Superficial siderosis of the central nervous system.

We report on three patients with superficial siderosis of the central nervous system. The main clinical findings were progressive dementia, hearing loss and ataxia in combination with repeated xanthochromia of the cerebrospinal fluid. Diagnosis was made in one patient with magnetic resonance imaging (MRI), which showed a hyperintense rim around the surface of the cerebellum and the spinal cord. In the two other cases necropsy disclosed superficial iron and hemosiderin deposits on the surface of the brain. The etiology either was idiopathic or secondary to chronic intracranial bleeding by an an

[Gunshot injury caused by a training bullet].

In the course of a scuffle a 50-year old man has been shot by a revolver. The culprit and witnesses state, there has been only one shot, whereas the victim declared having been shot twice. The medico-legal investigation revealed a gunshot wound that passed through the left thigh, a grazing shot of the right hand and a superficial lesion of the skin at the forehead with sprinkled gunshot residue in the epidermis. The investigation confirmed the statement of the victim: the first shot to the forehead was caused by a plastic training cartridge, the second to the right hand and thigh by a cartridg

Death due to superficial soft tissue injuries.

A series of deaths due to multiple superficial injuries is reported. These cases represented 5% of a consecutive homicide series in the Northern Territory of Australia. All cases were characterized by extensive superficial injuries, particularly involving soft tissue, which were present over much of the body including the face, head, arms, legs, and trunk. Upper-limb fractures were present in some cases, as were fractured ribs. The internal organs and tissues were pale, but there was either little or no organ damage. No significant volume of free blood was found in any of the body cavities. In

Dose-dependent smooth muscle cell proliferation induced by thermal injury with pulsed infrared lasers.

BACKGROUND: Recently, laser-heated and radio frequency-heated balloon angioplasty techniques have been proposed as a means to treat or minimize dissection and elastic recoil but have been associated with a high rate of clinical restenosis. Similarly, pulsed laser angioplasty techniques proposed to minimize thermal injury while ablating obstructing atheroma have failed to reduce clinical restenosis. Because "hot balloon" and pulsed laser angioplasty create both mechanical and thermal injury, it has been difficult to discern the cause of the smooth muscle cell (SMC) proliferation resulting in re

[Epidural "sulmycin implant" coverage for local prevention of infection in surgical management of open craniocerebral injuries].

In various series reported in the literature on the operative management of severe head injuries with compound depressed skull fractures and penetrating wounds of the brain, the rates of infection differ from 1 to 17%. In this paper the operative experience with 22 cases of penetrating head injuries is discussed. In conventional operative therapy, depressed skull fracture and lacerated dura were covered by "Sulmycin Implant" containing Gentamycin as a helpful bacteriological barrier. 18 patients survived, 7 patients had severe neurological defects, 5 patients had mild neurological deficits and

Head and face injuries in bicyclists--with special reference to possible effects of helmet use.

Fatal and nonfatal head and face injuries to unhelmeted bicyclists were analyzed to assess the injury-reducing potential of bicycle helmet use. Of the fatally injured, 64% (median age, 55 years) had fatal head and face injuries compared with 38% (median age, 18 years) with head and face injuries in the nonfatal injury group. The fatally injured often had multiple impact points, mostly to the occipital and temporal regions. Brain contusions, most often to the frontal and temporal lobes, were the most common cause of fatal injury, followed by subdural hematomas. In the nonfatal injury group abra

Complex femur fractures in children: treatment with external fixation.

External fixation was used to treat complex femur fractures in 10 children. These injuries were associated with head trauma, cerebral palsy, epidermolysis bullosa, open wounds, and failed casting. The duration of external fixation averaged 8 weeks, and the mean follow-up was 5 years 8 months. At follow-up, one patient had 2.6 cm of residual shortening, and two had overgrowth. Complications included one refracture. Three children developed superficial pin site drainage. Although most femur fractures can be treated with traction or casting, external fixation may simplify overall care in children

Management of facial injuries.

Because of the excellent blood supply to the head region, superficial lacerations to the soft tissue structures of the head generally heal rapidly without treatment. Lacerations of the equine tongue frequently go unnoticed because difficulty in eating usually is not apparent. The majority of lacerations heal if left untreated, with no loss in function. Surgical repair is indicated to promote healing and prevent deformity, to amputate a severely compromised apex, and to alter a scar or defect that is unacceptable to the owner. Surgical principles to be adhered to include thorough debridement an

Stairway injuries in children.

