Micropuncture Decompression With High-Dose Hyaluronidase for Early Reversal of Nasal Ischaemia from HA-Biostimulator Mixed Fillers: An Illustrative Case and Technical Algorithm.

BACKGROUND: Nasal filler-related vascular compromise is uncommon but may result in serious tissue injury. Mixed fillers combining hyaluronic acid (HA) with particulate biostimulators may create emboli with dual-phase rheologic behavior that is not fully addressed in current management algorithms derived largely from HA-only events. OBJECTIVES: To present an illustrative case and case-derived technical algorithm integrating high-dose hyaluronidase with superficial micropuncture decompression for early nasal ischemia following injection of a mixed HA-biostimulator filler. METHODS: We describe

Penetrating maxillofacial trauma with transmaxillary migration of a metallic foreign body to the sphenoid sinus: A rare case.

This case report details a rare and potentially life-threatening injury of an initially overlooked maxillofacial trauma sustained during agricultural activity. A 19-year-old male presented to the emergency department with a headache and fatigue, 2 days following a right malar injury that had been previously sutured without advanced evaluation. A thorough clinical examination was conducted, which revealed the presence of periorbital swelling on both sides, accompanied by a right subconjunctival hemorrhage and chemosis. Radiographic imaging revealed the presence of a substantial, crescent-shaped

[About the color of the skin outside the livor mortis marks in cases of death from general hypothermia].

This study examines the phenomenon of focal skin discoloration observed on corpses that died from general hypothermia, outside the areas of classic livor mortis. The authors note that these discolorations are more common in areas with less intense blood flow and less muscle tissue. The severity and number of these discolorations correlate with the intensity and duration of cold exposure, the condition of the clothing, and the degree of alcohol intoxication. A hypothesis on the mechanism of these spots formation is presented, based on the in vivo freezing of blood in superficial vessels with su

Anatomical Site-Specific Microbiological Divergence in Combat-Related Injuries During the Ukraine Conflict: A Comparative Analysis of Head, Torso, and Extremity Wounds.

INTRODUCTION: Combat-related infection research is overwhelmingly skewed toward extremity injuries because of the effectiveness of modern body armor. This creates a critical knowledge gap regarding the microbiological dynamics of non-limb anatomical sites, particularly in the context of intense warfare. This study evaluates the wound microflora of 51 Ukrainian soldiers during the second phase of the conflict with Russia, focusing on head and torso injuries. A key objective was to investigate how blast-induced acoustic barotrauma-including tympanic membrane perforation and mucosal damage-acts a

Trigeminal trophic syndrome: recognising a pattern.

Trigeminal trophic syndrome (TTS) is an unusual presentation of painless facial ulceration with associated facial anaesthesia and paraesthesia. The syndrome stems from injury to branches of the trigeminal nerve. TTS can also be triggered by conditions such as ischaemic strokes and herpes zoster virus. The altered superficial sensation to the skin causes irritation along the distribution of the trigeminal nerve, usually described as burning and tingling. This sensation leads to repetitive scratching and rubbing over the affected area. Self-manipulation injuries then progress into an erosive ulc

Microsurgical Repair of Large Nasal Septal Defects.

BACKGROUND: Nasal septal defects cause considerable morbidity and represent a challenging reconstructive problem. Traditional repair techniques have employed local intranasal tissues and allograft adjuncts. For large septal defects (>4-5 cm 2 ), less than half are successfully resolved. METHODS: We conducted a retrospective review of patients presenting with large (>4 cm 2 ) nasal septal defects treated by microsurgical septal reconstruction using various engineered or prelaminated flaps. RESULTS: Nine consecutive patients were included in this study. Defect size averaged 7.89 cm 2 (range 4.

Complex Nasal Reconstruction With Free Superficial Circumflex Iliac Artery/Inferior Epigastric Artery Flaps and Exteriorized Skin-grafted Pedicles.

Reconstruction of extensive nasal defects is technically demanding due to the nose's aesthetic importance and intricate 3-dimensional structure. The gold standard for nasal reconstruction typically involves local flaps, with or without the addition of free flaps, depending on the defect's extent and complexity. In cases where conventional reconstructive options are unavailable, innovative approaches may be considered. A 79-year-old woman presented with a large nasal defect following the excision of an extensive basal cell carcinoma and multiple failed reconstruction attempts. Due to the unavai

Can we predict the risk of esophageal stricture after caustic injury?

