Biomechanical assessment of repetitive brain injury in pedestrian collisions based on wavelet packet energy analysis.

PURPOSE: Existing brain injury criteria mainly focus on assessing injuries in a single collision and lack of assessment of injuries in real-world multiple collisions. METHODS: Based on the head kinematics signals of rats in repeated collisions and pedestrian accident data, the frequency domain characteristics of linear and rotational motion signals were analyzed by wavelet packet transform. Finally, the ROC curve was used to evaluate the effectiveness of injury index prediction. RESULTS: The results of animal experiments show that as the impact intensity increases, the energy in the frequenc

Preterm Risk Factors, Epigenetic Age, and Neurodevelopmental Outcomes.

IMPORTANCE: Studies examining biological factors associated with neurodevelopment in the preterm population are limited. It was previously demonstrated that epigenetic age acceleration was associated with better neurodevelopmental outcomes among children born preterm. It is unclear whether this association is modified by clinical and environmental preterm risk factors. OBJECTIVE: To examine whether the association between epigenetic aging and neurodevelopmental outcomes is modified by preterm brain injury and socioeconomic status. DESIGN, SETTING, AND PARTICIPANTS: This was a prospective coh

The CBI-M framework for characterization of traumatic brain injury: Evaluation of performance using data from the CENTER-TBI and TRACK-TBI prospective cohort studies.

BACKGROUND: The CBI-M Framework, incorporating Clinical, Biomarker, Imaging, and Modifier pillars, was recently proposed to provide a comprehensive characterization of patients with acute traumatic brain injury (TBI). Such detailed characterization reflects the clinical and biological heterogeneity of TBI, and is considered essential to inform appropriate clinical management. Still unclear is the extent to which CBI-M captures clinically relevant heterogeneity in relation to diagnosis, clinical care pathways, and longer-term outcome. Prior to broad implementation into clinical practice and res

[Protective effect of bloodletting at twelve Well-points of hand on neurological function by regulating cortical microglia/macrophage polarization through the IL-10/STAT1 pathway in mice with traumatic brain injury].

OBJECTIVES: To observe whether pricking blood therapy at the twelve Well points of the hand (Well-point bloodletting) can regulate the cortical phenotypic polarization of microglia and macrophages via the interleukin-10 (IL-10)/signal transducer and activator of transcription 1(STAT1) pathway, suppress excessive neuroinflammation and neuronal apoptosis, and thereby alleviate neurological deficits induced by traumatic brain injury (TBI) in mice. METHODS: Male C57BL/6 mice were used in the present study which is composed of 3 experiments. The TBI model was established by using an electronically

Cardiac troponin T and 28-day mortality in traumatic brain injury: a retrospective cohort study and machine learning prediction model.

BACKGROUND: Traumatic brain injury (TBI) frequently induces cardiac dysfunction via the brain-heart axis, yet the prognostic value of cardiac troponin T (cTnT) in TBI remains incompletely characterized. This study investigated thye association between cTnT levels and 28-day all-cause mortality in TBI patients and developed a machine learning-based prediction model. METHODS: This retrospective cohort study included 837 adult TBI patients from the MIMIC-III/IV databases (development cohort) and 121 patients from a single Chinese center (external validation cohort). The first cTnT measurement wi

Pre-surgery statin use and outcomes in operated chronic subdural haematomas: a nationwide cohort study.

BACKGROUND: Chronic subdural haematoma (cSDH) is common in older adults, and medication exposures may influence postoperative outcomes. We assessed whether pre-surgery statin use is associated with short- and long-term outcomes. METHODS: We conducted a nationwide registry study of adults undergoing surgical evacuation of traumatic or non-traumatic cSDH (6/2004-12/2018) in Finnish university hospitals. Pre-surgery statin use was defined from national prescription records using a pill-counting approach. The primary outcome was 30-day all-cause mortality. Secondary outcomes were 30-day reoperati

Advanced CT Myelography for Spontaneous Cerebrospinal Fluid Leaks.

Spontaneous intracranial hypotension is a frequently debilitating condition that is most commonly caused by loss of cerebrospinal fluid (CSF) from the spine. The most frequently occurring types of spontaneous spinal CSF leaks that cause spontaneous intracranial hypotension are ventral dural tears, lateral dural tears, and CSF venous fistulas, but other leak types are recognized including sacral dural tears or fistulas into vascular malformations. Localization and characterization of spinal CSF leaks with CT myelography facilitates subsequent targeted treatment. A tailored approach based on the

Indications for flexible bronchoscopy in elderly patients (≥ 75 years) with hypoxemic respiratory failure not eligible for intensive care: Expert opinion based on a Delphi study.

BACKGROUND: The indications for flexible bronchoscopy (FB) remain poorly defined in elderly patients (≥ 75 years) with hypoxemic respiratory failure (HRF) (O₂ ≥ 5 L/min) who are not eligible for intensive care. The lack of specific data highlights the need to develop evidence-based recommendations in this setting. STUDY DESIGN AND METHODS: A Delphi study was conducted among pulmonologists from the Groupe d'Endoscopie Thoracique et Interventionnel Francophone (GETIF). The first-round questionnaire included 18 statements. Consensus was defined as ≥ 70% of experts expressing agreement or disagre

Comprehensive approaches to Traumatic Brain Injury: bridging physical, emotional, and social dimensions for global health care.

