Frequency-specific alterations of spontaneous brain activity in stroke patients with upper limb motor dysfunction: A multi-metric rs-fMRI and machine learning study.

BACKGROUND: Post-stroke upper limb motor dysfunction is associated with complex alterations in brain function, but the frequency-specific characteristics of spontaneous local neural activity remain incompletely understood. This study investigated multi-metric resting-state functional magnetic resonance imaging (rs-fMRI) alterations in stroke patients with upper limb motor dysfunction and explored their discriminative value using machine-learning classifiers. METHODS: A total of 70 stroke patients and 45 healthy controls (HCs) underwent rs-fMRI. Fractional amplitude of low-frequency fluctuatio

Current Management of Extremity Lymphedema.

Extremity lymphedema is a common condition resulting from impaired lymphatic transport after oncologic treatment, trauma, infection, or congenital abnormalities. Modern management has evolved from conservative therapy with compression garments to physiologic interventions guided by integrated clinical and functional staging system. Accurate diagnosis includes objective circumferential or volumetric measurement, lymphoscintigraphy and indocyanine green lymphography. Physiologic interventions such as lymphovenous anastomosis and vascularized lymph node flap transfer are effective and selected on

Lymphoscintigraphic Findings in Patients With Lipedema: A Systematic Review.

BACKGROUND: Lipedema is a chronic disorder characterized by excessive, bilateral, and symmetrical deposition of subcutaneous adipose tissue, predominantly in the lower extremities. Despite its prevalence, lipedema is frequently misdiagnosed as lymphedema, and lymphoscintigraphy is commonly used to differentiate between the two. This systematic review synthesizes the available evidence regarding the most common lymphoscintigraphy findings in patients with lipedema. METHODS: This systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Ana

Hypothenar hammer syndrome: An expanded Kaji angiographic classification based on 91 cases.

BACKGROUND: Hypothenar hammer syndrome (HHS) is characterized by ischemic symptoms of the hand related to a stenotic or occluded ulnar artery. The disease is considered rare, although there is evidence of underdiagnosis. Digital subtraction angiography (DSA) is the primary diagnostic tool, although staging is typically based on the Kaji classification, which captures most abnormalities, though some may be overlooked. This study proposes an all-encompassing angiographic classification system for HHS. METHODS: In this multicenter retrospective case series, electronic medical records were search

Tattoo Pigment Migration Within the Breast and Axilla: Radiologic-Pathologic Correlation.

Tattoo pigment migration can appear on breast imaging, including mammography and ultrasound, where it may mimic malignant findings. In the literature, 19 cases have been reported. Here, we describe an additional three cases. Most published cases involve women undergoing screening mammography with incidental identification of pigment migration. On imaging, pigment can present as high-density deposits on mammography and echogenic foci on ultrasound, mimicking calcifications. Reported sites of deposition most commonly include axillary lymph nodes and intramammary nodes. If no other morphologic ab

Anti-NF155 antibody-positive autoimmune nodopathy with myocardial 123I-MIBG scintigraphy abnormality.

A 68-year-old man presented with numbness and pain of the hands. Neurological examination revealed distal limb weakness, diminished tendon reflexes, sensory disturbances in all extremities, tremors, ataxia, and orthostatic hypotension, which caused repeated fainting spells and falls. The nerve conduction studies revealed definite demyelinating abnormalities. The cerebrospinal fluid protein levels were markedly increased at 638 mg/dl. The contrast-enhanced MRI neurography revealed nerve root enlargement and enhancement, and the 123I-metaiodobenzylguanidine myocardial scintigraphy revealed abnor

Clinical value of MRI in central lymphatic vessel evaluation and grading in primary lower-extremity lymphedema.

Introduction: The manifestations of the central lymphatic vessels in primary lower-extremity lymphedema (LEL) remain incompletely understood. Advanced magnetic resonance imaging (MRI) techniques offer the potential to noninvasively evaluate central lymphatic vessels and their relationship with disease severity. Our objective was to investigate the differences in imaging performance of central lymphatics and inguinal lymph nodes in primary LEL of different clinical stages, and to assess the ability of MRI to grade primary LEL. Methods: Patients were staged according to the International Society

Unusual presentation of Currarino syndrome in an infant with right gluteal abscess.

