Sudden natural death in a suicide attempt.

In this article, we report an unusual case of sudden death arising from a ruptured basilar artery aneurysm during a suicide attempt. A 72-year-old male was found dead in his home after sustaining superficial flesh gunshot wounds (entrace and exit) involving the forehead. The victim had been depressed for several months and had frank suicide intention. The pertinent autopsy findings revealed a superficial nonpenetrating bullet path of the scalp with no skull fractures or brain injuries. However, brain examination identified subrachnoid hemorrhage and subdural clot about the brainstem and within

[Traumatic aneurysm of the superficial temporal artery: a case report].

Traumatic aneurysm of superficial temporal artery is an uncommon lesion. Blunt traumas of war, firearm, motorized vehicle and sport injuries are common causes. In this case, traumatic aneurysm of superficial temporal artery that occurs after scalp trauma in 74 years old male patient, was reported. Patient who suffers from a mass in his forehead ten days after trauma was admitted to our clinic. Selective angiography of the right temporal artery was performed as traumatic aneurysm was suspected upon the findings of physical examination and cranial computerized tomography studies. The aneurysm wa

Cerebral venous sinus thrombosis presenting as subdural haematoma.

The authors report a 39-year-old woman who presented with intermittent, excruciating nuchal and occipital headache. Magnetic resonance imaging and magnetic resonance angiography scans showed bilateral subdural haematomas with veno-occlusive disease of the superficial and deep venous systems. There were bridging collaterals with scalp veins, bleeds from which could explain the subdural haematoma. There was acute on chronic veno-occlusive disease with an acute rise in intracranial pressure and a bleed from the vein of Galen.

Eyebrow reconstruction following burn injury.

Facial burns represent between one-fourth and one-third of all burns. Absence of the eyebrows or distortions in their position alter the character of the face. Thus, eyebrow repair or reconstruction can be an important "finishing touch" in the overall reconstruction of a burned face. Generally, there are three ways to reconstruct the eyebrow: use of superficial temporal artery island flap; composite graft from scalp; and mini or micrografts from scalp. This report presents 20 patients reconstructed with the above techniques. Eleven patients (eight male and three female) with superficial tempor

Endovascular embolization of a traumatic arteriovenous fistula of the superficial temporal artery.

Arteriovenous fistula of the superficial temporal artery is a rare condition most commonly caused by trauma. Traditional surgical treatment has been superseded by endovascular embolization. We present the case of a 40 year-old man with a traumatic arteriovenous fistula of the superficial temporal artery who was treated by endovascular embolization. The advantages of this approach are discussed, along with a brief history of the condition.

[The use of superficial cervical artery island skin flaps in the repair of the maxillofacial and cervical scar].

OBJECTIVE: To investigate the method of harvesting of superficial cervical artery island skin flap, and its clinical application in the repair of the maxillofacial and cervical scars. METHODS: The origin, course, branches and distribution of the superficial cervical artery were studied in 10 adult cadavers. The superficial cervical artery could be divided into three segments: i.e. the segment before entering the trapezius muscle, the segment in the trapezius muscle, and the segment emerging from the trapezius muscle. Fourteen patients with maxillofacial and cervical scar contracture were enro

Pseudoaneurysms of the superficial temporal artery: treatment options.

OBJECTIVES: Pseudoaneurysms of the superficial temporal artery present as slowly growing masses of the face or scalp. They may arise as a result of infection or autoimmune disease but most commonly are sequelae of blunt, penetrating, or surgical trauma. We describe their clinical presentation and differential diagnosis and compare several treatment options. STUDY DESIGN: Observational case series. METHODS: We retrospectively reviewed the presentations, diagnostic evaluations, and management in three young men who presented with pseudoaneurysms after trauma. A computerized review of the world

First successful replantation of face and scalp with single-artery repair: model for face and scalp transplantation.

Successful replantation of the scalp with microanastomosis of a single artery and vein has been reported to produce reliable results. In fact, there have been several reports of scalp replantations based on one-artery and vein repair. There has been a face and scalp replantation reported in the literature, but this was as two separate parts and was based on several arterial and venous repairs. The authors performed the first successful replantation of a face and scalp with repair of a single artery and, of course, two veins. A 21-year-old man presented after his face and scalp were completely

Unmyelinated trigeminal pathways as assessed by laser stimuli in humans.

Laser pulses excite superficial free nerve endings innervated by small-myelinated (Adelta) and unmyelinated (C) fibres. Whereas laser-evoked scalp potentials (LEPs) are now reliably used to assess function of the Adelta-fibre nociceptive pathways in patients with peripheral or central lesions, the selective activation of C-fibre receptors and recording of the related brain potentials remain difficult. To investigate trigeminal C-fibre function, we directed laser pulses to the facial skin and studied sensory perception and scalp evoked potentials related to Adelta- or C-fibre activation in heal

A new retroauricular flap for facial reconstruction.

