The volume limitation of the galeal temporalis flap in facial augmentation.

Galeal temporalis flaps based on the superficial temporal vessels have been used for facial augmentation and can be extended to the contralateral side beyond the midline in order to achieve maximum volume. In five patients, the volumes of extended galeal flaps were measured intraoperatively using a water displacement method. The calculated volume varied between 28 and 38 cm3. Experience with this flap showed satisfactory results with no complications; therefore, it is concluded that the extended galeal temporalis flap may be a first choice in the correction of facial soft tissue deficits less

Late bleeding after rhytidectomy from injury to the superficial temporal vessels.

Five healthy, normotensive women, whose mean age was 49.8 years, developed expanding hematomas between 8 and 10 days (average 9 days) after rhytidectomy. In each patient, the bleeding vessel could be identified: In two, it was the parietal branch of the superficial temporal artery; in two, it was the parietal branch of the superficial temporal vein; and in one, it was the superficial temporal artery immediately before its branching. Contributing factors may have been sudden physical exertion in four of the five patients and in another salicylate ingestion. Several measures can help avoid late

Avulsion of the scalp: which one is the best artery for anastomosis?

We present a patient who underwent successful replacement of a large avulsed scalp with microvascular anastomosis of one artery and three veins. The left occipital artery in the avulsed scalp and a frontal branch of a right superficial temporal artery in the recipient forehead were anastomosed. Three veins in the occipital area in the avulsed scalp were anastomosed via vein grafts to three veins in the frontal area, respectively. For successful replacement, the selection of the vessels depends on each individual case. To give easy anastomosis, a good cosmetic result, and a comfortable postoper

Microsurgical replantation of two totally avulsed scalps.

We describe our experience with two patients in whom total avulsion of the scalp occurred. Successful replantation was carried out by microvascular surgery, with 90 and 95% survival of the scalp. Normal hair regrowth and ability to raise the eyebrows was achieved in both cases. A single superficial temporal artery was anastomosed, providing excellent perfusion of the entire scalp. Microvascular replantation should be attempted with all scalp avulsions, regardless of appearance and mechanism of injury. The patient must be transferred to a medical center with comprehensive microvascular surgery

Total scalp, ear, and eyebrow avulsion: aesthetic adjustment of the replanted tissue.

A case of successful replantation of a total scalp avulsion, including both ears and one eyebrow, is presented with bilateral anastomoses to the superficial temporal vessels. While over 90 percent of the replant survived, partial loss of occipital and posterior neck skin and the inferior half of the left ear occurred. The patient subsequently required additional procedures, including skin grafting, scar revisions, and staged ear reconstructions. Thus, while microsurgery often provides the only hope for tissue salvage, there is a frequent need for additional revisions and aesthetic adjustment o

The vascularized fascia of the scalp.

The fascial layers of the temporal and occipital regions of the scalp were examined in 11 fresh cadavers. In the temporal region, three independently vascularized layers were isolated, all of which could be elevated on a single superficial temporal artery, but separated to remain independently vascularized from specific branches of this parent trunk. In the occipital area, the occipital vessels could be dissected to yield a long pedicle for an independent, fascial territory that could be transposed locally or elevated as a free flap and that will, in all likelihood, carry vascularized occipita

Evaluation of skin stapling for wound closure in the emergency department.

Automatic skin staplers have been commonly used for surgical wound closure for many years. The efficiency and ease of placement of skin staplers make them an attractive alternative to suture repair of selected lacerations in the emergency department. Emergency physicians, however, have been reluctant to use staplers in the ED. We evaluated skin staples in 76 patients presenting with 87 lacerations to the scalp, trunk, or extremities, excluding hands and feet. Patients returned to the ED in two and seven to ten day for wound check and staple removal. Skin stapling was assessed for efficiency, c

Upper lip reconstruction: use of the free superficial temporal artery hair-bearing flap.

The use of the superficial temporal artery hair-bearing flap applied to the repair of burn injuries to the upper lip of two male patients is described. It is a relatively simple method of providing tissue to the injured lip with advantages over traditional methods in that it is single-staged and provides the possibility for hair growth in the form of a camouflaging moustache. Donor site morbidity is minimal. When combined with additional reconstructive procedures in the multiply-injured male face, it provides a reasonable expectation for the patient of an acceptable appearance.

[A case of multiple superficial temporal artery pseudoaneurysms following craniotomy].

A case of multiple superficial temporal artery (STA) pseudoaneurysms following craniotomy is reported and a review of the literature is made. The patient was a 17-year-old male who was hospitalized as he developed right hemiparesis 4 hours after golf ball injury to the left temporal region. He had been diagnosed as hemophilia A one year prior to the injury. CT scan showed right parietal intracranial hematoma with mass effect when he was submitted to emergency craniotomy. At surgery a linear scalp incision was made over the left temporal area after high factor VIII concentrates (con-VIII), 2,00

Facial and scalp burns from hair activator products.

