localized flow disturbances. It is important to report such cases, as they present unique diagnostic challenges and clinical implications, particularly when associated with systemic vascular risk factors such as diabetes and hypertension. Differentiating internal carotid artery fenestration from other conditions, such as internal carotid artery dissection, is crucial for appropriate management and patient outcomes. CASE PRESENTATION: A 78-year-old Iranian

Uncommon Presentation of Giant Tophaceous Gout in the Hand: A Case Report.

BACKGROUND: Tophaceous gout masses are characterized by the accumulation of monosodium urate crystals in peripheral joints and soft tissues. The most commonly involved areas are the metatarsophalangeal and knee joints. Finger/hand localization is uncommon. If not correctly treated, a finger tophaceous mass can grow and, in rare cases, reach an abnormally large size, termed "giant." AIM: The aim of our study is to present a rare case of a large tophaceous mass of the hand, localized in the fourth finger, and to highlight the role of surgical excision combined with a multidisciplinary team appr

Less is more: The Role of preoperative ultrasound as a risk stratifying tool in identifying low volume axillary disease in breast cancer.

OBJECTIVES: Axillary lymph node dissection (ALND) in early breast cancer causes lymphoedema in up to 20% of women and may be unnecessary in those with low axillary disease burden. This study evaluates whether preoperative axillary ultrasound (AUS) can stratify nodal burden preoperatively. METHODS: A retrospective single-centre observational study was conducted using records from January 2023-December 2024. Among 193 patients with abnormal AUS, those with negative axillary biopsy, neoadjuvant therapy, no surgery, or distant metastases were excluded. Data on AUS findings, tumour size, and posto

[A case of anti-myelin-oligodendrocyte glycoprotein (MOG) antibody-associated disease without abnormal findings on spinal MRI and cerebrospinal fluid examination].

A75-year-old man presented with recurrent episodes of myelopathy since youth, previously responsive well to steroid therapy. n Beginning at age of 72, he developed repeated episodes of gait disturbance suggestive of myelitis. Although spinal MRI and cerebrospinal fluid (CSF) analysis were normal, somatosensory-evoked potentials (SEP) were absent in both lower extremities. He showed a favorable response to steroid pulse therapy and tested positive for anti-myelin oligodendrocyte glycoprotein (MOG) antibodies. Additional treatment with double filtration plasmapheresis improved his mobility, enab

Phosphaturic mesenchymal tumour, connective tissue variant: a rare radiological presentation involving the radius mimicking fibrous dysplasia with oncogenic rickets.

Phosphaturic mesenchymal tumours (PMTs) are rare neoplasms that secrete fibroblast growth factor-23 (FGF-23), causing tumour-induced osteomalacia (TIO). Histological overlap with juvenile psammomatoid ossifying fibroma (JPOF) can lead to diagnostic difficulty, particularly outside the craniofacial skeleton. A 4-year-old girl presented with a painful forearm swelling. Radiographs demonstrated a fibro-osseous lesion in the radius and biopsy was initially non-diagnostic, leading to a presumptive diagnosis of fibrous dysplasia. Over the next 6 years, the lesion enlarged, and at age 10, the patient

Glial fibrillary acidic protein antibody-associated astrocytopathy presents isolated spinal cord enhancement: A case report.

RATIONALE: Glial fibrillary acidic protein antibody-associated astrocytopathy (GFAP-A) is an autoimmune central nervous system inflammatory disorder diagnosed via cerebrospinal fluid (CSF) glial fibrillary acidic protein-immunoglobulin G detection. Clinically, it typically presents with meningoencephalitis or myelitis, accompanied by periventricular perivascular enhancement on brain magnetic resonance imaging (MRI) (findings that guide most routine diagnoses). Yet isolated spinal cord abnormalities (with negative brain MRI) in GFAP-A remain rarely reported, especially when occurring in patient

Non-invasive Reduction of Accessory Breast Fat Using Multilayer High-Intensity Focused Ultrasound: A Quantitative Ultrasound-Based Study.

