Irreducible fracture-dislocation of the hip: a severe injury with a poor prognosis.

OBJECTIVE: To determine the outcome of and prognostic factors associated with irreducible fracture-dislocations of the hip. DESIGN: Retrospective review of a prospectively gathered trauma database. METHODS: Using a prospectively gathered trauma database, we identified twenty-five patients with fracture-dislocations of the hip that were irreducible with closed means under a general anaesthetic. All were victims of high-energy trauma with severe injuries (mean Injury Severity Score = 25, range 9 to 54). Eighteen patients had multiple fractures, fifteen had associated systemic injury, and only

Aerobic and anaerobic microbiology of infections after trauma in children.

OBJECTIVE: To review the recovery of aerobic and anaerobic bacteria from infections after trauma in children over a 20 year period. METHODS: Only specimens that were studied for both aerobic and anaerobic bacteria were included in the analysis. They were collected from seven separate centres in which the microbiology laboratories only accepted specimens that were properly collected without contamination and were submitted in appropriate transport media. Anaerobes and aerobic bacteria were cultured and identified using standard techniques. Clinical records were reviewed to identify post-trauma

Current trends in the treatment of femoral shaft fractures in children and adolescents.

Healthcare cost containment and a desire for early discharge of the pediatric patient to the home environment have become important factors in the treatment of femoral shaft fractures in children. As a result, newer techniques of treatment have become popular. The immediate hip spica cast remains the primary method of treatment for most children 6 years of age and younger. The treatment for children between the ages of 6 and 10 years is the most controversial. Many such patients may be treated successfully with immediate hip spica casts. However, external fixation and flexible intramedullary r

Rural hospital transfer patterns before and after implementation of a statewide trauma system. OHSU Rural Trauma Research Group.

OBJECTIVE: To evaluate trauma transfer practices in rural Oregon before and after implementation of a statewide trauma system. METHODS: A pre- vs post-system implementation (historical control) analysis of trauma transfer practices was performed using a sample of rural ED trauma patients from 4 Level-3 and 5 Level-4 trauma hospitals. Medical records of patients with specific index injury diagnoses in 4 anatomic regions (head, chest, liver/ spleen, and femur/open-tibia) were reviewed for a 3-year period before statewide trauma system implementation and 3 years after hospital trauma designation

[Case report of severe multiple trauma. Anesthesiologic techniques].

The modern anaesthetic devices have now put an end to the sixties-fashioned aphorism: "Closed circuit=open coffin". The currently renewed interest in the recirculation systems is justified by the following reasons: economic benefit; environmental advantages; clinical features, such as: 1) better humidification of gases; 2) better preservation of warmth; 3) monitoring of O2 consume; 4) gas analysis on each respiratory act. This allows the use of fresh gases at low flow without the risk of hypoxia or overdosage in patients and it guarantees for safe anaesthesia. The following clinical case clear

Double type III epiphyseal separation of distal ulna associated with ulnar nerve palsy.

This article presents a previously unreported double type III epiphyseal separation of the ulnar head associated with ulnar nerve palsy. The ulnar epiphysis burst into volar and dorsal segments, the latter giving rise to the ulnar nerve palsy. The patient was treated by open reduction of the translocated epiphysis and neurolysis of the ulnar nerve. Although good clinical results were obtained after surgery, forearm rotation remained limited, confirming that open reduction immediately after injury is mandatory in treating type III epiphyseal separations.

The spectrum of pediatric injuries after a bomb blast.

The spectrum of pediatric injuries seen after a bomb blast is poorly documented. The pathophysiology of blast injuries differ significantly from other forms of trauma and typically result in large numbers of distinctly patterned injuries. On April 19, 1995, a truck bomb was detonated directly adjacent to the Alfred P. Murrah Federal Building in Oklahoma City, Oklahoma. A total of 816 adults and children were injured or killed as a direct result of the blast. Twenty infants and children were seated by the window of the second floor day care center at the time of the explosion. The injuries incu

[Analysis of the S.O.T.EST series].

UNLABELLED: 126 records were studied retrospectively. The average age was 31.8 years (15-80 years) with 81% men which corresponds to other reports in the literature. Nearly half of the cases involved an accident of a two wheeled vehicle. The fractures were in the middle third in 50.1% of cases, the lower third in 33.6% and the upper third in 11.4%. 4.9% of fractures were bifocal. 46% had additional injuries (34% multiple injuries, 8% head injury). 4% were true polytrauma cases. The fracture was treated by external fixation in 103 cases and by intermedullary nailing in 23. Skin cover was by ped

Rehabilitation and reintegration of multiply injured patients: an outcome study with special reference to multiple lower limb fractures.

In this study a group of 104 multiply injured patients (mean ISS = 34) was examined for physical status and social reintegration. Essentially it was noted that 76 per cent of all patients were able to return back to work, although this appears to be age dependent. Open fractures of the leg seem to be the dominating prognostic factor for reintegration resulting in a disability rate of between 20 and 50 per cent. Disability of more than 80 per cent was only due to major head injury. Apparently reintegration is linked to the pattern of injury and is less related to the overall injury severity. We

[External humeral fixation in war injuries].

Type III open fractures of the humerus caused by high energy missiles or shrapnel in 26 male soldiers were treated by external fixation. 12 suffered from associated injuries involving the head and chest, 12 had 14 nerve injuries, and 4 required simultaneous repair of the brachial artery. Results in 19 examined after an average of 6.5 years were excellent in 22%, good in 52%, fair in 16%, and poor in 10%. The aim of external fixation is functional limb salvage. We think high-energy gunshot fractures should be classified as III-d because of the severe damage to the soft tissue, which differs fro

[Femur head dislocation fractures. Long-term outcome of conservative and surgical therapy].

