How Imported Pathology Becomes Overlooked as Resources Increase: An Example of a Neglected Tropical Disease (NTD).

This case report highlights how neglected tropical diseases (NTDs) may remain underdiagnosed in high-resource settings despite advanced healthcare systems. We describe an 18-year-old woman from Equatorial Guinea living in Spain who presented with chronic pruritus and episodic ocular symptoms and was ultimately diagnosed with ocular loiasis after the incidental extraction of a conjunctival filaria. Diagnostic evaluation included microscopy, serology, and molecular techniques, confirming Loa loa and Mansonella perstans co-infection alongside evidence of exposure to other parasitic infections. Incidence of loiasis clinical manifestations in a rural area of the Republic of Congo: Results from a longitudinal prospective study (the MorLo project).

BACKGROUND: Loiasis is endemic in Central Africa. Despite evidence of clinical complications and increased mortality, it remains excluded from the list of neglected tropical diseases. The main manifestations are Calabar swellings (CS), Eyeworm (EW) and non-specific general symptoms such arthralgia and pruritus. We calculated incidence rates for clinical manifestations of loiasis from a 13-month study on clinical manifestations in 991 individuals living in Loa loa-endemic areas in the Republic of Congo. METHODOLOGY: From September 2022 to September 2023, community health workers collected week

Imported loiasis: Diagnostic and therapeutic challenges.

Loiasis is a vector-borne parasitic disease that has long been neglected due to its benign nature and its geographical distribution, which is restricted to the forest regions of Central and West Africa. However, it is now recognized as a significant public health concern in endemic areas, where chronically infected patients face organ damage and an increased risk of mortality. Loiasis presents several challenges, from diagnosis to treatment, as inappropriate therapies can lead to severe and at times fatal side effects. Although loiasis has low prevalence in Europe, historical connections with

Revisiting the Loa loa microfilaremia thresholds above which serious adverse events may occur with ivermectin treatment.

BACKGROUND: Loiasis was long deemed to be a benign condition, but individuals with high Loa loa microfilarial densities (MFD) are at risk of serious adverse events (SAEs) including encephalopathy following ivermectin (IVM) administration. The risk of marked AE or SAE is usually considered when MFD exceeds 8000 microfilariae (mf)/mL or 30,000 mf/mL, respectively. There are no international guidelines on the treatment of loiasis, resulting in a variety of practices worldwide for the treatment of infected individuals outside endemic areas. Our objective was to determine the probabilities of SAEs

Treatment of loiasis: a review of clinical management recommendations.

BACKGROUND: Loiasis affects more than 20 million residents of endemic regions in Central and West Africa causing chronic and often lifelong disease. Antifilarial treatment options for loiasis include diethylcarbamazine, ivermectin, and albendazole. Safe and effective management requires classifying patients into occult, microfilaremia, and hypermicrofilaremia categories. Treatment is complicated by the risk of severe adverse events, particularly encephalitis. Clear guidance on the appropriate use of antifilarial therapy is therefore of utmost importance. The aim of this review is to evaluate c

Antifilarial treatment strategies: a systematic review and network meta-analysis.

BACKGROUND: The World Health Organization (WHO) prescribes mass drug administration (MDA) to eradicate lymphatic filariasis within endemic populations. The WHO endorsed using ivermectin with diethylcarbamazine and albendazole (IDA) for MDA in specific settings devoid of onchocerciasis or loiasis. Still, the utilization of IDA in sub-Saharan Africa is restricted due to the potential of diethylcarbamazine to induce severe adverse ocular events in individuals with onchocerciasis. AIM: We aim to investigate all documented combinations of antifilarial drugs available in the literature using a netw

Diagnosis, management and prevention of loiasis: guideline of the German Society for Tropical Medicine, Travel Medicine, and Global Health (DTG).