OBJECTIVES: Accessory breast tissue often presents as localized axillary subcutaneous fullness, presenting as a palpable soft-tissue bulge composed of adipose or glandular elements, causing mechanical discomfort and esthetic concern. While surgical excision remains effective, it ma result in scarring, postoperative pain, and risks of seroma/hematoma or contour irregularity, leading to growing interest in noninvasive approaches. The objective of the study is to investigate the effect of noninvasive high-intensity focused ultrasound (HIFU) on subcutaneous fat thickness in axillary accessory brea

3D Radiomic Texture Analysis of Quantitative Muscle MRI Enhances the Distinction Between Myotonic Dystrophy Type 1 and Charcot-Marie-Tooth Neuropathy Type 1A: A Proof-of-Concept Study.

BACKGROUND: Differentiating myogenic from neurogenic neuromuscular diseases (NMDs) can be clinically challenging. While quantitative muscle MRI (qMRI) with proton density fat fraction (PDFF,%) quantifies fat replacement, it misses micro-spatial patterns linked to underlying pathology. This study investigates whether qMRI with 3D radiomic texture analysis (TA) might improve differentiation between myogenic and neurogenic diseases, using myotonic dystrophy type 1 (DM1) and Charcot-Marie-Tooth neuropathy type 1A (CMT1A) as proof-of-concept models. METHODS: Thirty-three adults with DM1, 33 with C

Structural brain analysis in focal upper limb dystonia.

Prior structural magnetic resonance imaging (MRI) studies that investigated structural alterations in focal dystonia have reported inconsistent findings, potentially due to methodological limitations or sample heterogeneity. This work investigates gray and white matter changes in patients with upper limb dystonia using T1-weighted images and diffusion tensor imaging (DTI). Imaging data collected with MRI at 3T from 28 right-handed individuals with right upper limb dystonia and 29 healthy controls were analyzed. T1-weighted images were analyzed using FreeSurfer to examine cortical thickness, vo

Spinal Cord Injury Without Radiographic Abnormality Complicated by Acute Cholecystitis: A Case Report Highlighting Diagnostic and Therapeutic Challenges.

A man in his early 90s was brought to our emergency department after a fall in which he struck his left shoulder and subsequently became unable to move his left upper extremity. On arrival, neurological examination revealed muscle weakness and paresthesia predominantly affecting the left upper and lower extremities below the C6 level. Cervical magnetic resonance imaging demonstrated narrowing of the spinal canal at the C4/5 and C5/6 levels, corresponding to the level of neurological deficits, along with multilevel left-sided foraminal stenosis, leading to a diagnosis of spinal cord injury with

Expanding the phenotypic spectrum of COL6-related diseases: Motor neuropathy-like and neuromyopathy associated with COL6A3 c.7447A>G.

Collagen VI is a key component of the extracellular matrix. Murine models suggest that it regulates Schwann cell differentiation, peripheral nerve myelination and regeneration after injury. Neuropathic findings in COL6-related dystrophies (COL6-RDs) have been rarely reported, often based on electromyography findings without nerve conduction abnormalities. We report 37 patients (1.5-79 years), from 32 different families, carrying the COL6A3-c.7447A>G variant, presenting with neuropathic/neuromyopathic findings. Most patients presented in the first decade of life with an abnormal gait and freque

Temporary stent fixation combined with craniotomy for managing microcatheter fracture during cerebral AVM embolization.

A 54-year-old male patient presented with impaired consciousness and weakness of the right limbs, and was clinically diagnosed with ruptured cerebral arteriovenous malformation(AVM), thus undergoing endovascular embolization. During the operation, the embolic microcatheter fractured, with its residual segment retained in the left posterior cerebral artery and basilar artery. To prevent migration of the residual catheter segment that may lead to secondary embolism or hemorrhage, a 6 × 30 mm stent was urgently deployed intraoperatively to fix the proximal end of the catheter. An immediate cranio