From 1974 to 1989, we treated 32 patients with a femoral head fracture: 28 were associated with a traumatic posterosuperior dislocation of the hip (6 Pipkin type I, 7 type II, 4 type III and 11 type IV), 1 with a posteroinferior and 3 with an anterior displacement of the femoral head. Twenty-four patients had been involved in a traffic accident, 23 had associated injuries, and the average polytrauma score (PTS) was 21. All dislocations primarily treated at our hospital were reduced by closed methods within 4 h (mean 105 min). Eleven patients received no further operative treatment, 21 were tre

Plate osteosynthesis of diaphyseal fractures of the radius and ulna.

Between January 1980 and December 1989, 133 consecutive patients were treated for a fracture of the shaft of one or both forearm bones (134 forearms in total). All fractures were stabilized with AO/ASIF 3.5 mm stainless-steel dynamic compression plates. The 1 year follow-up rate was 99 per cent; the long-term follow-up rate was 92 per cent (the mean long-term follow-up was 10.2 years (range, 2.7-15.2)) so there were 96 men and 35 women, with an average age of 37.5 years (range, 16-63). Twenty-two per cent of the forearms had open fractures, 26 per cent of patients had sustained multiple injuri

[Open post-traumatic anterior luxation of the hip in children. Apropos of a case and review of the literature].

INTRODUCTION: Anterior open traumatic dislocation of the hip in children is rare. MATERIALS AND METHODS: We report the case of a child aged 9 years, who presented following a motor vehicle accident an anterior open dislocation of the left hip associated with a fracture separation of the trochanter, a fracture of the homolateral humerus and a fracture of the contralateral femur. He was treated by open reduction of the dislocation and pinning of the various fractures. RESULTS: After 15 months functional result of the hip was good. The radiograms showed early signs of necrosis of the femoral he

Treatment results of patients with multiple trauma: an analysis of 3406 cases treated between 1972 and 1991 at a German Level I Trauma Center.

The quality and progress of treatment for 3406 multiple trauma patients was reviewed retrospectively. Two periods (1972 to 1981, the first decade, and 1982 to 1991, the second decade) were compared. Sixty-nine percent of patients with multiple trauma had cerebral injuries, 62% thoracic trauma, and 86% fractures (40% open fractures). Concerning injury combinations, there was an increase of head/extremity injuries and thoracic/extremity injuries, whereas all combinations with abdominal injuries decreased. The relation between severity of injury as well as number of injured body regions and the m

[External fixation of the lower limb in children].

18 children (13 boys, 5 girls) with displaced fractures of the femur (13 cases) and tibia (5) after road accidents were treated with Wagner external fixation. 5 fractures were open and 13 closed. 9 also had head injury and were unconscious. In 3 fractures the popliteal artery was torn, requiring arterial reconstruction. 15 were examined an average of 2.3 years after injury and there was full range of motion in 13. In 2 after severe injury, range of motion was 80% of normal. There was refracture after removal of external fixation in 1. We conclude that external fixation is stable and has minima

Internal fixation of proximal humeral fractures.

Fractures of the proximal humerus are common, especially in elderly individuals, and are usually classified according to Neer into fractures having one, two, three, or four parts with or without an associated dislocation or an injury splitting the humeral head. This article is intended to assist the surgeon in selecting and using techniques for internal fixation of these fractures. The use of open reduction and internal fixation using plates and screws has become less popular because of the extensive dissection required and the tendency for the plate to impinge. Use of multiple percutaneous pi

Unreamed intramedullary nailing of femoral fractures.

Seventy-five patients with 81 femoral shaft fractures were treated with unreamed antegrade intramedullary nailing using a titanium alloy implant (AIM femoral nail, ACE Medical) with static interlocking. There were 73 closed fractures and 8 open fractures. Six patients had bilateral femoral shaft fractures. In two cases there was an ipsilateral fracture of the femoral neck, two patients had an ipsilateral intertrochanteric fracture, in one case there was a concomitant subtrochanteric fracture, and four patients had sustained an ipsilateral fracture of the acetabulum in addition to their femoral

Fractures of the radial head treated by internal fixation: late results in 26 cases.

Twenty-six patients, ranging in age from 14 to 57 years (average 29 years), were evaluated an average of 7 years and 4 months (range 1-14 years) after open reduction and internal fixation of a displaced radial head fracture. Using Mason's classification, there were 11 type II fractures, 9 type III fractures, and 6 type IV fractures with associated dislocation of the elbow. Seven patients had ipsilateral extremity injuries that included fractures of the coronoid process, capitellum, humerus, and distal radius. Using the Broberg and Morrey elbow score, good or excellent results were achieved in

Ipsilateral femoral neck and shaft fractures.

Ipsilateral femoral neck and shaft fractures occur in 2.5% to 6% of femur fractures. The injury results from high energy trauma. Victims are usually young, with multiple associated injuries. The diagnosis of the neck fracture is delayed in 19% to 31% of patients. The neck fracture line is almost vertical and nondisplaced, or minimally displaced in 26% to 59% of cases. The shaft fracture is often midshaft and open, and/or comminuted in 47% to 67% of cases. Ipsilateral knee injuries occur in 20% to 40% of patients. Two major complications, osteonecrosis of the femoral head and nonunion of the ne
