A review of loiasis and its vectors in Gabon.

Loiasis colloquially known as tropical eye worm is a neglected tropical helminthic disease that affects the rural poor population of Gabon. A systematic review of fifty-one documents (forty-five peer reviewed papers and six dissertations) over four decades has documented many knowledge gaps on loiasis and its vectors. Higher prevalence and high microfilaremia is typical wherever Chrysops vectors occur. The two main vectors were C. silacea and C. dimidiata along with the savanna species C. longicornis . The two main vectors were only caught around forested national parks at low density (<1 Chry

Reported healthcare-seeking of loiasis patients and estimation of the associated monetary burden in Gabon: Data from a cross-sectional survey.

BACKGROUND: Loiasis is a disease of relevance in endemic populations and there has been advocacy for its inclusion on the World Health Organization's neglected tropical diseases list. As loiasis-related healthcare-seeking behaviors and related costs are unknown, we aimed to evaluate these aspects in a population residing in an endemic region in Gabon. METHODS: Data were collected during a community-based, cross-sectional study assessing the disease burden due to loiasis. Diagnostics for microfilaremia were performed and a history of eyeworm was obtained. In addition, a standardized questionna

Focal spleen lesions in loiasis: A pilot study in Gabon.

BACKGROUND: Infection with the filarial nematode Loa loa, endemic in Central and Western Africa, has been associated with increased morbidity and mortality. A number of reports described the presence of spleen nodules, originating from degenerating microfilariae, in humans and animals infected with L. loa. The long-term consequences of this process on individuals chronically exposed to infection in terms of spleen function and possible link with excess mortality are unknown. The aim of this study was to evaluate the prevalence of focal spleen lesions, their evolution over time, and markers of

A novel antigen biomarker for detection of high-level of Loa loa microfilaremia.

BACKGROUND: Loiasis is a disease caused by the nematode Loa loa. Serious adverse events sometimes occur in people with heavy L. loa microfilaremia after ivermectin treatment. In regions of Central Africa where loiasis is endemic, this significantly impedes global elimination programs for lymphatic filariasis and onchocerciasis that use mass distribution of ivermectin. Improved diagnostic tests to identify individuals at increased risk of serious adverse events could facilitate efforts to eliminate lymphatic filariasis and onchocerciasis in this region. METHODS AND FINDINGS: We previously iden

Evaluation of a novel biplex rapid diagnostic test for antibody responses to Loa loa and Onchocerca volvulus infections.

BACKGROUND: Endemic to Central Africa, loiasis, caused by the vector-borne worm Loa loa, affects approximately 10 million individuals. Clinical manifestations include transient angioedema (Calabar swellings), migration of the adult worm under the eye conjunctiva (eye worm) and less specific general symptoms. Loiasis presents a significant public health challenge because L. loa-infected individuals can develop serious adverse events after taking ivermectin, the drug used to combat onchocerciasis. In this context, alternative interventions and rigorous diagnostic approaches are needed. Diagnosin

Variability of Loa loa microfilarial counts in successive blood smears and its potential implication in drug-related serious adverse events.

BACKGROUND: The standard method to diagnose Loa loa infection and quantify microfilarial density (MFD) is the microscopic examination of calibrated thick blood smears (TBSs). In 1950, it was noticed that successive L. loa MFD samples from a single capillary puncture could exhibit up to 20% variation. Although loiasis treatment allocation is based on MFD to prevent serious adverse events (SAEs), data on this variability are scarce. There are also no guidelines supporting the collection and analysis of one or two TBSs. METHODS: We assessed the variability of two successive L. loa MFD samples (M

Approaching onchocerciasis elimination in Equatorial Guinea: Near zero transmission and public health implication.

BACKGROUND: Onchocerciasis and lymphatic filariasis (LF) are endemic in Equatorial Guinea with notable variations in disease incidence between island and mainland regions. Historically, efforts to control and map these diseases were concentrated in Bioko Island, where loiasis is absent, allowing for targeted onchocerciasis interruption strategies. With the cessation of onchocerciasis transmission on Bioko and no reported cases on Annobon island, assessing the transmission status in the previously unaddressed mainland region has become imperative. Mapping efforts in mainland Equatorial Guinea h

Inflammation and hemostasis in loiasis before and after ivermectin treatment: A biological pilot cross-sectional study.

We assessed the impact of loiasis and its treatment with ivermectin on hemostasis and inflammation in 38 adults in Cameroon. Participants were divided into four balanced groups based on their Loa loa microfilarial densities. At baseline, eosinophils and platelets increased with microfilarial densities (p<0.001 and p = 0.044, respectively). At day 4 following ivermectin administration, median eosinophils rose from 171/μL to 1,095/μL (p<0.001) and median D-dimers increased from 582 ng/mL to 745 ng/mL (p = 0.024) with a median relative variation of +34.7%. Evaluation of knowledge, attitude and practices towards loiasis in the rural community of Sindara, in central African Gabon.

BACKGROUND: More than 20 million people are infected with L. loa, and around 40 million live in high or intermediate-risk areas in West- and Central Africa. Although loiasis is associated with significant morbidity and excess mortality, little is known about the perception of loiasis by affected communities. This study assessed the knowledge, attitudes, and practices in the rural population of Sindara, Gabon, a region characterized by high loiasis prevalence. METHODS: A community-based cross-sectional survey was conducted in Gabon between January and June 2022. During systematic door-to-door

A picture speaks a thousand words: A case of ocular Loa loa.