Children frequently injure themselves falling down stairways, but the characteristics of these injuries are not well described. A total of 363 consecutive patients seen in a pediatric emergency department were studied. The majority of patients had minor superficial injuries. Bony injuries occurred in 7% of patients. Head and neck injuries occurred in 73% of patients, extremity injuries, which were predominantly distal, in 28%, and truncal injuries in 2%. Children younger than 4 years of age were more likely to sustain head trauma than children older than 4 years of age (P less than .005). Inju

Death by lightning.

Deaths from lightning injuries are infrequent, amounting to 0.2-0.8 per million people per year, and are associated with climatologic conditions. The victims are mostly young, active people who are struck during various outdoor activities during the summer months. Even though many people can be struck by the same bolt of lightning, multiple casualties are uncommon. Among the fatalities, various types of skin burns are regularly present, often at an entry site on or near the head, and at an inferiorly located exit site. These skin injuries often consist of superficial burns and singed hair; dee

[Malignant melanoma in the area of the head and neck].

The incidence of malignant melanoma is rising worldwide. Recognition of the biological types of malignant melanoma (nodular melanoma, superficial spreading melanoma, lentigo maligna melanoma, and acrolentiginous melanoma) have greatly helped to improve the prognosis in recent years. Determination of the depth of invasion in histological serial sections has proved to be of great prognostic value. Melanomas with minimal invasion have an excellent prognosis. Wide excision of the primary tumour is of utmost importance. The excisional margins should be 3-5 cm, but in superficial melanomas the width

Cutaneous malignant melanoma in association with other skin cancers.

A case-control study of 232 cases of cutaneous malignant melanoma and 232 matched controls was performed to assess the association of the disease with nonmelanotic skin tumors--basal cell carcinomas, squamous cell carcinomas, and solar keratoses. There was a fourfold increase in risk of melanoma of all types when actinic tumors were present on the face. The risk was not restricted to the lentigo malignant melanoma class as might be expected but was significantly raised for superficial spreading melanoma and nodular melanoma as well (relative risk, 2.8; 95% confidence interval, 1.1-7.2). Pathogenesis, diagnosis and treatment of acute gastric mucosal lesions.

Stress ulcers are multiple superficial mucosal lesions which occur mainly in the fundus of stomachs of seriously ill patients and should be differentiated from reactivation of a pre-existent ulcer diathesis, Cushing's ulcer following head injury, or drug-induced gastritis. It is generally agreed that luminal acid and pepsin are required for ulceration to develop. Experimental evidence suggests that backdiffusion of acid is closely related to the formation of ulcers. In the absence of overt disruption of the gastric mucosal barrier, ischaemia appears to compromise the ability of the gastric muc

[Craniocerebral injuries in childhood].

Causes of head injury in childhood depend on age. In contrast to the adults there are differences in pathophysiological mechanisms and in the site of lesions. Brain stem disorders are more frequent, superficial brain damage is less frequent than in children. Multiple lesions of the limbic system are as well important as those ones of the mesencephalon. Most of the cases of apallic syndromes in childhood can be due to these lesions.

[Association of a melanoma and a spinocellular epithelioma on a burn scar of the scalp].

A thirty-nine-year-old caucasian man presented with an ulceration of the scalp, developing on an alopecia. The scalp indeed had been burnt by boiling water when the boy was two. During childhood and youth microtraumatisms induced superficial wounds which always healed easily. Nine months ago, a new ulceration followed a new trauma; the wound did not heal, and lay on a dark brown area. A biopsy diagnosed a malignant melanoma. The whole alopecic area was resected, under the ulceration and the pigmented area, as well as the external surface and the diploe. The defect was covered with a free trans

The surgical closure of exposed skull.

The closure of scalp defects is still surrounded by many uncertainties. Free skin graft has proved most successful. Thiersch graft, split skin graft and full thickness skin graft have their indication for use. Thiersch skin graft is used as a temporary dressing, in order to achieve aseptic wound conditions, but cannot be looked upon as a lasting solution cosmetically and functionally. Split skin is used only for the closure of smaller defects. Full thickness skin grafts are most suitable, because they offer a lasting coverage, which meets to all demands. If the outer table of the skull is expo