Loiasis is a complex filarial infection endemic in Central Africa and parts of West Africa. Loa loa is transmitted by the deer fly Chrysops dimidiata and C. silacea. The clinical manifestation of the disease is highly variable ranging from asymptomatic infection, symptomatic disease, to life-threatening complications. The diagnosis of L. loa infection is challenging due to a significant proportion of occult infections and a lack of reliable point of care tests. While diethylcarbamazine is the gold standard for curative treatment in many non-endemic countries, its use is limited in endemic regi

Fortuitous discovery of a microfilaria of the genus Loa loa during a routine blood smear at the Hematology Laboratory of the Mohamed V Military Instruction Hospital in Rabat: a case report.

Loiasis is a parasitic infection transmitted by a vector, specifically through the bites of Chrysops genus tabanid flies. It is often associated with marked and persistent eosinophilia in affected individuals. We report the case of a 28-year-old Cameroonian male patient. His medical history includes an episode of malaria treated on an outpatient basis. As part of a diving internship in Morocco, the young serviceman underwent a medical fitness examination at the Medical Expertise Center for Aircrew Personnel of the Mohamed V Military Hospital, which included a biological assessment. This reveal

Serendipity in Emergency Diagnostics: Near Miss of a Loa loa Case in a Swiss Hospital.

A 31-year-old Cameroonian man presented to the emergency department (ED) with a productive cough, exertional dyspnoea, and asthenia for a few days. The initial evaluation was limited due to cultural barriers, lack of known medical history, and the patient's migration journey through Libya, Tunisia, and Germany over the past 5 years before recently arriving in Switzerland. Clinical examination revealed no red flags. Blood counts were normal, C-reactive protein (CRP) was elevated at 91 mg/L, and a chest X-ray was unremarkable. A diagnosis of influenza-like syndrome was made. The patient returned

Efficacy and safety of albendazole 400 mg for 30 days compared to single dose of ivermectin in adult patients with low Loa loa microfilaremia: A non-inferiority randomized controlled trial.

BACKGROUND: Loa loa infection is endemic in central African countries and particularly in Gabon. Treatment typically involves the use of ivermectin and albendazole, with albendazole often administered to reduce microfilaremia in individuals with high microfilaremia before taking ivermectin. This study aims to evaluate the efficacy and safety of albendazole in patients with low microfilaremia. METHODOLOGY AND PRINCIPAL FINDINGS: The study was conducted from November 2021 to April, 2022 in the Woleu-Ntem province of northern Gabon. Following a questionnaire, direct examination of 10 µL of blood

A cross-sectional investigation of the ophthalmological impact of loiasis in Cameroon, Central Africa.

BACKGROUND: Current knowledge of ocular manifestations of loiasis is limited to the transient subconjunctival passage of the adult filaria and anecdotal reports of posterior segment lesions. Therefore, the ocular burden of loiasis is likely underestimated since it has never been systematically assessed at the population level. We aimed to evaluate the relationship of Loa loa microfilaremia and recent eye worm passage with chronic ocular lesions identified through comprehensive ophthalmological assessment in an endemic area. METHODOLOGY/PRINCIPAL FINDINGS: Subjects aged ≥ 15 years, residing in

Safety and efficacy of 3- and 5-day regimens of levamisole in loiasis: a randomized, placebo-controlled, double-blind clinical trial.

Individuals with high Loa loa microfilarial densities (MFD) risk serious adverse events (SAEs) following ivermectin treatment. A single dose of levamisole (LEV) induces a temporary, progressive MFD decrease. This double-blind, randomized, placebo-controlled trial evaluated the safety and efficacy of 3-day and 5-day LEV regimens for the treatment of loiasis in the Republic of the Congo. Participants were randomly assigned to receive placebo (PLA), LEV-3, or LEV-5. Safety was assessed by the occurrence of SAEs and the frequency and severity of AEs. Efficacy was measured by changes in Loa loa MFD

Delayed-Onset Loiasis: A Case of Loa loa Infection Diagnosed Six Years Post-exposure.