The 'African eye worm', also known as Loa loa, is filarial nematode that is commonly, transmitted by deer flies. A history of recent travel to endemic countries near western, Africa is common in patients with Loiasis. While a majority of patients infected with Loa, loa are asymptomatic, common presentations of Loiasis include localized areas of, edema, Calbar swellings, and ocular symptoms secondary to the migration of worms, through the spaces within the eye. This case illustrated presents a healthy 31-year-old, man with no history of recent travel who presented after his friend reported that

Profile of loiasis infection through clinical and laboratory diagnostics: the importance of biomarkers.

BACKGROUND: Detection of Loa loa microfilariae in peripheral blood is insensitive given only 30% of individuals are microfilaraemic while 70% are amicrofilaraemic with a variety of clinical signs. Biomarkers may improve the diagnosis of loiasis. METHODS: A total of 545 individuals exposed to L. loa were analysed using clinical data collected through a questionnaire (requesting information on eye worm, Calabar swelling, pruritis) and detection of microfilariae, immunoglobulin G4 (IgG4), DNA and antigens using microscopy, enzyme-linked immunosorbent assay (ELISA), quantitative polymerase chain

Longitudinal study of cross-reactive antigenemia in individuals with high Loa loa microfilarial density reveals promising biomarkers for distinguishing lymphatic filariasis from loiasis.

BACKGROUND AND METHODS: Circulating Loa loa antigens are often detected in individuals with heavy L. loa infections by diagnostic tests for lymphatic filariasis (LF) caused by Wuchereria bancrofti. This is a major challenge to LF mapping and elimination efforts in loiasis co-endemic areas. However, it also provides an opportunity to identify antigen biomarkers for loiasis. To determine which L. loa antigens might be promising biomarkers for distinguishing true LF from loiasis, we screened for L. loa antigens in a group of individuals with heavy L. loa infections living in the Okola Health Dist

Evaluating post-treatment Loa loa microfilarial densities to classify serious adverse events caused by ivermectin: a retrospective analysis.

BACKGROUND: The elimination of onchocerciasis requires increasing ivermectin treatment coverage in communities hypoendemic for onchocerciasis. In areas where loiasis is co-endemic, this approach is complicated by the risk of serious adverse events following treatment with ivermectin in individuals with a high Loa loa microfilarial density (MFD). We aimed to evaluate the extent to which the pre-treatment MFD can be inferred from post-treatment MFDs. METHODS: For this retrospective analysis, we used data from seven clinical or community trials (six were used for the main analysis and one for th

Temporal variability of Loa loa microfilaraemia.

BACKGROUND: The diurnal periodicity of Loa loa microfilaraemia is well known but few studies have documented the short- and long-term stability of microfilarial density. It seems stable over time at the community level, but significant variations have been observed at the individual level. METHODS: We assessed the temporal variability of L. loa microfilaraemia at 5-day, 1-month and 16-month intervals and analyzed the influence of sex, age, level of microfilaraemia, temperatures and time of sampling on this variability. RESULTS: At the community level, L. loa microfilaraemia is very stable ov

Development and validation of small animal models for onchocerciasis and loiasis microfilaricide discovery.

BACKGROUND: Onchocerciasis (river blindness) caused by the filarial worm Onchocerca volvulus is a neglected tropical disease that affects the skin and eyes of humans. Mass drug administration with ivermectin (IVM) to control the disease often suffers from severe adverse events in individuals co-injected with high loads of Loa loa microfilariae (mf). Thus loiasis animal models for counter-screening of compounds effective against onchocerciasis are needed, as are the corresponding onchocerciasis screening models. The repertoire of such models is highly limiting. Therefore, this study was aimed a

Filariasis research - from basic research to drug development and novel diagnostics, over a decade of research at the Institute for Medical Microbiology, Immunology and Parasitology, Bonn, Germany.

Filariae are vector borne parasitic nematodes, endemic in tropical and subtropical regions causing avoidable infections ranging from asymptomatic to stigmatizing and disfiguring disease. The filarial species that are the major focus of our institution's research are Onchocerca volvulus causing onchocerciasis (river blindness), Wuchereria bancrofti and Brugia spp. causing lymphatic filariasis (elephantiasis), Loa loa causing loiasis (African eye worm), and Mansonella spp causing mansonellosis. This paper aims to showcase the contribution of our institution and our collaborating partners to fila

Cases of Loiasis Among a Congolese Family: Screening for Loiasis Among Newly Arrived Refugees From Endemic Areas.

Loiasis is a parasitic infection caused by the filarial nematode Loa loa within endemic regions of West and Central Africa. These regions include areas co-endemic for other nematode infections. Although loiasis is rarely seen in the United States (US), primary care providers who regularly see refugees from endemic areas should be aware of its clinical presentation, diagnostic work-up, and initial management. Given the challenges of diagnosing loiasis, especially in low prevalence settings, we present cases of four family members, two of whom were diagnosed with loiasis, and discuss an approach

Excess Mortality Associated With Loiasis: Confirmation by a New Retrospective Cohort Study Conducted in the Republic of Congo.

BACKGROUND: Loiasis (Loa loa filariasis) is considered a benign disease and is currently not included in the World Health Organization's (WHO's) list of Neglected Tropical Diseases, despite mounting evidence suggesting significant disease burden in endemic areas. We conducted a retrospective cohort study to assess the mortality associated with L. loa microfilaremia in the Southwestern Republic of Congo. METHODS: The cohort included 3329 individuals from 53 villages screened for loiasis in 2004. We compared mortality rates in 2021 for individuals initially diagnosed as with or without L. loa m
