The prefabricated temporal island flap for eyelid and eye socket reconstruction in total orbital exenteration patients: A new method.

Anophtalmic socket reconstruction is a challenging problem in plastic surgery. We had described a prefabricated superficial temporal fascia island flap and used this technique in >50 enucleation patients with severe socket contraction ending in excellent or good results for 28 years (Altindas- 1 procedure). However, the flap was not suitable for the exenteration patients with complete eyelid loss. The technique was modified and used in exenteration patients (Altindas-2 procedure). In this 2-staged procedure, the temporoparietal fascia is prefabricated with a full-thickness skin graft from the

Current concepts in the management of Marjolin's ulcers: outcomes from a standardized treatment protocol in 16 cases.

Marjolin's ulcer is a malignant lesion observed in chronic wounds and in areas where the integrity of the skin is compromised because of any one of several reasons. The aim of this study was to define etiology, topography, and histopathology for Marjolin's ulcer and its surgical management. Sixteen cases were diagnosed and treated as Marjolin's ulcers. The mean age was 57.1 years (range, 32-85 years) and 15 of the patients (93.75%) had history of ulcer of 30 years or more. In 10 cases (62.5%), Marjolin's ulcer occurred after a flame burn and in 6 cases (37.5%) after a scalding burn injury. In

Reconstruction of temporal and suprabrow defects.

Large temple and suprabrow lesions can pose a reconstructive challenge. When the lesion extends anterior to the hairline, esthetically acceptable local flaps may be difficult to design. We describe a modified scalp flap (ie, part Converse scalping flap and part scalp rotation flap) that can be tailored to reconstruct a variety of difficult temple and suprabrow lesions while simultaneously maintaining eyebrow position. The modified scalp flap is raised in a subgaleal plane until approximately 2.5 cm above the brow. At this level, dissection proceeds in the subcutaneous plane to protect the fron

Head injury depth as an indicator of causes and mechanisms.

OBJECTIVE: The goal was to measure differences in the causes, mechanisms, acute clinical presentations, injuries, and outcomes of children <36 months of age with varying "greatest depths" of acute cranial injury. METHODS: Children <36 months of age who were hospitalized with acute head trauma were recruited at multiple sites. Clinical and imaging data were collected, and caregivers underwent scripted interviews. Neurodevelopmental evaluations were completed 6 months after injury. Head trauma causes were categorized independently, and subject groups with varying greatest depths of injury were

Versatility of free SCIA/SIEA flaps in head and neck defects.

Reconstruction of head and neck defects may require replacement of the bony structures, external soft tissue, and intraoral mucosa. Most cases, including maxillary defects, often require repair using only soft tissue flaps. Recently, the authors used free superficial circumflex iliac artery/superficial inferior epigastric (SCIA/SIEA) flaps for head and neck reconstruction. This was their first choice over other free flaps due to its versatile advantages. Fifteen patients underwent head and neck reconstruction with free SCIA/SIEA flaps (n = 16). No flap loss was observed; however, emergency vas

Occipital artery island V-Y advancement flap for reconstruction of posterior scalp defects.

BACKGROUND: The management of the posterior scalp defects with 'similar' tissue can be challenging. Currently available techniques of transposition/rotation result in creation of unwanted dog ears, change in direction of hairs and patches of skin-grafted areas with alopecia. We describe a new method of reconstruction of full-thickness scalp defects in the occipital region by moving the locally available scalp tissue in a V-Y advancement manner. The islanded flap is based upon the ipsilateral occipital artery in the substance of occipitalis muscle. The donor site/s can be closed primarily and t

Reconstruction of full thickness scalp defects after tumour excision in elderly patients: our experience with Integra dermal regeneration template.

BACKGROUND: Scalp reconstruction after wide tumor excision is particularly challenging. Free tissue transfers, local flaps, or skin grafts can be used but present some disadvantages especially with old patients with local advanced cancers, systemic diseases and in patients with a prior history of recurring scalp skin cancers in which the risk of burying a recurring tumor with a flap is likely. The Authors expose their early experience with Integra dermal regeneration template for scalp reconstruction after scalp tumor excision. METHODS: Eight patients with primary or secondary scalp tumor und

Aesthetic full-perioral reconstruction of burn scar by using a bilateral-pedicled expanded forehead flap.

Perioral scar after burn usually not only results in the certain degree of functional loss of the mouth, but it also destroys the facial appearance. This study aims to restore the perioral scarring lesion after burn as a whole unit to be achieved the functional and cosmetic outcomes in this area, by using a bilateral-pedicled expanded forehead flap with minimal donor morbidity. Six patients with perioral burn scar were undergoing the treatment. The technique was carried out by first positioning a tissue expander in the forehead pocket under the frontalis of the forehead and serially inflating

A trial on subcutaneous pedicle island flap for eyebrow reconstruction.

INTRODUCTION: Severe burn is a common problem affecting victim's eyebrows. This study aims to assess the effectiveness, complications and patients' satisfaction in two eyebrow reconstruction surgical methods. Up to now, many reconstruction methods, their specific advantages and complications have been introduced. This study compares a new method (subcutaneous pedicle island flap), which is scarcely discussed, with a standard method (superficial temporal artery island flap). METHOD: This is a clinical trial conducted during 2003-2005 at the Al' Zahra' Hospital. Forty patients with eyebrow defe

[Eyebrow reconstruction with a scalp island flap based on the superficial temporal artery].