A retroauricular reverse flow flap based on the anastomosis of the frontal branch of the temporal superficial artery with the corresponding vessel of the opposite side as well as with the ipsilateral supraorbital and supratrochlear arteries, was used in two clinical cases for forehead and facial reconstructions. The arc of rotation associated with the length of the pedicle allows its transfer, in a one-stage procedure, to cover small to medium defects of any part of the forehead, face and parts of the scalp. The procedure is safe, simple and causes minimum donor site morbidity.

Traumatic pseudoaneurysms of the superficial temporal artery.

Persistent scalp swelling following blunt head injury may be the presenting feature of a pseudoaneurysm of the temporal artery. This paper describes three patients who sustained injuries to the temporal region, which resulted in the formation of pseudoaneurysms. All patients had a delayed presentation to the Emergency Department. We describe their clinical presentations, subsequent management and pitfalls encountered when dealing with this condition.

Prefabricated galeal flap based on superficial temporal and posterior auricular vessels.

Scalp layers are widely used in reconstructive procedures. The authors used prefabricated galeal flaps based on the superficial temporal or postauricular vessels for ear, cheek, mandible, and cranium reconstructions in three cases. In case 1, synchronous beard and ear reconstructions were accomplished by using the temporoparietal and retroauricular flaps. In case 2, a buccomandibular defect was reconstructed by transposing the supra-auricular and retroauricular galea with prefabricated bone and skin. In case 3, an epidural hematoma in the left frontoparietal area was evacuated after a circular

Total upper lip reconstruction with a free temporal scalp flap: long-term follow-up.

BACKGROUND: In men, reconstruction of large full-thickness defects of the upper lip requires both an inner layer to replace the mucosal lining and an outer hair-bearing layer. METHODS: When locating the superficial temporal vessels, the design of the temporal flap is marked following the hairline needed. After meticulously dissecting the flap, it is inset and microanastomosed with the facial blood vessels. The internal mucosal layer of the flap is grafted on. During the follow-up period, the sensory recovery and motor functions are examined and recorded. RESULTS: The postoperative courses we

Is androgenetic alopecia a photoaggravated dermatosis?

Progressive thinning of the scalp hair in androgenetic alopecia (AGA) results in a gradual decline in natural protection of the scalp from ultraviolet radiation (UVR). A number of pathologic conditions of the scalp are evidently related to UVR, particularly photosensitive diseases and disorders of the chronically photodamaged bald scalp. The most important chronic effects of UVR are photocarcinogenesis and solar elastosis. Besides these, erosive pustular dermatosis and 'red scalp' are distinct disorders peculiar to the balding scalp. While the consequences of sustained UVR on the unprotected s

Significant selective head cooling can be maintained long-term after global hypoxia ischemia in newborn piglets.

OBJECTIVE: Selective head cooling (SHC) combined with mild body cooling is currently being evaluated as a potentially therapeutic option in the management of neonatal hypoxic-ischemic encephalopathy. It is proposed that SHC enables local hypothermic neuroprotection while minimizing the deleterious side effects of systemic hypothermia. However, there is little evidence that it is possible to cool the brain more than the body for a prolonged period of time. The aim of this study was to examine whether the brain (T(deep brain)) could be cooled to below the rectal temperature (T(rectal)) in our pi

Aesthetic and functional advantages of the anterolateral thigh flap in reconstruction of tumor-related scalp defects.

Eleven patients underwent free-flap reconstruction of tumor-related defects of the scalp, forehead, and temporal region. Flap selection aimed at achieving acceptable functional and aesthetic results combined with negligible donor-site morbidity. Ten males and one female, aged 61.3 +/- 14.3 years, were included in this study. Eight patients presented with tumor recurrences after previous surgery, irradiation, and/or chemotherapy. The average extension of defects was 169.5 (range, 30-600) qcm. Free flaps employed for reconstruction included antero-lateral thigh flaps (8), suprafascial radial for

Pseudoaneurysms of the superficial temporal artery secondary to placement of external ventricular drainage catheters.

BACKGROUND: Pseudoaneurysms of the superficial temporal artery have been described following trauma and various surgical procedures. There are no reports in the literature of these lesions following the placement of external ventricular drainage catheters. This article describes two patients and their successful treatment, and reviews diagnostic and treatment strategies. CASE DESCRIPTION: Two patients developed tender, nonpulsatile masses in the scalp along the former subcutaneous tract of an EVD catheter. Both lesions were successfully excised in the operating room and found to be thrombosed
