Functional Characterization of the PAX8 p.Leu264Pro Variant Identified in a Patient with a Müllerian Duct Anomaly.

BACKGROUND/OBJECTIVES: Müllerian duct anomalies (MDAs) are congenital structural abnormalities of the female reproductive tract with heterogeneous and incompletely defined genetic contributions. PAX8 is a developmental transcription factor implicated in thyroid and urogenital development. This study assessed the functional consequences of a rare PAX8 variant identified by whole-exome sequencing (WES) in an MDA cohort, without presuming a causal relationship. METHODS: WES was performed in 150 patients with MDAs. The PAX8 c.791T>C (p.Leu264Pro) variant was evaluated using computational predicti

Prevalence of Thyroid Dysfunction in Patients With Type 2 Diabetes Mellitus and Its Impact on Glycemic Control.

Background Type 2 diabetes mellitus (T2DM) and thyroid dysfunction are common endocrine disorders. Thyroid hormones play an important role in glucose metabolism, insulin secretion, and insulin sensitivity. Disturbances in thyroid function may worsen glycemic control and increase the risk of diabetic complications. This study aimed to determine the prevalence of thyroid dysfunction in patients with T2DM and evaluate its impact on glycemic control. Methods This cross-sectional study was conducted at Nawaz Sharif Medical College/Aziz Bhatti Shaheed Teaching Hospital, Gujrat, Pakistan, from Januar

Myxedema Coma Despite Mild Biochemical Hypothyroidism: A Case of Profound Clinical Decompensation.

Myxedema coma is a rare endocrine emergency with high mortality that is typically associated with severe biochemical hypothyroidism. However, profound clinical decompensation may occur despite only modest abnormalities in thyroid function tests, making diagnosis challenging. We report the case of an 87-year-old woman with long-standing hypothyroidism, primary adrenal insufficiency, pituitary pathology, and ischemic heart disease who presented with an Escherichia coli urinary tract infection and subsequently developed profound hypothermia, severe bradycardia, hypotension, reduced consciousness,

Functional Characterization of a Novel Variant of the Thyroid Hormone Receptor Alpha in a Child with Developmental Delay and Abnormal Thyroid Function.

BACKGROUND: Resistance to thyroid hormone α (RTHα) is a rare disorder caused by pathogenic THRA variants. We investigated the molecular basis of RTHα in a child with developmental delay, dysmorphic features, and a suggestive biochemical profile. METHODS: Whole-exome sequencing identified a de novo THRA variant. Cell-based transcriptional assays assessed thyroid hormone responsiveness, coactivator dependence, and dominant-negative activity. RESULTS: A novel heterozygous frameshift variant, c.1125_1132dup (p.Gly378Alafs*2), truncating the ligand-binding domain, was identified. The mutant recep

Comparative Analysis of Hyperbilirubinemia Between Congenital Hypothyroidism and Euthyroidism in Neonates in Rawalpindi, Pakistan.

INTRODUCTION: Neonatal hyperbilirubinemia is a prevalent clinical observation that, if persistent or severe, may suggest underlying endocrine abnormalities such as congenital hypothyroidism (CH). Thyroid hormones play a crucial role in liver function and the metabolism of bilirubin. OBJECTIVE: This study aimed to evaluate blood bilirubin and thyroid stimulating hormone (TSH) levels in neonates diagnosed with CH compared to those with normal thyroid function (euthyroid). MATERIAL AND METHODS: A comparative cross-sectional observational study was conducted at Majeed Medical & Gynae Complex and

International Registry of NKX2-1-Related Disorders: Clinical, Genetic, and Imaging Perspectives.

BACKGROUND: NKX2-1-related disorders result from heterozygous variants in NKX2-1, a gene crucial for brain, lung, and thyroid development. Although movement disorders, hypothyroidism, and neonatal respiratory distress are recognized, the full phenotype and genotype-phenotype relationships remain incompletely defined. OBJECTIVES: To delineate neurological, respiratory, and endocrine features across ages, characterize movement disorder trajectories - particularly chorea - and explore genotype-phenotype associations with clinical relevance. METHODS: We conducted a multicenter, cross-sectional s

Empirical Thyrotropin-Free Thyroxine Relationship Across the Thyrotropin Spectrum and Implications for Reference Intervals.

BACKGROUND: The relationship between thyrotropin (TSH) and free thyroxine (FT4) is commonly assumed to be log-linear. However, this assumption may oversimplify hypothalamic-pituitary-thyroid axis regulation. Characterizing the empirical structure of this relationship could inform the interpretation of thyroid function tests and the validity of current TSH reference intervals. Segmented regression was used to examine the TSH-FT4 relationship to determine whether statistically derived TSH breakpoints corresponded to the current reference limits. METHODS: This study retrospectively analyzed 782

Therapeutic effect and mechanism of different doses of aspirin on preterm delivery in pregnant mice.

BACKGROUND: Preterm birth is a major cause of perinatal mortality and complications, with inflammation being a key contributing factor. Current treatments, like uterine contraction inhibitors and antibiotics, are unsatisfactory. Aspirin, a cyclooxygenase inhibitor, shows promise in treating infectious preterm labor but has limited in vivo studies and an unclear mechanism. METHODS: In this study, a mouse model of infectious preterm birth was established via lipopolysaccharide (LPS) injection, and the aspirin doses used in these animals were converted from the recommended human doses by the bod

Thyroid Hormone Levels and Its Co-relation With Human Chorionic Gonadotropin in Patients With Molar Pregnancy.

Introduction Gestational trophoblastic disease (GTD) is characterized by abnormal trophoblastic proliferation, with hydatidiform mole being its most common form. Markedly elevated serum human chorionic gonadotropin (hCG) concentrations in molar pregnancy can stimulate thyroid-stimulating hormone receptors, resulting in thyroid dysfunction. This study aimed to evaluate thyroid hormone levels and their correlation with serum β-human chorionic gonadotropin (β-hCG) concentrations in patients with molar pregnancy and to compare thyroid function between complete and partial hydatidiform mole. Materi

Patterns of thyroid dysfunction in children with trisomy 21: an observational study.

BACKGROUND: Thyroid dysfunctions are the most common endocrine abnormalities observed in trisomy 21 (T21). The prevalence of thyroid disorders in children with T21 varies between 4% and 40%. Dysregulation of thyroid function, which is specific to T21, is a possible etiology. Thyroid dysfunction can have a significant impact on cognitive development and growth. Transiently abnormal thyroid function tests (TFTs) are frequently observed in T21 and may cause diagnostic confusion. There is a lack of literature on several specific thyroid conditions (such as CH, transient abnormal TFT and thyroid dy

Nails in osteoporosis and bone metabolism: a narrative review of current evidence and future directions.

BACKGROUND: Osteoporosis is a widespread public health problem, yet Dual-Energy X-ray Absorptiometry (DXA)-the current diagnostic gold standard-has significant limitations, including high cost, radiation exposure, and inability to assess bone organic phase changes. As an easily accessible epidermal derivative, nails share high molecular homology with bone tissue and offer unique advantages of non-invasive collection and stable composition reflecting medium- to long-term metabolic status. This narrative review aimed to systematically collate the pathophysiological associations between nails and