Currarino syndrome is a rare condition with an incidence of approximately 1 in 100,000. It is characterised by the triad of a sacral defect, a presacral mass and anal atresia or stenosis, and it is commonly associated with genetic mutations in the MNX1 gene.We report a case of an infant presenting with recurrent right gluteal abscess secondary to an infected sacrococcygeal teratoma, associated with anal stenosis. Extensive workup was performed to investigate for the source of her recurrent gluteal abscess and anal stenosis, leading to the diagnosis of Currarino syndrome. She underwent multiple

Innovative femoral vein valve repair combined with deep femoral vein ligation for treatment of chronic venous insufficiency: a case report.

Chronic deep venous insufficiency (CVI) of the lower extremities results from structural or functional abnormalities of the venous valves, leading to blood reflux and increased venous pressure. Common clinical manifestations include edema, varicose veins, skin hyperpigmentation, and ulcers. The primary etiologies of CVI include congenital valve defects and secondary damage from venous thrombosis, both of which significantly impair quality of life. Surgical treatments for CVI include valve repair, valve transposition, and valve transplantation. Valve repair is effective when the valve structure

Identification of peripheral pain generators with sigma-1 receptor Positron Emission Tomography/Magnetic Resonance Imaging in complex regional pain syndrome: initial study in a prospective trial.

Complex regional pain syndrome (CRPS) is a poorly understood chronic pain condition of the extremities presenting severe pain disproportionate to the causative injury. Owing to its heterogeneous clinical presentation and the lack of specific diagnostic tests, CRPS is often a diagnostic and therapeutic challenge. Early diagnosis and treatment of CRPS improve quality of life and delay disease progression. Identification of focal peripheral pain generators would provide an opportunity for targeted treatments with more limited side effects. A highly selective sigma-1 receptor positron emission tom

Assessment of Neuro-Musculo-Vascular Activity in Hemiplegic Shoulder Pain: A Multimodal Study With HD-sEMG, Ultrasonography, and Laser Speckle Imaging.

Hemiplegic shoulder pain (HSP) is a common complication following stroke, significantly affecting upper limb recovery and quality of life. However, the underlying pathophysiological mechanisms of HSP remain poorly understood, which poses a major obstacle to the development of effective therapeutic strategies. This study aims to investigate the underlying mechanism of HSP by evaluating neuro-musculo-vascular activities using high-density surface electromyography (HD-sEMG), musculoskeletal ultrasound, and laser speckle contrast imaging (LSCI). A total of 12 HSP patients and 12 hemiplegic control

Integrative management of microvascular spasm-driven Raynaud's-like digital ischemia in an elderly smoker with mixed-pattern vasculopathy: A case report.

BACKGROUND: Chronic smoking is a key driver of endothelial dysfunction and microcirculatory impairment, contributing to "mixed-pattern vasculopathy," a condition characterized by coexisting macrovascular stenosis and microvascular tone dysregulation. This pathology significantly increases the risk of refractory distal ulceration, dry gangrene, and amputation. This case report aimed to describe the clinical course and therapeutic outcome of integrative treatment with Chinese herbal medicine (CHM) in a patient with refractory ischemia associated with mixed-pattern vasculopathy. Nail-to-bicondylar ratio, but not CPAK classification, is associated with arthrosis and deformity progression after retrograde fixator-assisted nailing for distal femoral deformity in metabolic bone disease.

BACKGROUND: Distal femoral deformities are common in metabolic bone disease and often require surgical correction. Retrograde Fixator-Assisted Nailing(r-FAN) stabilizes the femoral axis, yet the influence of nail-to-bicondylar ratio (NBR) and coronal-plane knee alignment patterns(CPAK) on deformity and osteoarthritis(OA) progression remains unclear. METHODS: We retrospectively evaluated 30 limbs from 17 patients with distal femoral deformities treated with r-FAN, with a mean follow-up of 10.5 ± 4.6 years. Pre- and postoperative radiographs were analyzed for mechanical lateral distal femoral a