The Authors describe the reconstruction of an eyebrow, in a case of complete traumatic avulsion, using a scalp island flap based on the superficial temporal artery, in a young person aged 32 years. The lesion represented a serious aesthetic disablement capable of causing disturbed psychic equilibrium and individual relationships, in view of the fact that the eyebrows play a decisive role in the characterisation of the face and expressive gestures. The strategy of using a scalp island flap with a limited amplitude only slightly larger than that of the site in which it was inserted enabled optim

Management of class IV skull burns using the bipedicled superficial temporal artery scalp flap.

Calvarial burns are extremely rare and pose a difficult challenge for both the burn and reconstructive surgeon. Reconstruction of these injuries is dependent on the depth of invasion and the amount of tissue loss. Fourth-degree burns include damage to the calvarium and the underlying dura and or cerebrum. Historically, these wounds have been treated conservatively. Two cases of electrical fourth-degree calvarial burns with large soft tissue defects as well as loss of calvarium and dura with cerebral herniation are presented. Each patient presented to Shriners Burn Hospital in a delayed fashion

Aesthetic pubic reconstruction after electrical burn using a portion of hair-bearing expanded free-forehead flap.

BACKGROUND: Electrical burn in the pubic region usually results in a severe and contractive scar with pubic hair loss. The aesthetic restoration of this area often has become very difficult. METHODS: A 22-year-old male electrical engineer experienced severe pubic scarring with hair loss after electrical burn. He was treated successfully with an expanded free-forehead flap including a portion of hair-bearing scalp after microsurgical vascular anastomoses between the bilateral superficial temporal vessels and the bilateral deep inferior epigastric vessels. The donor forehead site was closed dir

Case report: successful use of hyperbaric oxygen therapy for a complete scalp degloving injury.

We report a case of a complete scalp degloving injury in a 43-year-old previously healthy, tobacco-using female whose hair was caught in a tractor power take-off causing an instantaneous total scalping injury. Microsurgical replantation was started 4 hours after injury with the surgical procedure lasting 4 hours. Only the superficial temporal arteries were reanastomosed with no venous anastomosis possible. Hyperbaric oxygen (HBO2) treatment at 2.5 atmospheres absolute (ATA) for 90 minutes was administered immediately after surgery due to duskiness of the flap. Further HBO2 treatments were admi

Superficial temporal artery pseudoaneurysm: a conservative approach in a critically ill patient.

A 71-year-old man affected by cardio- and cerebrovascular disease experienced an accidental fall and trauma to the fronto-temporal area of the head. A few weeks later a growing mass appeared on his scalp. A diagnosis of superficial temporal artery pseudoaneurysm was made following CT and color Doppler ultrasound. His clinical condition favoured a conservative approach by ultrasound-guided compression and subsequent surgical resection. A conservative approach should be considered the treatment of choice in critically ill patients affected by superficial temporal artery pseudoaneurysm.

Traumatic arteriovenous fistula of the superficial temporal artery.

Arteriovenous fistula of the scalp is relatively rare. The superficial temporal artery is particularly vulnerable to trauma due to its long and relatively exposed course in the scalp. We report a traumatic arteriovenous fistula of the superficial temporal artery treated by complete surgical excision and review the literature with regard to etiology, clinical manifestations, pathogenesis, diagnosis and management of these unusual lesions.

Delayed intracerebral hemorrhage from an extracranial ball bullet pellet.

The evaluation of superficial penetrating injuries to the face and scalp is common in the emergency department. We present a patient who presented with a ball bullet pellet to the forehead with an initially negative computerized tomographic (CT) scan of the head, who represented 18 hours later with nausea, vomiting, and mental status changes. A repeat CT scan of the head revealed intraparenchymal hemorrhage immediately posterior to the site of pellet entry. Upon review, the initial CT scan was limited by scatter artifact. In this case report, we discuss the potential for significant injury wit

[The pedicled superficial fascia temporalis flap].

INTRODUCTION: The technique used for reconstruction of a cervicofacial defect depends on the extent of the tissue loss. Locoregional or free flaps are commonly used for this type of reconstruction. The type of flap used depends on donor site availability and morbidity. The aim of this technical note was to present appropriate use of the fascia temporalis flap in maxillofacial surgery. ANATOMIC BASIS: The superficial fascia temporalis lies just under the scalp. Blood is supplied via the superficial temporal artery which widely irrigates this anatomic region. Venous blood is drained by the super

Free anterolateral thigh flap for reconstruction of major craniofacial defects.

Free-tissue transfer has revolutionized skull-base surgery by expanding the ability to perform cranial base resection and by improving the quality of reconstruction. The anterolateral thigh flap has come recently into use in the field of head and neck reconstruction. Its role in craniofacial and midface reconstruction has not been specifically defined. This study involved a total of 18 patients who were treated over a 5-year period from 1998 to 2003. Seventeen patients had locally advanced head and neck cancer, requiring craniofacial resection, and one patient had a complicated gun shot wound
