Between 1972 und 1993 a total of 68 patients were treated at the Department of Surgery of the University Clinic of Mannheim for peripheral vascular injury resulting from multiple trauma. Between 1968 and 1990, 26 patients (medium age 24.2 years) with a traumatic hip dislocation without fracture were treated at the Dept. of Trauma Surgery of the University Erlangen-Nuremberg. In the frame of multiple injuries we had 30% of the fractures of the femur. We treated 113 patients with polytraumas with fractures of the femur and with the plate. We had 25% AIS grade 1, 48% AIS grade 2, 21% AIS grade 3 and 6% AIS grade 4. Mortality was 4%. Indications for osteosynthesis with plate are: severe head injuries, fractures of the pelvis, open fractures, injuries of the nerves and blood vessels and severe trauma of the abdomen.

A whole-body bone scan was performed to search for undetected fractures in forty-eight patients who had multiple injuries or a head injury, or both, and who were less than twenty-two years old. During 6 years we treated nearly 500 patients with pelvic fractures. Three hundred forty-eight were admitted directly to our hospital following blunt injuries; these patients formed the basis of this review. There were 220 men and 128 women with an average age of 31 years, a mean Injury Severity Score of 21.8, and an average hospital stay of 16.5 days. Blunt pelviperineal trauma has been associated with mortality rates of 32% to 58% in recent literature. A review of our institution's experience revealed a much lower mortality rate, prompting further investigation. The treatment data on 3,406 patients with multiple injuries were analyzed retrospectively from the viewpoint of quality control. The diagnosis, therapy, complications and course were documented and the treatment results of two decades (1972-1981 and 1982-1991) compared. We analyzed 54 civilian patients (1988-1992) with gunshot wounds (GSWs) of the face to review the management principles and results. Urgent airway control was needed in 18 cases (33%): by orotracheal intubation in 13, cricothyroidotomy in two, tracheostomy in two, and nasotracheal intubation in one. A study was done of twelve patients (seven boys and five girls) who, because of multiple injuries or a head injury, had been managed with compression plating of a unilateral or bilateral femoral-shaft fracture at a level-I trauma center from 1986 through 1990. The patients had a total of fifteen fractures. The average age at the time of the injuries was eight years (range, five years to nine years and eleven months). From 1986 to 1991 we treated 63 patients with subtrochanteric fracture of the femur. The etiology in 53 patients with 40 fractures mostly pedestrians, hit by a car; or motor vehicle accidents, in other words high velocity injuries that resulted in a significant number of multiply injured patients of whom 38 were polytraumatized, by our definition a multiply injured person with a life threatening injury of at least one of the following body parts: head, neck, thorax, abdomen, spine, extremities with neurovascular compromise and pelvic girdle.

Pedestrians struck by motor vehicles have the highest mortality and morbidity rates of all motor-vehicle traumas. Fracture patterns and mechanisms were reviewed in a retrospective study of 115 consecutive patients.