Review of 1,008 admissions over the past two years to the Detroit Medical Center Burn Unit revealed 227 patients with facial or scalp burns. Of these, 12 patients sustained burns directly as a result of accidentally igniting a hair activator product called "Jheri-Curl." Eleven patients sustained superficial partial-thickness burns and were adequately treated with Dermaide. One had deeper burns requiring split-thickness skin grafting.

Successful replantation of a totally avulsed scalp.

A case of successful replantation of a totally avulsed scalp in a 19-year-old woman is presented. Although severely traumatized, about 90 percent of the surface of the replanted scalp survived. Only one artery (left superficial temporal) and four veins were anastomosed, sufficient enough to revascularize the entire scalp.

Clinical applications of free temporoparietal flaps in hand reconstruction.

Vascularized temporoparietal fascia can be used to cover deficits up to 14.0 X 12.0 cm in the hand or fingers. The tissue is thin, pliable, and has a smooth gliding surface advantageous to tendon function. Through scalp incisions, with meticulous yet straightforward dissection, this fascia can be isolated as a vascular island based on the superficial temporal artery system. Vessels are large and the anatomy is constant. Technique and clinical applications are discussed in detail.

Cosmetic repair of chronic head restraint defects.

Surgically implanted head restraint devices have been used in nonhuman primate research for more than a decade. Superficial scalp defects are created by the implantation procedures and remain after the devices are removed, a factor that occasionally hinders continued use of the animals by investigators on esthetic grounds. A technique is described whereby the scalp defects are surgically repaired, resulting in a good cosmetic appearance and effectively increasing the potential for reuse of these animals.

Soft cup vacuum extraction: a comparison of outlet delivery.

One hundred eighteen nulliparous patients delivered under conduction anesthesia who met the standard criteria for low forceps delivery were randomly assigned to one of three groups to be delivered by low Tucker-McLane forceps, Silastic vacuum extraction system, or Mityvac vacuum extractor. Significant maternal soft tissue trauma was identified in 48.9% of the forceps group, 36.1% of the Silastic group, and 21.6% of the Mityvac group. Superficial fetal scalp changes were found in 71% of the forceps group, 44% of the Silastic group, and 46% of the Mityvac group. Of these, cephalhematomatas were

Fatal complication of the "bonnet bypass": case report.

A patient with transient ischemic attacks and an occluded right common carotid artery underwent a left superficial temporal artery to right middle cerebral artery vein graft (bonnet bypass). One week after operation, minor trauma to the scalp caused avulsion of the anastomosis and a massive fatal acute subdural hematoma. The mechanism of the vascular avulsion and precautions to be taken to avoid such problems in other patients undergoing this procedure are discussed.

[Association of a melanoma and a spinocellular epithelioma on a burn scar of the scalp].

A thirty-nine-year-old caucasian man presented with an ulceration of the scalp, developing on an alopecia. The scalp indeed had been burnt by boiling water when the boy was two. During childhood and youth microtraumatisms induced superficial wounds which always healed easily. Nine months ago, a new ulceration followed a new trauma; the wound did not heal, and lay on a dark brown area. A biopsy diagnosed a malignant melanoma. The whole alopecic area was resected, under the ulceration and the pigmented area, as well as the external surface and the diploe. The defect was covered with a free trans

Scalp avulsions and review of successful replantation.

Microvascular surgery has greatly altered the handling of scalp avulsions. Review of the injury mechanism and of successful replantations has yielded some basic axioms concerning treatment of this tragic injury. Generally, larger avulsions contain vessels that are easily identified and anastomosed, and therefore have a better prognosis. Long periods of ischemia do not appear to threaten graft viability. The superficial temporal artery is the most reliable artery for scalp replantation; accompanying veins can normally be used for venous drainage, though vein grafts are often needed due to vesse

Traumatic arteriovenous fistulae of the scalp.

Two cases of traumatic arteriovenous fistula of the scalp are described. To our knowledge, only six such cases with angiographic correlation have been previously reported in the literature. While the clinical diagnosis of arteriovenous fistula of the scalp is often not difficult, angiography plays an important role in guiding the surgeon by demonstrating multiplicity of feeding vessels and their origin from various trunks. One of our cases showed feeding vessels from the occipital artery and cervical branches of the vertebral and subclavian arteries. Our second case revealed superficial tempor

Superficial temporal arteriovenous fistula.

A case of post-traumatic arteriovenous fistula involving the superficial temporal artery is reported. The traumatic episode antedated the clinical manifestation and diagnosis by 28 months. The salient feature of the physical examination was the presence of a pulsatile mass with detectable bruit. This lesion, which was confirmed by angiography and computerized tomography, was treated by direct excision.

15 patients with superficial basaliomas or premalignant epithelial neoplastic disorders (actinic keratosis, leukoplakia, Bowen's disease) were treated locally over three weeks with retinoic acid (RA) alone or in combination with 5-fluorouracil (5-FU). In 11 cases the lesions disappeared clinically, however, only 5 patients proved to be cured by histological examination. 4 of them were treated with RA combined with 5-FU. There was a decrease of the 3H-index after the first week and an increase after the third week of treatment. These findings suggest, that RA inhibits the proliferation of neopl