Nonoperative management of severe caustic injuries has demonstrated its feasibility, avoiding the need for emergency esogastric resection and resulting in low mortality rates. However, leaving superficial necrosis in place could increase the risk of esophageal stricture development. Data on the risk factors of esophageal stricture secondary to caustic ingestion are scarce. The aim of our study was to identify the risk factors for esophageal strictures after caustic ingestion at admission. From February 2015 to March 2021, all consecutive patients with esophageal or gastric caustic injury score

Asphyxia with a blunt instrument: which forensic evidence to use in complex cases of murder.

BACKGROUND: The blunt injuries may be heterogeneous. Due to the diversity, it is often difficult to establish the type of weapon used. Particular attention must be paid to the circumstantial data and previous diseases because the injuries often could not correspond to the presumed instruments used. CASE REPORT: A man was found dead in his bedroom wearing pajamas. On the bed there was a blanket with visible traces of blood, which had dried. There were around 10 dogs in the house, poor hygienic and sanitary conditions, widespread excrements and unbreathable air. Testimonies from general practit

Anatomical consideration and techniques in thread nose augmentation with mesh scaffold implant.

INTRODUCTION: Non-surgical rhinoplasty has evolved with the introduction of volumizing threads, which offer a less invasive alternative to traditional methods by enhancing nasal contours while minimizing filler use. This technique is gaining popularity, particularly in Southeast Asia, due to its shorter recovery time, reduced risk profile, and ability to prevent the 'Avatar nose' effect. However, there is limited anatomical guidance available for its application, especially in the Asian population. MATERIALS AND METHODS: This study analyzes the anatomical layers of the nose to guide the prope

Surgical management of a patient with severe and complex maxillofacial injuries caused by a bear mauling in the forest: A case report from Thailand.

A 60-year-old Thai male sustained severe and complex maxillofacial injuries, along with superficial neck and abdominal injuries, from being mauled by a black bear in Thailand. The accident caused massive injury to the face, including the right eye and periorbital soft tissue, right cheek, total nose, total upper lip, partial lower lip, and underlying bone structures. A computed tomography scan showed comminuted pan-facial fractures. Lifesaving procedures were performed at nearby hospitals, and then he was referred to our center one month after the injury. Our initial multi-stage surgical manag

Total lower eyelid reconstructions with septal chondromucosal graft and pedicled flap on the superficial temporal artery.

BACKGROUND: Subtotal and total lower eyelid tissue loss after tumour excision is a complex issue for reconstructive surgeons. The anterior and posterior flaps must be restored to avoid compromising the functional and aesthetic prognosis of the eye. This study used a septal chondromucosal graft for the posterior lamella and a fasciocutaneous flap pedicled on the temporal artery for the anterior lamella. METHOD: We conducted a 10-year monocentric retrospective study, including 18 patients who were treated using a septal chondromucosal graft with a temporal artery pedicle flap following tumour e

The Africa Temporal Scalp Flap: A Novel Flap for Facial Reconstruction.

Although a number of flaps exist for nasal reconstruction, severe scarring of the forehead after burn injury led to the development of a novel two-stage flap based on the superficial temporal artery. The Africa Temporal Scalp (ATS) flap is composed of an axial ascending part on the superficial temporal artery, and a descending anterior extension for reconstruction of the midface. This is a retrospective analysis of all patients who underwent ATS flap surgery on the MV Africa Mercy. During the 7.5-year period, the ATS flap was applied to 45 facial reconstructions, with a median age of 28 years

Rare Oral Hemangioma in Pregnancy: A Case Series Providing Clinical Insight into Patient Care.

BACKGROUND Hemangiomas are defined as benign soft tissue vascular tumors that are histologically classified as capillary, cavernous, or mixed types. Hemangiomas can also be described based on clinical appearance as superficial, mixed, or deep lesions. Following a thorough search, only 3 case reports of superficial protruding lip mass were found in the literature. Other cases of tongue hemangioma have been reported in infants or young toddlers, and only rarely in adults. CASE REPORT The first case was a 43-year-old pregnant woman, with an unremarkable medical and surgical history, in the second
