Is Varicocele Truly Unilateral? Contralateral Involvement and Bilateral Testicular Effects in Male Infertility: A Narrative Review.

Background/Objectives: Varicocele is usually clinically left-sided, but imaging and mechanistic studies suggest that contralateral venous reflux or bilateral testicular effects may be more common than physical examination indicates. This narrative review critically examines whether varicocele-associated infertility is better conceptualized as an asymmetric disorder with potential bilateral anatomical or functional involvement and considers the implications for diagnosis and treatment. Methods: PubMed/MEDLINE and Scopus were searched from inception to 21 June 2026 for studies on varicocele late

Non-thyroidal illness syndrome subtypes and mortality in sepsis: associations with thyroid autoantibodies.

BACKGROUND: Non-thyroidal illness syndrome (NTIS) is frequently observed in patients with sepsis; however, prognostic differences among its subtypes remain poorly defined. This study aimed to evaluate whether different NTIS subtypes and the presence of thyroid autoantibodies are associated with mortality in septic patients. METHODS: We conducted a retrospective cohort study including 871 patients with sepsis admitted to a tertiary hospital in Shanghai between 2015 and 2019. Thyroid function tests were performed within 24 hours of sepsis diagnosis. NTIS was categorized into four subtypes accor

Confirmation of frameshift variants in the last exon of FGFR1 as a cause of multiple epiphyseal dysplasia.

Multiple epiphyseal dysplasia (MED) is a genetically diverse skeletal disorder characterized by abnormal and delayed epiphyseal ossification, early-onset osteoarthritis, joint pain and mild short stature. MED is primarily caused by pathogenic variants in genes involved in cartilage and bone development, including COMP, MATN, COL9A1-3, CANT1 and SLC26A2. FGFR1 was suggested as a cause of MED in a study published in 2020, which reported a frameshift variant in the last exon of this gene. We assembled an international cohort of 13 individuals with MED carrying C-terminal frameshift variants in FG

Thyrotropin Controversy in Subclinical Thyroid Disorders.

AIM: The thyrotropin controversy in subclinical thyroid disorders (STDs) is among the most common endocrine disorders globally. In India, approximately 42 million people suffer from thyroid disorders, with subclinical hypothyroidism (SCH) affecting about 9.4% of the population. SCH is more prevalent in females (11.4%) compared to males (6.2%). DISCUSSION: The diagnosis and treatment of SCH and subclinical hyperthyroidism (SHT) are controversial. SCH is often diagnosed based on biochemical markers, as patients may be asymptomatic or exhibit vague symptoms. Thyrotropin (TSH) levels may be eleva

Thyroid dysfunction and Research Domain Criteria framework-based depression phenotypes: Insights from a historical longitudinal cohort.

BACKGROUND: Depression is a clinically heterogeneous disorder, making it difficult to identify its biological underpinnings, including potential links to dysregulation of the hypothalamic-pituitary-thyroid axis, which plays a central role in the pathophysiology of depression. The Research Domain Criteria (RDoC) framework provides a multi-dimensional approach to parsing depressive phenotypes. This study investigates the association between thyroid-stimulating hormone (TSH) abnormalities and depressive symptoms mapped to RDoC domains. METHODS: In a retrospective cohort from 2000 to 2021, adults

Endoscopic endonasal management of sellar/suprasellar arachnoid cysts: tailored surgical strategies and systematic review.

INTRODUCTION: Sellar and suprasellar arachnoid cysts(SACs)are rare lesions that may cause headaches, visual impairment, and endocrine dysfunction. While endoscopic endonasal approaches(EEA)are increasingly preferred, optimal surgical strategy-simple fenestration versus wide cyst-subarachnoid space(SAS)communication-remains debated. This study reports one of the largest single-center series of SACs treated via EEA for SACs and integrates these findings with an updated systematic review of the literature. METHODS: A retrospective review was conducted on 6178 endoscopic endonasal surgeries perfo

Temporal changes in cortisol secretion and their association with long-term outcomes in benign adrenal incidentalomas: a retrospective cohort study.

BACKGROUND: Mild autonomous cortisol secretion (MACS), the most common hormonal abnormality in adrenal incidentalomas, is associated with increased cardiometabolic risk. MACS is defined by cortisol concentrations higher than 50 nmol/L after a 1-mg overnight dexamethasone suppression test (1-mg DST). The prognostic value of a single test remains uncertain. We examined longitudinal changes in 1-mg DST results, cumulative cortisol exposure, and their associations with cardiometabolic outcomes and mortality. METHODS: In this retrospective cohort study conducted in 25 adrenal centres that are part

Recurrent Hyponatremia With Central Endocrine Dysfunction: A Diagnostic Challenge.

A 55-year-old man with a history of chronic hyponatremia presented with recurrent nausea, vomiting, and severe hyponatremia. Initial evaluation revealed features consistent with the syndrome of inappropriate antidiuretic hormone secretion (SIADH), including hypotonic hyponatremia with inappropriately concentrated urine. Further workup demonstrated concurrent central hypothyroidism and hypogonadotropic hypogonadism, prompting neuroimaging. MRI identified a 1.7 cm suprasellar cystic mass compressing the pituitary infundibulum. The patient underwent transsphenoidal cyst drainage, resulting in the

Luteolin alleviates PCOS by inhibiting AR/STAT3/NLRP3-mediated granulosa cell pyroptosis.

BACKGROUND: Polycystic ovary syndrome (PCOS), a common endocrine-metabolic disorder, is driven by hyperandrogenism and chronic low-grade inflammation that impair follicular development. Granulosa cell pyroptosis is increasingly recognized as a key pathogenic mechanism in PCOS. Luteolin (LUT), a natural flavonoid found in many traditional medicinal plants, exhibits potent anti-inflammatory properties. However, its role in regulating granulosa cell pyroptosis within the context of PCOS has not been elucidated. METHODS: We established a dehydroepiandrosterone (DHEA)-induced PCOS rat model to eva
