Reference values for cardiac dimensions and function in a rural population living in a loiasis-endemic area of the Republic of the Congo: An echocardiographic study.

Echocardiography is crucial for diagnosis and management of cardiac diseases. Existing reference values may not be applicable to rural African populations due ethnicity related variability and limited data. This study aimed to establish reference values for echocardiographic parameters in a rural population. We assessed echocardiographic parameters in the adult population ( ≥ 18 years) from rural areas endemic for loiasis in the Republic of Congo. Reference values were established using participants with normal BMI, no hypertension/diabetes, and no significant echocardiographic abnormalities.

Female Loa loa worm polyinfection in human hosts.

Loiasis is widespread in Central Africa. Some acute symptoms are associated with high Loa loa microfilaraemia, but the relation between the latter and the adult worm burden infecting an individual with loiasis is still unclear. This study aims to determine whether polyinfection by several reproductive female worms could be assessed using genetic variation in the mitochondrial genome of microfilariae. Microfilariae were collected from the individuals' blood. An optimization of the DNA extraction method that provides enough genetic material and minimization of human host contamination was the fi

Loa loa Encephalopathy Following Treatment With Benzimidazole Derivatives: A Systematic Review.

Loiasis, a filarial vector-borne disease, is common in rural West and Central Africa. Benzimidazole derivatives albendazole and mebendazole are recommended as alternative treatments due to their perceived safety in hypermicrofilaremic patients. There is growing evidence that benzimidazoles might also lead to Loa loa-associated encephalopathy. In this systematic review we analyzed all available evidence of benzimidazole-associated encephalopathy. Literature was systematically searched in PubMed, Google Scholar, and WHO-VigiBase®, including conference abstracts and consultation of experts. Six p

Infectious Diseases in the Head and Neck with Eosinophilia.

Eosinophilia, defined as an elevated eosinophil count either in blood or tissue, has diverse implications for diagnosing and managing various diseases. Elevated eosinophil levels are often associated with conditions such as allergic reactions, autoimmune disorders, and specific infections. In the realm of head and neck pathology, eosinophilia can offer valuable insights into underlying infectious processes, which are often challenging to diagnose due to their overlap with other inflammatory and allergic conditions. This review describes the roles of blood and tissue eosinophilia in several inf

Association between Loa loa microfilaremia and anatomical hyposplenia in a rural area of the Republic of Congo: a population-based cross-sectional study.

BACKGROUND: Data suggest excess mortality is associated with loiasis, which is endemic to Central Africa, although the underlying mechanisms remain unknown. We hypothesized that there could be an association between Loa loa microfilarial densities (MFD) and lower spleen volume (SV) due to micro-obstruction linked to circulating microfilariae (mf). This could result in functional hyposplenia and a higher burden of infections. Our objective was to investigate the impact of L. loa MFD and malaria on spleen's bi-dimensional dimensions, volume, and parenchymal lesions. METHODS: We included 981 par

Loiasis: the missing piece in the neglected tropical disease agenda.

Loiasis remains one of the least prioritised filarial infections, yet its neglect undermines onchocerciasis elimination and limits progress toward global NTD goals. Recognising Loa loa as a neglected tropical disease would unlock resources, promote safe mass drug administration, and accelerate the World Health Organization's 2030 targets.

Activity of antifilarial drugs on microfilaremia in the treatment of loiasis: a systematic review.

BACKGROUND: Loiasis, caused by the nematode/filaria Loa loa, presents a major health burden in Central and West Africa. Despite the growing recognition of loiasis' medical significance, current antifilarial drugs remain inadequate in terms of efficacy and safety, particularly for individuals with hypermicrofilaremia. This systematic review aims to evaluate the efficacy of antifilarial treatment regimens for reducing L. loa microfilaremia and provide guidance on treatment strategies. METHODS: A systematic review was conducted to evaluate the efficacy of antifilarial treatment regimens on reduc

Association between arterial stiffness and Loa loa microfilaremia in a rural area of the Republic of Congo: A population-based cross-sectional study (the MorLo project).

BACKGROUND: Loa loa filariasis (loiasis) is still considered a relatively benign disease. However, recent epidemiologic data suggest increased mortality and morbidity in L. loa infected individuals. We aimed to examine whether the density of L. loa microfilariae (mfs) in the blood is associated with cardiovascular disease. METHODOLOGY: Using a point-of-care device (pOpmètre), we conducted a cross-sectional study to assess arterial stiffness and peripheral arterial disease (PAD) in 991 individuals living in a loiasis-endemic rural area in the Republic of the Congo. Microfilaremic individuals w

Development and validation of a long-read metabarcoding platform for the detection of filarial worm pathogens of animals and humans.

BACKGROUND: Filarial worms are important vector-borne pathogens of a large range of animal hosts, including humans, and are responsible for numerous debilitating neglected tropical diseases such as, lymphatic filariasis caused by Wuchereria bancrofti and Brugia spp., as well as loiasis caused by Loa loa. Moreover, some emerging or difficult-to-eliminate filarioid pathogens are zoonotic using animals like canines as reservoir hosts, for example Dirofilaria sp. 'hongkongensis'. Diagnosis of filariasis through commonly available methods, like microscopy, can be challenging as microfilaremia may w

Microfilariae Prevalence and its Association with Anemia Among First-time Blood Donors in Lambaréné, Gabon.

