An unusual case: repeated nocturnal biting of a bedridden human by a nymph of Tettigonia viridissima (Orthoptera: Tettigoniidae).

Cases of adventitious biting on humans by non-hematophagous insects are rarely reported but hold significant medical relevance. This case report documents the first instance of repeated nocturnal biting in a bedridden, tetraparetic 59-year-old patient by a fifth-instar nymph of the great green bush-cricket, Tettigonia viridissima L. (Orthoptera: Tettigoniidae). The patient sustained four superficial skin lesions on the left ear and forehead, accompanied by distinct blood smears. A live T. viridissima nymph was captured at the end of a dark red blood streak nearby. Biting activity was likely an

Three-Stage Prelaminated Radial Forearm Flap for Total Auricular Reconstruction Following Scalp Avulsion Injury.

Loss of mastoid skin and temporoparietal fascia after complete scalp avulsion precludes conventional ear reconstruction. We report a total auricular reconstruction using a remote, prelaminated radial-forearm flap. A 42-year-old woman with prior scalp avulsion and right ear amputation underwent (1) 200-mL forearm expansion, (2) autogenous costal-cartilage framework insertion and maturation, and (3) free transfer to the mastoid area anastomosed to the preserved proximal superficial temporal vessels. At 6 months the neo-auricle showed symmetric contour, color match, and defined helix/anti-helix;

Treatment of parotid gland oncocytoma: a case report from a radiation oncologist's perspective.

BACKGROUND: Parotid oncocytoma is a rare benign neoplasm for which surgical excision remains the standard treatment. Although the recurrence rate is generally low, it increases significantly in cases with multifocal lesions or incomplete surgical excision. In such scenarios, postoperative radiotherapy (RT) has been shown to improve local control, reduce recurrence rates, and minimize the need for reoperation. This report presents a representative case of parotid oncocytoma, with a focused discussion on therapeutic decision-making from a radiation oncologist's perspective. CASE REPORT: We repo

Anatomical Site-Specific Microbiological Divergence in Combat-Related Injuries During the Ukraine Conflict: A Comparative Analysis of Head, Torso, and Extremity Wounds.

INTRODUCTION: Combat-related infection research is overwhelmingly skewed toward extremity injuries because of the effectiveness of modern body armor. This creates a critical knowledge gap regarding the microbiological dynamics of non-limb anatomical sites, particularly in the context of intense warfare. This study evaluates the wound microflora of 51 Ukrainian soldiers during the second phase of the conflict with Russia, focusing on head and torso injuries. A key objective was to investigate how blast-induced acoustic barotrauma-including tympanic membrane perforation and mucosal damage-acts a

Trigeminal trophic syndrome: recognising a pattern.

Trigeminal trophic syndrome (TTS) is an unusual presentation of painless facial ulceration with associated facial anaesthesia and paraesthesia. The syndrome stems from injury to branches of the trigeminal nerve. TTS can also be triggered by conditions such as ischaemic strokes and herpes zoster virus. The altered superficial sensation to the skin causes irritation along the distribution of the trigeminal nerve, usually described as burning and tingling. This sensation leads to repetitive scratching and rubbing over the affected area. Self-manipulation injuries then progress into an erosive ulc

External Auditory Canal Transillumination-Guided Middle Fossa Approaches: An Anatomical Feasibility Study.

BACKGROUND AND OBJECTIVES: The middle fossa approaches are tremendously versatile for treating small vestibular schwannomas, selected petroclival meningiomas, midbasilar trunk aneurysms, and lesions of the petrous bone. Our aim was to localize the internal acoustic canal and safely drill the petrous apex with these approaches. This study demonstrates a new method to locate the internal acoustic canal during surgery in the middle fossa. METHODS: The microsurgical anatomy of the middle fossa floor was studied in 11 formalin-fixed and silicone-injected cadaveric heads. Extradural dissection of t

Exploring Van Gogh Syndrome: A Case Report on Schizoaffective Disorder and Self-Harm.

Deliberate self-harm (DSH) is defined as the intentional infliction of physical injury on oneself without the intent to end one's life. Common behaviors associated with DSH include cutting with a knife or razor, scratching or hitting oneself, and intentional drug overdose. Additionally, these behaviors may encompass restricting food intake and engaging in other risk-taking activities, such as driving at high speeds and participating in unprotected sexual activities. DSH is a strong indicator of suicide risk, particularly in individuals with schizophrenia. The death of Vincent Van Gogh on July

The Course of the Main Trunk and Parietal Branch of the Superficial Temporal Artery for a Pterional Scalp Flap with Superficial Temporal Artery Preservation: Cadaveric and Clinical Study.

BACKGROUND: The pterional incision is usually performed near the course of the superficial temporal artery (STA), which carries a risk of injury to a branch or even the main trunk of the STA (mSTA). In this study, we assessed the usual course of the mSTA and its parietal branch of the STA (pSTA) and evaluated the efficacy of a modified pterional scalp incision for the preservation of all STA branches. MATERIALS AND METHODS: Sixteen sides of cadaveric heads were dissected to study the location and paths of the mSTA and pSTA in the vicinity of the ear cartilage and the oculomeatal (OM) line. We

Introducing a novel technique for auricular replantation following subtotal amputation combining Sexton and Mladick pocket methods: a case report.

