In over 3,700 facelifts in the last 40 years, the author has evolved a simple technique with a natural look. All cases were done with conscious sedation and local anesthesia. Patients are ambulatory immediately following surgery and have a rapid recovery with little edema or ecchymosis. Nerve injuries are avoided, and the temple hairline and earlobe clefts are preserved. There are no ridges, dimples, or "joker lines." Osteomas of the middle ear are rare lesions. We report the case of a 20-year-old male with osteoma originating from the promontory coincident with tympanic membrane perforation. Histopathological diagnosis was performed by means of a superficial curettage during surgery, to avoid a cochlear fenestration. Postoperative closure of the air-bone gap after myringoplasty confirmed the presumed asymptomatic nature of these lesions.

A free full-thickness chondrocutaneous flap from the auricular concha for the repair of large tracheal defects was transferred successfully. We present new material in the surgical repair of the perforated tympanic membrane. Forty-two patients with a unilateral tympanic membrane perforation and eight patients with bilateral perforations had their perforations closed with peri-umbilical superficial fascia via a transtympanic route. Of the 58 tympanic membranes repaired, 51 were successful while seven failed. The technique is cost-effective and quick, making bilateral repairs possible under the same anaesthetic.

From 1990 to 1991, 6 avulsed scalps (total avulsing in 5, partial avulsing in 1) were replanted successfully. Follow-up for over 6 months showed that the hair of the replanted scalps grew normally in all the 6 patients. We think that the key to successful replantation is to anastomose the vessels effectively. The scalp can survive just with its superficial temporal vessels on both sides and with an artery-vein ratio of 1:1. The management of postoperative complications is discussed.
