L-shaped scalp flap for moustache reconstruction in a patient with an acid burn of the face.

An L-shaped scalp flap based on the parietal branch of the superficial temporal artery is described to reconstruct the moustache in a young male patient who suffered from nitric acid burns of the face. This provides a simple technique as an alternative to microvascular technique for moustache reconstruction. The donor area morbidity was minimal.

Radiation-induced changes in the central nervous system and head and neck.

Radiation therapy of the brain, neck, and spine can be associated with radiation-induced changes that are increasingly evident radiologically. These changes affect the central nervous system (focal necrosis, diffuse white matter injury, atrophy, mineralizing microangiopathy, telangiectasia, optic neuropathy, large vessel vasculopathy), bone (fatty replacement of marrow, osteoradionecrosis, sinus inflammation), and superficial and deep soft tissues and include neoplasia. Focal necrosis, which most commonly occurs at the treatment site, is seen at computed tomography (CT) and magnetic resonance

Pseudoaneurysm of the anterior superficial temporal artery.

Pseudoaneurysm of the superficial temporal artery must be considered in the differential diagnosis of masses of the lateral forehead and temporal fossa. While the first reported case of a temporal artery aneurysm was reported by Thomas Bartholin in 1740, there is scant mention of this lesion in the plastic and maxillofacial surgical literature. Pseudoaneurysms can arise in the forehead and scalp as a result of blunt traumatic impingement of the superficial temporal artery against the calvarium. The anterior branch of the artery is most vulnerable, because in the lateral forehead it courses ove

Free temporoparietal fascia flap in reconstruction of the lower extremity.

Twenty-one patients with open wounds of the non-weight-bearing foot or the ankle underwent coverage with a free temporoparietal fascia flap and split-thickness skin graft. Our indications for this thin, well-vascularized flap included osteomyelitis and exposed tendon, bone, or fixation hardware. The pliability of the flap allowed superior restoration of natural anatomical contours. Only one flap was completely lost (95.2% overall success). There were four cases (20%) of partial flap loss, none of which required flap revision or a new flap. Four patients (19%) experienced transient donor-site a

Entire scalp replantation: case report and review of the literature.

A case of successful replantation of a totally avulsed scalp in a 40-year-old woman is presented. During a 19-hr surgical procedure, bilateral superficial temporal arteries and veins were anastomosed. The postoperative course was uneventful, except for partial necrosis of the distal end of the avulsed flap, i.e., the occipital region. Since the first successful scalp replantation using microsurgical technique was reported in 1976, the authors have found at least 32 subsequently reported cases. Of these, there were 22 cases of entire scalp avulsion (more than 80 percent of the scalp), including

A study of the supraorbital nerve.

A detailed description of the anatomical relationships of the supraorbital nerve as it courses across the forehead and under the scalp cannot be found in most anatomy textbooks, and illustrations of the nerve beyond the superior orbital rim frequently misrepresent its course. Because the supraorbital nerve is a structure at risk in many plastic surgical techniques, the plastic surgeon would benefit from a clearer understanding of its anatomy and function. The supraorbital nerve was studied anatomically in 12 (24 half-head) fresh cadaver specimens, and its sensory distribution was studied in 30

Microvascular replantation of a completely avulsed scalp.

Total scalp avulsion is an unusual injury. To obtain good functional and cosmetic results, immediate revascularization using microsurgical techniques and replacement of the scalp in the correct anatomical position is the procedure of choice. This article describes a case report of successful replantation of a totally avulsed scalp from a 26-year-old male. Bilateral superficial temporal vessel anastomoses were performed using reversed autologous long saphenous vein grafts on the right side and direct repair on the left. Points of technique are discussed and the literature is reviewed.

[Cross transposition of expanded scalp flaps for the treatment of postburn cicatricial alopecia].

When the area of alopecia is very large, the normal scalp is not sufficient to cover the defect resulting from excision of the alopecia. From August 1987 to December 1989, 8 cases with large defect of scalp are repaired by means of expanded cross scalp flaps with good result. Among 8 cases, 7 are male and 1 female, and the largest area of alopecia is 300 cm2 and the smallest 112 cm2, with a mean of 191 cm2. Two expanders are buried under the galea aponeurotica on both sides of alopecia, and the scalp is expanded. The central part of the expanded scalp is chosen to form the "major flap", the la

Blinded comparison of cranial CT and MR in closed head injury evaluation.

PURPOSE: To compare CT and MR in the evaluation of acute head injury. METHODS: One hundred seven consecutive patients who were referred to the emergency department and underwent both MR and CT cranial examinations within 48 hours were retrospectively reviewed. The films were interpreted by two neuroradiologists blinded to all patient information. RESULTS: The sensitivity of MR was significantly higher than that of CT for the detection of contusion, shearing injury, subdural and epidural hematoma, and sinus involvement. The sensitivity of CT was significantly higher than that of MR for the ev

Operative sepsis in neurosurgery: a method of classifying surgical cases.