Loiasis, caused by Loa loa, is a filarial infection endemic to Central and West Africa, increasingly recognized in non-endemic regions due to global migration and travel. Diagnosis in non-endemic regions can be challenging due to delayed symptom onset. We report a case of a 40-year-old woman presenting with ocular discomfort and a migrating subcutaneous parasite six years after residing in Gabon. Laboratory evaluation revealed eosinophilia and microfilariae on peripheral blood smear, confirming loiasis. Initial treatment with albendazole led to inflammatory symptoms, highlighting the risk of p

Plasma Proteomics Reveals Distinct Signatures in Occult and Microfilaremic Loa loa Infections.

BACKGROUND: Loiasis, caused by the filarial nematode Loa loa, imposes a significant disease burden in endemic regions in West and Central Africa. Manifestations include adult worms in soft tissue (e.g., the conjunctiva of the eye) and microfilaria in peripheral blood, with clinical presentations ranging from asymptomatic infections to life-threatening organ involvement. Diagnosis remains challenging due to variable microfilaria counts, frequent amicrofilaremic occult infections, and unreliable serological tests. The untargeted plasma proteome reflects broad (patho-)physiological responses, pro

A Novel Filarial-Multiplexed Probe-Quantitative PCR for the Advance in the Diagnosis of Multiple Infections With Human Filariasis.

The routine diagnostic method used for clinical samples suspected of filarial infection in our laboratory is the Filaria-real time-PCR (F-RT-PCR). The drawback of this method is the need for melting temperature analysis and PCR products' electrophoresis to identify the filarial species. Therefore, the aim of this study was to design a quantitative real-time PCR (qPCR) assay using filarial-specific hydrolysis probes, targeting 18S rRNA and ITS1 genes, allowing the simultaneous diagnosis of loiasis, mansonellosis and other human filariasis without the need of electrophoresis or melting temperatu

Evaluation of IgG4 response against Chrysops spp. molecules in long-term stocked plasma from humans exposed to Loa loa.

Chrysops silacea and Chrysops dimidiata are the main vectors of the human filarial disease caused by Loa loa but also a potential carrier of other parasites, bacteria, or viruses. The female feeds on blood from humans and from non-human species. Studies about African Chrysops species are scarce. In this context, we evaluated the detection of IgG4 against Chrysops molecules in samples from humans exposed to Loa loa infection stored for 20 yr. Fresh extracts of proteins from uninfected and unfed C. silacea were analyzed through enzyme-linked immunosorbent assay (ELISA) and western blotting for t

Chronic kidney disease related to Loa loa microfilaremia in a rural area of the Republic of Congo: a population-based cross-sectional study.

BACKGROUND: Loiasis affects millions in Central Africa and, though historically considered benign, emerging data suggest possible renal involvement. This study investigated the association between Loa microfilaremia and renal function. METHODS: We conducted a cross-sectional study in the Republic of Congo in May-June 2022. Renal function was assessed via estimated glomerular filtration rate (eGFR) using Chronic Kidney Disease-Epidemiology Collaboration (CKD-EPI) and European Kidney Function Consortium (EKFC) equations, and proteinuria and/or haematuria (renal abnormalities, RAb). Protective efficacy of skin-applied arthropod repellents against Chrysops bites in a Loa loa hyperendemic region in Gabon: A placebo-controlled randomized clinical trial of DEET, icaridin, citriodiol, and IR3535.

BACKGROUND: Loa loa - a filarial pathogen causing loiasis -is transmitted by deer fly species Chrysops dimidiata and C. silacea. There are no established control tools available to reduce loiasis transmission in endemic regions. Although the use of repellents has been explored against other arthropods, their protective efficacy against Chrysops bites in humans has not yet been systematically evaluated. The aim of this randomized controlled clinical trial was to assess the efficacy of four commercial skin-applied arthropod repellents against Chrysops bites in a hyperendemic region in Gabon. ME