INTRODUCTION: Traumatic Brain Injury (TBI) is a significant global health issue, affecting millions annually and resulting in physical, cognitive, emotional, and social challenges. While medical management, including neurosurgical interventions and rehabilitation, is well-documented, the emotional, psychological, and social dimensions of TBI remain underexplored. This gap in understanding hinders the development of holistic approaches to care. METHOD: This article adopts an interdisciplinary approach, synthesizing existing literature to analyze the multifaceted impacts of TBI, as a narrative

A Rare Encounter With Peripheral Dentinoma: Clinicopathological Insights.

Odontogenic tumors comprise a diverse group of lesions arising from tooth-forming tissues and often show overlapping clinical, radiographic, and microscopic features, creating diagnostic challenges. Peripheral dentinoma is an exceptionally rare dentin/dentinoid producing odontogenic tumor characterized by dysplastic dentin or dentinoid formation, with odontogenic epithelial and ectomesenchymal components that may vary according to the histologic pattern. We report a case of peripheral dentinoma in a 24-year-old male patient in the lower right posterior tooth region. Radiographic examination re

Traumatic Brain Injury in First Responders: From Hidden Burden to Occupational Health Action.

Traumatic brain injury (TBI), including concussion, is an important but often underrecognized occupational injury among first responders. Despite frequent exposure to circumstances that increase the risk of TBI, law enforcement officers, firefighters, and emergency medical services personnel often lack consistent pathways for injury recognition, reporting, clinical assessment, rehabilitation, and safe return to duty. This topical issue of the Journal of Head Trauma Rehabilitation brings together emerging evidence on TBI prevalence, underrecognition, mental health, functional recovery, and occu

Moderate and Severe Traumatic Brain Injury Outcomes-A 20 Year (2002-2021) Analysis.

OBJECTIVE: Although recognised as a leading cause of death and disability, it is unclear whether current treatment strategies have improved outcomes from blunt moderate to severe traumatic brain injuries (msTBI) due to a lack of available epidemiological data. This study sought to determine if in-hospital mortality and early functional outcomes of msTBI patients have changed over 20 years. Early functional outcomes were determined through the surrogate measure of the proportion of patients discharged directly to home. METHODS: A longitudinal cohort study using The Alfred Health Trauma Registr

Cinchonidine Ameliorates Traumatic Brain Injury-Related Neurobehavioral Deficits via the SRC/RAF1/NF-κB Pathway to Attenuate Mitochondrial Dysfunction.

Cinchonidine (CD), a metabolite derived from gut microbiota, shows promise in protecting against traumatic brain injury (TBI), where mitochondrial dysfunction is a key driver of secondary damage. Using a mouse model of controlled cortical impact (CCI) and an in vitro primary neuron injury model, CD administration resulted in improved physiological status (weight recovery, stabilized heart rate) and behavioral outcomes (increased travel distance and speed) in TBI mice. Histological analysis showed reduced hippocampal neuronal damage, decreased pro-inflammatory cytokines (interleukin‑1β, interle

Rural-Urban Parity and Socioeconomic Gaps in Traumatic Brain Injury Outcomes.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of death and disability in the United States. Rural residence and low community income are independently associated with poorer health outcomes, mediated by variations in injury mechanism and healthcare access. We evaluated the impact of geographic location on injury patterns and acute-care metrics in adults with TBI treated at Maryland's statewide Level I trauma center. METHODS: Adults (≥18 years of age) with radiographically confirmed TBI admitted between 2017 and 2021 were identified retrospectively from the institutional trauma r

Biomaterial characteristics and biofluid composition differentially modulate protein adsorption and macrophage phenotype on common surgical polymers with proteome-transcriptome concordance.

Dysregulated macrophage responses to common surgical polymers contribute to implantable medical device failure. While biomaterial characteristics are known to influence macrophage behavior, the contribution of the local biological milieu and the adsorbed proteins remains poorly understood. Here, we investigate how biomaterial physicochemical properties and biofluid composition differentially modulate adsorbed proteomes and macrophage transcriptomes on common surgical polymers as well as explore the link between the latter two. We first quantify the stiffness, roughness, wettability and protein

Head, face, and neck injury patterns across the life course: a 20-year NEISS analysis of 2,345,287 emergency department visits in the U.S.

BACKGROUND: Head, face, and neck injuries impose a substantial burden for emergency departments (ED); the mechanisms driving them across the life course however remain poorly understood at the population level. Large-scale, integrated analysis spanning anatomical regions, injury mechanisms, age groups, settings, and associated products or environmental surfaces offers an opportunity to identify the patterns and vulnerable populations that smaller or shorter-term studies cannot resolve. METHODS: We queried National Electronic Injury Surveillance System (NEISS) records from 2006 to 2025 for inj

Retained Colorectal Foreign Body After Suspected Chemical Incapacitation: A Surgical and Forensic Challenge.

Retained rectal foreign bodies are an uncommon but clinically significant surgical emergency. We report the case of a 74-year-old male with multiple medical comorbidities who presented with progressive rectal pain and obstipation several days after an episode of suspected drug-facilitated assault. Plain radiography identified a large metallic aerosol can, and attempts to retrieve it under general anesthesia using surgical clamps and a magnetic probe failed. Intraoperative fluoroscopy demonstrated that the can had migrated proximal to the rectosigmoid junction into the left colon, precluding tr