BACKGROUND: Anemia remains a significant public health concern in Gabon, particularly among children, adolescents, and females. Gabon is also home to two major species of filarial worms, Loa and Mansonella spp., which cause microfilaremia. The epidemiological nexus between hemoglobin (Hb) concentrations and microfilaremia in Gabonese first-time blood donors remains unknown. AIMS: To understand better the epidemiological relationship between anemia and microfilaremia to improve donor selection and management protocols. STUDY DESIGN: A retrospective cohort study. METHODS: This study was condu

Impact of repeated mass ivermectin administration using a community directed approach on L. loa infection in Chrysops silacea of the rain forest and forest savanna of Cameroon.

BACKGROUND: Loiasis is an endemic filarial infection in the rainforest zone of West and Central Africa. Repeated annual community-directed treatment with ivermectin (CDTI) delivered for several years to control onchocerciasis has been shown to reduce the prevalence and intensity of Loiasis in some Loa loa-Onchocerca volvulus co-endemic areas. However, the impact of these multiple rounds of CDTI on entomological indicators of loiasis transmission is not known, and was therefore assessed in this study in areas with contrasting histories of CDTI. METHODS: The study was conducted in the East, Nor

Analysis of diagnostic test outcomes in a large loiasis cohort from an endemic region: Serological tests are often false negative in hyper-microfilaremic infections.

BACKGROUND: The parasitic disease loiasis is associated with significant morbidity and mortality. Individuals with hyper-microfilaremia (greater than 20,000 microfilariae per mL of blood) may suffer from serious treatment-related or spontaneous adverse events. Diagnosing loiasis remains complex and primarily relies on direct parasite detection. In this study, we analyzed the performance of various diagnostic tests and the influence of parasitological and clinical factors on test outcomes in samples from individuals living in an endemic region. METHODS: Data and samples were collected from rur

Low diagnostic performance of thick blood smears of 50 µl in comparison with direct examination of 10 µl blood and the leukoconcentration technique of 5ml blood among loiasis-suspected patients with low microfilaremia in Gabon, Central Africa, using the STARD-BLCM guidelines.

BACKGROUND: The aim of this study was to determine performance indicators of thick blood smears of 50 µl (TBS-50), following the Standards for the Reporting of Diagnostic Accuracy Studies-Bayesian Latent Class Model (STARD-BLCM) guidelines. TBS-50 was compared with two common parasitological techniques-direct examination of 10 µl blood and a leukoconcentration of 5 ml-for the diagnosis of microfilaremic loiasis. METHODS: The study population was recruited among patients of the Department of Parasitology-Mycology-Tropical Medicine over a period of 1 year. Age, sex, symptoms, and eosinophilia v

Eosinophils, basophils and myeloid-derived suppressor cells in chronic Loa loa infection and its treatment in an endemic setting.

BACKGROUND: Chronic infection by Loa loa remains an unsolved immunological paradox. Despite harboring subcutaneously migrating adult worms and often high densities of microfilariae, most patients experience only relatively mild symptoms, yet microfilaricidal treatment can trigger life-threatening inflammation. Here, we investigated innate cell populations hypothesized to play a role in these two faces of the disease, in an endemic population in Gabon. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed numbers and activation of eosinophils and basophils, as well as myeloid-derived suppressor cell (MD

Safety and Short-term Efficacy of a Single Dose of 2 mg Moxidectin in Loa loa-Infected Individuals: A Double-Blind, Randomized Ivermectin-Controlled Trial With Ascending Microfilarial Densities.

BACKGROUND: In 2018, the US Food and Drug Administration approved the macrocylic lactone moxidectin (MOX) at 8 mg dosage for onchocerciasis treatment in individuals aged ≥12 years. Severe adverse reactions have occurred after ivermectin (IVM), also a macrocyclic lactone, in individuals with high Loa microfilarial density (MFD). This study compared the safety and efficacy of a 2 mg MOX dose and the standard 150 µg/kg IVM dose in individuals with low L loa MFD. METHODS: A double-blind, randomized, ivermectin-controlled trial of a 2 mg moxidectin dose was conducted in Cameroon between May and Ju

Imported Loiasis at a Clinical Reference Center in Germany: A Retrospective Case Series.

Loiasis is a rarely imported infectious disease that is often difficult to diagnose and treat. Here we describe clinical features and treatment outcomes of 11 patients with imported loiasis seen at a German reference center between 2013 and 2023. Clinical presentations varied by patient origin, with eye-worm migration and ophthalmological symptoms being more common among patients from endemic areas and Calabar swelling, subcutaneous swelling, and pruritus more prevalent among returning travelers from nonendemic regions. Eosinophil counts were higher in returning travelers. Diethylcarbamazine w

Clinical and epidemiological features of imported loiasis in Beijing: a report from patients returned from Africa.

BACKGROUND: Loiasis is one of the significant filarial diseases for people living in West and Central Africa with wide endemic area but is not seen in China. As economy booms and international traveling increase, China faces more and more imported parasitic diseases that are not endemic locally. Loiasis is one of the parasitic diseases that enter China by travelers infected in Africa. The better understanding of the clinical and laboratory features of loa loa infection will facilitate the diagnosis and treatment of loiasis in China. METHODS: The study targeted travelers who were infected with

Recurring Transient Tooth Pain as Newly Described Symptom of Migratory Loiasis: A Mixed-Methods Study in Rural Gabon.

Loiasis, a filarial pathogen exclusively endemic in central and western Africa, causes a wide spectrum of symptoms. Understanding the breadth of its clinical manifestations is of importance for adequate patient care and to understand its disease burden. Recurring transient pain in the oral cavity was reported as a self-perceived symptom of loiasis in in-depth interviews of patients in a high transmission region in Gabon. Pain was described as stabbing in character and transient for a few days in its temporal course. A quantitative epidemiological survey indicated that transient tooth pain was