BACKGROUND: Traumatic auricular amputations present a unique surgical challenge, particularly in cases where microsurgical anastomosis is not feasible due to vessel damage or absence. Traditional reconstruction techniques often yield suboptimal aesthetic or functional results. The pocket technique, as described by Sexton and Mladick, has shown promise in such scenarios, particularly in partial auricular amputations. CASE PRESENTATION: We report a case of a 52-year-old Caucasian male who presented with traumatic amputation of the cranial third of the auricle following a dog bite. The injury wa

Thermal touch and anatomical insight: Roman terracotta vessels as proto-clinical maps of the human body.

In the Archaeological Museum of Nicosia, Cyprus, a collection of Roman terracotta vessels-dated to the 1st century BCE to 1st century CE-offers a compelling view into the convergence of empirical healing and early anatomical reasoning. These vessels, likely used for localized heat therapy, are distinctly shaped to contour specific body parts: ears, hands, feet, knees, abdomen, and thorax. With spouts for pouring warm liquids and cavities for radiating retained heat, they appear to have functioned as reusable "hot bags" for therapeutic application. Their deliberate anatomical shapes hint at a p

Sonographic anatomy and imaging of the great auricular nerve.

PURPOSE: The great auricular nerve (GAN) provides sensory innervation to the region of the parotid gland, angle of mandible, external ear and mastoid process. Sonographic imaging can allow lesions, injury and entrapment of the superficial GAN to be diagnosed. To sonographically image the GAN, its detailed relative anatomy, how and where it can be demonstrated with ultrasound, and its sonographic appearance should be appreciated. METHODS: A review of the literature was conducted to unpack the path and course of the GAN, its areas of sensory innervation and how it should be clinically and sonog

Immunomodulation by the combination of statin and matrix-bound nanovesicle enhances optic nerve regeneration.

Modulating inflammation is critical to enhance nerve regeneration after injury. However, clinically applicable regenerative therapies that modulate inflammation have not yet been established. Here, we demonstrate synergistic effects of the combination of an HMG-CoA reductase inhibitor, statin/fluvastatin and critical components of the extracellular matrix, Matrix-Bound Nanovesicles (MBV) to enhance axon regeneration and neuroprotection after mouse optic nerve injury. Mechanistically, co-intravitreal injections of fluvastatin and MBV robustly promote infiltration of monocytes and neutrophils, w

Can we predict the risk of esophageal stricture after caustic injury?

Nonoperative management of severe caustic injuries has demonstrated its feasibility, avoiding the need for emergency esogastric resection and resulting in low mortality rates. However, leaving superficial necrosis in place could increase the risk of esophageal stricture development. Data on the risk factors of esophageal stricture secondary to caustic ingestion are scarce. The aim of our study was to identify the risk factors for esophageal strictures after caustic ingestion at admission. From February 2015 to March 2021, all consecutive patients with esophageal or gastric caustic injury score

Auricular Reconstruction.

The auricle is susceptible to various injuries due to its location and projection from the scalp. The unique contours of the external ear makes reconstruction a challenging endeavor. This chapter provides a comprehensive overview of auricular reconstruction, based on the depth, location, and size of injury. Strategies for reconstructing superficial, composite, and full-thickness injuries are discussed in relation to the location of the defect. Targeted reconstructive options for injuries of the upper, middle, and lower thirds of the auricle are outlined. Methods for total auricular reconstruct

Total lower eyelid reconstructions with septal chondromucosal graft and pedicled flap on the superficial temporal artery.

BACKGROUND: Subtotal and total lower eyelid tissue loss after tumour excision is a complex issue for reconstructive surgeons. The anterior and posterior flaps must be restored to avoid compromising the functional and aesthetic prognosis of the eye. This study used a septal chondromucosal graft for the posterior lamella and a fasciocutaneous flap pedicled on the temporal artery for the anterior lamella. METHOD: We conducted a 10-year monocentric retrospective study, including 18 patients who were treated using a septal chondromucosal graft with a temporal artery pedicle flap following tumour e

A Novel Classification System and Surgical Strategies of First Branchial Cleft Anomalies.

OBJECTIVE: To define a novel classification of first branchial cleft anomalies (FBCAs) based on the relationship between lesions and the facial nerve in terms of radiographic imaging findings and to introduce the corresponding surgical managements. METHODS: The clinical data were retrospectively reviewed. Novel classification was proposed according to the head-neck MRI findings and surgical records. FBCAs limited in the cartilaginous segment of external auditory canal (EAC) or superficial parotid gland capsule were classified as type A. Lesions close to the FN and(or) involved into the paroti

Lipoma in Parotid Gland: A Rare Case Report.

Lipoma is a benign, rare, mesenchymal tumor found in the head and neck region, especially in the parotid gland. It thus requires a careful and precise examination to establish a diagnosis. A surgical procedure of the parotid gland is challenging due to the associated risk factor of facial nerve injury. We report a rare case of head and neck region lipoma between the superficial and deep lobe of the parotid gland. A 44-year-old female patient was presented with the chief complaint of a painless lump on the left front ear to the left cheek for about 1 year. There were no complaints of tooth pain