Neurosurgical operations have traditionally been classified along the lines of general surgical procedures. A prospective study, during an 18-month period, was undertaken in 2249 patients undergoing neurosurgical procedures to establish and evaluate a method of classifying surgical cases by the use of specific neurosurgical criteria. Patients were placed in one of five categories according to the level and type of contamination at the time of surgery. Infection included all abnormal wounds and was documented as deep when infection occurred beneath the galea (subgaleal pus, osteitis, abscess/em

[Fascia superficialis temporalis in facial reconstructive surgery. Apropos of 35 cases].

The fascia superficialis temporalis (FST), one of the planes of cranial cover, can be raised to form an axial pedicle flap or a vascular island flap on superficial temporal vessels. Its fibrovascular structure, the area able to be raised and the possible addition of other superficial tissues (scalp, forehead skin) and deep tissues (calvarial bone) mean that this flap can be used in surgery of the external and intracavitary facial contours and shapes. The authors report their experience of 35 facial reconstructions: 7 ear reconstructions, 5 suspensions of a paralysed labial commissure, 10 conju

Total scalp replantation based on one artery and one vein.

A successful replantation of a totally avulsed scalp, including both eyebrows, with only one arterial and one venous anastomosis to the superficial temporal vessels is described. Apart from a small partial skin necrosis of the right eyebrow, the entire transplant survived. Subsequently, the patient required only minor additional skin grafting, correction of scars with a skin expander, and reconstruction of the eyebrow with hair micrografts. Replantation of the total scalp based on two or more vessels has previously been recommended and reported elsewhere. In the present case, complete survival

Clinical experience with electron pseudoarc therapy.

Between November 1986 and June 1990, 24 patients were treated with electron pseudoarc therapy at McGill University. There were 21 females and three males aged 27 to 81 years (median 62 years). 17 patients, nine of whom had received previous conventional locoregional irradiation, were treated palliatively for locally extensive breast carcinoma. Eight of these 17 patients achieved a complete, and six a partial, response to treatment; nine subsequently developed evidence of progressive disease within the treatment field after intervals of one to 27 months (median 5 months) following therapy. Eigh

Microsurgical replantation of the avulsed scalp: report of six cases.

This article reports the successful microsurgical replantation of six scalps avulsed for over 3 to 11 hr, after trauma and before repair, including five total and one partial avulsion. The authors believe that one key to successful replantation is effective vessel anastomoses. In the reported series, three scalps were reconstructed with two superficial temporal vessels and in five cases, with a 1:1 ratio between arteries and veins. The partially avulsed scalp was replanted successfully with only one artery and one vein. The management of postoperative complications is discussed.

Hair growth following scalp microvascular flap transfer for baldness due to burn injury.

A female patient, 8 years of age, presented with baldness of the right scalp following deep scalds from boiling soup landing on the head, neck, and chest. The depth of the burn was severe enough to cause baldness. She was primarily advised to wear a wig to address the problem of baldness on one side. Surgery was planned to use uninjured scalp skin to offer hairy skin coverage of the bald site. A left scalp skin flap (2.5 by 7 cm) based on the superficial temporal artery and vein was transferred to the bald area, with microvascular anastomosis to the superficial temporal vessels on the right si

Cutaneous complications of artificial hair implantation: a pathological study.

Five patients who developed severe cutaneous complications after artificial hair implantation were subjected to a scalp biopsy. The pathology showed the presence of hyperplastic epidermal proliferations that produced infundibulum-like structures around the implanted fibers in the superficial dermis. A dense acute inflammatory infiltrate surrounded the artificial fibers in the superficial dermis. In the deep dermis a granulomatous infiltrate was present whereas in the hypodermis the inflammatory infiltrate was sparse and the fibers were embedded in fibroplasia. The pathology of a patient who di

[Jump flap with pedicle containing superficial epigastric artery for treatment of exposed cranium after electric burn. Report of 9 cases].

Nine patients with denuded cranium after electric burn were treated by using jump flaps with pedicle containing the superficial epigastric artery. The pedicle flap was first transplanted to the burned area on the wrist, and then retransplanted to the cranial defect. It was successful in all cases. The healing time was only one half of that for the traditional method consisting of multiple trephinings through the outer table and then autoskin grafting on the granulations. It also created a better ground for hair grafting later on. Comparing with free flaps or scalp flaps, it was easier to opera

Subperiosteal subtemporal approach: technique and applications.

The authors describe a craniofacial surgical approach that they have used since 1987. Based on existing anatomical continuity between the subtemporal plane and the cranial periosteum, it consists of a direct subperiosteal subtemporal cleavage. Preservation of continuity between the muscle and the periosteum allows the operator to return the temporalis muscle to a normal position at the end of the operation when the scalp is sutured. This approach avoids superficial cleaving of the temporalis muscle or section and reinsertion to the fronto-orbital rim. It also allows access to the upper third o

[Microvascular anastomosis in replantation of 6 avulsed scalps].

From 1990 to 1991, 6 avulsed scalps (total avulsing in 5, partial avulsing in 1) were replanted successfully. Follow-up for over 6 months showed that the hair of the replanted scalps grew normally in all the 6 patients. We think that the key to successful replantation is to anastomose the vessels effectively. The scalp can survive just with its superficial temporal vessels on both sides and with an artery-vein ratio of 1:1. The management of postoperative complications is discussed.
